RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT XXXX XXXX data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX XXXX XXXX XXXX and XXXX. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with XXXX XXXX XXXX XXXX, Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C . 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. 1. TSC A/R SOLUTIONS bal. {$150.00} I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from alleged identity theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer that the information relating to the transaction by the consumer. I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. In XXXX my Primary Physician 's office, of which I've been a patient for over XXXX years, XXXX XXXX XXXX, incorrectly charged my expired insurance for a doctor 's visit I paid for upfront ( with debit/cash. ) In other words, even though I paid for the visit at the Front Desk, before seeing the doctor, {$120.00} with my debit card ( because I no longer had insurance ) the person I SCHEDULED the appointment with over the phone had automatically clicked on my OLD insurance, so the bill came back unpaid, as I no longer had this insurance. When I got a a bill in the mail a few months later, took it with me to my next doctor 's visit. When I called to schedule this visit, the receptionist told me that I had an outstanding balance and that I would need to pay it before the appointment when I got there. I explained that I had paid upfront, and that the bill I got was because they 'double charged ' me by charging an old insurance I no longer had ( for several years. ) She looked it up in the system and saw my upfront payment of {$120.00} made with my debit card for that visit. She apologized, took the incorrect charge off, and I continued my doctor 's appointments, paying up front each time. However, in XX/XX/XXXX, I got a call from and a letter from TSC ACCOUNTS RECEIVABLE. I spoke with them over the phone, told them to contact XXXX XXXX, XXXX XXXX XXXX, so they could explain that they had made a mistake. They said that they would look into it. Several more times throughout XXXX, I got letters and calls. It is in my credit report. I have disputed this with XXXX and XXXX 5 times in the last XXXX XXXX years. Each time, the dispute results come in as " it comes back as, " XXXX investigated your dispute but the information was verified as accurate. '' or " XXXX investigated your dispute but the information was verified as accurate. '' Each time I get a notification or a call from TSC about this debt collection, I contact XXXX XXXX XXXX, XXXX XXXX XXXX XXXX, XXXX, XXXX XXXXXXXX, at which I am STILL a patient, to tell them the TSC is STILL stating I owe. And each time, XXXX XXXX XXXX assures me that there is NO RECORD of my account or a payment being in collections. XXXX XXXX XXXX says they will explain this to TSC, if they contact them, but that they show nothing about me owing anything outstanding or being in collections. Therefore, TSC Accounts Receivable is NOT doing their part to look into it and my credit report is suffering BADLY. If XXXX XXXX XXXX is being dishonest or not taking the time to speak with them, that is equally bad. But I doubt this is the case, since you can NOT have a doctor 's visit there with an unpaid balance, and I visit this doctor XXXX times a year, for XXXX XXXX XXXX XXXX XXXX XXXX Please help me. TSC ACCOUNTS RECEIVABLE and this charge have been on my credit report for XXXX and XXXX XXXXXXXX years, my credit score is affected as this is over {$350.00}. Thank you!

Good Morning, I called TSC Accounts Receivable Solutions and spoke with a representative named XXXX, who did not disclose himself as a debt collector. He told me the balance of my $ XXXX debt was $ XXXX. I asked him about the additional {$400.00} dollars added to my medical bill and he stated that the medical bill accrued interest and he is allowed to do this by law. I asked what law defends adding an interest amount to a medical bill when I never initially agreed upon having interest accrued on any medical bill I attained on my date of service with my medical provider. He stated that civil code- Article 15 Section 1 Part 1 supports a 10 % interest is due per year on any unpaid medical bills once collection efforts begin. I researched the statute while on the phone with XXXX and explained that after reading the statute, it states that this accrual of interest is for : For any loan or forbearance of any money, goods, or things in action, if the money, goods, or things in action are for use primarily for personal, family, or household purposes, at a rate not exceeding XXXX percent per annum ; provided, however, that any loan or forbearance of any money, goods or things in action the proceeds of which are used primarily for the purchase, construction or improvement of real property shall not be deemed to be a use primarily for personal, family or household purposes ... and that it did not state this was allowed by law. He then became very rude and stating that " thats neither here or there but you can either pay in full or it can stay on your credit ''. I stated that I would like to pay my balance in full but not {$400.00} more than initially agreed upon my date of service. He again became rude on the phone and began to bully me into paying, at which time I asked for a supervisor and XXXX declined. I disconnected the call and called back to speak with a supervisor at which time I spoke with XXXX who supported the disrespect and sarcasm of her employee and stated there is no supervisor for her although she disclosed that the owner is who I would need to complain to. I asked for her email, she stated it was XXXX. XXXX could not explain the interest due or the law that supports interest on medical bills, however out of desperation I paid the principal amount due and the illegally accrued interest. I am writing you today because I feel that I have been taking advantage of and refused the right to escalate the matter to the company and I do believe that their business practices should be investigated. In XXXX XXXX, XXXX XXXX XXXX refused care to me as a patient because she felt I owed her money even though I was up to date on payments based on the itemized billable account ( see attached billing ledger from her office manager, XXXX XXXX ). Even though it was BEYOND what was due, I paid the {$300.00} bill in full to the penny on XX/XX/XXXX. I have attached copies of the receipt of payment and email confirmation thank yous for full bill payment from XXXX XXXX and XXXX XXXX, the doctor 's office managers. In addition to attached copies of the ledger from XX/XX/XXXX, I have attached evidence of yet an additional payment I had made of {$150.00} on XX/XX/XXXX prior to the XX/XX/XXXX {$300.00} payment. The truth is : I paid WELL BEYOND what was due on the account according to the XX/XX/XXXX ledger balance. I did not see the doctor in XX/XX/XXXX AT ALL because the doctor still refused services to me after I paid my bill in full on XX/XX/XXXX. After I paid my bill in full on XX/XX/XXXX, she abandoned me as a patient despite the fact that I had a time-sensitive FDA regulated XXXX. It took me four months to receive much needed care and refill the XXXX. I was XXXX by this doctor when she refused patient care to me and yet I have not yet taken any legal action against her for my XXXX ( although I should ). TWO YEARS (! ) later, in XX/XX/XXXX, XXXX XXXX XXXX reported a collection of {$42.00} ( {$51.00} with interest ) to my credit bureaus -- over and beyond the final bill which was paid. This is fraudulent billing -- my account was paid in full in XX/XX/XXXX for {$300.00} and I have the receipt along with email confirmation that the bill was paid in full by her office manager. I have tried to contact the doctor 's new office manager 17 times to resolve this issue and he continues to hang up on me. I have asked for the doctor to call me back ten times and she has n't returned my calls. I have even tried to connect with the doctor on XXXX to reach her but she has rejected my invitation. I have tried to call the respective collection agency phone number listed as reference for this account on my XXXX credit report, and their number is " out of service. '' XXXX XXXX XXXX is a bad doctor with illegal billing practices. Her team has zero concern for patients or resolving patient billing issues fairly. She fraudulently charges patients and her billing procedures need to be investigated. Despite providing documentation about this issue in great detail including receipts, email confirmations, and more to XXXX, they refused to remove the Collection from my account. Both XXXX and XXXX have removed the Collection from my accounts, declaring it to be fraudulent billing. In XXXX of XXXX I was admitted into an emergency room ( I have XXXX XXXX XXXX XXXX ) so I made sure before I was admitted it was an in network facility so that my insurance would cover my stay. Luckily I am fine. After a few months I started receiving my bills from XXXX and paid my portion in a timely matter. It is now Late XXXX and have just received notices from a collection agency that I owe {$630.00} plus interest on a debt I had no idea I owed. After contacting them regarding this notice -TSC Solutions ( the credit agency ) sent me a bill summary from my emergency stay. Apparently during my emergency stay an out of network doctor performed a procedure and this is where the bill is from? At no time did I EVER receive a bill from this doctor/or his affiliate company for my portion of the bill. Ive spoken to my insurance ( XXXX ) regarding this and my advisor stated this happens quite often to patients. XXXX sent my explanation of benefits from this bill to me and I forwarded to TSC SOLUTIONS. Initially {$630.00} was submitted by the out of network doctor- but due to the fact that {$120.00} from the claim is considered incidental to the primary procedure only {$510.00} was owed to the doctor ( or his office. ) I emailed this information to the credit agency as I do owe a portion of the bill ( {$510.00} ). But they are insistent that the FULL BILLED AMOUNT of {$630.00} IS due plus interest. Ive tried unsuccessfully to resolve this matter with XXXX at TSC Solutions even offering to pay more than what was owed ( I offered {$550.00} to TSC but was told in an email that the full amount of {$630.00} plus interest of {$88.00} was owed -now totaling {$720.00}. ) I am upset that as a patient I was diligent to go to an in network facility so that I wouldnt have any problems with my insurance but apparently this type of medical billing is done at the expense of the consumer. I am also bewildered that a credit agency Does not have to abide by the law that protects consumers from over billing. At no time should a consumer have to pay the FULL amount owed from medical billing just because the bill is now in a collection agency hands. As I said I have tried to resolve this matter with TSC SOLUTIONS with no success. I will not stand by and be intimidated to over pay just because it may affect my credit report. The difference of {$120.00} may not be a lot to some people but its a matter of principle. At no time should a collection agency or other entity feel that the law that protects over billing to consumers does not apply to them. As I said I tried to resolve this in a timely matter with TSC Solutions but have not been successful.

To describe what happened I will state that I have been in contact with " TSC Accounts Receivables Solutions '' since the XXXX or beginning of XXXX quarter of XXXX. The amount of the debt is incorrect by the amount of {$260.00}. Our communications have been very upfront from my conveyances. Upon contacting them this past Wednesday I recorded the following information. This was in attempts to close the account in my name to transfer to the promised payee - my biological mother XXXX XXXX XXXX. The companies representatives would not oblige and failed to offer me the best genuine care. I do have proof the debt does not belong to me per a government agency in my local area and because of this I rightfully claim to be discriminated against and defamed as an XXXX & XXXX with the XXXX XXXX of which this country does not take lightly. Please know the medical provider is also at fault for malpractice being not solely but in addition me receiving permanent damage to my brand, body, and career in attempts to receive a more clear and desirable aesthetic required for my work. XXXX XXXXEST XXXX XXXX stated that I would need to speak to his manager regarding successfully completing my request and to wait for a callback from XXXX XXXX XXXXXXXXEST XXXX XXXX I never received a callback so I followed up and was informed that he does not perceive the situation to not be my responsibility and they will not accept the police reporting to remove ownership of my account XXXX. Then asked if I still wanted to speak to a manage even though I never had requested that. XXXX States that it would be my responsibility on the basis that I was the patient even though there was a legal agreement that I would not be responsible for paying for the services from XXXX XXXX XXXX XXXXXXXX. Would not accept police report and did not have any genuine care.

I went to the emergency room listed on my medical insurance card. I paid my copay. About a year later I got a bill from a collection agency with limited info about what the bill was for. I called and they said it was for an ER visit about a year ago. I paid the bill with my FSA. A few days later i received an email from our payroll company questioning the validity of the charge. I called the collection agency and asked for a description of the charge to provide to my payroll company as I needed to prove it was for medical expenses. They said that I would need to obtain this information from the creditor. I have not been able to get a hold of the creditor and have been put off by everyone else involved. The debt collector said its not their responsibility to provide me with this information. This bill was for a private contractor doctor who never submitted it to my insurance or myself. I never received a bill. The first time I received anything in regards to this was from a collection agency! This seems to be unethical at best. Now my payroll company is threatening to suspend my FSA and my wife is XXXX XXXX XXXX XXXX XXXX in less than a week. I XXXX the company who sent my debt to collections and they have lots of negative reviews from other people experiencing the same problem. They never send a bill, just send people straight to collections! There is something wrong with both of these two parties and they are working together to overcharge consumers. I have spent about 2 hours every day for a week trying to obtain a receipt from this charge to provide my payroll company proof this was a valid charge. Please help.

TSC Accounts Receivable Solutions, claims that I was admitted to XXXX Hospital XXXX for medical treatment on XXXX XXXX, 2011. As I have no record of this, I have requested XXXX items from TSC Accounts Receivable Solutions : 1 ) A signed copy of the admission and/or release form showing I was admitted on this date. 2 ) A signed form showing they are assigned this debt and have authority to collect on it. To date, TSC Accounts Receivable Solutions has not provided either item, nor any other signed document showing I am responsible for this debt. TSC Accounts Receivable Solutions has provided me what they call an " itemized bill '' and a " Hospital XXXX Admit sheet '', stating these " as proof of this debt and his responsibility for payment '', but if I was truly admitted to the XXXX, I would have had to sign forms to allow treatment and for financial responsibility. To this point, TSC Accounts Receivable Solutions has not provided any signed documents. At this time, TSC Accounts Receivable Solutions, and indirectly XXXX XXXX XXXX XXXX, has and continues to cause me financial harm by providing credit reporting companies with negative information that they either can not, or will not, prove. Reporting false information to credit reporting companies is a violation of the law, and both parties continuing refusal to provide the above information makes this matter even worse. I am trying once again to get TSC Accounts Receivable Solutions and XXXX to provide this supportive information before I pursue my rights under FCRA regulates.

TSC is non-responsive and is not providing any form of verification, either in this or the other responses, nor over the phone. What XXXX XXXX provided is simply a copy of a bill in my name. I believe this bill was made in error and have requested proof that I was the one that received this service. A simple bill with my name on it does not prove this is my bill. As it is not my responsibility to prove this debt, I consider this debt invalid until TSC can prove that this is my debt and not an error on their side or the originator of this bills side. My Rights Under California 's Fair Debt Collection Practices Act and TITLE 15 -- COMMERCE AND TRADE, state that I must be provided proof that I owe the debt. Simply providing a bill with my name on it is not proof of responsibility. The fact that TSC states that they " believe '' this " account is accurate '' is just their opinion, not proof or validation of the debt. As such TSC is violating my rights. In addition, TSC has not proven that if this is even a valid debt, that they have been assigned the debt for collection. As I stated before, the continued reporting of account, # XXXX, for the current amount of {$220.00}, without all the above validation has caused me harm, and will continue to cause harm. As I have now brought this to their attention multiple times, and their refusal to remove this from my report until such time as they can provide debt validation, is a violation of my rights under the California 's Fair Debt Collection Practices Act.

In XXXX I had XXXX XXXX followed by intensive XXXX to recover. My bills were numerous, and I reached my out-of-pocket limit on my insurance. I had several rounds of XXXX that were over my insurance allowance and these series were " re-upped '' by my carrier on multiple occasions for continued treatment. My last treatment in XXXX was on XX/XX/XXXX, which should have been covered by my policy. Instead, the final treatment was declined by insurance as being over my XXXX limit. After spending over XXXX in one year on deductibles and copays, the additional {$200.00} was an additional burden I didn't want to incur. I resubmitted and appealed to my insurance several times, but finally ended up paying the bill out of my own pocket to avoid having this go to collections. However, a collection still landed on my credit report, which was pristine ( not even a late payment ) prior to this. First, it was for the entire bill amount, which I disputed and had removed from all three credit reports, then this faux debt was reinserted on my XXXX report for a {$22.00} fee from the collection agency itself. I have tried to dispute this, had it deleted and then reinserted on my report again. I paid the original creditor ( which I never should have had to do in the first place ) but I was trying to avoid having this bill go to collections. I have never had any business with this collection agency, and they are in violation of the FDCPA by continuing to reinsert this bogus {$22.00} collection on my credit report.

I went for XXXX XXXXXXXX which was paid off. In the beginning when I was going for XXXX, they gave me a break down of the items that I would be billed for. I asked several times to make sure that that was the final cost and that there wouldn't be any extra charges incurred. I saved up the amount that I needed to pay for the XXXX and even paid it ahead of time. I have proof of payment and receipts from the same company that keeps on sending me invoices. I contacted TSC Accounts Receivable Solutions at the phone number given in response to another complaint. I agreed that account # XXXX, for date of service XXXX/XXXX/XXXX, with a current balance is {$270.00}, including principal and interest, was indeed mine, even though they did not have anything with my signature on it as such. I paid this in full and was told it would be totally removed off all my credit reports. The account, # XXXX, for date of service XXXX/XXXX/XXXX, for {$220.00}, including principal and interest ( the lessor of the XXXX reported ), is unknown to me, so I asked for debt validation. At this time, TSC does not have anything with my signature on it proving it is indeed my debt and not an error. I requested debt validation and was told I will have to call back in a month to find out the results of the dispute. TSC Accounts Receivable added a collections account to my credit reports on XX/XX/2021. I had perfect credit before this and paid all of my bills on time, so I was shocked that I was not contacted nor aware of any debt owed by the original creditor, XXXX XXXX XXXX. I was never contacted by TSC regarding the debt before it was published to all 3 credit bureaus, which is a violation under the Fair Debt Collection Practices Act. Had I been properly contacted and they provided proof that I owe this debt ( see below ), it would have been settled before they took extreme measures to ruin my credit. I received services from a medical group. The claims were paid by my health insurer. They call repeatedly and send physical bills in what appears to double collect for the bill. They mumble the name of the company and refuse to give you any identification name.
