I sent a dispute letter on XX/XX/2022 to United Revenue Collection Service, I asked for proof of practice and proof of any legal obligation to pay them, Ive asked them to provided a copy of my signature with the provider of service to release my information to them. Ive asked them to please provide me with a verified certificate of authority, and proof that their company is licensed to collect debt in my state, also, Ive ask them to provide me with a license number. XXXX XXXX sent a response to me and only provided me with description of charges, amount, patient number, date of call, reason of transport, and nothing else of what I asked for. More over, I have never lived at the address they have on file. I receive numerous calls from this agency in a day to day basis. I would like this off my credit if this company can not provide me with the information I asked for. Im hoping to receiving the documentation I asked for by the owners XXXX XXXX, XXXX XXXX, or XXXX XXXX XXXX from United Revenue Collection Service.

On XX/XX/2019 United Revenue collections added a derogatory mark on my credit report per XXXX XXXX Upon further research, the account was transferred/sold and I am not responsible for the account. The debtors creditor did not verify this collection. Also, there was no written form of communication, contract, or signature for this accused debt at all. A bill without a written signature does not validate the debt and anyone can say its mine. According the FCRA, XXXX XXXX XXXX has violated my rights as a consumer, and I am requesting legal action to remove this account immediately.

We notified the gym owner we would be ending our gym membership at the end of the month, we were on a month to month plan with no contract. Upon letting the gym owner know he became very irritated and would just leave in the middle of us talking to him. At the end of the month we had repeatedly told him not to charge us anymore, and that we were not returning.
He then reached out to a collections company where XXXX XXXX another phony alias would call and harass us 30 times a day. He threatened us verbally as well.
