Hello, Ive been charge the total of {$410.00} from services of NorthShore XXXX XXXX XXXX on XX/XX/. On this day I had to go XXXX Room with intolerable levels of hardship, due to an accident during my first day of work. The companys insurance was supposed to provide help. I never heard back from them and I dont have the money to pay for it. Im still injured.

On my credit reports via XXXX XXXX they have ONE DEBT " Northshore Financial '' listed three times. One ( # XXXX ) as " Paid and Closed '' and two others ( # XXXX & # XXXX ) as late. That account was only one account and was also discharged in bankruptcy. That company doesnt even exist anymore so I am not sure who is reporting it.
