On XX/XX/XXXX I received a call from RevMD Partners, attempted to collect a debt for a medical bill in the amount of {$100.00}. They shared with me that the bill was for original creditor XXXX XXXX / XXXX XXXX XXXX, and that it covered three dates of service, XX/XX/XXXX {$17.00}, XX/XX/XXXX {$40.00}, and XX/XX/XXXX {$45.00}. I informed them that I am just XXXX years old now, and that for the dates of service mentioned, I would have been a minor and not the financially responsible party. They told me that since I am XXXX now, I am legally responsible for these bills ( from when I was XXXX years old ) and threatened that if I didn't pay it they would take collections action. I asserted my belief that they were mistaken on the legal responsibility for any alleged balance, and asked to speak to a supervisor. In talking to supervisor ( " XXXX '' ) I was again told that because I am XXXX NOW, that I am responsible for this balance from 5 years ago. I asked for the details for the collection agency that was contacting me and expressed my intention to file a formal complaint for the misinformation and legal threats made against me. RevMD Partners, XXXX XXXX XXXX XXXX XXXX, XXXX, IL XXXX, phone # XXXX, tax ID # XXXX. Additionally, I was provided with a phone # of XXXX to contact the original creditor. I have now spent more than 6 hours on the phone attempting to uncover what this alleged balance is for. I have talked with representatives at the phone # that RevMD provided ( XXXX ) who have assured me there is no open balance in collections. I have taken it upon myself to further contact other publicly available phone numbers for XXXX XXXX XXXX - both hospital related patient accounts and physician group related accounts - they have told me the same and that XXXX XXXX XXXX is not any provider they recognize. Proactively, I thought XXXX may be related to " XXXX XXXX XXXX XXXX XXXX - so I proactively have contacted every provider 's office I can think of whom I have ever had services from. Again, both on the physician 's side, and hospital side of all accounts - they all tell me there is no open balance, and nothing has been turned over to collections. I called RevMD back to share this information with them, and was told that there is no additional information they can give me about this debt to identify the provider, but that perhaps since I was a minor at the time of service, I should call again to these providers I reached out to and ask them to search under my mother 's name as she would have been the guarantor. I did so - again, no one has any record of any outstanding balance being turned over to a collections agency. I have now spent more than 6 hours trying to get to the bottom of this, and RevMD 's position on it remains that I am legally responsible for this balance and if I don't pay it they'll take collection action against me. I'd like to formally request they be reprimanded for their unethical and invalid remarks that I bear any financial responsibility for a medical debt incurred at such a young age. For their inability to substantiate the debt, or provide even the factual original creditor. I request that this balance be removed from any collections activity with no further threat to resume such unethical and illegal acts.

In addition, I don ; t quite understand why this was billed as XXXX Office Outpatient New when I had been there before. That billing code does not seem to be consistent. The unpaid item was not my responsibility, it was a Employment Requirement- And you XXXX was told this on the day of service and afterwords via phone. None of my actual patient notes, visit records are available via XXXX ( XXXX : XXXX ) so I can not contrast the billing verses the actual dates of service. The part that was not in dispute was paid in full : The Payment of {$97.00} was paid towards the visit XX/XX/XXXX with XXXX, XXXX XXXX at XXXX XXXX XXXX. That's paid in full. This is how you treat a XXXX XXXX. To the best of my knowledge, I was required to have this test, as being a XXXX XXXX, and XXXX of first aid in XXXX County Parks. As you may be aware the Parks are currently closed so to further complicate the matter, being unemployed and seeking work, I now have this matter incorrectly being reported to my credit report. Defamation of Character : Your reporting this matter as if I am easy to let some obligation not be taken care of, multiple phone calls and letters were sent to XXXX. I have not received any letters from this collection agency. A matter of fact this matter is still showing as a bill on XXXX. I have screenshots of the patient account screens and from the way it looks to me, an average patient and customer, with average education, there is no reason for me to believe that the matter was not billed to third party. It clearly states that I owe no money to see this on my credit report, does upset me. ( SEE ATTACHED ) Using the current pandemic as an excuse for not picking up the phone is just pathetic, people are answering phones at home at some companies, I expect better from you. In XX/XX/XXXX XXXX XXXX XXXX ( XXXX ) charged me for services connected to the care of my minor child. The charges were incurred due to inadequacies on the part of XXXX. Specifically, my child had XXXX, and the charges for the XXXX are a " global rate '' to include a required XXXX follow up office visit. These rates are contracted rates between the service provider and the insurance carrier, but there are timeframes in which the service provider is to arrange the follow up visit. XXXX intentionally pushed our follow up appointment outside of the scope of that timeframe, citing that due to the XXXX XXXX holiday, various doctors were going to be out of the office, and that they needed to push us to their next available appointment time. I explicitly discussed with them at the time the appointment was set, that I would accept the later appointment time, with the understanding that we would not be pursued for any financial difference beyond our obligations connected to the XXXXXXXX XXXX global rate. The appointment occurred, we carried on as normal, and for nearly two years since it has been a battle over a lousy co-pay for that follow-up office visit. They were paid for the visit by the insurance outside the global rate, however they are still trying to collect the copay from me despite that I explicitly addressed this issue before it even occurred. Without substantiating my dispute claims, they then turned the account over to collection agency " Rev MD Partners '' who has contacted me on repeat occasions " in attempt to collect the debt ''. I formally disputed the debt with them in writing on XX/XX/XXXX and have not been provided any further substantiation of the debt, yet they have reached out to me by mail and by phone with continued efforts to collect. Each time I have addressed the situation, and each time I am met with assurances that they do show the balance in their system to be disputed and that no further collection action will be taken - but that ultimately results in a subsequent phone call, or letter, to which I proactively call them to discuss said letter. The debt has never been substantiated subject to the nature of my dispute. By REV MD 's own admission ( recorded phone call ), XXXX has not responded to their efforts to contact THEM related to my dispute. Under the FTC guidelines and Fair Debt Collection Practices Act, no further collections contacts or actions can occur until this happens. I am unfamiliar if there is a timeframe at which point a dispute must be validated for lack of response from the creditor - but I do know that it is inappropriate for me to have to keep investing hours and hours out of my own day ( during my working hours while they're open ), to tend to this ongoing unresolved issue.

RevMD Partners received a validation notice and a notice of account settlement with XXXX XXXX from me via priority mail no XXXX XXXX XXXX XXXX XXXX on XX/XX/2022. I have not received a valid response to validate that RevMD Partners are Holders in Due Course. In the notice received, Ive kindly requested for RevMD Partners to cease and desist. Revmd Partners ignored my request and during the validation process RevMD Partners changed their original reference number from : XXXX ( see letter date XX/XX/2022 ) to reference number : XXXX, with a new balance of {$490.00}, and new HIPPA violating documents. Due to the change, it yielded no additional fees with the XXXX XXXX. This account was settled with the XXXX XXXX on XX/XX/2022. Please be advised that RevMD Partners are not Holders in Due Course ; no services or goods were received from RevMD Partners and I have not agreed to pay/tender anything to RevMD Partners on behalf of their alleged client XXXX XXXX for any reason. RevMD Partners have not provided any documents or itemized statements VIA MAIL proving I owe a debt to them. This proves that RevMD Partners is not an honorable or honest business that allows their alleged clients the opportunity to defend themselves. I have not provided signed any wet-ink releases of my private medical information to RevMD Partners. This is proof that my HIPPA rights have been violated according to the letter from RevMD Partners providing a list of unverified medical service fees provided via US mail on XX/XX/2022 RevMD Partners do not have my permission to contact me on my private phone line attempting to collect on debts that do not belong to me. I will reply VIA MAIL and I can only be contacted via mail. RevMD Partners reply outside of the CFPBs portal, must be delivered via the postal service. If the envelope is not properly stamped by the postmaster ; it will be returned to sender due to mail code violations. Please abide by all mail codes. All calls are recorded for my safety.

My XXXX is XXXX years old and not capable of entering into a legally binding contract. I handle his finances for him. All of a sudden his credit score as shown on a credit card statement has dropped XXXX points. I suspected Identity theft and contacted all of the credit bureaus. The XXXX XXXX report shows XXXX adverse collection accounts from " REVMD Partners '' with no other information. I have never heard of these people, there has been no collection activity, and no communication of any type from them, written or oral. Dad does not owe anyone any money and this is clearly an attempt to XXXX by holding their credit report XXXX. We need to protect our XXXX citizens, and in this case an impaired veteran from XXXX. Totally intolerable. And I was so upset when I got this information that I was quite upset and awake all night with night sweats. These people should not be allowed to do this and instead should be making license plates or pounding big rocks into little rocks, if you know what I mean.

I have a debt from a company called RevMD partners. I noticed on my credit report I had a few open accounts and I reached out to them to start paying to get the amounts to XXXX. I call every few days and every time they tell me I owe different amounts than whats on my credit report. I ask for statements habitually by mail and I never recieve them. I just called again this morning and I gace them a small payment and told them I refuse to pay anymore until I get statment. The staff is very small there and I always get the same two people on the line and they already know who I am and still, they say they are sending statements and I never get them. They keep mentionng different accounts and I have no idea where my money is going as the amounts on my credit report are not reflecting what I owe and they cant explain or give me a statement. They blame my mail, but I have no issues getting my mail. I WANT A STATEMENT

I disputed the account and ask that they validate this account, and they fail to respond to the action. The law states they have 30 days to do so if not or over the 30 days this account must be removed from the credit file and in this case, they fail to respond to the matter. It has been over 30 days with no response, and I did not open or authorize this account, and I therefore request that it be closed immediately. By the federal law requirement this account must be removed because you do not have proof or any evidence to support this matter and failure to respond.

The debt of {$110.00} was paid directly to the medical provider. I mailed a letter stating that to the debt collector. On XX/XX/ I called the number listed on the debt collection letter and the number did not work, so I am unable to confirm the receipt of my letter or to speak with a real person to explain that the debt has been paid.
