Several months ago, I went to an urgent care facility to take a XXXX test, which was required for work. I had asked the receptionist whether the test that was going to be administered was par with my insurance, and she is assured me that it was. Weeks later, I received a letter from sad establishment indicating that I had a bill in the amount of {$920.00} for the XXXX test that was taken. I was under the impression that XXXX tests were covered by my insurance and that the testing facility that was used was par with my insurance, making it a free test. The creditor, XXXX XXXX lab was apparently, not par with my insurance and sort to charge me the above amount. They have since saw the help of legal aid by way of the The Levinbook Law Firm P.C. to collect on my supposed debt. I have told XXXX XXXX XXXX that I was unwilling to pay the debt because I was told that the insurance was par with their facility. So, I am still unwilling to pay any debt that is owed. I hope that you can help in this matter, as it has caused me great stress in my older age, and in my retirement. I have limited funds, and XXXX not in a position to pay close to {$1000.00} for a test that was supposed to be free. The date of service was XX/XX/2020. I am aware that I was violated. As a natural person who is federally protected. I am notifying you that you have help damage my name and my reputation. Your corporation has offensive activities towards me as a consumer, such unauthorized use that did not serve me any benefit. I have been violated as such, the truth and lending act 15 USC 1692C ILLEGAL TO COMMUNICATE, 1 5 USC 1692 ABUSIVE PRACTICES OF DEBT COLLECTIONS you violated my privacy and only I deem what is private to me and 15 USC 1692 G VALIDATION OF DEBTS you do not have permission to say I owe anything because as natural person and according to the TILA I validate debts because I am original consumer. This does NOT allow debt collectors to obtain a copy of " original contracts '' as I DO NOT have contracts with MIDDLE COUNTRY COLLECTION SERVICES, INS, you do not have granted that permission. As per congress, you violated me, your corporation is criminally liable pursuant 15 USC 1611 because your corporation willingly knew that you were complying to a transaction that is fraud. It is a violation as per 18 U.S. Code 241 for two parties to come together and conspire against MY rights as consumer.

I went through a series of treatments and I was mislead to think I was " covered '' and I was told " no worries you are covered ''. After procedure done, bills started coming like oops we forgot to tell you. I was never quoted or asked if I agreed on the amount for the treatment. I know insurance paid the the debt and now they want me to pay it again. They send me 4 FDCPA letters alike and with absolutely no description of what it is.

The medical company submitted incorrect insurance information and is trying to collect the balance which they claim is owed. The insurance should have covered the bill. I have 2 insurance providers. Additionally this was a yearly physical which is provided free of charge.
