In XXXX XXXX leaked the data of 143 million Americans, I was also one of those 143 million. My personal information was used to open several accounts in my name, as a result I have become a victim of identity theft, my overall credit score as well as mental health has suffered greatly.
The following accounts should be removed from my credit file.
1.Interstate Credit Collections {$780.00} 2.Interstate Credit Collections {$780.00} I have disputed this account twice ; this company has not verified this account as belonging to me. The Fair Credit Reporting act requires you to remove this account from my credit file as soon as possible. Under the " Fair Debt Collection Protection Act '', by law, your company should have sent me the following information.
1. An explanation of what this alleged account balance is for.
2.A calculation of the balance, including the complete payment history.
3.Documentation that shows I have agreed to pay this debt.
4.The agreement that grants you authority to collect on this alleged debt.
5.A copy of your state license, including license number.
Your company has failed to provide me with this information, my lawyer has advised me that we will move forward in court in exactly 4 business days if this information is not removed today.
In the event that you do not remove this account from my credit file, I will move forward with the necessary steps to sue you for actual damages and declaratory relief. According to the FCRA regulations, I may sue you in qualified state or federal court, including small claims court in my area.

XXXX/XXXX/XXXX XXXX XXXX referred XXXX bills for services to a collection company, Interstate Collection Inc. After examining the explanation of benefits and speaking to a representative from my health insurance company, it was determined that the dates of service and fees are erroneous. I wrote a letter to the collection company denying bill. I also spoke directly to a customer service representative from XXXX hospital billing office by phone. When I quoted the dates of service and fees listed on bill the representative stated that she did not have that information in the system. Letters and phone conversations have been unsuccessful in getting this matter resolved. This negative information will affect my credit rating and impact the interest rate.

I reiterate -- the insurance company has stated that they do not have any information for dates and fees listed above. I am responsible for deductibles and copays.

This account is involved in litigation for fraudulent practices according to my FCRA rights. I have not supplied proof under the doctrine of estoppel by silence, XXXX XXXX XXXX ( Mo ) 281 SW 715, 719, I may presume that no proof of the alleged debt, nor therefore any such debt, in fact exists. Note that the collector has to stop all collection activities until it can provide all the evidence. Under the FDCPA Section 809, Validation of debts [ 15 USC 1692g ] I have the right to verify any alleged debt on my credit report and if the alleged debt is being disputed, the debt collector shall cease.

I was never sent any bills, but a medical debt from 2011 for {$930.00} is on my credit report. I was XXXX years old in 2011 and on medical assistance for mental health issues.
I asked the collections agency, Interstate Credit & Collections Agency to send me proof of financial obligation. They refused unless I gave them my financial information. I would like it removed from my credit report. They wo n't even tell me what it is for and I am not even sure it is real. I was denied private loan for college and a credit card. I was humiliated.
