RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT XXXX XXXX data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX XXXX XXXX XXXX and XXXX. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with XXXX XXXX XXXX XXXX Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C . 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. 1. NATIONWIDE CREDIT & CO bal. {$8700.00} I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from alleged identity theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer that the information relating to the transaction by the consumer. I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. I went to XXXX hospital around XX/XX/XXXX. After receiving treatment and leaving the hospital. I provided the hospital with my medical insurance information. After about a full year later I received a letter attempting to collect a debt from said emergency room visit. Attached to the letter was a voucher where you input your insurance information and send it to the hospital for manual processing. I completed the form and submitted it to the hospital ( on TWO separate occasions via USPS business reply mail ) About 12 months ago was the last time I sent a written business reply including my health insurance policy for the billing dept to bill my health insurance so they have had AMPLE time to process the request. Their department is now criminally negligent. I called the XXXX XXXX XXXX ( in two separate quarters of XXXX, Q2 & Q3 ) and provided them VERBALLY in addition to, in writing, my insurance information, TWICE, over the phone with a customer service representative who stated that I was on a recorded line and she was representing the credit department at XXXX XXXX XXXX. I was verbally informed, TWICE, that the insurance company would be billed for the visit to the Emergency room. After contacting my insurance company to identify if there was a clerical error in paying out to the hospital I was informed that the hospital NEVER FILED AN INSURANCE CLAIM with my carrier XXXX insurance. Nor did I authorize a company name XXXX XXXX XXXX to operate on my physical person. I authorized XXXX XXXX to provide me with emergency care after an incident and they have clearly failed me as they must be failing other patients in a similar fashion. After they attempted last month to file this collection account I filed a separate CFPB complaint with the original creditor that they attempted to fool into pursuing this debt. The previous debt collector immediately dropped the case. I also filed complaints at the state level, and I filed consumer complaints on XXXX and social media. The hospital has not reached out to me with a comment or to resolve the issue. I retain audio recordings of my conversations with customer service reps where I provided them my insurance information for the final time.

XX/XX/XXXX I had a medical procedure completed at XXXX XXXX in XXXX, IL ( part of XXXX and recently acquired by XXXX XXXX ). At the time I had the procedure done, I thought I had insurance. Since I had not received a bill from the hospital, I did not know there was anything still outstanding. In XX/XX/XXXX ( over 2 years later ), XXXX sent me a bill for the first time stating that XXXX XXXX XXXX XXXX ( XXXX ) had paid the bill, then took the money back on XX/XX/XXXX. I immediately called XXXX and filled a hardship application, which was approved for 100 % financial assistance on XX/XX/XXXX. I also filed a complaint with the Illinois Department of Insurance ( IDOI ) against XXXX. My claim against XXXX was reviewed, and they repaid the insurance portion on XX/XX/XXXX. I immediately contacted XXXX XXXX to inform them that my efforts had resulted in them recouping the {$4400.00} back from XXXX. XXXX XXXX acknowledged that they received the XXXX payment. On XX/XX/XXXX, XXXX XXXX issued me an invoice for the 20 % co-pay that XXXX did not cover {$240.00}, even though the entire amount of {$4700.00} had previously been written off. On XX/XX/XXXX, I received a collection notice from Nationwide Credit and Collections for the same {$240.00} - even though I have a written letter from XXXX XXXX stating that I owed nothing. The financial assistance letter states it is good for XXXX months, so clearly it is still in force. In addition, my account was sent to collections apparently the same day I was sent the invoice ; and I was never even given an opportunity to resolve the issue with XXXX XXXX before they sent my account to collections. Also on XX/XX/XXXX I attempted to contact a manager at XXXX XXXX / XXXX XXXX, because clearly there is a procedural problem with their billing department. When I called the billing number XXXX, I was told that I could not have that information. I have tried searching online for the information and there is no clear way to contact anyone at the executive level to bring this to their attention.

Today, XX/XX/XXXX, I received an email from my bank that something had gone to collections. I am unaware of any bills I have not paid, so I filed a report about it. On XXXX, XXXX, I received a credit alert that there had been medical collections placed on my credit report. I'm closing on a home XX/XX/XXXX and these medical collections has hauled my closing. On XX/XX/XXXX, I called Nationwide and spoke with XXXX I asked about the collections and explained I had not received notice in the mail. I explained that I need to have these removed because I was in the process of closing on a home. I asked about date of service she stated in XX/XX/XXXX. I did not argue about the bill because I needed these to be removed so that I could proceed with my closing. I asked XXXX if I paid would these collections be removed, she stated that they would be. I asked if she would be able to fax me proof of payment so that I could submit to the loan officer. She stated that should could only give me proof if I come into the office in XXXX XXXX, Illinois and made the payment. I then reached out to XXXX on XX/XX/XXXX and explained that I would be coming into the office to pay. Once I arrived I spoke with XXXX, the receptionist. I explained that I need to pay collections and I needed itemized bill from hospital showing what I owed. She stated that the lady that handled that account is out of town and would return next week, but XXXX would fax over the itemized bill and the collections would be removed the XX/XX/XXXX which is when Nationwide ran information to credit bureaus to be removed. On XX/XX/XXXX I received another alert that Nationwide had placed the same collections again on my credit reports after I had paid the accounts off. Now I can't move forward with closing, in fact the deal may fall through because of these four collections which two of the collections are duplicate? The way that Nationwide had damaged my credit is unfair and it could cost my dream of being a first-time home owner to be lost.

I was contacted by Nationwide Credit in XX/XX/2019 by phone. Nationwide credit stated that I owed a debt and that a debt was owed on behalf of my ward. The debt amount was not recognized by me. I requested validation of the debt. I was informed that I needed to put this request in writting. I verbally told them that I would be doing that immediately, and asked them not to further contact me by phone. I immediately wrote to the company and put this request in writing no more than a week after receiving that call. I did not receive anything in the mail. After that initial call, I continued to receive calls and voice messages at least once a day over a two week period. I usually let these go to voice mail. On some occassions I answered and again stated that I was questioning the debt and had requested validation of the original debt, which I still had not received. The agents each time stated that they were making notes in the account and that the calls would stop while that information was being sent as I requested. The calls stopped for a little while, but then continued again. I also received mailed correspondence about the debt, but never received validation or information about the original debt. Meanwhile I was told that the debt was being reported on my credit. In XX/XX/2019, I again put it in writing and stated that this was now my second written request for validation of the debt. Again I received nothing. It is now XX/XX/XXXX and I am still receiving calls from this company. I have now written for the third time for validation of the debt, and have not received anything other then them sending out their own one page demand for payment with no validation of the debt.

XXXX has been harassing me by inaccurately trying to collect debt when I never signed up with them for any type of services. I called XXXX a while back to explain that they have the wrong person for the debt and i wish to not be contacted but XXXX continued to pursue me for debt that I don't know and has even turned it over to a third party Nationwide Credit , Inc in the amount of {$130.00}. XXXX and Nationwide Credit , Inc have violated the law by pursuing me when I never signed any documents or spoke to someone regarding the address I stay at. Furthermore I am not the lease holder nor do I hold a mortgage for the address they have on file so I don't know why they would be billing me especially since I never signed any documents nor did I have a written or verbal agreement. This is a violation of my rights from both of the companies for harrassing me and disrupting my right to privacy. This is a violation of my rights 15 US Code 1681 A4 There is a need to ensure that consumer reporting agencies exercise their grave responsibilities with fairness, impartiality, and a respect for the consumers right to privacy. 15 US Code 1 A ) the consumer authorizes the agency to provide such report to such person. 15 US code 1692d ( 2 ) The use of obscene or profane language or language the natural consequence of which is to abuse the hearer or reader.

XXXX XXXX XXXX XXXX ( visit on XX/XX/23 ) billed me for {$28.00} on XX/XX/23 which was paid on XX/XX/23. They then billed me on XX/XX/23 for {$77.00} but the total charges are {$430.00} and the total payments are {$430.00} which equals XXXX so how can I owe {$77.00}? You can not bill for an amount due when debits and credits net to XXXX. Called XXXX on XX/XX/23 they assured me my account was XXXX and sent me an email. I got another bill dated XX/XX/23 with the same amount due. I called on XX/XX/23 and asked for a supervisor and was told no they could help me and said the account would be reviewed by special claims. Then on XX/XX/23 I received a collection letter from NCC Nationwide. I contacted them and they noted the account regarding the dispute. I called XXXX again on XX/XX/23 and asked again for a supervisor and was told that they didn't have any. That has to be a lie. Call Centers have supervisors. I was told the account was fixed and pulled back from collection. I asked for a new bill showing the account had a XXXX due and was told I would get an email with the bill. I have not gotten an email and I also got a collection call. I consider what they are doing to be called surprise billing and I would think this is fraudulent. I want to know why this has not been caught in an audit. If this has happened to me it has to others.

I have sent you guys a letter dated XX/XX/2022 & still have not got a response. XXXX Nation Wide used my identity and fraudulently opened this account and reported these derogatory accounts to all credit reporting agencies. They've violated my rights pursuant to FDCPA section 807 by false and misleading reporting. I've also been violated of my rights pursuant to 15 USC 1692D ( 4 ) just for parking the debt on my credit report trying to get me to pay it. I do not have any contract with my signature on it with this company. I've never done any business with XXXX nor have I ever given them any of my information, so this is also a violation of my rights pursuant to 15 USC 1681A ( Q ) ( 3 ). I also never gave this company consent to report this alleged owed debt to my credit report, so this is another violation of my rights as a consumer pursuant to 15 USC 1692c ( b ). This alleged owed debt is furnished on my consumer report in a positive amount. It is impossible to pay on a positive balance. This is false and misleading and a violation of 15 USC 1692e ( 2 ) ( A ). Because this debt does not belong to me, nor am I obligated to pay it pursuant to 18 USC 8, I demand that this company cease and desist all communication and fraudulent activity reporting ASAP. Nationwide Credit called my cell phone on XXXX and asked for my mother, XXXX XXXX. I explained that she is XXXX XXXX XXXX, very ill and in hospice care and she does not live here, and her only income was social security that she does not even get anymore. I explained that I am her daughter, this is my personal cell phone number and please do not call me again. On XXXX XXXX called again, I explained again that I am not XXXX XXXX and that is my mom and she does not live here, very ill, etc. the representative was very rude and called me a XXXX as she disconnected the line. On XXXX Nationwide called again, I explained for the XXXX time that I am not XXXX XXXX, this is my personal cell phone. I also told them that is this is the XXXX time that you have called asking for XXXX XXXX and to not call me anymore. Approximately XXXX minutes later I called Nationwide Credit to make sure that they remove my number from there database. They said I am not in there system, I explained I know I am not in your system but my mother XXXX XXXX is and she is who keep calling my cell phone for. Nationwide said they can not remove me as I am not in there system. I am tired of Nationwide harassing me. It is difficult enough to face the upcoming passing of my beloved mother, I should not be subjected to XXXX from Nationwide Credit.

I keep getting auto dial phone calls that are designed to go straight to voicemail and can not be picked up from a company called Nationwide Credit. I have spoken to them repeatedly denying any debt as I have never received an invoice, they say I have a debt, but I have never received anything nor have i ever been to the place they say I owe. I have repeatedly stated that Until I see an Invoice I don't owe anything, They say that's not correct and THEY have determined I owe this debt, Now they continue to call after they have been told to stop and they are sending letters to my home address. When I try to contact directly to say they have to stop calling they reply they will keep chasing me for a debt I don't owe, they don't listen and cut me off mid sentence It is continued harassment and they should be held accountable. I have NEVER received any Invoice nor have I ever been to the place that Nationwide Credit say I owe, I DONT owe this debt that Nationwide Credit keeps Harassing me for. The last XXXX I spoke to was XXXX, After telling them to stop calling yet again I received two calls to my voicemail while I wrote this complaint. They need to be held accountable for this Harassment.

I was seen At XXXX XXXX XXXX and gave all the proper insurance information, address, numbers, etc. The billing department wrote my address incorrectly and I never received a bill. I was put into collections and XXXX XXXX XXXX called to collect the debt. It was then I learned that : 1. I owed the debt 2. My address was written incorrectly and i had never received a bill from the doctor 's office, nor from XXXX XXXX XXXX I asked the girl on the phone to send me a copy of the bill, so I can pay it- which she did not do - she sent me a copy of whatever she was looking at with no address to pay the bill. She also changed my billing address to the correct address. At that time I took the paper she sent to the XXXX XXXX XXXX and paid the bill. I was told I would absolutely be taken out of collections by the office manager, as well as the billing manager, as well as Nationwide Credit. A week later, Nationwide is still trying to collect money for a debt that should have never been mine, and has since been paid. I have left another message for them and have not heard back from them yet. I do not want this to affect my credit score, which is in the top tier.

I sent a letter to Nationwide Credit & Collection , Inc., located in XXXX XXXX, IL dated XX/XX/2020. I requested for them to allow me to verify this account. I have not received anything from them. However, I received a phone call asking for this account. This person was not a very nice person. She wanted for me to verify my birthdate before she was able to speak to me. I have had these accounts on my credit that I have been trying to get off and they have not been getting taken off for some reason.
