I am receiving collections phone calls when I have a {$10000.00} credit in my name. I should owe no money to any medical as I received {$10000.00} in an account for Co-pay assistance for XXXX and reached 100 % on my health insurance early in XXXX. XXXX XXXX was enrolled in the XXXX XXXX-Pay assistance program ( XXXXXXXX XXXX XXXXXXXX ) All monthly bills were sent XX/XX/XXXX according to XXXX at XXXX XX/XX/XXXX {$10000.00} was placed in a fund with my name. Contacts included XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX at XXXX assured me month after month that her records showed I was paid up 100 % My husband and I have been receiving harassing phone calls repeatedly from an anonymous numbers. We do not pick up unknown or anonymous numbers. We live in Alaska so often these calls have been as early as XXXX in the morning disrupting our sleep, repeatedly throughout the week. These sleep disrupting repeat phone calls have added to our high stress level. Several times when we did pick up the call on the old phone with no phone id we asked who was calling and learned it was from a company called Miramed, a collection company. Miramed will not discuss the account except to arrange payment, which I do not owe. No one will transfer me to a manager or other person that has any decision making authority. No contact phone numbers or address for additional help was given. A little history, on XX/XX/XXXX I wrote an email to XXXX requesting assistance. We did not hear back. Saturday XXXX XXXX. I picked up the phone upstairs, it is a landline that does not show who called. The toll free number was said so fast you can not write the number down. I pressed one and asked what the number was XXXX XXXX XXXX, what company was calling? MiraMed and to please spell it. What does this company do? Accounts receivable Management in XXXX, Illinois. I do not owe this alleged debt. I had in XXXX a {$10000.00} XXXX credit on account for the XXXX XXXX-Pay assistance XXXX and reached 100 % coverage on health insurance early XXXX due to XXXXXXXX XXXX at the hospital XXXX XXXX. The contact person is XXXX XXXX XXXX XXXX and contact information is XXXX XXXX XXXX. The XXXX XXXX-Pay Assistance program XXXX XXXX ( XXXX ) I have provided this information to employees several times starting XX/XX/XXXX. They refuse to accept the information, and refuse to provide contact information to reach someone who is empowered to act on this information. XXXX at XXXX assured me month after month that I was covered 100 %. I received a form letter regarding an alleged debt. I responded immediately in XXXX in writing disputing this inaccurate, erroneous debt. I responded with a letter disputing the debt and provided information relating to a {$10000.00} XXXX on account with Co-pay assistance with XXXX on XX/XX/XXXX XXXX at XXXX XXXX XXXX XXXX XXXX XXXX. MiraMed did not respond. I never received verification of the debt from MiraMed. MiraMed has been and continues to be ; unprofessional and violate my rights under FDCA by illegally harassing me by calling before XXXX. I informed them I live in Alaska explain the time differences. I have asked repeatedly for them to stop doing so, and to stop calling multiple times in a row. The robo caller talks so fast as to be not understandable. I have been hung up on several times. I asked for a written statement from them. She refused. I am continually asked for personal information, which I will not provide. Messages are too fast to get contact information and the employees are not clear when speaking. Asking for repetitions elicits anger, hang ups, but never, clear information. Callers refused to give information about contacting MiraMed on several occasions. I advised callers several times that they were not understandable and to make contact by mail or provide me with information for contacting MiraMed by mail. Anonymous calls persist at XXXX repeatedly even on weekends. MiraMed blatantly violated my consumer rights and continues to do so. This alleged debt lacks veracity and verification. Please assist me in this matter. There should be no need for the use of an Accounts receivable management company as all medical bills should have been paid by a XXXX for deductible/copay & /or covered by XXXX XXXX insurance XXXX Any credit rating impacts should be rectified. Thank you for your support.

I was contacted by a collection agency, MiraMed about a colonoscopy that was performed by XXXX XXXX in XX/XX/2020. They had purchased what XXXX XXXX 's office sold to them as a bad debt. His charges for this service were {$1600.00} I have no dispute that these services were rendered, but I did inform MiraMed that XXXX XXXX 's receptionist demanded the {$1600.00} the day I showed up to get the results of a biopsy that was performed on a XXXX XXXX XXXX XXXX, and his office advised that they wouldn't give me the results of the biopsy until I paid them the {$1600.00}. It was the first I had heard of any denial by my insurance company, and since my last visit to XXXX XXXX 's office I had incurred hospital charges as well for that procedure, and if they had told me when they were denied initial visit charges, the problem could have been handled long before it became a {$1600.00} mess ( or potentially a {$10000.00} mess with possible hospital charges for all I knew at that point ). I immediately contacted my insurance company and they told me that I was fully covered for the procedure. In fact, they had already paid the hospital in full. My insurance company told me that XXXX XXXX 's office had been denied because they filed my insurance wrong. They filed it as the wrong XXXX type of insurance, and they couldn't pay the doctor 's office until they resubmitted the charges correctly. I contacted XXXX XXXX 's office to explain the problem. Now, in the weeks that followed, XXXX XXXX 's office in XXXX XXXX, Missouri refused to file my insurance correctly as well as refused to forward those biopsy results to my regular doctor 's office. All they have done, in so far as I am aware, is to sell this completely unnecessary debt to a debt collector. Now, when I told MiraMed this, they immediately contacted XXXX XXXX 's office with my dispute to the debt so that they could answer my concerns, and three days ago MiraMed contacted me with their answer. MiraMed has officially informed me that XXXX XXXX 's office did render me services ( something I have never denied ), and therefore, I owe MiraMed {$1600.00}. They did not address the fact that XXXX XXXX 's office never filed my insurance correctly or that I was fully covered for the XXXX as a routine screening procedure. All MiraMed did was to advise me that I should make arrangements to pay MiraMed the {$1600.00} in the next 10 days or they will " proceed '' whatever that means. I am NOT paying MiraMed XXXX cents let alone {$1600.00} under any circumstances. This is a matter of principle. XXXX XXXX 's office could have been paid in full more than a year ago. All they had to do was click the right sequence of numbers on a screen and punch send, but they chose to sell this debt to a collection agency instead. I don't know if XXXX XXXX 's billing worker was badly trained or if someone in that office is just too lazy to file my insurance, but I do know that it was never filed properly. The doctor 's office simply chose to not get paid, and in spite of this refusal, XXXX XXXX 's office ended up selling my debt to a company that spent my XXXX sending me badly veiled threats if I don't pay up now. Now, I don't know what you can or can not do for me, but I am going to fight. It is not right. It is not right for them to hit my credit report or write me or call me or threaten me over a debt that could have already been paid. This is not how we heal patients. This is how we stress people out. I mean it was enough that I never got the biopsy report, but this is ridiculous. There has got to be something I could do. I am not paying XXXX because they purchased this unnecessary debt. This bill should already be paid. My insurance company did not deny the charges for any other reason than an incorrect filing. All they ever had to do was file it correctly.

My EOB from my insurance company shows my hospital bill was covered and I only owe {$29.00}. After multiple phone calls over the past year between the insurance company and hospital, I have since mailed by certified mail and faxed my EOB and an appeal form from my insurance company to the hospital ( because they would not communicate with each other ), yet the hospital continues to claim they never received my last EOB and tell me I owe them {$4000.00}. The hospital and my insurance company will not communicate and I continue to be stuck in the middle with a very large wrong amount. Now the hospital has sent me to a collection company. The debt collector first contacted me through a text message which I assumed was a scam. After the XXXX text, I called them and was given an email address to send my proof and was told it would take them 2 days to process documents I send. So on XX/XX/XXXX, I emailed my EOB and appeal form from my insurance to the debt collectors showing insurance covered the hospital bill - no response. I then received my first letter from the debt collector dated XX/XX/XXXX ( which tells me they did not process the documents of proof I sent before responding ). This first letter I received was missing information they are required to include by law. They did not include : the amount owed ; that I can dispute the debt and if I don't dispute within 30 days the debt collector will assume the debt is valid, that if I dispute the debt in writing within 30 days the debt collector will provide verification of the debt, and that if I request the name and address of the original creditor within 30 days that information will be provided. On XX/XX/XXXX, I then emailed the debt collector the same documents again and verified they were received over the phone. I then paid to fax a copy of my EOB to the hospital along with the appeal form from my insurance company. On XX/XX/XXXX, I sent a dispute letter to the debt collector asking for the missing information I never received which included : the original contract with my signature, the creditor information, the payment history/itemized list, and proof of ownership. On XX/XX/XXXX, they mailed me the same letter as the first time still missing the information I requested along with a copy of my original contract containing my signature. On this original contract, it states the contract is valid for 1 year dated XX/XX/XXXX. Now on XX/XX/XXXX, I still have not received the following information from the debt collector : 1 ) an itemized list of the current amount of the debt that reflects interest, fees, payments and credits since a particular date ; 2 ) the current amount of debt when the notice was provided ; 3 ) information I can use to reply to the debt collector such as if I believe the debt is not mine or of the wrong amount ; 4 ) an end date for a 30-day period when I can dispute the debt According to the CFPB, this lack of information from the debt collector puts them in violation and invalidates the debt they are trying to collect. I have also filed a complaint with the No Surprise Act and Attorney General 's Office which is currently investigating my claim and proof of coverage by my insurance company showing I only owe {$29.00} and not the {$4000.00}

I am writing to alert the Consumer Financial Protection Bureau of a collections company that is violating my rights and not using proper communication methods when showing up on my credit report. I am pertaining to 2 MIRAMEDRG accounts that were previously reported in XXXX XXXX XXXX ACCOUNTS. The balance {$960.00} and {$68.00} was previously reported with XXXX XXXX XXXX ACCOUNTS in XX/XX/XXXX. After I disputed this account these were removed XX/XX/XXXX and replaced to MIRAMEDRG in the same month. These items are left reported in my XXXX credit report.I need the CFPB office to please look into my case. This account has just popped up on my credit report without ever communicating to me. There was no written letter to me or calls to me so I can explain that they have the wrong person. They have recently just popped up on my credit report and I am not familiar with how they got my information or how they have my social security number. I do not owe this company and I Right now I am getting an alert on my XXXX XXXX and credit monitoring service saying my credit score has dropped tremendously. I can furnish the images of this alert showing the collections company. They are hurting me financially and this is a really big issue. To my understanding they must be compliant with laws and regulations that say they must write the consumer within days of the first communication. They are using my credit to communicate with me yet they NEVER wrote me before.. If they don't write me then how could send me a right to dispute or explain my rights? Please review the original paperwork for this account. I request they send me a copy of this information to my address. I also request the original contract signed showing my signature long with all other pages of the original agreement. I am suffering damages because of this error. I have been a victim of ID theft before and I monitor my financial affairs very closely. To me this seems like an instance of debt parking that I read about on the FTC website. The Federal Trade Commission has taken action against debt collection companies that allegedly placed bogus or highly questionable debts onto consumers credit reports to coerce them to pay the debts. They label this as DEBT PARKING. The Federal Trade Commission recently took action against a Missouri collection company and its owners, alleging that they collected more than {$24.00} million from consumers, largely by placing bogus or highly questionable debts on their credit reports. The defendants used this illegal debt parking to coerce people to pay debts they didnt owe or didnt recognize, XXXX XXXX XXXX director of the XXXX bureau of consumer protection , said in prepared remarks about the agencys settlement with the company, XXXX XXXX XXXX XXXX I DO NOT HAVE ANY BUSINESS RELATIONSHIP WITH THIS COMPANY and I am concerned they have unethicality acquired my information and they are acting unethically with their debt collections. There is a process in law that protects consumers from these illegal actions. This account is lowering my credit score and if I am declined for anything, I will seek financial remedy by talking to an fdcpa lawyer. CFPB Complaint ExampleTOP

XXXX XXXX called me from MiraMed and did not give the mini miranda. She told me that I owed a debt in the amount of over but around {$410.00}. She verified my name, address, zip code, and XXXX. She asked me if I would be able to pay on the debt and I explained to her that I was not able to as of todays date being that I have been in and out of the hospital recovering from 3 XXXX XXXX within a 6month period, with my first XXXX back in XX/XX/XXXX, 2nd was in XX/XX/XXXX, and the XXXX on was in XX/XX/XXXX. I am going to have my 4th XXXX in XX/XX/XXXX. I also told XXXX XXXX that I was not aware of any medical debts that I owed because I have not been back to that address which she verified. I told her that Ive been in recovery from a different address. I also told her that I had insurance at the time and was under the impression that my insurance company paid for my Dr. visit. She became rude and wanted to put me off once I explained to her that I am not working at the time because my Dr wont release me to go back to work being that another XXXX XXXX is imminent. She continued to ignore me. I advised her that I am not refusing to pay but I dont have a job, I have no income and I would have to speak with someone to get help. She said that {$25.00} was the minimum payment so I advised that I would try to resolve it if she would notate that I would call back and make the payment in a couple of weeks. She got rude with me and said that she cant do and told me to call back once I could make a payment on the account. I advised that I needed some kind of future documentation and that I am not refusing to relsolve this debt but I had to get help being that I dont work and I was not aware of this debt being that I was insured. She literally ignored, I told her she was not listening to me, then I asked to speak to her supervisor. She got rude saying call back once I can pay. Again I asked to speak to her supervisor I asked for her name earlier. I aso wanted her to know I was not refusing to pay on the debt, I just needed more information and time to speak with someone to help me pay on the debt. As I was talking she hung up on me. I called back spoke with another representative and asked for a supervisor. She told me her name but refused to give me the name of the rep by the name of XXXX XXXX who hung up on me. She place a male by the name of XXXX XXXX on the phone i advised to him that I was hung up on. I also advised I was not refusing to pay or refusing to resolve this debt. He told me that he would listen to the call and then he apologised. He also told me that he would remove my number from their call list, I advised him that I never asked him to do that but he could if he wanted too. He then asked me when would I be ready to pay and I advised him the same. I asked him who the VP of th agency was and he told me that the name is XXXX XXXX. I told him that being that its my 1st time speaking and knowing about this debt,, I would need to get some help and I let him know that I would pay the $ XXXXminium within 2weeks. He did say that he would send me a document showing my promise to pay, he referred to it as something. We ended that call.

I received a letter from MiraMed Revenue Group regarding an alleged debt owed to XXXX XXXX XXXX. XXXX. To that end, I communicated in writing ( see attached ), regarding this inaccurate, erroneous, unverifiable entry. Miramed responded, see attached Please note, MiraMed 's letter is dated XX/XX/2022 ; however, the envelope containing the letter was sent XX/XX/2022, please see attached. This is very deceitful on their part, especially given the content of their letter states they will proceed with collection activity after the 10-day period. Please note, I am a XXXX consumer and receive Medicaid. All of my bills are covered and paid my Medicaid at 100 %. My disputes to this company were an effort for them to communicate with their client because I do not owe XXXX XXXXXXXX. Additionally, I requested they cease any and all collection efforts and return this alleged debt to XXXX XXXX. Their aggressive tactics to collect on this alleged owed debt continually cause me stress due to the fact that I know I am XXXX and my medical bills are fully covered. MiraMed has very detailed information about my treatment and medical care that violate my HIPPA rights. A third party company should not be privied to any information regarding care and services that are explicit to any medical conditions. Please see attached. This is illegal as information shared with third party collection agencies should be limited in nature and not explicit to the services so as to mitigate discrimination or bias. See attached whereby, MiraMed is clearly able to determine medical conditions. It is puzzling to say the least, that MiraMed can threathen to proceed with collection activity when they are at fault and wrong. MiraMed did not furnish an agreement reflecting their authority to collect on this alleged debt in the state of Texas. MiraMed did not provide a copy of all terms and conditions of the account from its alleged inception. They did not provide the name and address of the bonding agent in case legal action becomes necessary. MiraMed blatantly violated my consumer rights and continues to do so. This alleged debt lacks veracity and verification ; further since they are violating FDCPA mandates when they received the lettes of dispute, MiraMed should return this alleged debt to XXXX XXXX and remunerate me for the violations. Again, I am a XXXX consumer receiving XXXX benefits and Medicaid. I know my rights and I know that my insurance covers my bills at 100 percent.

On XX/XX/2020. I took steps to try and re-pay old debts on my credit history. I was trying to do the right thing and repair past medical bills. I contacted the debt company " Miramed '' Group. I actually had my report in front of me. Right from the start, the amounts they had did not match with mine. I questioned this many times. I kept being told that it all added up to the same amount. I did the math and noticed it did not. I still offered to pay a large amount if I could get verification that the 4 separate accounts would be updated. They stated that they would be re-done into one acct. Showing the amount paid and that I was current. I still was not feeling great about the situation. But, with this being my very first conversation with a debt collector and not educated about my rights and what they are allowed to do and not do. I continued on. I did state over and over I did not like or understand how the amounts they had did not even match to one of the amounts on my credit report. Once I decided I was going to pay in good faith. As soon as I stated this. That I would pay as long as it would be updated on my report. The woman IMMEDIATELY started saying just so you know all calls are recorded. And can be used in legal litigation. After this i was placed on hold. When the hold was lifted the woman I was speaking to said she was going to transfer me to her supervisor to complete transaction. When I got on with " supervisor '' the woman asked me how much I could pay? The total amount they had was around {$2300.00} if I rember correctly. Where my reports showed around {$1500.00}. Being still weary of this, I said I would pay {$600.00}. The supervisor then swayed me into paying {$730.00}. She said that this would take care of 3 of the accts. And she could then just make the rest to one acct. With a current paid status. I paid {$730.00}. Since then, NOTHING has changed on my reports. Not the balances or number of derogatory accts. ( 4 ) from Miramed group. I want to know what happened to the money I paid. Why did nothing change. Is this not theft??? Or at the least, misinformation by predatory tactics? I tried to do the right thing and all that happened was I lost {$730.00}. As if I tossed it out the window. I am trying to correct these pasts to try and build up my credit to buy a home for my wife and XXXX children. This company should not be allowed to operate with such tactics and dishonesty.

I had a XXXX back on XX/XX/2016. I was assured that there would be no out of pocket cost. That it would be covered 100 %. It was a preventative procedure. I have been getting calls from Miramed Revenue Group threatening me. That if I don't pay {$480.00}, it will go on my credit report. That was after I told them I have a recording of Dr XXXX 's staff telling me there would be NO OUT OF POCKET COST TO ME. Based on that information, I went ahead had the procedure done. Under the Affordable Care Act, services recommended by the U.S. Preventive Services Task Force are covered with no out-of-pocket payments. To whom it may concern : In a recent evaluation of my credit on XXXX/XXXX/XXXX, it has come to my attention that XXXX XXXX XXXX XXXX negative reports were filed on my report. According to the reports I have reviewed, the reports were filed in XXXX by a " XXXX '' on behalf of " XXXX XXXX XXXX '' and a collection agency " MIRAMED REVENUE GROUP '' on behalf of a " XXXX ''. In further research of the alleged collections, XXXX XXXX has numerous complaints of other false collections under investigation. Miramed Revenue Group appears to have mistakenly filed the same collection twice which has further damaged my credit unnecessarily and also reported inconsistent reports with different agencies bringing their entire credibility as an accurate reporting entity into question. Finally, it is my assertion that XXXX agencies ( and therefore all the negative reports ) are completely fabricated, fraudulent, or otherwise false. Whatever the case may be, rest assured that I in no way agreed to or had knowledge of either Miramed Revenue Group OR XXXX XXXX XXXX, XXXX or XXXX. These collections were all filed without my knowledge and devoid of any warning from any of the above entities. I have, by certified mail, notified Each of the above collection agencies, all XXXX credit, submitted an official complaint with the Consumer Financial Protection Bureau in hopes of immediate resolve. I am also notifying the office of XXXX XXXX, the United States Treasurer and the Illinois XXXX in hopes of further oversight in debt collection practices. I am not alone in what appears to be a growing list of victims in my community and in across the country and I demand these records be completely removed so I can benefit from the advantages I deserve as someone who made the decision to never utilize their credit at all. I am simply attempting to start the process to own my own home and these, false reports stand in my way of that human need. I am looking forward to your response!

Recvd phone call in XX/XX/XXXX from a company called MiraMed Revenue. A man named XXXX called to inform that I owed {$930.00} for a XXXX XXXX procedure done by my XXXX XXXX XXXX XXXX back in XX/XX/XXXX. I told him we have recvd nothing regarding an outstanding debt from our XXXX or the XXXX XXXX company. I told him then that means if it was owed thennit was also paid already. He assured me it wasnt paid and that it was outstanding and owed. And that he had not the bad debt on my credit report yet. ARE YOU KIDDING ME????? Thats harassment. I told him we have received nothing on this balance!!!! I told him then I would need to check with my doctors billing Dept as well as my husband and would call him back. He proceeded to tell me he would be willing to negotiate the {$930.00} if I was willing to pay today. A red flag went off right there. I did call my doctor and they proceeded to call the XXXX XXXX company and indeed, the bill had long been paid. This is ridiculous. Why are they not required by law to confirm and reconfirm before a single phone call is made that a debt is indeed outstanding?? Unless they are intentionally hoping to scam someone out of money already paid. These are the kind of companies that take advantage of elderly people or non English speaking, or even inexperienced people because of their persistence as well as their lack of desire to check on a balance a customer says has been paid. Why wouldnt they be required then and there to check with company they are collecting for??? Because its just as easy for them, when they get caught collecting again, to just plead ignorance or call it a misunderstanding. Baloney. Stop these companies from being lazy and require them to do their due diligence before placing a single call. It was a waste of two hours out of my day trying to gather information from a year ago to prove to them this balance had been paid. Infuriating. XXXX XXXX

RE : Intent to file lawsuit, HIPPA Privacy Violation Validation Letter sent to : MiraMed Revenue Group To Whom It May Concern I checked my credit reports for accuracy as I am in the process of being pre-qualified for my very first mortgage through the First Time Homeowners Buyers program and I saw two incorrectly trade lines for an account from XXXX for XXXX XXXX XXXX XXXX XXXX XXXX for {$200.00}. Both account statuses show past due and they should not say that as these debts are not owed. I am requesting both a validation and " verification '' as well as the signed HIPAA release with my signature as the item being reported to the credit bureaus by XXXX is a VIOLATION of the HIPAA Act of 1996. XXXX did NOT provide me a HIPPA release that releases my medical information to them, therefore by XXXX XXXX XXXX XXXX XXXX providing such information they are in VIOLATION of my HIPPA rights. I am proceeding with legal action as prescribed by law against XXXX as well as XXXX XXXX XXXX XXXX XXXX should this item not be deleted within 10 business days and until the debt has been validated and verified. XXXX is in direct violation of HIPAA regulations, and they are facing fines up to {$1000.00}, even possible jail time. I am seeking a legal remedy immediately as I have contacted Legal Services to obtain an attorney for free to file suit against XXXX, XXXX XXXX XXXX XXXX XXXX, and all 3 credit bureaus : XXXX, XXXX and XXXX. I urge you to take this extremely seriously as I have documented my case without error. I encourage a response from XXXX expeditiously as I have no idea where this debt is coming from and I do not owe this company any money. Additionally, I can prove that at the time of service, I was both unemployed and homeless. I completed the financial assistance forms, which should have taken care of any costs associated with my medical care.

I paid the original debtor ( XXXX XXXX Hospital ) the following amounts : XX/XX/XXXX - {$10.00} XX/XX/XXXX - {$10.00} XX/XX/XXXX - {$10.00} XX/XX/XXXX - {$10.00} XX/XX/XXXX - {$10.00} XX/XX/XXXX - {$10.00} XX/XX/XXXX - {$25.00} XX/XX/XXXX - {$25.00} XX/XX/XXXX - {$140.00} I was contacted by MiraMed on XX/XX/XXXX by mail that I owed {$60.00} on XX/XX/XXXX. I contacted XXXX at XXXX who verified a XXXX balance with them. on XX/XX/XXXX. I contacted XXXX at MiraMed who said she'd send a balance verification to XXXX XX/XX/XXXX. I contacted XXXX at MiraMed who was disgustingly rude and disrespectful when I tried to discuss the issue again. XX/XX/XXXX. I contacted XXXX at XXXX, she verified the XXXX balance and agreed to escalate the issue with MiraMed and request a Paid In Full letter be sent. MiraMed would call various times between then and now and I would explain it again and they would agree to look into it. Between XX/XX/XXXX - XX/XX/XXXX, MiraMed called ( 5 ) times about this debt. On XX/XX/XXXX, I mailed XXXX and MiraMed copies of the cleared checks my bank sent to XXXX electronically. Since XX/XX/XXXX, they have only left two messages attempting to collect a debt. I've paid my debt in full to XXXX, MiraMed isn't owed any money. These two companies had a miscommunication and refuse to resolve it and meanwhile Miramed continues to harass me over the issue. I'm requesting debt collection practices be stopped and a paid in full letter be sent. Thank you In the attached file below, my bank could not separate the payments I made to XXXX during this time period but each check ( upper left corner ) and letter ( Re : ) is marked with the correct account number.

On XX/XX/2023 I had XXXX at XXXX XXXX, XXXX, XXXX. My surgeon was an XXXX XXXX XXXX physician. I receive my billing on the XXXX XXXX, and claims are submitted through my health insurance company, XXXX XXXX XXXX. All claims were submitted and processed by XXXX, 2023. I set up payment plans for the remaining amounts due via clinic billing services according to XXXX. I received a bill from XXXX XXXX XXXX XXXX dated XX/XX/2023 for {$1800.00}. I called XXXX insurance as I was confused as to why I received a separate bill in the mail and that I was under the impression that this debt was inclusive with the current payment plans I have in place. I called the number on the bill to discuss the amount due and make payment arrangements. The woman was hard to understand, but we did our best to communicate. I was told that I should received a call in within 2 weeks to set up a plan. I never received a call back. On XX/XX/2023, I contacted my insurance company again to resolve the billing issue. They told me to file with XXXX if it doesn't get resolved. That the XXXX XXXX XXXX program implies that I should not hqve been billed twice. Today, I learned that the XXXX XXXX XXXX XXXX sent me to collections with XXXX, A global services company, XXXX XXXXXXXX XXXX XXXX, XXXXXXXX XXXX XXXXXXXX. I received a call today, XX/XX/2023. This is not best practice, nor, did they prove I owed this outstanding amount which is already being paid by my payment plan with my hospital. I want this removed from collections and investigated as to why I was double billed and not offered an opportunity to pay my debt through a payment plan prior to sending it to collection a mere 6 months after XXXX.

MiraMed Revenue Group has called me over and over again for the last 60 days - but I paid the bill directly to XXXX ( the original company who invoiced me ) on XXXX/XXXX/2016 and have confirmed that directly with them. MiraMed called on XXXX/XXXX/2016. I returned the call on XXXX/XXXX/2016 at XXXX and informed them that the bill had been paid and that they were pursuing the debt collection erroneously. I provided receipt information and asked that they never call me again. MiraMed call me again, despite being told not to ever call again, on XXXX/XXXX/2016 at XXXX. Again, I informed them that the bill had been paid and to never call me again. Mid-sentence, the operator hung up on me. MiraMed called me again on XXXX/XXXX/2016 at XXXX. I did not answer the call. MiraMed called me again on XXXX/XXXX/2016 at XXXX. I did not answer the call. MiraMed called me again on XXXX/XXXX/2016 at XXXX. I answered the call and told them for the 3rd time that the bill had been paid and to stop calling me. I told them that I would report them to the FTC and said to never call me again. The operator continued to push me to make a payment and I refused. The operator then hung up on me. MiraMed has harassed me over a debt that should never have been in collection in the first place. They 've called me a total of 6 times - or 5 times after being notified to never contact me again. I 've communicated with them multiple times to stop calling. I contacted the original vendor and asked them to make sure I was removed from collection, as well, and they 've indicated that I should have been removed. MiraMed continues to call on a debt I do not owe.

Re : MiraMed Revenue Group. Account Reference # : XXXX and any other one of my accounts with MiraMed Revenue Group. First, MiraMed Revenue Group ( thereafter " MiraMed '' ) has continuously violated my rights under the FDCA by illegally harassing me ( by calling me before XXXX XXXX. or after XXXX XXXX., calling me during unreasonable hours when I have repeatedly asked MiraMed to stop doing so, making my phone ring for long periods of time ) and my family; and failing to respect my privacy. MiraMed alleges that I owe debt it wishes to collect. I have repeatedly requested that MiraMed send me an itemized list detailing all such debt and stating with detail and specificity what exactly each itemized charge pertained to. MiraMed has repeatedly refused to do so. Without such an itemized, detailed and specific list, it is impossible to verify MiraMed 's allegations as to the alleged debt. Thus, MiraMeds billing as to the alleged debt is intentionally misleading, in violation of the Fair Debt Collection Act ( FDCA ). Furthermore, I have repeatedly informed MiraMed that the hospital ( XXXX XXXX ) to whom MiraMed alleges I owe debt has illegally billed me, by virtue of discriminating against me with respect to other similarly situated patients by not granting me the same financial aid it grants to indigent individuals such as myself. The hospital unjustifiably denied me the same financial benefits it grants to other similarly patients, thus violating its duty to only charge patients in good faith.

I pulled a copy of my credit report around the beginning of XX/XX/XXXX. I noticed that I had 3 accounts showing on my credit report from Miramed Revenue Group. All 3 accounts had different account numbers and had totals of {$50.00}. I called them they informed me it was copays from varies dates with XXXX XXXX. I advised I would pay off the accounts that was showing on my credit report and set up payment arrangements for the rest of the accounts. I made my payments on those 3 accounts showing on my credit report on XX/XX/XXXX. I called them today ( XX/XX/XXXX ) because the information still hasn't been updated on my credit yet. I was advised by a rep. that they would not update my paid accounts to the credit bureau 's until I paid all of my accounts off with them. I advised them that all the accounts with them have separate account numbers and are all for different dates of service. So they needed to update the information according they refused and stated they aren't sending anything to the credit bureau 's until I satisfy the other accounts that aren't showing on my credit report. I haven't received anything from them In regards to any of the accounts. Please help I dont think this is fair. I am trying to repair my credit and the fact that these may show for a longer time even though they are paid isn't fair to me.

From XX/XX/XXXX to XX/XX/XXXX I had XXXX XXXX XXXX thru XXXX, XXXX XXXX XXXX XXXX XXXX which both of my insurances covered. XXXX filed my primary but did n't file my secondary. I called them after I started receiving a bill for the part that my secondary insurance was to pay. XXXX said they did n't have my secondary information so I again provided it to them. A month after I received another bill. Same story but this time I took copies of both insurance cards and mailed it in to them. This went on for a year. Each time XXXX told me they would file it. They never filed my secondary. XXXX only had 1 year to file my secondary. On XX/XX/XXXX I called my secondary to find out if the claim was ever filed, I was told no. While I was on the phone with my secondary insurance they asked me to stay on the line while they tried to call XXXX, there was no answer. My secondary insurance also told me that I was n't responsible for the bill because they would have paid it in full. About 6 months or so ago is when a debt collector started calling me, at least once a week. I would explain to the same person, every time, what happened with that bill and what I was told by my insurance company. It 's not like I did n't try to settle a bill they just refuse to hear me or ignore what I tell them and continue to call.

XXXX XXXX XXXX hired MiraMed Revenue Group, LLC to collect a {$57.00} debt for my young daughter rather than contacting us directly. This week MiraMed has contacted relatives of ours using a robodialer and left personal account information for my daughter on their voicemail. The automated message indicated that the person listening should hang up if they are not the parents because it contains " personal and private '' information ( I do have this recording saved ). The message continued to provide the private information on the voicemail without a prompt for consent ( for example, pressing a key to continue ). ANYONE is able to enter the reference number they provided on MiraMed 's website, mmrgpayment.com, to easily access account information with my childs name, address, debt amount, and medical creditor information. It's like openly handing out a password! Hopefully MiraMed did not contact any other third parties and provide private information about my family because it would be just as easy for them to access this private data. This goes against the Fair Debt Collection Practices Act ( 15 U.S. Code 1692c ) as they should not be disclosing private information to third parties. Additionally, I have not yet received any written notification regarding this debt.

XXXX XXXX sent their invoice to my secondary insurance carrier ( XXXX ) for more than a year -- and not to my primary insurance XXXX. I made two phone calls and sent FIVE letters telling XXXX that they had to bill the PRIMARY insurer first. I even made sure this was changed when I was AT the doctor 's office. I do n't know if they never did this, or if they waited the year and a half before they did so, but at that point ( 1 1/2 years after the dr visit ) my company changed insurace to XXXX. Apparently now XXXX is not paying since it is so long since the original visit -- and I ca n't blame them. XXXX is at fault for not billing the correct insurance company in a timely manner. I would be willing to pay what I would owe AFTER what the insurance would have covered, but I am not willing to pay this entire $ XXXX bill that is XXXX 's fault. They have now filed with MiraMed Medical Group, a collection agency to collect this bill and this company is not interested in how this came to be. I 'm about to buy a house -- and I have a STELLAR credit rating -- so I do n't want this diminishing my credit score! Please help! I do have copies of the letters I have written, but would need to take cell phone photos and email them to you, I do n't have a scanner.

This company claiming to be MiraMed Revenue Group robocalls me from a spoofed number XXXX XXXX XXXX XXXX and left a voice mail saying to call XXXX XXXX XXXX XXXX regarding case # XXXX. The robocaller speaks so fast I had to play back the message multiple times to get the information. I called back the XXXX number and when they asked how they could help me, I said, " I don't know. You called me. '' The first person hung up on me. The second person claimed they were representing XXXX XXXX. I told her to send me anything in writing because all bills that I received from XXXX were paid. She refused and wanted personal information from me. I am in the midst of dealing with identity theft and do not want to give out information. I requested she send out the information she has to the address she got from XXXX, if they are truly who they say they are. The information I need is the date of service, diagnosis code, and the provider. She refused unless I confirmed my personal information ; which I don't want to give out to someone I don't know and don't do business with. This company should be above board and not spoof their telephone number. The caller ID should accurately say who is the company or person calling.

XX/XX/2021 I checked my credit report and it has been brought to my attention that Miramed Revenue Group , LLC has 2 negative remarks on my report of {$120.00} and {$120.00}. The total debt they are claiming that I owe them is {$250.00} ( see neg_remark.jpg ). On XX/XX/2021 I mailed Miramed Revenue Group , LLC a registered letter via certified mail, requiring a signature of receipt, requesting competent evidence bearing my signature, showing that I have ( or ever had ) some contractual obligation to pay them. In short, I asked them to validate the debt they claimed I owed them and if they could not validate the debt they must request that all credit reporting agencies delete the entry ( see Miramed_validation_letter.pdf ). I mailed Miramed Revenue Group , LLC the registered letter to its listed address on XXXX and from their main website and I have yet to receive a response. The registered letter was sent by Certified Mail via USPS ( see certified_mail_receipts.jpg ). These letters are tracked and they show that they have been delivered, signed for, and received by a Miramed Revenue Group representative. It has been over 30 days and I have yet to receive any response from Miramed Revenue Group.

Over the course of 2 years, my XXXX paid my bills to XXXX XXXX XXXX and they returned the payment to my account each time. I would pay the bill- they would return it. Over and over. I paid all past balances- everything they said I owed, even things I thought I already paid many times and I have had a balance of {$0.00} on my XXXX account since XXXX of 2022. I am still receiving collection letters from 2 different collection companies. I disputed one through XXXX and successfully had it removed from my credit report. The second continues to send letters and emails. I would like XXXX XXXX XXXX to STOP reporting my debt as unpaid as I have paid all debts and my balance is {$0.00}.

I request for debt validation as well as proof of date of service for so call medical bills this bill is for. The rep. " XXXX '' informed me that MIRAMED does not have nor provide this information for date of service etc. I inquired how can you bill me if you are not providing me with all the details etc. She informed this is their collection policy etc. I told her to have MIRAMED send me a debt validation letter along with the detail for the date of service they are now attempting to collect for etc. She said they will not be able to provide that information that I would need to call my insurance company. Why would I need to call another entity when MIRAMED REVENUE GROUP is the one stating I owe the bill not my insurance company. Please have this company stop their bad debt practice. I want this information removed from all credit files XXXX XXXX and XXXX FROM UNDER MY NAME ETC.

On XX/XX/2018, I was notified by XXXX XXXX, a credit monitoring company, that I had a debt that went into collections. At complete disbelief, I went to check out what this debt was in regards to. It was a collections from a company called " Miramed Revenue Group ''. On XX/XX/2018 I had called Miramed to find out what this debt was for. They refused to validate the debt or tell me any information as to why this was sent to collections. I wasn't going to pay on a debt I had no idea what I was paying for, or be scammed not knowing if it was even for me or valid of me to pay, let alone even notified for. I was never once notified of an outstanding debt or sent an invoice or even notification that I was going into collections. I found out the debt went into collections in XXXX for {$150.00}. I still wanted to see an invoice to see who I was paying or what I was paying for. I never received any of that. The collections agent said I couldn't get any written validation of the debt and I couldn't dispute the debt. How am I supposed to know what I am paying for?

Miramed Revenue Group alleges it has an account pertaining to XXXX XXXX in their possession. Attached is the billing statement from that provider office. All dates of service were covered by insurance, however, the insurance company indicated that the provider coded improperly/different for the service date of XX/XX/2017. A co-payment was made as required by XXXX XXXX. Again, neither XXXX or the provider office submitted bills to the insurance company regarding XXXX. It can be ascertained that an adjustment was made to one bill by XXXX but not the other and neither were insurance adjustments. Therefore, these bills have been sent back to XXXX as well. Both the providers office as well as XXXX was addressed prior to this and no changes were made as of this date. However, I paid for insurance, therefore the insurance will be the primary payment and as already stated, I will be responsible for co-payments. Both dates of service provided the same care and XXXX, so there should not be any significant balances between the two dates of service.

Earlier in 2021 I received a call from Miriamed Revenue Group indicating I had an outstanding debt with XXXX XXXX XXXX. I informed the agent calling that I believe I had already entered into a payment arrangement with another agency, therefore disputing the validity of the debt. I never received any information about my dispute, and months later learned they had placed the account on my credit bureau, lowering my score almost 30 points! Today, XXXX, I received another call from them. They told me they would be willing to settle my debt for 75 % of the balance, and if I paid, they would remove it from credit. The agent proceeded to over talk me, and tell me I am wrong as well. This agency never extended me credit, and it is clear now after this conversation, they placed this on my credit for one reason, as a weapon to use against me and force my hand to pay the debt. I will pay the debt, but i want this removed from my credit immediatley and I would like this company sanctioned.

They call repeatedly with a robocaller that tells you to wait for the next operator. I waited on the line and instead of getting an operator I was hung up on. This happened several times. Finally after a couple months I received a letter from them stating I owe a debt to pain management, I pay them every time I go into the office, if I was delinquent they would not make appointments with me. I attempted to reach them at the number on the letter and was told that I had to call back as all operators were busy. Finally a real person called me and berated me for being so delinquent on this debt, I told them had they bothered to send a letter or actually put a human on the line earlier so I could see what the debt even was I would have handled it already. I have a call into the doctors office who stated they should not be pushing collections on people without contacting them for verification first.

A collections notice was received by XXXX XXXX XXXX for an unpaid medical bill for a foster child. When the collections agency was contacted for the purpose of providing insurance billing information, they claimed that they are not in the business to bill insurance, do not have the ability to bill insurance, and were requiring that XXXX provide the insurance company address, contact information, and other items that are not available to this worker. The information on the insurance ID card, used by medical providers everywhere for the purpose of billing medical charges, could not be used by the collections agency. They have intentionally created obstacles that prevent them from billing insurance, and it is done for the purpose of pursuing payment of the full amount of the bill to increase the profit they make from collections.

Miramed revenue group. On XX/XX/XXXX I paid a {$90.00} collection bill with them. I recorded the call and was told that the bill would drop off of my credit if I paid it. It didnt drop off of my credit. The status was only updated to paid which still shows up as bad. The company wont answer the phone. I paid the full balance and the entry should be deleted completely from my credit. The exact same thing happened with XXXX XXXX a few years back. I agreed to pay the debt after the agent explained that it would be removed from my credit completely, not only updated to reflect paid. After years of going back and forth XXXX has still not removed the debt in its entirety off of my account. Legal representation had to finally be deemed due to this.

I received a letter in the mail today XX/XX/XXXX, from the MiraMed Revenue Group, LLC, claiming they represent XXXX XXXX For a date of service of XX/XX/XXXX. My husband called the MiraMed XXXX XXXX ) and asked them what this was about. They wanted extra information about me, so my husband told them that we are thinking they are trying to run a scam to collect money. That we have never done business with this XXXX. We also contacted our health care insurance and they told us any medical bill that we have not received for a service that is over a year old does not need to be paid and the insurance company will not pay on it either. That all medical groups know this. So that is why we feel a scam is trying to be run.

Hi, Over a year ago I gave birth to my son and paid in FULL all hospital bills. However, they have continuously called me over the past year trying to collect money from a bill that was already paid. Despite the fact I have sent several times proof of payment to the hospital they continue to call me and now they hired a collector. I would obviously pay if I actually owed the money but this isn't the case. This is bringing a lot of stress and could possibly hurt my credit. I'm attaching the hospital bill + a copy of my bank statement were you can see that the payment was made + the letter I received from the collection agency. I hope you can help me with my problem. Have a wonderful day! XXXX XXXX, XXXX

XXXX XXXX XXXX is re-reporting a collection that I disputed in XX/XX/2018. The collection in question was subsequently deleted after the collection agency failed to properly validate the debt in question ... now they have reported it again to the credit bureau and have still not validated the debt per the Fair Debt Collections Practices Act, 15 USC 1692g Sec 809 ( b ) ... Furthermore they are in direct violation of the Federal Hippa Laws ... A breach of protected health information ( PHI ) is defined as the acquisition, access, use, or disclosure of unsecured PHI, in a manner not permitted by HIPAA, which poses a significant risk of financial, reputational, or other harm to the affected individual.

XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX, Arizona XXXX {$1000.00} Original date of service : XX/XX/18 Sold to Miramed Revenue Group I went to the my Doctor 's office for something that should not have been sent to a lab for results. I was not aware of this cost of {$1000.00} or I would not have consented to the lab work being done. In the past I was treated with a prescription, but this particular nurse practitioner had repeatedly wanted to send things to the lab. I got the bill and called the Doctors office. They told me it was just too bad, & my responsibility to pay it. The results even indicated that all I needed was a prescription to remedy the problem.

Hi, On XX/XX/2021 Miramed Revenue Group placed a Collection item on my Credit Report for the amount of XXXX. I have NEVER EVER received a collection letter from this or any other entity regarding a medical bill for XXXX. I asked for any proof what-so-ever of debt or a copy of a bill sent to me and they were not willing to provide. The rep I spoke to admitted that they had the wrong mailing/billing address on file. It is not Okay to place an account in collection on a Credit Report without giving a chance for the customer notice of, chance to review or pay the claim. I kindly ask that this item be removed from my Credit Report ASAP.

Miramed, XXXX, and/or XXXX XXXX XXXX Hospital attempted to collect a debt without utilizing the insurance. Prior communications indicate that all parties were aware of the insurance in place and the purpose of me paying for insurance, is that it may be utilized. The original debt was indicated to be {$4800.00} and the reduced rate is {$580.00}. If they want only {$580.00}, then the insurance company can pay them that. Otherwise, the deductible and co-payment will need to be identified and ALL provider bills will need to be considered in the billing cycle. Not just this bill. Again, this is the reason I purchased insurance.
