On XX/XX/XXXX, I disputed inaccurate information reporting on my credit report by Puget Sound Collections. I was notified by the 3 credit reporting agencies, XXXX, XXXX and XXXX that the information was verified. I disagreed with this information, so on XX/XX/XXXX I sent a follow-up to the 3 credit reporting agencies that I disagreed with the information reported and requested their method of verification as in my research, I found they should be able to provide me. Again, they verified the accuracy of the reporting and made no changes. Ive also requested procedures in place for the bureaus because they have continually reported inaccurate information, knowingly and willingly. I sent a follow up on XX/XX/XXXX. I wanted to follow-up with the 3 credit reporting agencies, XXXX, XXXX and XXXX. I notified them of the errors with the reporting and made it evident that they were knowingly and willfully reporting inaccurate information. This is my exact reason for my dispute : Please verify this account and It's a shame how you have conducted and supposedly verified this account. The errors are evident as they are inaccurate in more ways than one. When reading up on the FCRA, I learned that if an account is reported on my credit report, everything within the reporting must be free from error which is not the case. You have failed to conduct a reasonable investigation and now I'm requesting from you the information you affirm to have firsthand knowledge of this account reporting with 100 % accuracy. Should you deny my request, I simply request that you remove this account in its entirety from my credit report. Based on my research, I'm requesting to further clarify your compliance with Metro2 Compliance. I don't agree with the Credit Limit listed and request that you please verify this information. I am also requesting that you provide verification of documents that you have reviewed. Please confirm the Status Code is also correct. I would like you to provide me with the Metro 2 Format, specifically Base Segment, Field 17A. Let me remind you that " parroting '' is not " conducting a reasonable investigation ''. Please take my request seriously. You have again confirmed to me in writing that the information reported is in fact accurate. It is now my opinion that you may have mix or mis-merged this file onto my credit report. After my research, I have found that mis-merged files occur largely because the CRAs ' computers do not use sufficiently rigorous score or scale thresholds to match consumer data precisely, even when such unique identifiers as Social Security numbers are present. The nationwide CRAs rely on a match of only seven of nine digits of the SSN ( See Reeves v. Equifax Info. Serv. ) The CFPB has identified mixed files as a particularly challenging problem. I again request that you immediately remove this account from my credit profile. Last month, I requested you to provide me with the " reasonable procedures '' that you have in place, yet you didn't provide anything " specific '' with " first-hand knowledge '' about this reporting. Need I remind you that these procedures are designed to prevent inaccuracies. I've requested this in writing and once again, you have failed me. It is my right to know that you have actually followed YOUR own procedures. ( Rothery v. TransUnion, LLC. ) It's not enough to just have procedures put in place but that the employee also follows the procedures in each and every report they prepare ( See Konter v. CSC Credit Serv., Inc. ). You must not just have reasonable rules, but your employees must strictly follow those internal CRA rules ( Carroll v. Exxon Co. ). The FTC staff indicates that " [ o ] ne of the most significant compliance procedures to assure accuracy will be the training of new personnel and the retraining of current employees from time to time. Even isolated instances or error should be followed up and procedures adjusted in order to correct the cause of the error '' ( FTC, Compliance with the Fair Credit Reporting Act ). Since you have failed yet again, I implore you to do the right thing and remove this from my credit report. On XX/XX/XXXX, I sent another dispute to the 3 credit reporting agencies to let them know that I have not been able to obtain actual verification of what Puget Sound Collections is reporting. I notified them that the information is inaccurate and unable to be verified per my request. On XX/XX/XXXX, I sent another dispute to Puget Sound Collections to notify them that I've had my rights violated. My requests have gone ignored. In my research, I found something called Appendix B to Part 1022 - Model Notices of Furnishing Negative Information. My research shows that a financial institution that is subject to section 623 ( a ) ( 7 ) of the FCRA shall be deemed to be in compliance with the notice requirement in section 623 ( a ) ( 7 ) of the FCRA if the institution properly uses the model notices in this appendix. I would like to see the actual document that was sent to me to verify that my rights have not been violated. On XX/XX/XXXX, XXXX, I sent another dispute to the 3 credit reporting agencies letting them know that my report is inaccurate. I requested more information regarding the reporting of this account, yet all they did was verify that the information is accurate. My exact dispute was as follows : Please verify this account. I'm unaware of the information reporting on this listing. Please remove it in its entirety or show me what information you reviewed in order to report it in the first place. Based on my research, I'm requesting to further clarify your compliance with Metro 2 Compliance. I don't agree with the XXXX Type listed and request that you please verify this information. I am also requesting that you provide verification of documents that you have reviewed. Specifically, I would like to confirm the FCRA Compliance Date or Date of First Delinquency/Last Activity is in Base Segment, Field 25. Let me remind you that " parroting '' is not " conducting a reasonable investigation ''. Again, if youre unable to comply with my request, please simply remove this listing from my credit profile. On XX/XX/XXXX, I sent a request of verification to Puget Sound Collections so that I can see how and why they have verified this information with the 3 credit reporting agencies, yet, all 3 of them didn't report the exact same thing. Again, in my research, I found that the FACT Act allows me to go directly to the original creditor under Section 312. I requested a bunch of information ; however, they didn't provide me with the requested information. On XX/XX/XXXX, Puget Sound Collections sent me the requested " verification '' with a form that they have created stating the funds were due to XXXX XXXX XXXX. Maybe they are unaware that this is not sufficient verification and that they have to go to the original creditor and provide actual verification from the original creditor. They showed nothing proving that this account is mine nor that the information that is being reported is accurate. I also requested verification of the forms completed to the bureaus since information is different across the bureaus.

This account is in process by my health insurance carrier, XXXX XXXX XXXX. It NEVER should have been sent to a collection agency. Puget Sound Collection is pursuing the debt on behalf of XXXX XXXX XXXX and XXXX, a public organization. XXXX XXXX XXXX XXXX XXXX claims that they do not require residents of their district ( which I am a resident of ) to pay out of pocket for their services. Original Creditor : XXXX, XXXX XXXX XX/XX/XXXX - Date THIS account 1st became past due, was written off and sent to collection agency ( I don't know where I have no record of contact from them ). XX/XX/XXXX - Moved to Florida and have continuously updated and forwarded current addresses through USPS database from then to date. XX/XX/XXXX - Date this account was moved to 2nd and current collection agency ( Puget Sound Collections ). Never received any sort of notification or statement. XX/XX/XXXX - Date I began requesting credit reports from XXXX, XXXX, XXXX and PSC showing old XXXX debt erroneously as new debt opened on XX/XX/XXXX which caused extreme drop to credit scores appearing as new debt. Contacted PSC by phone to inquire about conflicting information in report, they confirmed this was account with 1st past due in XXXX and that they got it from a previous collection agency as of XX/XX/XXXX, so that is the date that becomes the 1st past due and original report date. Disputed this as accurate and requested proof of date account 1st became delinquent and that they correctly and completely report this account as past due from the XXXX date f from original creditor. XX/XX/XXXX - Received 1st actual statement and additional enclosed documentation that PSC stated was the response and provided by XXXX, XXXX XXXX - to include her provider office financial report ( all dates from XXXX only ) with not just dates and balances, this report included PHI I in no way conseted she could share or distribute to anyone, FULL ICD diagnosis codes with my name for each visit date some going as far as documenting XXXX next to the codes. She also chose to send even more PHI without consent to this collection agency, to include a had printed family history and information sheet I had completed that has my minor daughters names and D.O.B. and that they are treated and seen there ( for XXXX ). XX/XX/XXXX - Sent certified receipt letter to XXXX, XXXX XXXX attached here. No response, reply or contact as of today. - Sent PSC certified receipt letter to PSC attached here. No response, reply or contact from them as of today. XX/XX/XXXX - Since this date have submitted disputes to XXXX, XXXX & XXXX XXXX ( x3 ) that dates are incorrect, that due to the " date reported '' line information being XX/XX/XXXX, requested removal or correction. XX/XX/XXXX - XXXX has since deleted the account all together. - XXXX now only shows as a number with X at end, only information in Account type, Account Status ( closed ) the balance {$1200.00}. Your comment : Medical ( not my comment at all ), Coments : MEDICAL Unpaid. No Original creditor or business name or contact information at all. - XXXX - contacted today dispute completed and Meets all FRCA requirements and not being removed. Stated to manager that It is definitely not meting them and all other information relating to the actual date the account was 1st past due missing completely the date of this debt is GROSSLY misrepresented and continues to have a profoundly negative impact it would not otherwise have this many years later. Told by manager that the Date the account 1st became past due is on his screen as XX/XX/XXXX so even though that does show up in ANY field or at all on my actual report they have done their job and it will just fall off XX/XX/XXXX. He said they dont have to put complete information or input all of the data they can see on their screen and if I have a problem with that I should make a report to you and or contact the Attorney General.

In XX/XX/2017, I underwent a XXXX by and through XXXX XXXX XXXX XXXX. After the XXXX, I owed a balance to XXXX XXXX in the amount of approximately {$2200.00}. At the time, I was working with the billing office attempting to set up a payment plan. However, due to the amount of the payments for the plan, I was unable to continue paying the full amount and was attempting to pay what I could afford. The bill was sent to a collection company Puget Sound Collections ( hereinafter referred to as PSC ). PSC contacted me and discussed the situation. We agreed to a payment plan, and I was to receive a promissory note from the company that confirmed the payments, amount, etc. At no time did I receive a promissory note from PSC, and under the law, I am protected from making payments without validation and verification of the debt. I contacted PSC and did not receive a very nice response. In fact, I was threatened with legal action. I filed a complaint with the State regarding the threatening message, and heard nothing further. A year and a half later, I was served with a lawsuit. This complaint is in regards to that lawsuit and PSC 's deliberate attempt at destroying me both financially and personally. Due to some hard times my family has fallen on ( 4 XXXX XXXX in less than 6 months between my husband and I ; job loss for both my spouse and I ; two XXXX XXXX, one life threatening that almost killed my husband ) I hired a company called XXXX XXXX XXXX to assist me in being responsible, and cleaning up all of our debts. XXXX XXXX XXXX is an alternative to Bankruptcy, which I am fully qualified for right now due to the extreme hardship I am under. It is my understanding from XXXX XXXX XXXX and my attorney by and through XXXX XXXX XXXX that my file with PSC has been noted as " no settlement/no negotiation ''. XXXX XXXX XXXX has made a diligent effort to resolve this matter with PSC and they are refusing. Stating that they want to obtain a judgment as opposed to working out a payment plan through my Debt company and attorney. PSC is deliberately attempting to destroy my credit, and my future by obtaining a judgment. PSC has been offered an amount to settle, and I have offered to pay the full amount through XXXX XXXX XXXX in order to resolve this debt, and pay my obligation, however PSC continues to state they will not settle, or accept any payments and thus are going to move for a judgment against me wherein they will receive {$0.00} as I am unemployed. I would like action taken against PSC for unfair tactics and deliberate abuse towards a person. If PSC is unwilling to accept a payment plan for the full amount, I am not above filing for Bankruptcy and wiping out the debt completely. However, I would prefer to continue working with XXXX and continue being responsible and paying my obligations so that my future is bright, not destroyed by a medical debt and an irrational, unprofessional debt collector.

XX/XX/XXXX - Initial Dispute with Credit Bureaus requesting Verification ( no verification supplied ) XX/XX/XXXX - Method of Verification Request Sent to Credit Bureaus ( no details regarding creditor, dates, added info supplied - no change ) XX/XX/XXXX - Wrote to bureaus again requesting verification of the debt and no verification received regarding this account again. XX/XX/XXXX - Escalated and sent Validation Letter directly to the collection company ( no true validation has occurred up to this point ) XX/XX/XXXX - I received a bill from the collection company, not validation - it is attached XX/XX/XXXX - Again, I requested full validation on this account again- they supplied a letter stating the sent full validation ( the letter I received on XX/XX/XXXX attached, but again this this is not validation of a debt, it's merely a bill ) XX/XX/XXXX - I received the same bill as a response, with the statement that the debt had been validated back in XXXX ( as you can see from the letter they sent in received XX/XX/XXXX - it is a bill, not legal validation/ Per my consumer rights there are details legally they should be able to provide me the consumer that has not happened. A bill is not validation. I will attach proof of the validation requests I have sent. Legally they should be able to supply the following upon request, I do not know what this is for and I am trying to find out and still have not found out. What they sent me does not prove that I owe this debt. I requested multiple times they provide legal validation to include the following : ( 1 ). Proof that you either own or were assigned the debt from the creditor. ( 2 ). Proof that you have the right to collect this debt in my state. ( 3 ). Proof that your company carries a bond. ( 4 ). Proof of how your company assessed the outstanding debt. ( 5 ). Payment history from the original creditor. ( 6 ). Name and address from the original creditor. ( 7 ). Copy of the original contract bearing my signature. ( 8 ). Chain of title on this debt. ( 9 ). Proof that you can collect this debt from me. ( 10 ). Do you own or is this debt assigned? ( 11 ) .Copies of all letters sent to me from you and the original creditor regarding this debt. ( 12 ). How much did you buy the debt for? ( 13 ). When was the last time a payment was made on this debt? ( 14 ). What fees/interest were tacked onto the debt? ( 15 ). How were the fees determined? It has now led me to file a complaint with the CFPB, because I have been requesting information and it has yet to be provided. I have followed all channels from disputing/investigating validity with the bureaus and the creditor and no one has validated or supplied any of the actual supporting documentation that by law they should be able to supply.

XX/XX/XXXX : I was seen in a hospital XXXX XXXX for an injury and after 90 minutes I was discharged home. XX/XX/XXXX : I received an Explanation of Benefits from my medical insurance provider, XXXX XXXX, which indicated I had {$0.00} patient responsibility to the provider ( s ) for my treatment. XX/XX/XXXX : I received a vaguely-worded letter from a debt collections agency claiming I owed {$810.00} to XXXX XXXX XXXX XXXX . XX/XX/XXXX : I sent a reply to the collections agency via USPS Certified Mail that I wasn't familiar with the charge ( s ) they were referencing. XX/XX/XXXX : I received another memo from the collections agency ( dated XX/XX/XXXX ) that these charges were associated with the hospital XXXX visit on XX/XX/XXXX. They provided a statement dated XX/XX/XXXX from XXXX XXXX XXXX XXXX ( XXXX XXXX XXXX, XXXX XXXX XXXX XXXXXXXX ) claiming that the insurance company only paid a portion of the provider 's services. Needless to say, this was a " SURPRISE BILL ''! I had received ZERO contact or correspondence from this group and the mailing address they provided is 400 miles away. They also didn't include a telephone number for correspondence. XX/XX/XXXX : I called my health insurance HMO to investigate what they have documented on their files. They verified that the claim from the " emergency medical group '' had been processed and paid in early-XXXX XXXX. The representative from my HMO researched the medical group and located a telephone number ( XXXX ) for them. The representative placed me on-hold, then called the medical group and left a voicemail message with both the HMO phone number and my telephone number, requesting that they please call back at their earliest convenience. XX/XX/XXXX : I sent a second reply to the collections agency via USPS Certified Mail stating that I had never received any statements or memos from this medical group and that I had been in contact with my medical insurance provider who indicated the medical group had received payment for services rendered. XX/XX/XXXX : I have made several subsequent attempts to contact the medical group by telephone ( XXXX ) but incoming calls are immediately routed to voicemail and I have yet to receive a call back. XX/XX/XXXX : This seems like a scam to me and I would like assistance on how to resolve this situation. I would be happy to provide documentation, but will first need the opportunity to redact the documents for PHI and PII.

I've been trying to resolve this issue with this company and it has been 3 months now and they are not taking this matter seriously. I contacted them about validation of debt and when they provided it, it proved based on laws in the state of Washington that there debt is illegal. Based on the documentation it affirms my move out date of XX/XX/XXXX and this would mean based on the law that 21 days later the final move out the landlord must provide the statement as to the disposition of the deposit along with any charges. My previous offer is rescinded and I am now fully disputing this entire collection balance. This is in regard to our phone conversation regarding the itemized statement you provided in reference to the debt assigned to you by XXXX XXXX. As mentioned, the packet of documents you sent over had several addendums and revisions to the final move out report and deposit worksheets that have been presented to us since our move out XX/XX/XXXX. Specifically, the final revision the following line items dated after the 21 days XXXX XXXX had as prescribed by XXXX XXXX which states ( 1 ) ( a ) Within 21 days after the termination of the rental agreement and vacation of the premises or, if the tenant abandons the premises as defined in XXXX XXXX, within 21 days after the landlord learns of the abandonment, the landlord shall give a full and specific statement of the basis for retaining any of the deposit, and any documentation required by ( b ) of this subsection, together with the payment of any refund due the tenant under the terms and conditions of the rental agreement. Note : 21 days was increased to 30 days as of XX/XX/XXXX but that does not apply to this case. XXXX XXXX failed to meet this requirement in order to have a legal claim to collect on my security deposit as they did not provide any documentation dated within those 21 days. The documentation provided is postmarked and dated well after the 21 days. In addition, there are several deposit worksheets and Move out reports that are conflicting. An examination of the itemization reveals that there was a charge of {$780.00} added on XX/XX/XXXX described as Replace a broken window which is a full 6 months after my move out statement. That is completely unreasonable, unacceptable, and rejected. I feel well within my legal rights to reject any of the line items after XX/XX/XXXX.

I, XXXX XXXX believes in the credit reporting system as the most important element to the success of the commercial banking system. I am also a proponent of the Consumer Financial Protection Bureau ( CFPB ) as a tool to streamline issues on credit reports that may be dragging down my overall scores. Accordingly, I address each particular issue directly with the creditor in question, before submitting any complaint to the credit bureaus if necessary. Therefore, I desire the following reinvestigation as follows : 1 ) On accounts where late payments are posted, I respectfully request a review of the past payment history and to ascertain if certain payments could actually have been posted late because of an overlap on the 30-days grace period. I realize that this may happen on occasion, and I request only a summary review internally, and do not require any documentation. 2 ) On any account that falls within the 84-month negative reporting timeline, I respectfully request either the deletion of the account if said account was closed, or the elimination of the prior payment if the account it is still active, this is well within the Fair Credit Reporting Act ( FCRA ) guidelines pertaining to obsolete information. 3 ) Any collections must be proved if either a direct collection and/or debt purchase. In either event I request that a true original copy of right to pursue collection be established with each respective credit reporting agency, and if a debt purchase the guidelines proving transfer of ownership must be submitted to all credit bureaus as well as me personally to validate ownership and the right to collect. I am well within my rights requesting proper documentation as administered through the Fair Debt Collection Practices Act ( FDCPA ), the Consumer Financial Protection Bureau ( CFPB ) and the Federal Trade Commission ( FTC ), in addition to the even stricter guidelines enforced by the office of attorney general in my state of domicile. I respectfully request each investigation to be reported to XXXX and XXXX XXXX within a reasonable time period, hopefully 30-days. I realize that the current Covid-19 crisis could cause delays, so the best efforts of each financial institution and collection agency will be appreciated under the current circumstances. XXXX XXXX

I work at XXXX XXXX, XXXX WA. About a month ago I received a phone call at my bosses office phone XXXX from XXXX Police. I was informed that there was a woman waiting for me and I had an appointment with her. I informed them that I had no appointments and did not know the person there. XXXX Police put the woman on the phone and she told me she was from Puget Sound Collections and wanted to meet with me. I told her on the phone that I was not allowed to receive calls at work and could not entertain her. I asked her to contact me at home and we could work out a solution to whatever problem there was. I then asked her not to call me at my work place again. I heard nothing from this company for about two weeks, until I received another call from them, again, at my workplace. I got there contact information and gave them mine and told them I could not receive phone calls at work and could they please stop and contact me at my home. I received a verbal warning from my supervisor about personal calls at that time. I had not heard anything from this company again for about XXXX weeks until today. I was at work, XXXX, XXXX. It was about XXXX when my supervisor came out to our jobsite and gave me a phone message that came into his office during the day shift. The message had my name on it and was from someone named XXXX from PSC, saying that they would be in the office until XXXX. I received another verbal warning from my supervisor about personal calls and was informed that if it continued I would get a written letter of warning. I feel that I am being harassed on my job and it could jeopardize my job. I also feel that this company contacting another shift supervisor could also impact my job. I have told the people from this company to please not call me at my work, but contact me at home and I feel that they are purposefully ignoring my request in an attempt to shame or embarrass me into paying. If they would contact me at home I am more than happy to discuss whatever bill they have and if it is legitimate then I will work out a payment plan with them, but if they continue to ignore my requests it could put my job in danger.

I pulled all 3 of my credit reports to find out what was showing on them, as I hadn't done it in a long time. I was shocked to see accounts that I do not recognize, including companies I've never done business with. One of these was : PUGET SOUND COLLECT. The address LISTED ON the credit report is : XXXX XXXX XXXX XXXX, WA XXXX. I was advised by an attorney to send a " Validation '' Letter to them to validate the debt under the FDCPA and FCRA. It has been 60 days since I mailed that out, and under the FDCPA, the debt collector had 30 days to respond, and did not, which is a violation of the FCRA and FDCPA. Please note that before sending the validation letter to this company, I also sent a dispute letter to all 3 credit bureaus ( XXXX, XXXX, and XXXX ) and PUGET SOUND COLLECT has NOT removed this account from my credit reports, or have sent me any kind of validation. I request for this company to remove this account from my credit reports immediately, as they have not followed the law, have reported inaccurate information on my credit reports, including FDCPA Section 809 ( b ), which carries a {$1000.00} fine for not validating the debt and still continuing to report it on my credit reports. In addition to this, they're reporting that they originally purchased this collection on XX/XX/2013. The statute of limitations starts when a debt FIRST becomes delinquent. Even if the first delinquency of this erroneous debt with the original creditor was on the SAME date that this debt collector purchased the debt, there would only be 9 months left in the statute of limitations. Title 4, Chapter 16 of the Revised Code of Washington ( RCW ) outlines the various statutes of limitations for debt collection in the state. RCW 4.16.040 gives written contracts and accounts receivable a statute of limitations of 6 years before the unpaid debt becomes time-barred. RCW 4.16.080 stipulates that oral contracts have a statute of limitations of 3 years. Thank you for helping me resolve this matter.

I had a judgment placed on me for medical bills that I never received on XX/XX/XXXX, 2017 by Puget Sound Collection Inc. ( PSC ) in the amount of {$5800.00} and PSC court Fee 's of {$530.00} which totals out for them to be {$6400.00}. I noticed the large amounts of medical bills and immediately contacted PSC Inc. and found out that most the bills were due to a L & I claim that I had years ago that multi-care never bill them for but knew it was an L & I claim. PSC and Multi-care both denied that it was but I had medical proof that they knew but did not disclosed this to me or the court. I began to investigate this insane amount. my findings through doing a discovery with a legal aid service & L & I showed that Multi-care knew that most of my bills that I was being persuade for were in fact a claim from years ago. I contacted PSC and was told to take it to court it does n't matter to them if I can prove that they falsified documentation or not it 's up to the court to decide and since judgement was already ordered against me I was told " GOOD LUCK '' after weeks of hard work and labor and taking days off work to attend to this matter I finally submitted my paperwork to the court on XX/XX/XXXX, 2017. I spoke with PSC Lawyer XXXX XXXX XXXX after I saw the judge on my case who set a court date for XX/XX/XXXX, 2017. we had a conversation and he emailed me the next day. and I responded to his email back in reference to my case. we came to an agreement on the amount owed. and paperwork was finalized in court on XX/XX/XXXX, 2017. I went to PSC Inc. dropped off the paperwork that day from the court and came back on the XXXX of XXXX and made a payment of {$250.00} in cash to PSC and noticed that they are charging me fees for court and there lawyer, this is not what I agreed to with the Lawyer that represented PSC Inc? I have attached the following PDF files in the order of events as they occurred.

I received a notice of debt from PSC regarding medical services provided by XXXX XXXX XXXX. I was shocked to see a collection notice as I never received a normal bill from them prior to that. I have disputed these charges through an online service that routed to the main credit bureaus for dispute but never heard from a response from PSC. Finally, I sent a certified letter on XX/XX/2020 requesting a correction of their charges as the doctor only saw me for 3 minutes in a bed temporarily in the hallway as the hospital was busy during my visit and even the medical provider spent a quarter or half an hour, {$500.00} is excessive. To my surprise, their response was a statement of account. This is not an acceptable response from PSC or XXXX XXXX XXXX as the billing practices are excessive and abusive. If they adjust their bill accordingly or even had the decency to explain their variance, I would have been more than happy to resolve this matter. XXXX XXXX XXXX 's attempt to collect excessive charges over actual across state lines from my insurance carriers constitutes insurance fraud and these practices should be further investigated. ( 1 ) Puget Sound Collections is in violation of the FDCPA 's section 807 by providing a statement of account instead of an answer to excessive billing practices as a response to a dispute. ( 2 ) Because of this, Puget Sound Collections is also in violation of the FDCPA 's section 809 subsection ( b ) for improperly attempting to validate the debt and not directly addressing the erroneous and excessive billing amount. ( 3 ) Puget Sound failed to respond within the 5 day period as stated by the FDCPA 's section 809 subsection ( a ). Please see attachments confirming the dispute letter was sent on XX/XX/2020 and their response was 36 days later on XX/XX/2020. The FDCPA does allow a consumer to recover up to {$1000.00} per violation

Original debt from XXXX XXXX XXXX, XXXX WA. Original XXXX XXXX ordered by XXXX XXXX XXXX, XXXX AZ. XXXX XXXX for several XXXX XXXX XXXX XXXX XXXX XXXX XXXX such as XXXX and XXXX etc.XXXX. XXXX sent specimen unrefrigerated in mail. Specimen arrived at XXXX 7 days after taken from patient making it a non viable specimen for testing. XXXX personnel knew this and conducted testing anyway. Charged customer enormous fees for XXXX XXXX unwarranted for XXXX vs XXXX XXXX which is a fraction of the cost. This was a common practice to do this unwarranted testing to overcharge patients. Department of Justice has charged XXXX XXXX XXXX with giving all its doctors, including XXXX XXXX XXXX KICKBACKS for referring services unwarranted and nonviable specimens for testing. I, as XXXX XXXX customer was duped. During a doctor visit with XXXX XXXX he said he was going to do a XXXX XXXX in office. I watched the nurse perform test. After I left office they took XXXX sample and mailed it to XXXX XXXX XXXX and authorized expensive XXXX XXXX which I would have never authorized. I had been tested many times only to pay no more than {$40.00}. When I got the bill from the XXXX for in the {$3000.00} range I was outraged and started investigating. Not only was it fraudulently sent but a nonviable specimen for testing being so old and exposed to heat!! I then found out that XXXX XXXX XXXX was getting a KICKBACK in the form of some kind of marketing check as found out by the Department of Justice. I then discovered that the XXXX had closed, moved a couple blocks up the street and reopened in a different name. I am a victim of fraud and a kickback scheme in the XXXX profession. The Department of Justice has arrested the top people at the XXXX and some have already pleaded guilty. This debt is a product of FRAUD!

I pulled all 3 of my credit reports, and my wife 's reports to find out what was showing on them, as I hadn't done it in a long time. I was shocked to see accounts that we do not recognize, including companies I've never done business with. One of these was : XXXX XXXX XXXX XXXXXXXX. The address LISTED ON the credit report is : XXXX XXXX XXXX XXXX, XXXX, WA XXXX. They're reporting an account on our credit reports, and we have NEVER done any business with this company, signed any contract with this company, or even know who they are! Upon calling the company, they could not even FIND our information, or the account that they're reporting on our credit reports. I was advised by an attorney to send a " Validation '' Letter to them to validate the debt under the FDCPA and FCRA. Under the law, they had 30 days after receiving the validation letter to respond to my letter and validate the debt that they're reporting to the credit bureaus. It has now been over 70 days ( they had 30 days under the law ) and they have NOT sent ANY kind of validation of this erroneous account, for both my wife and myself. Please note that before sending the validation letter to this company, I also sent a dispute letter to all 3 credit bureaus ( XXXX, XXXX, and XXXX ), and my wife did too, and XXXX XXXX XXXX XXXX has NOT removed this account from our credit reports, or have sent us any kind of validation. I request for this company to remove these accounts from my credit reports immediately, as well as my wife, as they have not followed the law, have reported inaccurate information on our credit reports, which carries a {$1000.00} fine, as well as FDCPA Section 809 ( b ), which carries a {$1000.00} fine for not validating the debt and still continuing to report it on my credit reports. Thank you for helping me resolve this matter.

XXXX XXXX had a prior collection account with XXXX XXXX XXXX for attorney fees that was then transferred to XXXX XXXX XXXX. Payment arrangements had been set up with XXXX XXXX including an agreement to hold interest. The account was then sold, when the original debt collection agency went out of business, without our knowledge. We had no idea who XXXX XXXX XXXX was or that they had assumed responsibility for the debt. Prior to their communications, a check in the amount of {$1000.00} was sent to and cashed by XXXX XXXX XXXX. XXXX XXXX XXXX has now contacted us on multiple occasions. Their most recent communications include contacting XXXX XXXX at XXXX on the dates of XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, and three times on XX/XX/XXXX. During each of these phone calls and voice mails they failed to notify us that this was an attempt to collect a debt. When returning their call to learn more about why they were contacting us, we became aware that an additional {$600.00} had been added on in " interest and fees ''. The gentleman on the phone stated he was " doing us a favor '' by applying our {$1000.00} payment to principal. We asked for an explanation regarding how the additional fees had been assessed stating that they could not just add fees without justification. The gentleman on the phone at that time stated " Actually we can and we will ''. We asked XXXX XXXX XXXX how we could arrange to pay on the account, what they were willing to do to work with us, and were told that we would have to make them an offer, then they would decide if they would accept. They failed to notify us in writing of the debt until XX/XX/XXXX. Further, on their statement they allege that the last payment received was XX/XX/XXXX although multiple payments have been made since XX/XX/XXXX.

I used to live in an apartment building ( XXXX XXXX, XXXX, WA ). One day there was water leakage from above apartment ( Apartment # XXXX ) causing my carpet wet for many days. Management fixed the leakage but did not do anything to dry carpet. Carpet was wet for many days got ripped as time passed. I requested manager to change or repair carpet but she did not listen. In XX/XX/2018 i finally told manager if you don't change or repair my carpet i will not pay rent or i will move to somewhere else. She ignored and I did not pay rent and asked to repair carpet but she sent me a notice to pay rent or vacate apartment. She sent me 3 days evacuation notice on XXXX XX/XX/2018 without helping me out. I decided to move and i moved on XX/XX/XXXX and left keys with my neighbor XXXX apartment # XXXX because it was night time. XXXX handed over keys to manager next day. I called manger next days and informed that I left apartment yesterday and she said alright. But after couple of days she sent me an invoice for rent and new carpet change charges etc. My lease agreement was already finished in XX/XX/2018 and i was not signing lease because they were not changing or repairing my carpet. I was paying just month to month rent. I did not break any rental agreement. I have a copy of written complaint which was sent to rental company 's website about leakage and carpet problem. They wanted me to live with ripped and moldy carpet with my kids. Now they want to charge me for new carpet, attorney charges and rent ( $ 2200+ ) .The management ( XXXX XXXX ) sent bills to collection agency ( XXXX XXXX XXXX ) to collect money which is impacting my credit score and stress. I explained to XXXX credit bureau but they did not reply. Management also did not refund my {$600.00} security deposit.

On XX/XX/XXXX I was informed by a lender that unpaid medical debt showed up on my credit report XX/XX/XXXX for services provided by a doctor in XX/XX/XXXX. As far as I know I paid all my copays and deductibles in XX/XX/XXXX. I was not notified by the doctor that any further debt was outstanding. I contacted Puget Sound Collections to inquire about the debt and to validate it was correct. They told me it was for unpaid medical bills to XXXX, but that is all they would tell me. They referred me to XXXX who told me I had a {$0.00} balance and referred me back to Puget Sound Collections. I 've spoken to XXXX people at Puget Sound Collections and XXXX people at XXXX and none can verify or validate the debt. The second woman I spoke to at XXXX told me that a letter is sent every month for XXXX months if a balance is due and on the XXXX month it automatically gets sent to collections. She did n't have an answer as to why mine would 've been sent almost XXXX years instead of XXXX months later. I asked Puget Sound Collections to take the negative marks off my Credit Report until the debt was validated and they refused multiple times. The reason they gave for not notifying me prior to sending the account to the credit Bureaus is that they are not licensed in my state. I verified with both XXXX and Puget Sound Collections that they have my current address and phone number. Puget Sound Collections has told me, in no uncertain terms, that I must pay them without verification of the debt before they will remove any negative remarks. I consider this extortion. It 's my understanding that if Puget Sound Collections reported inaccurate information to the Credit Bureaus that is considered fraud.

This company is incorrectly reporting a collection account in the amount of {$510.00} on my credit report. I have no knowledge of this account, have never had a late payment in my life. I contacted Puget Sound Collections to try to rectify this situation as it has dropped my XXXX+ credit score by 75 points! I was treated very rudely, and told they do n't have the phone number for the original creditor and all she can do is mark their file ( not my credit report ) as a verbal dispute. I finally researched this so called " original creditor '' and found a phone number myself. I contacted XXXX XXXX, whom Puget Sound Collections claims is the original creditor, and I spoke directly with the owner of the business. She stated that her company does not even use a collection agency. I did have her search her records for me, which she assured me she has no record of. To be honest, I would like to think I would notice if one of our two cars was towed, but they are both still here ... in my driveway ... where they belong ... because they WERE N'T towed! I informed XXXX XXXX, a manager a Puget Sound, but she also did not wish to provide any assistance. I have since filed a complaint with the credit bureau directly, but this company needs to have their collections practices reviewed and revised accordingly. It is absolutely unacceptable that I, a consumer with perfect credit history, should be treated with such disrespect and disdain by a company that has zero legal standing. I am appalled and disgusted and I intend to do everything in my power to force this company to abide by the FDCPA and follow legal collection practices, which do NOT include berating a consumer who has no business with them.

In XX/XX/2018, I received a document at my place of employment requesting verification of my income. Upon receipt of this document, I contacted P.S.C. to discuss the debt and my intent to pay. During this conversation, we discussed a promissory note for the debt in question, and I agree to review the document, and I made a {$50.00} good faith payment towards to debt after being told that I needed to do so. I was to receive a promissory note in the mail from P.S.C. in order to begin making payments on the medical debt. To date, I have not received the document, but I have continued to receive harassing phone calls. On Friday, XX/XX/XXXX, I received a call from " XXXX '' demanding that I return his phone call by XXXX XXXX that day. On that date, I also made another {$50.00} payment as good faith, expecting that I would at some point receive the agreed upon documentation that would afford me the opportunity to clear this debt. It is not my responsibility to ensure that documentation is received by me in order to clear the debt. It is the responsibility of the collection agency to honor their part of the arrangement and provide me with proper information. I am not going to pay on a debt, under a verbal agreement with no assurance that my credit will not be impacted. If PSC wants me to pay the outstanding debt, then I suggest they stop calling to harass me, and they provide me with the written documentation they themselves proposed so I can go ahead and issue payments to clear the debt. This will be my only attempt to resolve this matter with PSC. Should PSC continue to harass me, I will take this to the next level. Thank you.

Puget Sound Collections ( PSC ) is attempting to collect a debt that is invalid and previously paid. I have offered on numerous occasions to settle this alleged debt despite previously believing and now knowing that the balance had been previously paid. PSC has reported this alleged debt to the credit reporting agencies and caused me economic harm. Attached is a letter from them, a letter from the original creditor, a letter I sent them explaining how this has been paid and supporting bank statements. PSC has engaged in a pattern of deceptive behavior when I have attempted to ask them for validation of the debt they have sent me a multitude of conflicting account numbers and inaccurate service dates. I believe that they have not attempted to validate any of the debts as they are legally obligated to do so. Further, they have previously regened on promises to myself to remove this from my credit report, another practice that seems to be happening repeatedly. I believe they are engaging in a pattern of abusive practices as evidenced from complaints at the better business bureau. PSC has also previously offered to settle this matter with me for full payment and agreed to remove it from my credit report while on the phone with XXXX. She additionally stated they would send me a letter to the same effect and never did. When I called in subsequent to that they have told me that " its against office policy '' to remove entries from people 's credit reports. This same kind of behavior and worse is easily found on the XXXX for this business. This company is an example of the worst behaviors of debt collection agencies.

In XX/XX/XXXX I was treated for XXXX XXXX at XXXX XXXX. I received one bill for this visit from XXXX. It has been determined that this bill was paid. However, I still discovered that a collection of {$590.00} from this vist was put on my account on XX/XX/XXXX. I did not pay this because I believed I already had. Later on, I disputed this collection because I believed this debt was paid on XX/XX/XXXX. However, on XX/XX/XXXX I was informed by Puget Sound Collections that this was a separate bill from a XXXX XXXX XXXX XXXX, not the XXXX XXXX. The Collection agency claims to have sent a notice to me to XXXX XXXX XXXX in XXXX, WA. However, I am completely unsure of where they got this address from. At the time of this hospital visit, I was living in XXXX, WA which I had been since XXXX, XXXX. This can be seen through my personal information from the hospital and an addition to a lease agreement. The XXXX XXXX was able to have my address, because they sent my a bill and it was paid. I did move shorlty after this visit, but it was not to a XXXX XXXX . You can see I moved out of XXXX in XX/XX/XXXX. I moved to XXXX, WA but our mail was forwarded to XXXX XXXX XXXX. Again, I do not know where XXXX XXXX XXXX came from, but it was not the correct address to reach me at and it is not right that this is negatively affecting my credit when I did not have the opportunity to pay this in a timely manner. I did not provide this address and it is not an mailing address connected to me.

Puget Sound Collections is attempting to collect this debt in an unfair and unconscionable manner. I have stated numerous times the debt in question is unrelated to my XXXX accident, and therefore, I wanted to pay this debt immediately. Each and every time I spoke with a Puget Sound Collections representative, they advised me they would not speak to me as I had attorney representation. I have tried to take care of this debt however, Puget Sound Collections will not speak to me about the debt or tell me how much I need to pay to satisfy this debt. These actions are unfair and they are financially harming me. I hereby request that Puget Sound Collections take the following action : 1 ) remove this negative reporting from my credit bureau at once, 2 ) have a manager speak to me about my debt so I can make arrangements to satisfy this outstanding amount ( if the debt is truly non related to accident ). And last but not least they lied to me and my attorneys office stating this would not go on my credit until the accident has been resolved. They spoke with my attorneys office in XXXX 2015 and as of XXXX 2015 it has now been put on my credit report dropping me down XXXX points. I have worked to hard to have the cruel people ruin this. A company called Puget Sound Collections is reporting that I owe them a debt of {$1200.00} and keeps illegally increasing the debt owed on my report every month. The account number being reported to my credit is # XXXX. I have never had any dealings with this company and do not know why they want me to pay them this money. There is no reason for this debt to be on my credit report. I received no written notification that they were placing this debt in collections and I only discovered it was on my credit in recent months. I will also need to understand why they are increasing the balance owed on my credit every month. My XXXX XXXX credit report shows a balance of {$1100.00} and my new XXXX XXXX credit report shows a higher balance of {$1200.00}. Under the FDCA 808 which governs unfair practices, a debt collector is not allowed to add their own fees or expenses to any amount being collected on. It appears this is what this company is doing in trying to collect this debt and this is completely unacceptable and unethical. I 've done some research online and I have only come across negative reviews and complaints by other consumers who have been harassed and scammed by this company. They appear to be based in XXXX Washington and I have never lived in Washington in my life. I will not give my money to a company that I do not recognize and who will not tell me why they want me to pay them.

I spoke to Puget sound collections on multiple occasions and they said the 5 accounts that are showing XXXX balance and open have been settled and was promised to be removed and will be removed. They said the easiest way is to dispute and it should be removed with no problem being they are already at XXXX balance which means there is nothing owed and settled. I had a total of 11 accounts that reported at XXXX balance from Puget sound collections XXXX removed all and XXXX only removed 6 of the ones I disputed and left 5. I have disputed and requested for removal 5 times thru XXXX XXXX and they still will not remove. Puget sound collections agreed and promised removal so I would like the remaining 5 Puget sound collections accounts that is reporting at XXXX balance and open on XXXX to be removed like promised. I also have another account with Puget sound collections for {$76.00} that was paid in full on XX/XX/2019 that needs to be removed because it has been settled. I have called and also disputed thru XXXX customer service and they are rude and no follow up or they say there is nothing I can help you with. This company list everything that was on this letter on my credit report before my 30 days I can valid information. I send the register letter from the postal office these item is not my and I need the debt valid. I feel the company violated my rights under the FDCPA. The company got the company that I filed with consumerfiance department and they said We received the complainants dispute notice on XX/XX/. Our records indicate that the previous address we had on file as an incorrect mailing address on XXXX XXXX in XXXX. We only recently obtained updated address information from his estranged spouse and mailed the validation letter to the updated address. Since it appears the complainant did not receive previous communication, we will provide a deletion of accounts recently reported for 30 days. I have not received any letter from this company explain or etc and I notice that place everything back on credit report this month without validation letter.I am going to report this action back to Attorney General today to see if his office give me better answer on this matter.

Neither of these debts are mine - I am in excellent health and have been to neither of these doctors. Ever. I have disputed them many times with XXXX and XXXX ( they are not on my XXXX report ) and they always come back saying that XXXX XXXX XXXX verified that the debts are mine. So, I sent a certified letter to XXXX XXXX XXXX requesting that they either offer proof that the debts are mine or remove them from my credit reports and gave them 30 days to do so. Their 30 days were up on XX/XX/XXXX/17 and I did not receive a response, nor have I received one since. I then disputed the debts with XXXX and XXXX, explaining that I had sent a certified letter to XXXX XXXX XXXX and had not received a response within 30 days. I just received a response from XXXX stating that XXXX XXXX XXXX verified that the debts are indeed mine. No, they are not mine. They had 30 days to prove it, they did not, the debts are not mine, they need to be removed from my credit reports and XXXX XXXX XXXX needs to be punished for not following the law.

I am a XX/XX/XXXX that worked for aXX/XX/XXXX in XXXX called " XXXX XXXX XXXX '' in XXXX WA. They claimed that I owed them money for a Referral fee for a lead program that they owned which I did not use, the Business that I did which they want a referral fee for was unrelated to the Firm. I left the firm in XXXX and Unbeknownst to me the Firm sold to " XXXX XXXX '' and along with the sale they attached a debt stating that I owed them, I knew noting about this until seeing it on my credit report. The new company now claims to own the right to collect on this debt despite having any proof that I owe this debt. They do not have any evidence, contracts or proof of said debt but have managed to hand it over to a collection agency called " Puget Sound Collectors '' I hired a lawyer to get this information regarding this debt but the collection company ignored the lawyer. The debt is over {$35000.00} and is on my credit report. There is no proof I own this debt and none has ever been provided. A Judgment does not exist in any courts against me for this Debt, yet on my credit report they are calling it a Judgement. This is very frustrating and harming my credit.

The hospital has turned the medical bills over to a collection agency. I have tried to explain to them and the hospital that I am a XXXX XXXX and I am not responsible for the charges.. Also that I can not be sued or garnished because my only income is XXXX XXXXy XXXX and XXXX XXXX and it is exempt. I have also explained this to XXXX, XXXX and XXXX. I had to call the hospital and the hospital transfered me to XXXX XXXX XXXX who hired XXXX to handle their billing. I asked XXXX about what bill they are sending me to the collection and why I didn't receive any notification or invoice before my case was sending to collection, according to XXXX, XXXX customer representative, they couldn't get hold of me because they had my mailing address incorrectly. And they sent me to collection and can't not pull it back. Now I have 45 days to resolve the issue otherwise this will hit my credit record. For over 3 yrs and 3 months now. This company has been harassing me, as well as making a major delinquency red flag on my credit. XX/XX/2019. Started with the harassment phone calls, threatening. So I spent days, which than turned into weeks and months. Just trying to communicate with them civilly yet was tossed aside. Due to the fact that I went and got the proper medical documentation and proof of the account being fully paid off by my insurance. I mailed them in my documents of proof. They mailed it right back claiming it was the wrong address sent to. Durning those weeks and months. They were gradually adding on fees and lowering my score every single time tremendously. I than reached out to XXXX XXXX many times about how all of this is wrong. Every investigation has come back as the account is stil open and I owe. I reached out to my attorney generals office about this. They have told me that this company has been reported for their legally wrongful acts once or twice before. So it being over 3 yrs now Im over this fraud they are committing.

Puget Sounds Collect has been reporting 2 collections on my credit file that they do not belong to me. I requested via certified mail validations of the debt with a specific requirements which they did not meet and in response all they send was a letter with a bill in a positive balance and a copy of what it seems to be a bill with a positive balance from a medical facility which is clearly a violation of the HIPPA Law, I also send them a letter certified mail to stop harassing me or cease and desist and they continue to report the 2 collections accounts on my credit file, I noticed they are violating federal laws 15usc1692b ( 5 ) and 1692f ( 8 ) by sending me mail with their company logos and balances stating they attempting to collect a debt which is another federal violation 15usc1692b ( 2 ). they are currently sharing my information illegally by reporting it to the CRA 's because, I never gave consent for any of my personal information to be shared nor this ALLGED debt belongs to me. The amount of the alleged debts are {$740.00} and {$140.00}

I called the collection agency and when attempting to work out a deal with them, they refused to accept an offer less than the principal balance and directed me to contact the originator of the debt to work out a settlement offer. Upon contacting the originator they said that they don't develop those sort of deals, but that I could pay the balance owed. I paid the balance owed to the originator. On XX/XX/2019 I saw the collection balance increased and called both offices to see what the issue was. For the originator, I am in good standing, and they sent the notice of payment to the collections office. Upon calling the collections office they accused me of avoiding paying the interest due to their office. I was misinformed by their office as to what my options actually are and the person I talked to on the phone was unhelpful and accusatory and threatened that the interest would not be written off despite me fulfilling my obligation to the originator, whom I believe still owned the debt.

As of XX/XX/23 the amount owed to " Puget Sound Collections Inc '' is a total of {$3300.00} after interest. However, I never received any type of notice from the original biller that I had a bill that was owed, nor from the collection agency. I had medical coverage at the time of service, XXXX, in fact. I have proof of coverage. Had I actually received notice in a timely manner ( within 2 years from DOS ), I could of had this resolved with the original biller by providing proof of coverage. This is the most recent information on this collection due a recent dispute filed with the three credit bureaus. I will continue to dispute this as I am not responsible for it, this would have, absolutely been paid and taken care of had the original biller and the collections agency done proper due diligence in reaching me to get this taken care of. I have proof that the hospital bills were paid, which is the same time that these collections are showing for DOS.

In XXXX i entered into a consumer transaction with XXXX XXXX and it looks like now Puget Sound Collections has taken over the account. The big problem that i have is that i noticed this collection company pop up on my credit report, BUT i have never heard from them before and i don't know who it is. I don't know what the {$1800.00} charges are all for, what interest or fees are associated with that amount or anything. I was never given the right to dispute this account, and they haven't sent me any of the documents that i requested. I'm not refusing to pay but this account is inaccurate and never was properly validated for my consumer rights. I think Puget Sound is in violation of my consumer rights by illegally reporting this debt and hurting my credit and causing me stress. I sent them a letter over 30 days ago ( XX/XX/XXXX ) asking for ANY information that they had about this account and i'm being ignored. This is really messing me up.

They like I have 30 days to respond to me. They did not, I call them they do not respond, they are very rude. These are all medical and most of the medical work with my insurance company and make agreements to take certain dollars for the procedures. ANY AND ALL PUGET SOUND COLLECTIONS SHOULD BE REMOVED. PSC has not sent me my HIPAA release that I have signed. They also haven't successfully validated the services I supposedly received. Instead, they told me to call the hospital I supposedly received services at. Unfortunately, I am unaware of a debt that I owe to said hospital. It is Puget Sound Collections responsibility to validate the information they are reporting on my credit report, not mine. I intend to follow suit if they can not provide my HIPAA release and actually the following information within 30 days : 1. Provide my signed and dated HIPAA release stating that 3rd parties may collect on my supposed debt. 2. Provide proof that I received a service. 3. Provide services I am being charged for. 4. What the cost of each service is that I am being charged for. This company claims they have validated the debt, but they have not. They sent me basic information that can not be proved as mine.

I have had no mail, no phone calls, nothing from anybody. On XXXX is was sent a collection letter notifying me that I owed Thousands of dollars plus an ongoing interest charge. I was not even given a chance to pay for this bill, if it is even mine. PSC Inc. has marked my credit and started charging me Hundreds of dollars in interest before I even knew I had a bill. This is my second complaint against Puget Sound Collections. The attached judgement was filed on XX/XX/13, which is the past seven year mark for collections. This company can not collect this amount and it can not be reported to any credit bureau due to the age of the debt. In review my XXXX report ; I found this debt was from a eviction from a private landlord, i.e. Original Creditor : XXXX XXXX XXXX that was over 7 years ago ; it went to court in Washington State and there was a judgement ; to which was never resolved due to major discrepancies with this landlord that was not following laws. On XX/XX/2019 the original creditor XXXX XXXX XXXX sent this uncollectable debt to PUGET SOUND COLLECTIONS. This can not be collected, the original creditor knows that and the collection agency should know that.

I asked for verification of a debt that appeared on my credit report. To validate the debt, I simply asked the collector to send me my signed medical documents, HIPAA release, and an itemized list showing what the debt was for. They did not send me any methods of validation, only the date of the alleged hospital visit. Per the FDCPA, this needs to be removed unless they can furnish the requested documentation within 30 days.
