I received a bill from XXXX XXXXXXXX XXXX from a XXXX in XX/XX/XXXX in the amount of {$850.00} dated XX/XX/XXXX. I made the payment a day late in the mail but I sent them {$100.00}. I never got a bill after that not one. I thought the insurance company had picked up the remaining.

I then started to receive phone calls onXX/XX/XXXX from XXXX XXXX with IQMS, LLC stating that I owe {$750.00}? I was trying to explain to her that I do not owe {$750.00}, I owe {$650.00} now because I sent a payment in already and why was in collections? I did not skip out on the bill? She said if I had read the bill correctly from the Dr. 's office, bill states that services are due at the time rendered and that they do not send statements out showing my remaining balance from the one payment??. She then stated that I can set up payment arrangements or I will be sent to the " investigation department ''. I told her Ok, how much? She said that it would be {$200.00} something but that offer only stands till the end of the day. I did not have {$200.00} and something that I can send {$100.00}. She then got really cruel and just threatening me that I will go to the " investigation '' dept. once again.

I have been sending {$100.00} to this IQMS company for the last 3 months now, cashiers checks because I will NOT give them any information on me because she refused to send me an updated letter showing the correct amount. The first cashier check I sent was on XX/XX/XXXX for {$100.00} and it was cashed on XX/XX/XXXX, 2nd check was sent on XX/XX/XXXX and cashed on the XX/XX/XXXX. My 3rd check is going out today XX/XX/XXXX. I called XXXX on XX/XX/XXXX before I sent the letter asking if I could get a statement showing the correct balance and her was basically no. Since I refused to set-up payments she could not send one. That is incorrect. I did n't refuse to set-up payments, I just told her that I could n't due the {$200.00} something. Each time I send payment to iQuantified Management Services, I copy the old statement she sent me and cross out the date and the amount to place the correct amoutn and date I am sending my payment in.

I received a letter from XXXX XXXX and amount shows {$750.00} that I still owe and in capital letters tells me that if the account is not paid in full by XXXX XXXX XXXXey exercise their right to refer my account to an attorney and report as paid credit.

For {$550.00}, go ahead and report, I will dispute. And my account will be paid in full by the time they get a lawyer here in AZ. All I have been asking for from XXXX is a statement showing my remaining balance and she advised no, since I did not set up payment arrangements.

Now they have been cashing my cashier checks and I do not get a statement?? Tell me this is right? All I want is the threats to stop and an updated statement showing my correct balance. My account will be paid in full in XX/XX/XXXX which I have explained to her every month I send money with a letter attached to it.

I received a text message XX/XX/XXXX from Quantified Management Services stating they were trying to reach me to collect a debt and that I could pay online or call. I figured it was a scam since I don't have any outstanding debt. I got a second text XX/XX/XXXX stating the same thing. I went online and entered the text 's reference # XXXX and was prompted to enter the last 4 digits of my social security #. I was not comfortable doing that so I called the number in the text. I spoke to someone named XXXX, gave the reference # in the text and asked for the name of the provider, date of service and the amount. XXXX told me she couldn't give me that information because of HIPAA laws unless I identified myself. I said the reference # should be enough since they sent it to me, that I was not comfortable giving the last 4 digits of my social security # to her or online, and that I believed this to be either a scam or a mistake. I told her to send whatever she had in writing. She said that was already done once on XX/XX/XXXX and that they weren't required to send it again. She did reveal the address the bill was sent to. I asked the person still living at that address if anything came in the mail for me. I was told no and that no mail had come there for me in years. Based on the address XXXX mentioned ( where I no longer live ) I believe I am able to piece together what this is about. In XXXX XXXX XXXX XXXX XXXX XXXX ILLEGALLY sent me a bill I DO NOT OWE for services totaling {$4400.00}. I then contacted my insurance provider, XXXX XXXX, and sent them a copy of this ILLEGAL document. The XXXX XXXX person I spoke to explained to me that XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX ) was paid what they were contracted to be paid by XXXX XXXX and that XXXX XXXX was in fact ILLEGALLY sending me a bill for the balance. XXXX XXXX sent XXXX XXXX a letter on XX/XX/XXXX stating that they ( XXXX XXXX ), PER NEW YORK STATE REGULATIONS WERE PROHIBITED FROM REQUESTING COMPENSATION OTHER THAN ALLOWABLE CO-PAYMENTS FROM FIDELIS CARE MEMBERS. ( MY CO-PAY AT THAT TIME WAS {$0.00}. ) I have attached a copy of the original ILLEGAL bill I received from XXXX XXXX and the letter from XXXX XXXX. This letter also included a XXXX Update which states that a XXXX RECIPIENT " MUST NOT BE REFERRED TO A COLLECTION AGENCY FOR COLLECTION OF UNPAID MEDICAL BILLS OR OTHERWISE BILLED, EXCEPT FOR APPLICABLE XXXX CO-PAYMENTS, WHEN THE PROVIDER HAS ACCEPTED THE RECIPIENT AS A XXXX PATIENT. '' XXXX XXXX obviously knew I was a XXXX patient because they had my health insurance information. I trust that this will resolve this matter and that Quantified Management Services will cease and desist requesting payment from me, will not report me to any credit bureau and will not resell this ILLEGAL bill to another agency.

XX/XX/2019 - XXXX XXXX performed by XXXX XXXX XXXX at XXXX XXXX XXXX XXXX. XX/XX/2019 - Received the FIRST and ONLY bill from XXXX XXXX XXXX XXXX for {$230.00} XX/XX/2019 - Paid bill online with Visa Credit Card XX/XX/2019 - Received Voice Mail from Quantified Management Services about collecting an unpaid debt. XX/XX/2019 - Received second Voice Mail from Quantified Management Services about collecting an unpaid debt. XX/XX/2019 - Returned call to Quantified Management Services and inquired about the debt they are trying to collect on, and who the company was that I owed the debt to. They informed me it was for XXXX XXXX XXXX XXXX. I attempted to inform them I had no outstanding debts with XXXX XXXX XXXX XXXX but the individual on the other end would hear none of it and disconnected the call. XX/XX/2019 - Called the Billing Dept at XXXX XXXX XXXX XXXX. Spoke with XXXX. She informed me that the bill showed " paid '' as of XX/XX/XXXX. I informed her that I paid online with a credit card on XX/XX/2019, and also that I checked my Visa Credit Card Statement and it shows a posted date of XX/XX/2019. She informed me that the bill was 90-days over due and that was why it was sent to the Collection Service agency hired by XXXX XXXX XXXX XXXX. I informed her that I received the bill on " XX/XX/2019 '' and that the statement date shown on the bill is " XX/XX/2019. '' Therefore, how could this bill be 90-days overdue, especially when I paid it within 10 days of receiving it? She surmised it was because their billing dept was waiting for my insurance to pay their share of the bill and it went past the 90-days and thus triggered the collection agency into collecting a debt past 90 days. So why am I being penalized for their shortcomings in their billing dept.? XXXX XXXX XXXX XXXX obviously has internal problems with the procedures they follow for their billing protocols. XXXX said she would notify Quantified Mgmt Services that the bill was paid and I should not be getting any more calls from their collection dept. XX/XX/2019 - Received yet another follow up correspondence via usps from Quantified Mgmt Services dated XX/XX/2019 attempting to collect on this same debt of {$230.00}. XX/XX/2019 - Wrote a complaint letter to XXXX XXXX, the Administrator for XXXX XXXX XXXX detailing the harassment I have been receiving from Quantified Mgmt Services.

I was illegally balance billed by an XXXX ( XXXX XXXX XXXX of XXXX XXXX XXXX for an emergency XXXX XXXX XXXX done in the emergency room at XXXX XXXX Hospital on XX/XX/XXXX. He was paid by my HMO but balance billed me for more . California 's Knox Keene act prohibits this and sets settling it between the doctor and HMO insurer. I sent a letter to the doctors billing department quoting the California law and its subsequent California Supreme Court ruling in support of it. I received a phone call from IQMS ( iQualified management services in Colorado ) on XX/XX/XXXX advising that it had been turned over to them collections. I advised the individual I spoke with that this was an illegal billing. She hung up and sent me a collection demand. I sent a detailed letter to them on XX/XX/XXXX describing the situation and the relevant law, and told them to cease and desist all communications with me. On XX/XX/XXXX, I received a letter from IQMS with a bunch of gobbledegoop billing stuff irrelevant to my dispute, violating the federal Fair Debt Collection Practices Act ( FDCPA ) since I had asked them to desist in calling, letter writing or any other contact. Included was a copy of my XX/XX/XXXX letter stating a cease and desist. The Knox Keene act specifically states that ER doctors must accept the HMO patient and if they dispute it, its between the provider and insurer and NOT the patient. Some of the goop I got back from IQMS claims that this is a " coinsurance amount ''. I had already exceeded my out of pocket maximum with my insurer prior to XXXX XXXX bill to me, so there can be no " co-insurance '' charged to me directly. I also had two contacts with XXXX XXXX, both of which told me it was not legal for them to make this collection and XXXX XXXX sent the doctor a letter advising him of such, which he ignored. If you XXXX XXXX XXXX XXXX you will see nothing but one star reviews regarding incorrect billing and practices.

This is for Client Named ; XXXX XXXX XXXX XXXX XXXX ( a/c # XXXX dated XX/XX/XXXX ). I am Resident of California, and have never resided in any other state. My Jurisdiction is California Only. I remember after my accident in XX/XX/XXXX, i was Hospitalized at the XXXX XXXX XXXX XXXX at XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX, CA XXXX. I had XXXX ( on XXXX ), and qualified for Medi-Cal Benefits ( ID # XXXX issued on XX/XX/XXXX ). Until now! I was never informed of any pending BILL from ; XXXX XXXX XXXX XXXX XXXX ( client 's a/c # XXXX ). I have done some research for the COST of XXXX services and XXXX info reads that Cost are less than {$500.00}. The Question is, WHY so much {$1800.00}?? Financial Extortion is a criminal offense! Doesn't Medi-Cal reimburse for XXXX services?? Also, WHY didn't a California Collection company contact me?? I am NOT from XXXX, Colorado! This Collection BILL is way too, overstated vs the actual COST in question. We all know that the XXXX XXXX XXXX XXXX, XXXX, CA is under investigation for Penal Code : 550. They have been using POOR patients as Pawns to Fraudulently Bill the State of California for frivolous sums of monies! Medi-Cal benefits is a FREE benefit for qualified applicants under MAGI Program! Logic tells us that, medical services is reimbursed at Cost plus a small profit Margin less than 100 % on Cost Value, and NOT at 200 % or 300 % plus values. Grand Theft from the state of California is a High Criminal offense! These people have been screwing the State for Millions of US Dollars.

On two occasions I was sent a bill ( XX/XX/22 ) and then a final notice ( XX/XX/22 ) from XXXX XXXX XXXX XXXX. After receiving the first bill on XX/XX/22, I called them and had my XXXX XXXX XXXX representative, XXXX, on the phone. It was determinedthat I was billed for three claims with the same tax ID and two different doctors. Not only that, but the claim was for a hospital procedure when the actual procedure was done in a doctor 's office. Today, XX/XX/22, I received a debt collection bill from iQuantified Management Services stating that I owed for this exact claim. I then called XXXX XXXX XXXX XXXX on the phone for the THIRDtime. I spoke to a manager named XXXX, who went back over the claim that is in need of correction. I even provided him with the corrected claim number ( XXXX ) that XXXX XXXX XXXX XXXX provided me with after my very first call on XX/XX/22 and then again after the second call on XX/XX/22, when I spoke to a man named XXXX XXXX. XXXX XXXX XXXX XXXX never fixed the claim, billed me incorrectly twice, and then sent it to a collection agency. I find these practices unethical and therefore I reported them to the XXXX XXXX XXXX and to you, the Federal Consumer Affairs Bureau of Consumer Protection. As a final note, XXXX, the manager who I spoke to today, on XX/XX/22, stated that he is calling the collection agency and escalating this to his account management team who will, in turn, call my insurance. He assured me that I would not see another incorrect bill or a bill from your collections agency. If I see another bill, I will be seeking legal action. Services and the creditor XXXX XXXX XXXX XXXX XXXX XXXX XXXX WA My XXXX, through XXXX in XXXX XXXX WA was XX/XX/XXXX XXXX is my primary insurance. XXXX sent me 2 bills, saying that I owed them extra monies. I in fact set up a payment plan for the XXXX and XXXX amounts. The insurance to cover the rest. Then on XX/XX/ I received a Collection letter from IQuantified Mge. Services in XXXX, CO to collect for XXXX XXXX XXXX. for XXXX I called XXXX to please contact them, as I was not able to get them XXXX through phone or email. They said they would and that XXXX would get back to me in 5 days.No phone call, no text, etc. I in fact never received a written bill from XXXX. It looks like they just decided to send the bill directly to collections. I feel like I am being railroaded. I have an extremely high credit score and don't wish to have a blemish on my score. I always follow the rules, though some arbitrary, but I do. So I feel like I have been scammed by XXXX. Please, please help me with this problem. And did they even submit my XXXX XXXX??. Many issues have occurred with this vendor, first they called both my husband and I at XXXX local time today XX/XX/XXXX, secondly we had not received any notification in writing before these calls started ( she admitted the first date they sent an invoice was XX/XX/XXXX which is a Sunday ) and as of yet, I have not received it. Her words were also trying to deceive making it sound as though the debt was already in hard collections, and as if I did not have my 30 days to rectify before hitting my credit report, after pressing the issue ( since I work in XXXX XXXX XXXX ) she backed off a bit with the truth. Ultimately very dishonest business practices.

During the months of XXXX and XXXX 2015, I started receiving calls for some unknown debt that the company admits is not in mine. I do not know what this debt is, or who they are looking for. The have continued to call my personal cell phone 15-20 times during this period. I have talked with someone at their company at least eight different times letting them know my name and that they have the wrong phone number. Each time, the person apologizes and tells me they will take my phone number off their list. I have received three calls this week alone. Over the past few months I have been receiving calls from various numbers, including from ( XXXX XXXX XXXX on XX/XX/XXXX and XX/XX/XXXX and from ( XXXX ) XXXX on XX/XX/XXXX and XX/XX/XXXX. They are robo-calls claiming to be from iQuantified Management Services and they claim to be a debt collector. I do not have any unpaid debts so I do not know why they keep calling me. They claim I need to call the number ( XXXX ) XXXX. I checked the website of this company and I don't see that number listed as belonging to them, but I have seen that number described by others on the internet claiming it is a scam number.

in XXXX and XXXX i was contacted by iqmsllc.com. i called and requested a written copy of the debt in XXXX and XXXX ; i received none. on XX/XX/XXXX, i went online to get a copy. i found a notice which had no creditor name and no dates. it did identify {$90.00}. i believe the failure to identify a creditor is illegal.
