This information does not relate to any transactions I made. I did not authorize anyone to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim files an affidavit it shall be honored by all credit bureaus and all fraudulent information must be blocked within 4 days and proper notification shall be given to all furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with The XXXX XXXX XXXX, Consumer Financial Protection Bureau, and Attorney General with legal action of your companys unlawful collection practices, misrepresentation of reporting inaccurate debts, and noncompliance to adhere to laws enacted to help who are victims of identity-theft. I request that an extend fraud alert be placed in my file explaining that fraudulent applications may be submitted in my name. Do not extend credit without first contacting me personally and verifying all applications.
The below-listed accounts by the Original Creditors, who have reported these accounts within my credit reports, were not opened by me. I also did not give my authorization for anyone else to open these accounts. Someone else of who did these that I did not know.
Policy states.
According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy.
( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller.
Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts.
Finally, you are reporting the INQUIRIES that are the result of the same type of fraudulent activities.
It must be noted that I did not apply for credit accounts with the below-listed companies and creditors, some of these are the result of attempts to open accounts, but the companies did not approve the accounts. Therefore, these inquiries must be deleted from within my credit file. You can not delete the fraudulently opened accounts without also deleting the fraudulent inquiries.
It should be noted that an inquiry that was not made by me is not a factual record, just as a fraudulently opened account is not a factual record and not referred to the creditor to be disputed directly with them. The credit reporting agencies have attempted to address an inquiry as a factual record of file access, but if it was done through fraudulent means, it is not a factual record. As a consumer I should not be required to dispute each fraudulent inquiry directly with each company. These inquiries are the result of fraudulent activity and therefore, inaccurate in which the credit reporting agencies or the creditors can not verify, meaning that these inquiries are unverifiable information and must be deleted from out go my credit files.

I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted.
According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity-theft.
1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from an alleged identity-theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer that the information relating to the transaction by the consumer.
I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted.

Received a letter in XX/XX/XXXX that a medical bill was given to OAC Collection Specialists ( XXXX, WI ). On XX/XX/XXXX, I made an online payment of {$27.00} in an effort to pay down the debt which was {$270.00}.
On XX/XX/XXXX, I scheduled a payment plan and agreed to have a monthly payment taken from my checking account every month on the XXXX until the debt was paid off. The reason why I could not pay this medical bill is because I had over {$12000.00} in medical bills from XX/XX/XXXX through XX/XX/XXXX - the result of two surgeries and rehabiliatation costs from the surgeries. During this time, my 20 year career in the XXXX XXXX was eliminated due to Covid. I was trying desperately to make ends meet.
The payments were made automatically in XXXX, XXXX and XXXX and I also have the next one scheduled for tomorrow, XX/XX/XXXX. On XX/XX/XXXX, I reeceived a credit card notification that I had a change in my credit score. My score was lowered by 30 points. When I researched why this would have happened, I saw that OAC Collection Specialists reported my account as " delinquent '' even though I had a scheduled payment planned arranged and had been faithfully paying this since setting up the plan.
When I called and finally spoke to a XXXX XXXX, she said that they could report that this debt is owed until it is paid in full. None of the documents that I printed out with the payment plan confirmation pages state that the agency will report negatively to the credit bureau REGARDLESS if you are currently making payments - the purpose of creating this plan was to, in good faith, pay this debt. This is a deceiving scheme that is now impacting my ability to obtain full-time employment and to re-finance my home mortgage - over a medical bill. Companies that I have applied to require you to agree to a background check and credit check. I now have a negative mark on my credit report that the company will not remove. As a matter of fact, to add to insult, Ms. XXXX stated " well, this debt is from XX/XX/XXXX, it isn't like it was new. '' Ms. XXXX is lucky to be a healthy, able-bodied individual that is gainfully employed. It is unforunate that her origanization preys on people that can not afford to pay their medical bills in one lump sum. If I was told that my payment plan meant nothing, than I would have borrowed money from a relative or friend so that I could afford to pay this so it does not impact my credit and my ability to refiniance and to find full-time employment. I asked Ms. XXXX where it states that she would report to the credit bureaus even with a payment plan and place and she said that she does not have that on the letter sent to me or the payment plan confirmation page.
This is wrong - it is deceiving, it is unethical, it is abusive and I need help removing this from my credit report.

I am writing to alert the Consumer Financial Protection Bureau of a collections company that is violating my rights and not using proper communication methods when showing up on my credit report. I need the CFPB office to please look into my case. This account has just popped up on my credit report without ever communicating to me. There was no written letter to me or calls to me so I can explain that they have the wrong person. They have recently just popped up on my credit report and I am not familiar with how they got my information or how they have my social security number. I do not owe this company and I Right now I am getting an alert on my credit karma and credit monitoring service saying my credit score has dropped tremendously. I can furnish the images of this alert showing the collections company. They are hurting me financially and this is a really big issue. To my understanding they must be compliant with laws and regulations that say they must write to the consumer within days of the first communication. They are using my credit to communicate with me yet they NEVER wrote to me before. If they don't write to me then how could they send me a right to dispute or explain my rights? Please review the original paperwork for this account. I request they send me a copy of this information to my address. I also request the original contract showing my signature long with all other pages of the original agreement. I am suffering damages because of this error. I have been a victim of ID theft before and I monitor my financial affairs very closely. To me this seems like an instance of debt parking that I read about on the FTC website. The Federal Trade Commission has taken action against debt collection companies that allegedly placed bogus or highly questionable debts onto consumers credit reports to coerce them to pay the debts. They label this as DEBT PARKING. The Federal Trade Commission recently took action against a Missouri collection company and its owners, alleging that they collected more than {$24.00} million from consumers, largely by placing bogus or highly questionable debts on their credit reports. The defendants used this illegal debt parking to coerce people to pay debts they didnt owe or didnt recognize, XXXX XXXX XXXX director of the F.T.C.s bureau of consumer protection , said in prepared remarks about the agencys settlement with the company, XXXX XXXX XXXX XXXX I DO NOT HAVE ANY BUSINESS RELATIONSHIP WITH THIS COMPANY and I am concerned they have unethicality acquired my information and they are acting unethically with their debt collections. There is a process in law that protects consumers from these illegal actions. This account is lowering my credit score and if I am declined for anything, I will seek financial remedy by talking to an fdcpa lawyer.

I received a notice from Oliver Adjustment Company , Inc that I owed a XXXX XXXX XXXX SC {$15.00}. I have never been to this location, I have never had an appointment there, I have looked through all of my appointments on my health care app and none are tied to this company. Oliver Adjustment Company states on their website that they are for " healthcare professionals '' and " pro fee healthcare collection specialists, '' but do not at all appear to be legitimate or professional.
Their statement says they are located in the state, but their website domain ( .bz ) looks to be from another country and the site itself does not feel secure or professional. Plus the po box they want me to send payment to is on the other side of the state compared to their office locations if you XXXX the company. I tried to call to dispute my claim during their regular business hours, and they would not answer the call. The call booted me off and asked for a voicemail with more than 30minutes left to go for their business day, after having waited to speak with a representative. Their only other options to dispute is to pay to send a letter back, or use their website which authorizes them to use my information that I give them freely.
If Oliver Adjustment Company was a legitimate business, then I would assume they would help collect for other reputable businesses. Instead, they claim people have debt for companies like XXXXXXXX XXXX XXXX whose reviews are riddled with the same issue I have ; illegitimate billing practices for procedures that were never rendered or were already paid for.
The original bill was supposedly from XX/XX/2022. I have no appointments during that time period. All of my healthcare procedures for the year of XXXX were paid for. My healthcare app states all of my bills were paid, and my health insurance also has the matching information including my out of pocket maximum being reached so at the very least, if they were a third party, XXXX XXXXXXXX XXXXXXXX would have had to bill my insurance or the healthcare provider that asked for their services because I have already paid for all of my services. I have received no bills or late charges for any of my medical procedures. My credit report does not state that I have any accounts with XXXX XXXXXXXX XXXX
I invite you to look into either company and their reviews and see how many claims are made against them for being fraudulent or harassing for debt that should not exist whether because it was paid or because the procedure never happened.

I have maintain careful records and for your benefit, and as a gesture of goodwill final attempt to have you clear up this matter. You have 15 days to cure. The following on the letters needs to verified : In 2000, the FTC held that the FCRA barred a debt buyer from " verifying a tradeline disputed by a consumer when it didn't posses the original documents. XXXX XXXX XXXX XXXX called OAC. I disputed these on XXXX XXXX, they were removed, and then put back up again numerous times. How many times do I have to keep disputing them. I have XXXX and XXXX, my insurance XXXX covers the remaining balance on everything, so if it's going to collections, it's because they're not doing their jobs and billing XXXX. They have all my proper information, and I even confirmed it with their billing department over the phone. I finally got my credit to a good number, and these XXXX keeps billing me these charges that insurance has paid for already, and there should be a log of that. Isn't it illegal to do this to someone's credit? We're in 2021 now, I am on XXXX and have been for XXXX years.. I collect XXXX, and don't work, I make about {$500.00} a month, isn't there some sort of relief out there? We shouldn't even be having this conversation, because these were already paid off by insurance. Some of these date back to XXXX how is this possible?
Recently I reached out to the company on their site, but got no reply back, which I honestly wasn't expecting one. OAC seems to do this to a lot of people from what I've read in XXXX them for their last cent, and going after people who actually can't afford it.. harassing them.
I don't have anything that says they were paid for, unfortunately, but my insurance should have that.. most of it is done digitally now. I think the problem is that XXXX XXXX did not bill my XXXX, instead billed me, even though I told them to bill insurance, they cover whatever XXXX doesn't, and that's why it's in collections. However, I told collections this, and they are not doing their job either in getting the money from insurance.. I'm sure you understand what I'm saying. So I can't have documentation of something when people aren't doing their jobs and billing the right people.

I spoke with XXXX at this company back on XX/XX/XXXX around XXXX regarding a medical bill in the amount of {$390.00} for my son. After speaking with XXXX, she advised me that she will reach out to XXXX regarding submitting the claim to be paid and she also advised me that she will submit the information to the insurance and that I should follow back up in 30 days regarding the status. I followed up and was advised that they have not received a response yet. After a couple of months I received another letter on XX/XX/XXXX from OAC which is the collection agency again regarding the bill. I then reach back out to the collection agency OAC trying to get the status on the claim and was advised by a representative that the claim had not been submitted to medicaid and that they would submit it. I have been reaching out to this company for almost a year offering my assistance in anyway possible while they kept informing me that they were submitting the bill to medicaid and come to find out, they never did. I 've been misled as well as lied to for months and all I asked of the staff at OAC was to submit the claim to XXXX so that they could pay it before the timeframe expired which was a year from the date of service which was XX/XX/XXXX. When I spoke with the supervisor XXXX on XX/XX/XXXX I expressed how dissatisfied I was with how the staff handled my account and he did not seem to care nor did he try to offer a resolution. The way the office handled my file was unjust and as a result of their negligence regarding submitting my claim to XXXX in a timely manner, XXXX will not pat the claim because a year has passed from the date of service. the names of the other individuals I spoke to in that office were : XXXX, XXXX, and XXXX.

I received several letters in the mail inquiring on medical debt owed. There was not enough information on the statements verifying that these were accurate. Attached are the reference numbers and amounts owed with dates. I started a new job on XX/XX/XXXX and there was a lapse in my insurance. I found out I was XXXX and got several XXXXXXXX XXXX done, but those were paid. I had to get emergency XXXX on XX/XX/XXXX and I believe the {$510.00} bill is part of that but I am set up with an automatic debit of {$44.00} for that. In addition, I informed the collection agency that I was in the process of filing for Chapter XXXX bankruptcy. I filed for Chapter XXXX bankruptcy on XX/XX/XXXX. The Bankruptcy Court through my attorney notified the original creditor, XXXX XXXX on XX/XX/XXXX of my Chapter XXXX filing ( this was a little over 4 months after I received my first bill from the original creditor ). My bankruptcy was discharged in XX/XX/XXXX. On XX/XX/XXXX, the collection agency OAC unfairly added two collection accounts to my XXXX credit report for the first time ever. The collections are being added literally 7 months after the accounts were paid the bankruptcy protection. This is an unfair and deceptive tactic by OAC to ruin my credit rating. The accounts show on my credit file as closed paid accounts and included in bankruptcy. They appear as if they were on there the entire time, but were just added last week on XX/XX/XXXX.

After noticing OLIVER ADJUSTMENT COMPANY on my consumer report I contacted them and asked for validation of this alleged debt. When my request went unheard I contact the CFPB because they refused to give me the information I requested and I now see why. I received in the mail the alleged itemized statement showing literally nothing but a falsified piece of paper. Good day, I am filing a complaint against Oac, XXXX XXXX XXXX, and XXXX. I received a derogatory remark on my credit of owing {$88.00} and this has put me in collections with OAC. I refused. I then spoke to a person who identified herself as XXXX. She refused to give me her full name, claiming she was legally prevented from doing so unless I gave my SSN or DOB. OAC has been inaccurately reporting a paid off service on my credit report, and furnishing this information to credit bureaus since XXXX of 2019. I informed OAC that this information was incorrect, and asked them to verify with XXXX XXXX XXXX, the original creditor, that the information that they were reporting was accurate. The original service was paid by IDPH through XXXX insurance at the time of service. Attached is proof of coverage that was also sent to OAC. Looking at my credit report back in XX/XX/XXXX I saw accounts in collections and I never gave any company the right to share my information whatsoever. One for {$150.00} which is listed on my credit report as account XXXX. Healthcare provider is attempting to collect an amount from me that has already been paid by my insurance company. The amount billed conflicts with the Explanation of Benefits. This is the second time within two months that I have received notices from a collection agency for medical bills for XXXX XXXX XXXX. In both cases I was never billed by XXXX XXXX XXXX for their services. I replied to the first notice and paid the amount owed. In my reply I suggested that XXXX XXXX XXXX amend their billing process to ensure the customer was billed and provide a second notice before submitting to a collection agency.
My advice was ignored and I have now received a second bill with the same circumstances. The practice by XXXX XXXX XXXX of failing to bill the customer and sending the bill to a collection agency is unfair. I am now very concerned that my credit rating may be adversely affected for a bill I never received.

I requested information regarding the medical provider/facility to which I allegedly owe the debt and a detailed breakdown of the fees that I owe. As of today, I 'm still waiting on a response. I believe that this debt is not mine and the fact tha t OAC has never provided any verification of debt, further proves this for me. FCRA states that I have the right to demand that they provide documentation to verify debt and if they are unable to do, it is illega l for OAC to continu e reporting this debt.
