RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT METRO 2 data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX , XXXX , and XXXX This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with The XXXX XXXX XXXX, Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C. 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. Finally, you are reporting the INQUIRIES that are the result of the same type of fraudulent activities. It must be noted that I did not apply for credit accounts with the below-listed companies and creditors, some of these are the result of attempts to open accounts, but the companies did not approve the accounts. Therefore, these inquiries must be deleted from within my credit file. You can not delete the fraudulently opened accounts without also deleting the fraudulent inquiries. It should be noted that an inquiry that was not made by me is not a factual record, just as a fraudulently opened account is not a factual record and not referred to the creditor to be disputed directly with them. The credit reporting agencies have attempted to address an inquiry as a factual record of file access, but if it was done through fraudulent means, it is not a factual record. As a consumer, I should not be required to dispute each fraudulent inquiry directly with each company. These inquiries are the result of fraudulent activity and therefore, inaccurate in which the credit reporting agencies or the creditors can not verify, meaning that these inquiries are unverifiable information and must be deleted from my credit files. 1. RECEIVABLE RECOVERY bal. {$1800.00} I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from alleged identity theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer that the information relating to the transaction by the consumer. I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. Debt Collection claim was in the amount of {$460.00} from a XXXX that I haven't be too, nor use their service or any other similar service. This Collection has violated my rights, and adding someone else 's information on my credit report. I sent out a validation of debt and HIPAA authorization request in XXXX by USPS express 1 day delivery mail to the company and to the credit agency. In my letter I explained that this wasn't my debt, and please cease and desist all reporting activities within 14 days after receiving this information. Also, in my request I explained that they had 30 days to validate the request to show ownership of this debt. Neither party has responded and continues to have this information on my credit and recently updated according to my credit monitoring service. The company has failed to comply within my rights. I haven't received any information from this company for validation, nor a HIPAA Authorization with my signature that I had requested in writing.This company continues to practice fraud by placing un-authorize, and inaccurate/ invalid information on my credit without my consent. It has been over the time frame of my request. The company just continues to add fraud information. The credit agency themselves have violated my rights by having this information on my credit after I have requested and sent them the same information to validate this information with a 100 % accuracy, and the credit agency is in the violation of 15 USC 1681b, for even placing this on my account without my acknowledgement. I have disputed this information over 5 times and asked to personally send me validation that is my account. I haven't received anything from the credit agency nor the debt collector. I have asked the Credit Agency and the Debt Collector to send me a validation of debt with my signature from all parties. I have explained in writing that this is not my debt. I have asked them to cease and desist reporting until all information is received and validated This was the original request for the company to send the following information : Validation of Debt and HIPAA Authorization 1. I am requesting that the HIPAA covered entity or its business association provide debt validation for an account in question. Please provide that validation in the form of agreement, original bill, original date of closed account, and a detailed statement of procedures is required. 2. Please provide a breakdown of fees, including any collection costs, medical changes, and a contract or agreement with the original creditor and signed by all parties involved. 3. Please provide a copy of my signature with the provider of services to release my medical information to you. 4. Please provide a report of comparative analysis between the dubious signature and the signature recognized by the alleged signer. 5. Cease any credit bureau reporting until the debt has been validated by me. 6. Please provide me with a verified certificate of authority, and proof that your company is licensed to collect debt in my state. Kindly provide me with a license number and registered agent or agent of service. 7. Finally, please provide an affidavit signed under penalty of perjury that your organization has not violated any portion of the FCRA FDCPA or HIPAA. Please send this information to my address listed on this form and accept this letter, sent certified mail as my formal 14 days debt validation request and 24 hours cease and desist from manners of reporting and communication, of which I am allowed under the FDCPA. Under the FDCPA it is a false, deceptive, or misleading representation or means in connection with the collection of any debt to communicate to any person ( consumer credit reporting agency ) credit information that is known or which should be known to be fake. Under the FDCPA, it is unfair or unconscionable to attempt to collect an alleged debt in any amount that is not expressly authorized by an agreement creating the debt or permitted by law. First and foremost, I am not a dental provider, nor do I have any dental billing or legal training. If my claims are unsubstantiated, I sincerely apologize. I tried to work with XXXX in XXXX directly to resolve this matter to no avail. Therefore, Im asking The Pennsylvania XXXX XXXX XXXX to step in on my behalf. Tooth 14 I was placed in collection by Drs. XXXXXX/XX/XXXX ( XXXX ), XXXX XXXX XXXX ( XXXX ) andXX/XX/XXXX XXXX XX/XX/XXXX. I received collections notice from XXXX in XXXX in the amount of {$1100.00}XX/XX/XXXX was requesting {$590.00} ( XXXX Crown-Porcelain/Ceramic ) and {$97.00} ( XXXX XXXX XXXX, Including any pins ) for tooth number 14. Per collection notice/invoice these services were completed XX/XX/XXXX. XXXX XXXX paid XX/XX/XXXX{$550.00} ( Porcelain Ceramic Crown ) and {$100.00} ( XX/XX/XXXX ). XXXX was also billed and paid {$75.00} for XX/XX/XXXX on tooth number 14. XX/XX/XXXX, I became a patient at XXXX for XXXX and was quoted {$17000.00} to complete all work needed. XX/XX/XXXX, I had {$15000.00} of the work completed XX/XX/XXXX. With the plan of completing the remainder once my benefits renewed for XXXX. One of the items that were tabled for XXXX was the core build up and crown for tooth number 14. XXXXXX/XX/XXXXreatment plan for tooth number 14 in the amount of {$1000.00} is attached. XXXX and XXXX XXXX both paid for services rendered to tooth number 14. XX/XX/XXXX did not complete the services and reported me to collections for monies they received, and no services were rendered. Tooth 4 PerXX/XX/XXXX collections notice I owe {$150.00} for XXXX XXXX, Including any pins for tooth four completed XX/XX/XXXX. XXXX paid {$50.00} XXXX XXXX XXXX Any Pins for tooth number XXXX and XXXX XXXX paid {$100.00} XXXX XXXX for tooth 4. XX/XX/XXXX, my bridge from XXXX ( teeth 4, 5 and 6 ) had not been removed therefore I doubt any work was completed on tooth four. Bridge was not removed until XX/XX/XXXX, when implant was placed. Tooth 5 Elegance also reported me to collections for tooth number five implant in the amount of {$1500.00}. Which was supposed to be billed to XXXX XXXX before my XX/XX/XXXX, termination date. I had {$2000.00} in benefits left when I termed with XXXX XXXX because claims were never submitted. I paid {$1000.00} out of pocket because XXXX rejected the claim because XXXX refused to submit the proper documentation. XX/XX/XXXX, XXXX paid XXXX {$760.00} for tooth number five implant. If my calculations are correct that would give me {$230.00} credit. XX/XX/XXXX, bridge was removed from teeth four, five and six so implant could be placed in tooth XXXX. Once the procedure was completed my bridge for XXXX was reinstalled so that I could get crowns for teeth four, five and six once implant site was healed. XX/XX/XXXX, the bridge I had since XXXX broke. XX/XX/XXXX, I went to XXXX and was told they would have to make a temp bridge. I was never informed that it would be {$130.00} ( 3 ) for a temp bridge. XX/XX/XXXX, temporary bridge broke and XXXX would not schedule an appointment to repair. Tooth 3 During my consult with XXXX XXXX ( Periodontist ) atXX/XX/XXXX I was told I needed a {$1500.00} sinus lift for tooth number 3. XXXXXX/XX/XXXX XXXX said it was not necessary and completed my implant without any alterations of my sinus. To Whom It May Concern, In XXXX of this year, I started the house buying process, only to find out that there where medical collections on my credit report that I had no knowledge of. I had my wife, XXXX, reach out to the collection agency and the original creditor and XXXX of the bills are from the birth of my son at the hospital and the other is for a follow up visit at XXXX XXXX XXXX. All of the medical bills from the hospital where covered so I 'm having a hard time understanding how only these XXXX accounts where not covered. My wife started her investigations by calling Receivable Recovery on XXXX XXXX, 2015. She spoke to XXXX at ext XXXX then contacted XXXX XXXX XXXX XXXX. The conversations with XXXX XXXX XXXX XXXX continued on the following dates : XXXX, XXXX, XXXX, XXXX, XXXX, XXXX, XXXX, when finally my wife was put in contact with the manager named XXXX and went into the office and paid the collections in full, ONLY BECAUSE XXXX STATED SHE WOULD CONTACT XXXX AT RECEIVABLE RECOVERY AND HAVE THE ACCOUNTS DELETED FROM MY CREDIT REPORT. My wife works at a XXXX and by using the " What if Simulator '' the only way my scores were going to improve was if the accounts were removed completely from my report. There is no other reason for me to pay the accounts unless they were going to be removed. Now it 's a month later and the accounts are still remaining on my credit and Receivable Recovery has been telling me they ca n't remove it without the permission of their client ( XXXX XXXX XXXX XXXX ) and XXXX XXXX XXXX XXXX is now saying they are n't going to give them permission to remove it because it 's my fault I did n't give them the correct insurance information. When she spoke to XXXX XXXX XXXX XXXX in the past they told me they claimed it with XXXX and their claim came back as denied due to no coverage. I know for a fact that my son was covered at the time so she took the initiative to contact XXXX last week only to find out they have no claims in their system on the date the hospital claimed to have made them. For date of service XXXX ( Wife is still in the hospital mind you ) Claim was filed in XXXX of XXXXFor date of service XXXX Claim filed in XXXX XXXXFor date of service XXXX Claim filed in XXXX XXXXXXXX XXXX stated all of these claims where denied but XXXX stated they have no claims filed for any of the dates of service. This is a billing error on XXXX XXXX part or XXXX not on my part and I did my due diligence by paying the accounts when I was told they would be deleted and now everyone is going back on their word. Due to these errors, I will now no longer be able to purchase a house for myself and my family of XXXX. We will be homeless in a matter of XXXX weeks and no one is wanting to work with me or take the extra step to get the mistake rectified. I even have a voicemail from XXXX, the supervisor at XXXX XXXX XXXX XXXX billing from XXXX stating that Receivable Recovery would n't delete because of the circumstances and now that I have Receivable Recovery willing to do their part, now XXXX XXXX XXXX is not willing to do theirs. This is more than unfair and my wife and I have wasted hours and days of our time and efforts because of the all the runaround we 've been getting and nothing has changed. I ca n't imagine how hard this is for someone that does n't have the knowledge and education that my wife has in this field and to think that they only XXXX that is going to be out is us. They got their money now and are n't keeping up their end of the bargain. I was under the impression that if it 's the hospital or insurances fault that they have to delete the accounts once rectified.

Not verified, not liable,, no legal contract, not mine. I received a copy of my credit report XXXX XXXX at that point I saw a collection account from the Company Receivable Recovery Service, I sent out a letter in regards to this debt asking for verification and validation of this debt to the company .I never received a return response from the company via mail in regards to the letter I sent after 30 days. I sent another letter, still no response, But I did start receive ugly phone calls of harassment and threats early morning and night calls .I tried to explain that I had no clue of this debt and that this was not my account .The caller was very rude with a nasty attitude to the point I asked them to stop calling me because .I also sent out letters to the bureaus asking them for verification from Company. I never received the verification information, but I did see where on my report it was updated as verified .How can they verified something I did n't do .I HAVE HEALTH INSURANCE AND HAVE HAD INSURANCE ALL OF MY LIFE therefore it is no reason for me to owe a medical debt besides I never went to the hospital or was sick for any reason that 's why I requested verification when I say a medical charge. .Last month I decided to send off a letter asking for that verification again to the Credit bureaus soon after I received a letter from Receivable Recovery Services for the first time ever stating they mailed me papers back in XXXX of XXXX which I never received and they told me that my limitation for verification had expired at the end of XXXX XXXX .I HAD NO KNOWLEDGE OF THIS ACCOUNT AT ALL..How can my time expire about something I have no knowledge of. Receivable Recovery continues to blatantly report inaccurate, unverifiable entries on my credit report ( see attached ). The alleged entries reflect the original creditor is XXXX XXXX XXXX. The entry on my reports reflect that Receivable Recovery reported this entry on my credit report, yet they did not provide any communication to me. No dunning letter was received from this company. I have never did any business with this company, nor have I ever heard of the company. I have attempted on numerous occasions to gain clarity and insight on these erroneous entries by disputing in writing to them and the credit bureaus. My first disputes were with each of the credit bureaus and the responses provided from each bureau reflected that the entry was verified by Receivable Recovery. To that end, I sent a certified letter to this company. The tracking of this receipt reflects that they did indeed receive the dispute nor was the mail returned. To date, this company has not responded to my dispute of the validity of the entry they are continuing to negligently report. I requested : An agreement that states their authority to collect on this alleged debt ; Agreement with signature of the alleged debtor wherein he/she agreed to pay the creditor, Alleged account number, Date this alleged debt became payable Original delinquency date, Verification that this debt was assigned or sold to collector Complete accounting of alleged debt, and Name and address of the bonding agent for collection agency in case legal action becomes necessary. THEY DID NOT RESPOND. In my second request, given I never received any notification from them with regard to the first dispute, I requested they provide a copy of the financial responsibility page, provide a copy of my signature with the alleged provider of service to release the alleged medical information to them and cease any credit bureau reporting until the debt had been validated by me. They DID NOT. Consumer law provides that the creditor is expected to respond to your claim within 30 to 45 days and to inform you of the results of its investigation within five business days. Prior to reporting any alleged medical entry the ACA provides a collection company must wait 6 months. Receivable Recovery did not prove they met this requirement and to date has not furnished a copy of any documentation to support they provided the allotted amount of time to pass per law prior to inserting this derogatory, inaccurate entry on my credit reports. It is obvious that Receivable Recovery has no intent to honor the law nor respond to my previous disputes ; therefore, in the absence of them not responding with any documentation to validate the alleged debt, they must cease collections, return this debt to the original owner since they are attempting to collect on behalf of another company, and remove this entry from my credit reports immediately. This erroneous, inaccurate reporting is misleading and is further diminishing my creditworthiness.

I paid {$170.00} to my doctor 's office on XX/XX/XXXX ( cleared bank and posted to medical office account on XX/XX/XXXX ). This is a debt which I still believe is not owed but was willing to make payment so that it would not be reported on our credit report. Received 1st letter from Receivable Recovery on or near XX/XX/XXXX. Called the agency and informed them I did not believe I owed the debt and requested records from BOTH Receivable Recovery and the doctor 's office. I also informed Receivable Recovery that I would make payment in the meant time. To date, I have not received records from either entity. Received a call yesterday from Receivable Recover. Called them back and they told me I had not paid yet. ( Note that I had received multiple calls each day for a XXXX # with no voicemail on the Friday and Monday prior to speaking with them ). I informed them they were wrong and the operator proceeded to be very rude with me. I requested a supervisor and was sent to the voicemail. Immediately called the billing company for the doctor and was told that they showed the bill paid and that notice had been sent to the collection agency to that point. She assured me that they would send a second notice of payment to Receivable Recovery. Received another call today from Receivable Recovery that they had just received notice that the debt had in fact been paid in XX/XX/XXXX and to inform me that they were processing a deletion since it had been reported to the credit agencies. I am furious that this has been reported as the matter had been correctly taken care of. I spoke to a supervisor who was of absolutely no help. He basically said " We reported it, we are asking for a deletion, its over, not sure what you want me to do about it. '' I believe that this agency uses fraudulent practices in trying to collect debt. They are harassing and not willing to do what is asked of them. To date, I still have not received any documentation from them proving that we were notified that we had a past due balance with the doctor and would be sent to collections, nor that Receivable Recovery had attempted to contact us prior to the letter received in XX/XX/XXXX. ( They claim a letter was sent in XX/XX/XXXX. ) I was also informed by the supervisor that he did see where I had request an itemized statement, and that they had requested one from the doctor 's office ( because they accept files for collection WITHOUT a copy of the bill ), but that they never received it from the office. I maintain that I followed what was expected of me in 1 ) disputing the amount in question and 2 ) making a payment in the amount in question to the doctor 's office in order to prevent this from being reported while we tried to clarify the situation and Receivable Recovery did not do as they promised by reporting this to the credit agency while the debt was in dispute.

I was XXXX from XXXX consumption at a friend 's house for a get together on XX/XX/XXXX. An ambulance was called and I was picked up and brought to an emergency room and stayed for a few hours before being sent home. The ambulance company apparently sent the bill to the address they picked me up from and not the address on my license. I never received a bill due to this error. Months later, on XX/XX/XXXX, a resident at the house that the ambulance picked me up from let me know I had mail addressed to me at their home. I thought this was very strange and gave them permission to open it. They then sent me pictures of a " Notice of Default '' from a collection agency for my bill, it stated that the bill may be reported to the bureaus if it was not paid in 45 days from the date they first received my account from the ambulance service. I then called the ambulance company and informed them of their error fearing the impact on my credit since I never received a bill from them, never received a notice of my bill being sent to collections, and very luckily had the resident notice my name on a piece of mail at their house. I was told over the phone by their billing department that they would take 20 % off if I paid in full that day. As soon as I was about to pay over the phone the operator said she had to check with her supervisor to make sure I got the discount and she would call me back. She called back later to inform me that she could not give me a 20 % discount and corrected my address so I could receive the bill and get my invoice number to pay. Once I received the bill from the ambulance company at my address, I noticed I had a payment due date of XX/XX/XXXX. I then went to their website to pay. I started a payment plan with the first payment for my plan starting on XX/XX/XXXX. I received a credit score notice from XXXX XXXX for my credit report on XX/XX/XXXX. This stated I had a derogatory mark from the collection agency which was for the full amount left on my bill. I was under the assumption that paying my bill to the ambulance service would solve my issue. Apparently, the ambulance service did not inform the collection agency that they made a mistake of my address. Apparently, the collection agency opened it on XX/XX/XXXX, way before I was even informed of a bill, the bill being sent to collections, and a " Notice of Default '' from said collection agency. This derogatory mark was for the full collection and did not show my payment from XXXX as well. All because the ambulance service sent the bill from the location I was picked up from instead of my actual residence on my license and did not inform the collection agency of their mistake after they sent the bill to the correct address.

This company, RRS, LLC called me multiple times using different numbers ( XXXX ) XXXX and ( XXXX ) XXXX with the same call back number ( XXXX ) XXXX. It was a recorded message stating that this was in regards to a legal matter. I should have researched the number before I called back. Big mistake on my part! I called the number XXXX/XXXX/XXXX and requested this company never call me again. I also told them I was reporting them to DoNotCall.org. I have already reported the first number. The comment was that this company works with them. First of several more lies to come. The man I spoke to is XXXX XXXX. He was very evasive and rude. I asked him to please tell me what this was about several times. He would mumble his words ; and when I would react and say " excuse me '', he would keep telling me to stop interrupting him. He then accused me of being rude and told me to stop talking to him like he was my husband, child, girlfriend, boyfriend or fiancee. He first said he was tasked to find me regarding fraud and that my parents were looking for me. The second lie. My only living parent knows right where to find me. He told me I am being charged with fraud in the State of XXXX. Third lie. I have not lived there in 13 years. The case number the message referred to covered about XXXX people, all of which he disclosed to me since I would not tell him my name. This was a trick to try and get my name. When he thought he had the right person, he said that was all he needed to know. Fourth lie. He said I am being charged with fraud for an account from XX/XX/XXXX from XXXX XXXX XXXX. I informed him I have no knowledge of such an account. I never acknowledged anything to him. Not my name, social security number or anything else. I told him I have never defrauded anyone. I also advised him that I am aware that any debt this old is no longer valid since it has exceeded the statute of limitations in any state ( 3 yrs in XXXX and 4 yrs in XXXX ). He said that there is no statute of limitations on fraud. The Statute of Limitations for Fraud in both XXXX and XXXX is 3 years. He rudely called me ignorant. I informed him I was not ignorant ; or I would not have known about statutes of limitations. He clearly does not know the meaning of this word. The last thing he said was " I will see you in court ''. I hung up at that point.

I recieved a phone call from Recievable Recovery Services LLC on the XXXX of XXXX. They told me they were collecting debt in the amount of XXXX cents from XXXX XXXX Hospital services. I informed them if the debt was indeed real it was XXXX debt and I would make an effort to pay. I asked for an itemized bill to prove the debt was real. They tried to settle and I demanded a paper bill. They called me again today, on XX/XX/XXXX and again attempted to collect the debt. They said they had sent an itemized bill. I told them I didnt have that money and had not seen the bill yet. They offered again to settle for around 1400 dollars. The worker asked me to put it on a credit card, to cash put my 401k, and to borrow the money from a family member. I explained to her she was not my only creditor and was trying to collect the same debt another creditor had called about and that I could give her XXXX dollars a month until I was in a better position. She told me that was unacceptable. She informed me they would credit report no matter what I paid and when I asked what would happen other than that she refused to tell me. She also refused to allow me to speak to a manager. Different phone numbers were used for the phone calls and niether number works when dialed back. I never recieved a notice to dispute the debt and when I looked at her itemized bill it only shows the hospital name and the amount they say I owe with dates. There is no phone number on the bill or an account number specific to the company that I can tell and no information on how I can make a payment, just a stamp with their address and company name on it. She was abusive and inappropriate in her attempts to collect debt on XXXX XXXX without ever proving to me there was a real debt.

I received a letter from Receivable Recovery Service LLC dated XX/XX/2020. The letter stated Notice of Default with a balance of {$330.00}. I logged onto their system XX/XX/2020 disputing the account and asking for validity of debt. Please note that this company violated the FDCPA laws because for one they reported it on my credit XX/XX/2020. Which was before I even received the letter which stated I have 30 days from the date of notice to dispute the validity of this debt. I made aware of this being reported on my credit through an app called XXXX XXXX. This company made a negative remark on my credit and back dated it to XX/XX/XXXX with a mark saying disputed by the consumer! They did partially respond to my request for validation of debt but they did not provide all requested documentation for an account that does not belong to me. They responded on XX/XX/2020 and I asked for documentation on XX/XX/2020 to the person with an email address of : XXXX I never heard anything back from them. A copy of communication is attached to this complaint. They have violated a lot of my consumer right and have made a negative remark on my credit! They need to be fined and they need to delete this from my credit history! For one I dont owe and didnt owe the debt. Once I saw it on my credit report I freaked out!!! RRS is assuming that the debt is mine due to my lack of dispute to their initial communication letter. However, according to the FDCPA, my lack of response is not an admission of debt. On XX/XX/2018, when I called the representative admitted to not having any documentation on file about the debts. Afterwards, RRS voluntarily ( without my knowledge or permission ) obtained my records from XXXX which included itemization of services that listed the labs I took, an XXXX " Acknowledgment of Receipt of Notice of Privacy Practices '' with my signature, an XXXX " Patient Information Sheet '' with my signature, and a copy of my driver 's license. None are these are CONTRACTS stating I take responsibility for the bill. Also, RRS violated HIPPA by obtaining my private medical information without my permission because they admitted to voluntarily obtaining my files. This means that RRS cant produce a HIPPA Release form where I gave them permission to obtain my medical services. RRS reporting their accounts to the credit reporting agencies, is illegal due to them using information illegally obtained. Theyre saying my disputes have no factual or legal ground, however with RRS having my medical file and information without me knowing especially after their representative stated they had nothing on file for me is a clear violation of the HIPPA Laws. RRS is aware of this violation and refuses to remedy the situation. They are now subject to up to {$50000.00} in fines per violation according to the HIPPA Guidelines. By continuing to report the information they are exercising illegal debt collection. Also, I checked with the hospital and they confirmed that there is a balance of {$0.00}.

On XX/XX/, a letter requesting verification and validation was sent to Receivable Recovery Services ( RRS ) in regards to an unknown dept balance from XXXX XXXX XXXX. It was explained in the letter that I was unaware of the dept, provider where dept was stated to be from, and that there was no consent/authorization given for my personal information to be shared with a dept collector. The letter requested that RRS provide me with validation and verification of debt, HIPPA authorization signed by myself, a breakdown of fees including any collection costs and medical charges, a copy of my signature with the provider of service to release my medical information to the debt collector, and that credit bureau reporting cease until debt has been verified and validated. Two letters from RRS dated XX/XX/ was received by myself stating that my request for validation and verification was respectfully declined. The letters also included dates, times, and deadlines for correspondence and disputes that I was unaware of due to being unaware of this dept. A follow up letter was sent to RRS on XX/XX/ requesting validation, verification, and proof that the stated accounts were mine in the form of previous requested documents. I have mailed two certifed letters to Receivable Recovery Services , LLC requesting Validation of Debt and HIPAA authorization. RRS has responded to my letters twice but have failed to provide me with the requested information validating this debt. They have ignored my requests, yet this debt still shows on my credit report. I had noticed recently that my credit report had collections from a company named Receivable Recover services. On XX/XX/2019 I called Receivable Recovery services because in XXXX I had sent letters asking them to verify debt that they claim I owe and had not received a response. I spoke with 3 different people after calling 3 different times that day. I asked them to tell me what the debt was for and they gave me a list of account numbers and amounts that were owed. I asked if they were duplicates because the some of the amounts reported on my credit report are the same. They said they were not. I asked them to send me validation of the debt and the person I spoke to said that she would check with her manager. I noticed some inaccuracy on my credit report and disputed the account with the ( creditor ) XXXX XXXX XXXX, Also with the Debt collection agency Recievable Recovery Services. I also informed them that the information was invalid. They also took my information without permission, and made their own decison to report that debt on my credit which i do not owe. On XXXX XXXX, 2017, I sent a HIPAA validation letter to Receivable Recovery to validate a debt that they alleged I owed for XXXX XXXX. I asked them to provide proof of the debt and validation of my signature giving this provider permission to release my medical information to them. They in turn responding with a generic " patient ledger '' that included my daughter 's name, some charges, and procedure codes ( HIPAA violation ). This is not validation, as they could not provide proof of my signature giving the provider permission to release my personal protected health information to them. I sent a second certified letter on X/XX/2017 requesting proof of my signature and that if they could not provide me with this validation that they would need to stop reporting this debt to all three credit bureaus. My second letter advised that they had 15 days to respond from the date of receipt of my letter, which they did not. I sent this letter certified and I have attached proof of both letters and a screenshot showing where the second letter was received on X/XX/2017. They still have yet to respond my second letter, which proves they do not have adequate proof to validate this debt.

I disputed this dept on my credit report on XXXX XXXX, 2016and on XXXX XXXX, 2016 the company responded to my dispute as verified so I call the company on XXXX XXXX, 2016 at XXXX I asked the representative about the dispute and why is it not being reported on my credit report as a medical bill instead of placed in collection. The representative asked me why was I disputing this account anyway, I advised him that the insurance paid this account. On XX/XX/2019, I sent a validation of debt letter to Receivable Recovery Services, a collection company that now owns a supposed medical account of mine. I do not recognize this account, which is why I sent them a validation of debt letter. According to the FCRA, the collection agency was to provide me with : - what they say the money I owe is for ; - how they calculated what they said I owe ; - copies of any papers showing I agreed to pay what they say I owe ; - a copy of any judgment ( if there is one ) ; - identify the original creditor ; - proof the statute of limitations has not expired ; - proof they are licensed to collect in my state ; - their license numbers and registered agent. Not only did they disregard my requests and not provide me with any information, as a collection agency, they blatantly broke the law that is the FCRA. After reviewing my credit report and finding a balance owed by a past rental company, I reached out to them XX/XX/2019 to find out what the balance was for. I was told that I an additional balance after departing the premises. Before leaving the premises and turning in any property owed items, a walk through was taken by myself and manager of the property. I was told, I would forfeit my deposit and nothing else was owed. Five years later, I receive an alert from my credit report. When I spoke with the property manager and asked why wasnt I informed about the debt supposedly owed. She stated because there was no corresponding address listed, it was put in my old mailbox, Which is on their property. I recently viewed my credit report and found a debt that was being reported by Receivable Recovery LA XXXX XXXX XXXX XXXX. XXXX XXXX La XXXX. This debt does not belong to me. I do not have, nor have I ever had a contract with Receivable Recovery. I sent a U.S. Postal Certified Letter on XX/XX/XXXX ( # XXXX XXXX XXXX XXXX XXXX ) asking them to validate this debt and to send any signed contracts I have between myself and with this company. I do not have, nor have I ever had a contract with Receivable Recovery. According to the Fair Credit Reporting Act, the creditor has to prove this account indeed belongs to me. My letter to this company was sent more than 30 days ago. I'm also concerned with the Hippa Violation of this medical bill. I have called this company several times with no returned phone calls. On multiple occasions I have advised Receivable Recovery that they are attempting to collect on a debt the does not belong to me and May have stemmed from Identity theft. It is absolutely upsetting that someone can steal my information and this company harass my life for a debt that was not incurred my me. I have sent over documentation numerous times for the fraud department to review in reference to this account. But the collection agency continues to make false statements saying that they cant view or read the documents. The address on file is an address that I have NEVER been associated with. On XX/XX/XXXX of 2019 I went to have my braces removed because I could no longer afford to have them. I was made to sign a document stating that I was going to have them removed for that reason. Services were not provided until payment was made on that same day. After a brief review of my credit report, I discovered that XXXX XXXX XXXX XXXX placed my account with Receivable Recovery. Receivable Recovery has never called me, has never sent me validation of this debt, and has made derogatory marks to my credit without my knowledge. Under the FCRA, it is illegal to report a debt without first providing notice of your intention to do so. This information will be disputed as well through XXXX, which will be inclusive of proof of payment. Proof of payment will be provided to the CFPB as well. The agency has 15 days to correct their records before I consider legal action against blatant FDCPA and FCRA violations.

Receivable Recovery Services calls my phone and will either not say nothing ( so I just hang up ) or want me to tell them my personal information before even telling me what the call is about or where they are calling from. I have repeatedly asked them to send me an itemized letter detailing what they are refusing to tell me about and to send it certified to ensure I get it. I have told them not to to contact me until they can prove that I get a letter and yet they still call. I have told them I do not owe them anything and to stop calling me. When I do this, they become aggressive and rude. I refuse to give my personal information to anyone on the phone especially when I know I do not owe this company anything. I've sent multiple letters to this agency about this not being my account. After being advised by identitytheft.gov, I've completed and sent a fraud affidavit via certified mail. I can see they received it, but nothing has changed. See supporting documents. Even the USPS tracking. My official complaint is against XXXX XXXX. My information was used to obtain an apartment, cell phone, and an auto loan with my ex-partner. I am a victim of identity theft and have put this off for too long. I informed the hospital ( the original owner of debt ) that this was not my account. The account belongs to my daughter whose mom is responsible for financial matters as she has sole legal custody. I have written several letters to this company in regard to a medical collection that was paid in full to the original creditor before it became a collection. I have not received anything back from the collection company. I have even asked if the collection company was legal to serve as a collection agency in my state. I have not received validation as of yet. On XX/XX/XXXX I received a letter form Receivable Recovery Service LLC about paying a medical debt from XXXX. The letter stated that I must contact them within 45 days of this bill or they will report my account to the credit reporting bureau. On XX/XX/XXXX, this was reported to the credit bureau even though I set up a payment agreement online. There was no indication that they would report to credit bureau after the payment plan was set up. Also, the 45 day period that they stated in the letter was not honored. This account was put on my credit report twice after it was removed. i called the credit bureaus and they aid it looked like the account number was changed and dates that it was opened was different as well. So I opened up an investigation on those accounts. and they are reporting as closed now and paid but they are not suppose to be on my report at all. Also Receivable Recovery. XXXX continues to report two items on my credit reports from Receivable Recovery. I disputed both accounts numerous times but XXXX keeps reporting as verified. I called the Collection Agency at XXXX on XX/XX/2018 and the representative told me that their company does not have any documentation for the accounts they are reporting. Upon reviewing my credit report a few months ago i seen multiple accounts listed under MEDICAL. Under hippa my medical information is protected. Why does a third party have my medicam information? Where 's the medical release form? This is humiliating and embarrassing that a third party collection agency can have access to MY medical records. A validation of debt letter under Fair Debt Collections Practice Act was sent to Receivable Recovery Services but they did not follow the requests of the letter within 30 days. Up to present day they have not provided an original contract that bears my signature. They also failed to provide an affidavit of claim & a certification of authority. In the early Part of XXXX 2017 I attempted to contact receivable recovery to clear up my accounts with them 1 by 1 at which time I was told by the representative but I spoke to that I could not clear up one account that I would have to pay all accounts at 1 time before they would clear any account. If I 'm not mistaken by law I am able to say which account that I want my funds to be applied to. I noticed this company attempting to collect a debt after seeing my credit report. I do not recall ever getting a letter informing me of the debt so I sent a request for them to validate the data and amount for I had medical insurance and do not believe an outstanding debt was incurred. I have sent 2 requests, both by certified mail. I have yet to get anything from them validating this debt so they have ignored me twice yet continue to report such as I have just checked today.
