I received services from XXXX XXXX XXXX in XXXX XXXX, LA in 2015. I asked for them to tell me how much it was going to cost so I could budget in anticipation of the treatments. I received a quote for how much I would have to pay based on them saying they checked with my insurance company. After my treatments, I was presented with a bill for an additional {$450.00}, which I had not budgeted for. At the time, I was recently unemployed. My insurance covered whatever they covered, but the additional amount came due during the time that was recently unemployed. I tried working it out with XXXX XXXX XXXX, but my unemployment was for an extended period of time. The accounts receivable person finally said that they would send it to collections. I received a bunch of phone call messages over the past twelve months from a telephone number in the ( XXXX ) area code that left messages saying that " ... a claim had been filed against me and to prevent further action to please call this number ... '' and they provided a number. The message didn't say who they were or why they were calling of who filed a " claim '' against me. Last month, a new derogatory remark showed up on my XXXX credit report, and it was from XXXX XXXX XXXX on behalf of XXXX XXXX XXXX. I've been in the process of cleaning up my credit and paying off the three creditors I had past due balances with. This derogatory remark from XXXX XXXX XXXX has fouled up my otherwise newly-cleaned up credit. I want this remark removed or revised. I'd be glad to make arrangements to pay the balance. The menacing phone calls I received with no verification of who the calls were from or what the calls were for felt like a scam to me. I experience a breach of my bank account and credit card last summer and this past fall, so I am very wary of callers that don't identify themselves and those that want me to volunteer personal information over the phone to an incoming caller. Can you help with this?

The original company was XXXX XXXX in XXXX XXXX, LA. The collection agency that took over the debt was XXXX XXXX Collections. Between the time frame of XXXX when the file was opened. I reached out to the company, and they informed me that they were billing the wrong insurance. I was told by the original company that it would be taken care of, and they would process the correct insurance. At the time I was XXXX XXXX with XXXX XXXX XXXX XXXX. They were charging XXXX which is not XXXXXXXX XXXX XXXX for XXXX XXXX XXXX The total amount to be paid for the lab work that was done. I have documentation that states I had insurance and I also have military orders to prove I was a XXXX XXXX receiving benefits. I have asked Lake Area Collections to validate a collection they are reporting on my credit reports from XXXX/XXXX/XXXX to XXXX/XXXX/XXXX and they have failed to provide anything that would support the fact that they are attempting to collect on a debt without an agreement or contract. Nor have they supplied anything other than a statement on XXXX of the collections. I told them that I would be reporting them to you but they obviously do n't fear you because they failed to delete themselves or answer questions about the debt ( s ) that they are using to damage me financially. Its damaging my credit and it is important to me.

This company continues to report on my credit report after I sent them a letter telling them that this account was not mine and I have no idea what it is or who it belongs to! I asked for proof of a signed contract, I asked for a license to collect in my state, I asked for copies of all information referenced for this debt and still to date, I have not received anything but harassment from this company! THIS IS NOT MY DEBT! I WANT THIS ACCOUNT REMOVED FROM MY CREDIT REPORT AND THIS COMPANY TO STOP CONTACTING ME IMMEDIATELY!

The collection agency Lake area collection is violating my rights under the No surprise Act of 2022. I have a medical bill in the amount of {$330.00} from XX/XX/XXXX that an out of network provider is charging without my prior authorization. Lake Area Collection has placed an account in the amount of {$10.00} on my credit reports. When asked to validate the account they sent private medical data that was obtained without my consent. I have sent tried to dispute the information for violating my HIPPA rights and have received no response from the company.
