RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT METRO 2 data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX XXXX XXXX XXXX and XXXX. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with The XXXX XXXX XXXX Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C. 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. Finally, you are reporting the INQUIRIES that are the result of the same type of fraudulent activities. It must be noted that I did not apply for credit accounts with the below-listed companies and creditors, some of these are the result of attempts to open accounts, but the companies did not approve the accounts. Therefore, these inquiries must be deleted from within my credit file. You can not delete the fraudulently opened accounts without also deleting the fraudulent inquiries. It should be noted that an inquiry that was not made by me is not a factual record, just as a fraudulently opened account is not a factual record and not referred to the creditor to be disputed directly with them. The credit reporting agencies have attempted to address an inquiry as a factual record of file access, but if it was done through fraudulent means, it is not a factual record. As a consumer, I should not be required to dispute each fraudulent inquiry directly with each company. These inquiries are the result of fraudulent activity and therefore, inaccurate in which the credit reporting agencies or the creditors can not verify, meaning that these inquiries are unverifiable information and must be deleted from my credit files. 1. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. XXXX mailed to me a statement dated XX/XX/XXXX ( copy attached ) in the amount of {$45.00} due for services rendered XX/XX/XXXX. I checked this billing against my Claim Details with XXXX ( copy attached ), which indicated that I owe nothing, zero ( {$0.00} ) for those services. I chatted with an XXXX representative XXXX XXXX XXXX online XX/XX/XXXX, who contacted XXXX by phone at XXXX. XXXX told the XXXX rep. that she would have my " account adjusted to remove the patient balance of {$45.00}. She asks that you allow up to 45 days for completion. '' I hoped that the matter was resolved, but having had to contact CFPB in the past about XXXX, I suspected further trouble. I was correct. On XX/XX/XXXX I received a new statement from XXXX ( copy attached ) indicating that the {$45.00} charge had indeed been removed, but now XXXX was billing me for {$1100.00} for the very same services rendered on XX/XX/XXXX. Once again, sometime XX/XX/XXXX, I chatted online with an XXXX rep. XXXX XXXX XXXX, about the {$1100.00} that XXXX Claim Details had already indicated were not due. The XXXX rep. phoned XXXX at XXXX and advised him that I am not " responsible for the {$1100.00} that they are billing [ me ] for. Per XXXX request, '' said the rep., " I have e-mailed a copy of the explanation of benefits ( EOB ) for this claim to the address that he gave me. The EOB will show them that you are not responsible for this claim. Once they receive the e-mail it will be forwarded to their accounting department so that they can review and update file. '' Now I was really worried, for, as noted, I have had to get CFPB to investigate XXXX in the past to remove a similarly undue debt that the company had reported to the credit bureau. Now I suspected a whole lot more trouble XXXX. Again, I was not mistaken. XXXX continued to bill me {$1100.00}. So yet a third time, I chatted with an XXXX rep. ( XXXX again ) online XX/XX/XXXX. The XXXX rep. phoned XXXX at XXXX ; XXXX stated that " she sees a coding error on [ my ] claim. She sees the notes from XXXX call back XX/XX/XXXX. She apologized XXXX did not catch this at that time. She is sending your file to the reviewer to get corrected and re-filed to XXXX. She has taken your claim out of pre-collection status. '' Of course, I continued to worry. Every time an XXXX rep. contacted an XXXX rep. the XXXX rep. had a different story. I suspected a stall tactic. I also suspected that XXXX story would change. Again, of course, I was correct. The statements asking for payment of {$1700.00} continued to arrive in the mail. On XX/XX/XXXX I contacted an XXXX rep. XXXX XXXX XXXX for what I decided would be my final time. One : I had XXXX coverage with XXXX that would expire the next day ( XX/XX/XXXX ) ; and, Two : I had promised myself that I would contact CFPB if XXXX persisted, something I should have done from the beginning. The XXXX rep. phoned XXXX at XXXX, who stated that XXXX feels " the denial of the {$1700.00} charges XXXX was not correct and they state they have appealed with XXXX on! XXXX. '' The XXXX rep. said " the account is currently in a 'hold ' status until they receive a response to their appeal from XXXX XXXX '' To determine if XXXX was telling the truth, the XXXX rep. checked the appeal records. '' However, the rep. " did not find an appeal in the appeals system. '' The rep. concluded : " I am not so sure they have submitted the appeal, XXXX did n't sound so confident. She added that she needed to 'reach out ' to another department to find out the status of their request. '' The XXXX rep. finally concluded : " '' I regret to inform this seems like it will be an ongoing issue until they can get their billing in order. '' Once again, I knew XXXX would persist. Once again, I was correct : I received a collection notice from United Consumers , Inc. ( UCI ), dated XX/XX/XXXX ( copy attached ), for the {$1100.00}. This is the last straw.

XXXX XXXX XXXX XXXX contacted United Consumers to open a collection and put a deragorty mark on my credit, from a hospital bill that occurred in early XXXX, when I was still just a minor. They opened the collection in XXXX, around the time Id gotten my first credit card. I wrote the company several times in XXXX using the contact us option on their website. I finally contacted them again yesterday and asked why I was receiving this bill when at the time, I was a minor with insurance under both of my parents. They told me they billed one of the insurances, and the debt left over was my responsibility. They said Any balance of the bill that was not paid by insurance will be billed to the guarantor or patient .... I wrote back and told them : This should not have been billed to me, and I am not responsible for paying this bill. According to Maryland Code, Commercial Law 1-103, the age of majority is 18 years old in Maryland and anyone under 18 can not be legally bound to a contract unless they are legally emancipated, which I can assure you I was not. This means I lacked the capacity and legal ability to enter any contract. ( please refer to section C, parts 1 ) ( a ) The Maryland Uniform Commercial Code shall be liberally construed and applied to promote its underlying purposes and policies. ( b ) The underlying purposes and policies of the Maryland Uniform Commercial Code are : ( 1 ) To simplify, clarify, and modernize the law governing commercial transactions ; ( 2 ) To permit the continued expansion of commercial practices through custom, usage, and agreement of the parties ; and ( 3 ) To make uniform the law among the various jurisdictions. ( c ) Unless displaced by the particular provisions of the Maryland Uniform Commercial Code, the principles of law and equity, including the law merchant and the law relative to capacity to contract, principal and agent, estoppel, fraud, misrepresentation, duress, coercion, mistake, bankruptcy, or other validating or invalidating cause shall supplement its provisions, except that : ( 1 ) The age of majority as it pertains to the capacity to contract is 18 years of age ; and ( 2 ) No person who is at least 18 years old shall be considered to be without capacity by reason of age. I also want proof of this debt, showing the contract where I supposedly signed my name and consented for my own treatment, within 30 days. To attempt to collect the debt on which the creditor can not show a contract or an underlying basis would violate the Fair Debt Collection Practices Act. According the FDCPA under Unfair Practices [ 15 USC 1692f ] : A debt collector may not use unfair or unconscionable means to collect or attempt to collect any debt. Without limiting the general application of the foregoing, the following conduct is a violation of this section : ( 1 ) The collection of any amount ( including any interest, fee, charge, or expense incidental to the principal obligation ) unless such amount is expressly authorized by the agreement creating the debt or permitted by law. I was unconscious when I was brought to the hospital by my best friend, and my father was called specifically because I was underage so that they could get consent for treatment by a parent. This debt needs to be taken off of my credit immediately or I will take this to the FDCPA and we can ultimately take this to court.

I was contacted by XXXX XXXX at United Consumers Incorporated ( ucicollects.com ) in XXXX 2015 about a purported debt of {$130.00} that I had with a medical provider. As I explained to XXXX, that matter was bogus as the provider had erred on a previous procedure and asked me back for a redo, explicitly stating that there would be no charge for that redo. After speaking with XXXX, within days, I finally succeeded in getting that bogus charge removed. I e-mailed the company with my account reference number at XXXXXXXXXXXX about this being resolved, and never heard back from UCI ... for a while. Incidentally, in XXXX 2015 I received an invoice from that provider showing that I owed nothing. On XXXX XXXX, 2015, I received another phone call from XXXX about this purported debt, and he indicated no awareness that we had discussed this a few months earlier. ( At that time, I was n't sure if he was the person whom I 'd spoken to in XXXX. He said nothing when I told him that I 'd spoken with someone and did n't know if it was him or not. ) He asked me if I wanted to pay it through him right then and there -- trying to receive money from me over the phone which I did not owe. He gave me the same UCI account reference number that he 'd given me in XXXX. While I do n't blame UCI for trying to collect money from me in XXXX for a debt that they were told I owed, it is inexcusable that they would call me months later to receive payment for a " debt '' that even the provider had long acknowledged I did n't owe. Moreover, doing so -- telling me that I owed money which I did not -- was IN VIOLATION of 28-3814 ( f ) ( 5 ) of XXXX Code. Apart from the annoyance of harassment, my bigger concern has been a collections agency affecting my credit rating. I e-mailed XXXX and the company about this on XXXX XXXX, 2015, including a scan of the provider 's original assertion that I would not be charged for that procedure and also a scan of the XXXX invoice showing that that provider was requesting not one penny from me ( he had cited the same figure of {$130.00} that he had cited in XXXX, so it obviously was now resolved though he incompetently did n't confirm this before harassing me over the phone while I was at work ). On XXXX XXXX, I e-mailed the company at the XXXXXXXXXXXX address with my account reference number. I also called their phone number and left a message to alert them to all of this. I followed-up with another e-mail on XXXX XXXX, noting that I had already begun investigating legal action and became aware of that section of the XXXX code that they had unquestionably violated in harassing me by asserting that I owed money which I did not owe. On XXXX XXXX, I still had not heard from them with any assurance that they wold back off and that my credit rating would not be affected by any of this. Today is XXXX XXXX and I still have heard nothing. It has been three weeks since I last received communication from the company, when they insisted that I owed money and tried to have me pay it to them when I in fact owed nothing to that provider and had n't for months.

On XXXX/XXXX/XXXX a claim through XXXX in the amount of {$1100.00} from provider " XXXX XXXX XXXX XXXX '' was processed with {$770.00} applied by XXXX, it was then reprocessed that same day as {$580.00} applied instead, the difference being an over payment in the amount of {$180.00}. The above " XXXX XXXX XXXX XXXX '' is an in-network provider. This overpayment amount {$180.00} is to be paid by the provider and neither by XXXX or myself. " XXXX XXXX XXXX XXXX '' should therefore pay this without ever billing anyone. For reference purposes to distinguish this from proper balance-billing, a different situation which is common, arises for when an out-of-network doctor appears for example unexpectedly in an ER or XXXX setting ( e.g. XXXX ) but the provider overall is in-network, where balance-billing is invoked. Customer/patient pays provider, XXXX reimburses provider and we are done. I have experienced this before and it works pretty smoothly. NOTE balance-billing is a proper example. The addressed situation in question is NOT applicable to balance billing and is the improper use of billing an overpayment to customers. The reason for this request of investigation into intentional fraud and extortion, is that 16 months later from the date of XXXX/XXXX/XXXX " XXXX XXXX XXXX XXXX '' has fraudulently applied the over payment amount of {$180.00} to be paid by me. There was never any correspondence sent to XXXX or myself at all within this time. Instead a collection agency ( United Consumers , Inc ) was improperly contracted by " XXXX XXXX XXXX '' to handle this. The use of a collection agency without notifying XXXX or myself prior is an act of a ) fraudulently passing an overpayment as a balance owed by a patient and b ) extorting patients for the overpayment amount since their credit score will be effected quickly. Based on other reports against " XXXX XXXX XXXX XXXX '', I believe that these overpayments are intentionally fraudulently applied as balances, especially the smaller amounts being sent to collection so that patients pay providers without questioning the activity. I did not receive notice in the mail of my right to dispute this debt AND I did not receive any notice in the mail of an attempt by this company to collect a debt. The debt collection agency had an apartment number XXXX XXXX XXXX XXXX in my address thus making the address invalid/inaccurate. They claim to have sent me a letter but will not provide me any proof of a letter showing notice of collection of debt. I have called this agency on numerous occasions to try to get proof that they sent me a letter in the mail to no avail. Each time I speak with this office they are extremely rude, will not allow me to voice my concern and continuously speak over me and rush me off the phone. Nobody at this establishment will use ethical practices to realize they had the wrong address entered into the system and that I, the customer, is telling the truth that I never received notification. Once I told them of my address, they claimed to update their system and delete the " UNIT XXXX '' association off of my profile at the time when I paid the account in full. I have disputed this illegal, inaccurate, unethical and wrong information with all three credit bureaus to no avail. Each time I submit a dispute, XXXX XXXX XXXX, decides to reject my dispute of this account without researching their systems completely. If XXXX XXXX, can not review my phone call with them in XXXX XXXX to realize that they had my address wrong then they are discriminating against me and being extremely unethical. The situation is as follows : In XXXX 2015 I started a summer internship in XXXX. I was asked to do an electronic signature. I asked if there is a contract and they guy from the gym said that there is no contract. Per XXXX said that the signature is just so that I can get my pass to try the gym. He asked me to enter credit card info as a standard practice ( also as a requirement to do this free month ). I never signed any contracts and was never shown one. All I did in XXXX 2015 was my electronic signature on a machine to state that I agreed to one month free membership to try this facility. You can check their documentation - you will not see my signature in ink, because I never intended to be in this location for more they a month or XXXX. To Whom It May Concern : I am writing to dispute a new derogatory remark that appeared on my credit report on XX/XX/XXXX. This collection stems from an incorrect billing code that neither the doctor 's office nor the insurance company was able to fix after both parties acknowledged that it is a billing error and I should not be responsible for the payment. I received a letter on XX/XX/2018 from United Consumers , Inc. stating that I had been reported to them from XXXX XXXX XXXX XXXX ( actually XXXX XXXX XXXX, though that was not referenced in their letter ) for an amount of {$54.00} ( actual amount on every bill I knew about was {$55.00} ). The letter said that if I wished to dispute the collection, I needed to submit a written notice within 30 days. I sent a letter on XX/XX/2018 ( that is well-within 30 days ) with my written dispute, providing documentation as to why I was not responsible for the amount. I had reached my maximum out of pocket as of XX/XX/2018, which I had told XXXX XXXX XXXX on two occasions over the phone, as well as providing them with the letter on two occasions. I provided that letter, as well as my Explanation of Benefits from my insurance company where they clearly denied the claim and stated the provider ( XXXX XXXX XXXX ) was responsible for the charge. I sent it to the only address provided by United Consumers , Inc. on their letter. On XX/XX/2018, there was a voicemail left on my phone - no phone call, just a voicemail that bypassed any phone call - stating I needed to call XXXX about a debt. It did not say who I needed to speak to, did not reference an account number, did not reference an amount, did not provide any other information whatsoever. When I called, the person spoke too fast for me to understand his name ( he was male ), and he told me I had an outstanding bill that was due and needed to pay it. I told him I had sent my disputation letter within my 30 days, and he told me they never received it. He then proceeded to tell me the U.S Post Office is unreliable, and so I shouldn't have trusted it would have reached them. He said he would give me a fax number to fax my documents, but I was upset and demanded to know why they offered the letter as an option if they felt there was no point sending it. He refused to answer. I then demanded to know why a fax number wasn't provided from the beginning if the Post Office was deemed so unreliable, especially when they were making a requirement of a WRITTEN disputation. He refused to answer and continued to refuse to answer, before finally stating he was not going to deal with me and hanging up. I was never given the fax number, and it is most certainly not available on the paperwork provided.

Recently I received a notice from XXXX XXXX that a new item had appeared on my credit report. When I checked, it listed an account wi th United Consumers ( acc t. # XXXX ). I contacted United Consumers and spoke to XXXX XXXX who told me it was a charge of {$180.00} due to XXXX XXXX XXXX . She said that it was for natural gas charges owed on a previous address. I told her that I had my gas with XXXX XXXX XXXX and that when I moved, all charges were paid and my deposit was returned because all charges were paid. She explained to me that XXXX XXXX XXXX provides natural gas to XXXX XXXX XXXX , and that even though they were always charged on one bill, upo n termination the bills were separated. I never received a bill from XXXX XXXX XXXX , or I would have taken care of it at the time. I asked XXXX XXXX why I had not been contacted and she said they had sent a letter to my old address and had " assumed '' that it had been forwarded. I told her that I was willing to pay, even though there was no proof offered that I owed the debt, as long as it could be pulled from my credit report. She said that they are not able to pull information from my credit report that XXXX XXXX XXXX are the only ones that can do that. I then contacted XXXX XXXX XXXX XXXX to discuss this ( original acct # XXXX ). I spoke with XXXX and explained the whole situation. She seemed sympathetic but said that this would have to be handled by the " back office ''. I asked her if there was anyone I could talk to and she told me that I could only send my complaint to the back office via XXXX XXXX XXXX . I sent them an email with my concerns and request on XXXX / XXXX / XXXX . I have yet to be contacted by anyone regarding my request. I have since looked to find any office information where I can handle this in person, but XXXX XXXX XXXX has no public offices listed. It is very difficult to take care of an issue when the only possible way of dealing with it is to contact ( via faceless email ) some so-called back office. I am not trying to get out of paying a legitimate debt, but I was not properly notified and given an opportunity to take action. I am willing to pay what I owe, but due to the circumstances believe that it should be pulled from my credit report.

This company has been harassing me for almost a year now. My phone does not ring but instead goes straight to voicemail where they constantly leave robo messages. At first, I tried adding their number to my cellular phone 's " Block '' list but that didn't help. Out of complete frustration, I did some research on how to stop these calls. I found out about the National Do Not Call Registry and took the proper steps to register my phone number on XX/XX/XXXX. I am aware that it can take up to 31 days for the calls to stop but they were relentless. I then filed a complaint with the National Do Not Call Registry ( not sure of the date ). On XX/XX/XXXX, I filed a complaint with the FCC about the same company which is United Consumers. I was still being flooded with calls. On XX/XX/XXXX, I filed another complaint with the FCC. Since doing so, the calls continue. I have tried calling the number back to speak to someone and it's a robo message and only a message can be left. On XX/XX/XXXX, I left a message including my phone number and asked that they stop harassing me and stated to them that I am on the National Do Not Call Registry and that I also filed complaints with the FCC. I also told them that if they insist on continuing to call that I would take more drastic measures if the calls do not stop. Low and behold, the calls ( voicemails ) continue. I have no idea what this matter concerns because I can never get a live person. On the robo voicemail, it leaves another number to call to resolve the issue and I have tried calling that number, only to get the exact same robo message and no live person. Now, they have resulted to, not only calling from their " standard '' number, but have now started calling from another number as well from the exact same company. I am hoping that you can be of assistance and if not, I am only left with filing a class action suit. I have tried numerous times since XX/XX/2019 to make payment arrangements with this company. each time, the representatives are extremely rude, raising their voices and hanging up on me. I offer to make payments and they refuse to accept the amount that I am trying to give, even though I tell them that what they are asking for will put me in a tight spot. I find this to be very abusive and degrading. The first time I called, I asked if I can make payments online. I was told that they do not have a way for me to make payments online. and, even if there was, they will not honor any payments that I make because I am not " approved '' to make payments. The second time I called, I was over-talked, yelled at, laughed at and was told that there are no supervisors in the company. Then when I demanded that there has to be someone overseeing the office, I was just transferred to a different rep who laughed at me and told me to call back in the morning if I want to speak to someone else. The last person I spoke to on XX/XX/2019, who said she was a " supervisor '', implied to that I do not keep my word and that I do not pay my bills. She lectured me telling me that I am in this situation because I can not be trusted to keep my word. Then she tells me that, even though I made several attempts to make payments, it is my fault that no payments have been made and that her Company requested a lien on me because of it. i received a call on XX/XX/2019 around XXXX, and missed the call. I returned the call and questioned the call. the representative asked for my phone number to pull my file, and said she was calling for united consumers inc, I paused and she pressed that she was calling for a "medical file" reason. I questioned if she was calling from my hospital, and she pressed me for my date of birth to verify my information. I did not feel comfortable with this but she asked for my information again. Before she was able to get my additional information she never indicated she was a debt collector, or collecting information to pursue an alleged debt. After verifying my date of birth. She continued the conversation revealing she was calling to collect a debt alleged to be against me. I find that the way she started the call was deceptive and she should of revealed that she was attempting to collect on the onset of the call. I told her that I would like verification of the debt, and she argued that it was already sent, also she asked if I would be paying the alleged debt and i indicated i did not want to answer, so she said she would place in the file that I refused to pay which is incorrect. she asked for my confirmation of my employer for what I believe was an attempt to infer that she would let my employer know i have an alleged debt against me.in the end of it all i am disturbed at the way this was handled, she misrepresented herself at the onset of the call, asked for information before indicating it would be used in an attempt to collect an alleged debt, and spoke to me in an abusive manner.

I received a bill for medical services by XXXX XXXX rendered on XX/XX/XXXX a couple of months ago. I spoke with XXXX XXXX several times to have the remaining balance after insurance paid using my HSA card. On the third attempt with XXXX XXXX they were able to process a payment using my HSA card. I received the most recent bill dated XX/XX/XXXX which doesn't show the HSA card payment, but shows a balance of {$110.00}. This bill was received on XX/XX/XXXX. Last week I sent in using the remittance stub and enclosed envelope a payment of {$50.00} which as of today has not been debited from my bank account. I intend to pay the {$66.00} remaining balance when I am paid again on XX/XX/XXXX. Yesterday, I received a collection notice from United Consumers , Inc. dated XX/XX/XXXX which is less than the 10 business days the XXXX XXXX statement mentions possible collection action. Further it shows a balance of {$110.00} and {$12.00} dated XX/XX/XXXX from XXXX XXXX. These amounts do not align with the statement from XXXX XXXX XXXX dated just 7 days prior. Lastly, in the United Consumers , Inc. letter it shows two charges from XXXX XXXX dated XX/XX/XXXX for {$2.00} and {$37.00} ( 48 days ago ) which I have not even received a bill by XXXX XXXX XXXX. XXXX College of Virginia ( XXXX ) is a division of XXXX XXXX University and was changed to XXXX XXXX several years ago to reflect it is one institution.

I incurred medical costs out of pocket of {$1300.00} to XXXX hospital XXXX of XXXX VA for XXXX XX/XX/XXXX. I paid without missing about {$100.00} per month and the balance was down to {$260.00} by XX/XX/XXXX when monthly billing by XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX. suddenly stopped coming. I had to fight to get subsequent monthly billings with the return envelop since XX/XX/XXXX. Now the bill is down to {$65.00} and XXXX has forwarded this debt to United Consumers , Inc. ( UCI ). I explained to a rep at UCI identifying himself as " XXXX '' that I simply need a final billing statement with the {$65.00} outstanding balance and I can send them the money and close this debt but UCI over the past 2 months have been promising to send this but have not. I suspect that they 're trying to obtain attorney and collection penalties from me some time soon when I have not been negligent and have been trying to close this debt if only they will stop playing games and send me the final bill and return envelop immediately. This is all I ask so that they ca n't scam me with anything down the line in terms of late penalties, interest, or attorney fees on this {$65.00} debt. There 's also been some misrepresentations on the origins of this bill and the amount which I wo n't bother going into. I am writing to dispute a bill that I received from United Consumers Inc. on behalf of XXXX in the amount of {$990.00}. After I terminated services with XXXX, I never received a final bill ; they simply sent this account straight to collections! There is no way that I 'm going to pay for services that were not provided to my children as I had to pull both of them out less than a month into the XX/XX/XXXX-XX/XX/XXXX school year due to negligence on behalf of the staff at XXXX. Their father and I had to pay more than {$400.00} to get them enrolled at another center on very short notice. My XXXX XXXX XXXX was being bullied by an older boy at XXXX and even after complaints from my mother and myself, no one took action. After speaking with the regional mgr. at Corporate a day or two after the incident was initially reported by my child, I informed her that since the director of the XXXX XXXX did a poor investigation of the situation, my children would not be returning as I did n't feel like they were being protected there. I also explained to her that this has caused me to miss a day of work as I had to scramble to make new child care arrangements for my children. United Consumers, a Collections Agency does not do anything except post allegedly owed debts onto my credit report. Their client never sent us an unpaid bill in XX/XX/XXXX and in fact we never found out that we had a bill that I alledgedly owed for {$87.00} until it first appeared on my XXXX credit report in XX/XX/XXXX. Upong disputing the debt, the collection was removed. Then, after being turned down for credit, I sent for a Copy of my XXXX XXXX Credit report again in XX/XX/XXXX and found out that United Consumers was claiming that I owed {$87.00}. Although I did not owe the amount, it was not worth a fight with them, so I paid it off ( see attached -- Proof of my payment in XX/XX/XXXX ) for a " Bad debt '' that I never Received a written notice in the mail or even a phone call about, EVER! This year, I found out that my credit score was being harmed again by the the United Consumers, for reporting that I have a collection on file for {$0.00}, and this is harming my credit. I recently found out an unpaid {$450.00} from " United Consumers '' on my credit report. It turned out was an unpaid medical bill from a car accident of my niece. I have paid the bill right after I found it out the same day last week. And claimed the money from car insurance company. My question is this was billed since last year but I had never received any document/request from neither the hospital nor the collection company. Otherwise it was taken care of long time ago. So the debt was sitting there all these time and hurt my credit. I want the collection company to fix my credit ASAP because I am working on a finance my house right now. I went to XXXX to received the medical emergency help in XXXX 2015 due to the work injured and I had the claim number which I provide to hospital, but the bill was sent to me. I tried calling ( XXXX ) XXXX number in hoping to talk with XXXX ( the actual bill : proof file attached ) trying to explain to them to what was going on, but this number would never be working. United consumers has been leaving me voicemail on my work phone stating they are a collection agency and I need to contact them. I have contacted several times and asked what this is about since I am not aware of any outstanding bills. When I get in touch with somebody, they state my phone number I am calling from, provide me my name from my phone number and then continue to ask for more personal identifying information. I am NOT going to give some company I have no idea who they are any information since they seem to know who I am when I call. They have refused to provide me any reason for calling me and leaving the threats regarding unpaid bills. At this point I am furious and need an explanation. There are way too many scams out there and I will never provide any personal identifying information to some voice on a phone, If they have my phone number and my name and I return their call, I expect a quick response as to what the issue is, not asking for more information to add to their attack. I had a XXXX appointment on XX/XX/XXXX. I was contacted by United Consumers , Inc. in XXXX via mail stating that I owed four payments of {$28.00}, {$310.00}, {$81.00}, {$42.00} respectively. I contacted the company and was told the debt was for said XXXX appointment. I then contacted the XXXX who reported that the charge had long since been paid off, first by me and the outstanding balance by the insurance company, and that the last two charges were erroneous. I reported this to the company and told them to sort it out with the healthcare provider. I received another mailing on XX/XX/XXXX for the same charges and when I called ( took 4 calls to talk to a representative ) I was told that the XXXX had said the charges are still in effect, in direct contradiction to what was told to me. After issues a dispute with the insurance company paying a hospital debt, the debt was passed to a debt collector. I contacted the debt collector to clear the amount. The deb collector told me I could either work out a payment plan or I could pay a slightly lower one-time payment to clear the debt. I went with the second option ( total debt was {$1100.00}, I paid {$970.00} ). The company left several messages over a span of a month or two. The messages were intimidating and aggressive - the recorded voice emphasized that it was important that the listener be in a private location and only be the person intended for the message. There was no reference to a specific debt the company was trying to collect. When I called to inform United Consumers to report that this debt is not mine, I was told that the phone number is linked to the original account. United Consumers engages in harassing tactics to collect debt that has been paid. They have robe called my home repeatedly and when I press 2 to talk to a representative the call is dropped. XXXX I reached XXXX XXXX, who refused to contact the medical provider verify the debt, which was already paid a while ago. XXXX XXXX insisted that I had to prove to her that I paid the debt or the harassing calls would continue.
