NOT 4 TO 24 MONTHS IN A ROW LIKE YOU ARE REPORTING IT!!! AND NOT AS A BRAND NEW COLLECTION EACH MONTH ... YOU HAVE REPORTED THESE ACCOUNT INACCURATELY LONG ENOUGH.. .REAGING OF AN ACCOUNT IS A MISREPRESENTAION OF MY CREDIT HISTOR-Y ... XXXX so DELETE THESE ACCOUNTS NOW!!!! PLEASE!!!!! I have fully researched my rights and found that the Fair Credit Reporting Act does not allow you to victimize innocent credit worthy people by reporting erroneous false information. Under the FCRA, the following applies to me 2011 Louisiana Laws Civil Code CC 3494 Actions subject to a three-year prescription Universal Citation : LA Civ Code 3494 SECTION 2. THREE YEAR PRESCRIPTION Art. 3494. Actions subject to a three-year prescription The following actions are subject to a liberative prescription of three years : ( 1 ) An action for the recovery of compensation for services rendered, including payment of salaries, wages, commissions, tuition fees, professional fees, fees and emoluments of public officials, freight, passage, money, lodging, and board ; ( 2 ) An action for arrearages of rent and annuities ; ( 3 ) An action on money lent ; ( 4 ) An action on an open account ; and ( 5 ) An action to recover underpayments or overpayments of royalties from the production of minerals, provided that nothing herein applies to any payments, rent, or royalties derived from state-owned properties. Acts 1986, No. 1031, 1. civil code sec. 2 art. 3494 Louisiana Also I would like to state you keep deleting accounts and then the creditors add them back to my credit and re age debts that are in fact 3 + years older and also none of the creditors are registered in the state of Louisiana. I have provided proof were accounts have been deleted and creditors are illegally adding accounts back as new accounts. I have also provided proof that you are reporting accounts open that have been charged off and in my state prescription which is only allowed there Years ... do whats right and delete these accounts as you are reporting false documents!!! Procedure in case of disputed accuracy [ 15 U.S.C. 1681i ] 5 ) Treatment of Inaccurate or Unverifiable Information ( A ) In general. If, after any reinvestigation under paragraph ( 1 ) of any information disputed by a consumer, an item of the information is found to be inaccurate or incomplete or can not be verified, the consumer reporting agency shall ( i ) promptly delete that item of information from the file of the consumer, or modify that item of information, as appropriate, based on the results of the reinvestigation ; and ( ii ) promptly notify the furnisher of that information that the information has been modified or deleted from the file of the consumer. B ) Requirements Relating to Reinsertion of Previously Deleted Material ( i ) Certification of accuracy of information. If any information is deleted from a consumer 's file pursuant to subparagraph ( A ), the information may not be reinserted in the file by the consumer reporting agency unless the person who furnishes the information certifies that the information is complete and accurate. ( ii ) Notice to consumer. If any information that has been deleted from a consumer 's file pursuant to subparagraph ( A ) is reinserted in the file, the consumer reporting agency shall notify the consumer of the reinsertion in writing not later than 5 business days after the reinsertion or, if authorized by the consumer for that purpose, by any other means available to the agency. ( iii ) Additional information. As part of, or in addition to, the notice under clause ( ii ), a consumer reporting agency shall provide to a consumer in writing not later than 5 business days after the date of the reinsertion ( I ) a statement that the disputed information has been reinserted ; ( II ) the business name and address of any furnisher of information contacted and the telephone number of such furnisher, if reasonably available, or of any furnisher of information that contacted the consumer reporting agency, in connection with the reinsertion of such information ; and ( III ) a notice that the consumer has the right to add a statement to the consumer 's file disputing the accuracy or completeness of the disputed information. ( C ) Procedures to prevent reappearance. A consumer reporting agency shall maintain reasonable procedures designed to prevent the reappearance in a consumer 's file, and in consumer reports on the consumer, of information that is deleted pursuant to this paragraph ( other than information that is reinserted in accordance with subparagraph ( B ) ( i ) ). ( D ) Automated reinvestigation system. Any consumer reporting agency that compiles and maintains files on consumers on a nationwide basis shall implement an automated system through which furnishers of information to that consumer reporting agency may report the results of a reinvestigation that finds incomplete or inaccurate information in a consumer 's file to other such consumer reporting agencies. ( 6 ) Notice of Results of Reinvestigation ( A ) In general. A consumer reporting agency shall provide written notice to a consumer of the results of a reinvestigation under this subsection not later than 5 business days after the completion of the reinvestigation, by mail or, if authorized by the consumer for that purpose, by other means available to the agency. ( B ) Contents. As part of, or in addition to, the notice under subparagraph ( A ), a consumer reporting agency shall provide to a consumer in writing before the expiration of the 5-day period referred to in subparagraph ( A ) ( i ) a statement that the reinvestigation is completed ; ( ii ) a consumer report that is based upon the consumer 's file as that file is revised as a result of the reinvestigation ; ( iii ) a notice that, if requested by the consumer, a description of the procedure used to determine the accuracy and completeness of the information shall be provided to the consumer by the agency, including the business name and address of any furnisher of information contacted in connection with such information and the telephone number of such furnisher, if reasonably available ; ( iv ) a notice that the consumer has the right to add a statement to the consumer 's file disputing the accuracy or completeness of the information ; and ( v ) a notice that the consumer has the right to request under subsection ( d ) that the consumer reporting agency furnish notifications under that subsection. ( 7 ) Description of reinvestigation procedure. A consumer reporting agency shall provide to a consumer a description referred to in paragraph ( 6 ) ( B ) ( iii ) by not later than 15 days after receiving a request from the consumer for that description. ( 8 ) Expedited dispute resolution. If a dispute regarding an item of information in a consumer 's file at a consumer reporting agency is resolved in accordance with paragraph ( 5 ) ( A ) by the deletion of the disputed information by not later than 3 business days after the date on which the agency receives notice of the dispute from the consumer in accordance with paragraph ( 1 ) ( A ), then the agency shall not be required to comply with paragraphs ( 2 ), ( 6 ), and ( 7 ) with respect to that dispute if the agency ( A ) provides prompt notice of the deletion to the consumer by telephone ; ( B ) includes in that notice, or in a written notice that accompanies a confirmation and consumer report provided in accordance with subparagraph ( C ), a statement of the consumer 's right to request under subsection ( d ) that the agency furnish notifications under that subsection ; and ( C ) provides written confirmation of the deletion and a copy of a consumer report on the consumer that is based on the consumer 's file after the deletion, not later than 5 business days after making the deletion. I became a patient at XXXX XXXX in XXXX, XXXX after challenges I had with my previous dentist. The XXXX was so severe, my extractions required an XXXX XXXX and then dentures. I was referred out to a surgeon for extractions and XXXX was going to provide a lower immediate denture as it was a covered benefit under my insurance plan. XXXX submitted a claim for an exam and XXXX-XXXX which was only partially allowed and paid by XXXX, leaving a balance of {$55.00}. I was unaware of the prohibition against balance billing and I was told I had to pay my balance in full or I wouldnt be treated, so I charged the {$55.00} on my credit card when it was a prohibited practice by XXXX. After an initial denial for dentures because of the wrong coding applied in XXXX, they resubmitted for preauthorization and received a preauthorization on XX/XX/XXXX ( XXXX ). Impressions were taken for immediate denture then extractions were performed by another provider and I self paid because I chose to have implants at the same time on XX/XX/XXXX. I returned to XXXX and had denture fitted and claim was filed on XX/XX/XXXX ( Claim # XXXX ). That claim was denied on XX/XX/XXXX because insurance had no proof extractions were performed ( I had self paid another provider who performed additional work not covered by my plan ). I was made aware of non payment by XXXX and stated I would not pay as it was a covered service with preauth obtained, but I did contact my insurance carrier and requested an appeal. The XXXX billing statement from XXXX clearly shows a patient portion due of {$0.00} which is consistent with what the XXXX XXXX states. On XX/XX/XXXX, I returned to XXXX XXXX who performed extractions for next phase of implants and XXXX shows extractions completed however these were not submitted by XXXX to XXXX because they wanted me to come in to their office and pay them to take XXXX to submit with claim. So, the original decision to deny claim was upheld on appeal and letter was sent out on XX/XX/XXXX. I then received another Bill from XXXX on XX/XX/XXXX showing balance of {$770.00}, the full amount of a covered service, and then a letter not dated threatening to send bill to collections. This constitutes balance billing as I have a XXXX Advantage HMO ( XXXX XXXX XXXX XXXX ) and providers are prohibited under Federal law which bars providers and suppliers from billing an individual enrolled in the QMB program for Medicare Part A and Part B cost-sharing under any circumstances ( see Sections 1902 ( n ) ( 3 ) ( B ), 1902 ( n ) ( 3 ) ( C ), 1905 ( p ) ( 3 ), 1866 ( a ) ( 1 ) ( A ), and 1848 ( g ) ( 3 ) ( A ) of the Social Security Act [ the Act ] ). The QMB program provides Medicaid coverage of Medicare Part A and Part B premiums and cost sharing to low income Medicare beneficiaries. QMB is an eligibility category under the Medicare Savings Programs. Providers and suppliers may bill State Medicaid agencies for Medicare cost-sharing amounts. However, as permitted by Federal law, States can limit Medicare cost-sharing payments, under certain circumstances. Regardless, persons enrolled in the QMB program have no legal liability to pay Medicare providers for Medicare Part A or Part B cost-sharing. Medicare providers who do not follow these billing prohibitions are violating their Medicare Provider Agreement and may be subject to sanctions ( see Sections 1902 ( n ) ( 3 ) ( C ), 1905 ( p ) ( 3 ), 1866 ( a ) ( 1 ) ( A ), and 1848 ( g ) ( 3 ) ( A ) of the Act ). This bill should not be my financial responsibility when the provider failed to adequately complete claim process for a procedure that received preauthorization. The matter has been sent to collections and is now on my credit report which I just learned when pulling my credit report on XX/XX/XXXX. It is a negative item that potentially can keep me from obtaining housing and therefore a matter of considerable importance to have removed from my credit report. I have submitted a corrected claim on XX/XX/XXXX which included the documentation requested by XXXX that the extractions had been performed ( XXXX, treatment plan and receipts for payment of extractions ) in hopes the claim can be resolved.

The only number I received calls from in my complaint below was : XXXX On XX/XX/XXXX at XXXX XXXX ( AZ time ), I was contacted by HCI and when I asked who the call was from I was hung up on, so I called the phone number back at XXXX XXXX ( AZ time ) and was advised HCI Heath Care Collections was attempting to collect a debt. My call was to HCI was answered by an agent that goes by the name of XXXX. I was made aware that XXXX had sent a {$100.00} Emergency Room Co-pay from when when my toddler was sick back in XXXX of XXXX. I was given the login information to the online portal and advised the agent I would make the payment once I had the means to via the online portal. On XX/XX/XXXX, I received another call from XXXX at XXXX XXXX ( AZ time ). I called HCI back and again got XXXX, I had several questions regarding their collection practices and asked that he place a Cease and Desist on my account and went to speak again, but the call was disconnected by XXXX. I called HCI back at XXXX XXXX ( AZ XXXX ) and once again XXXX answered the call. I advised XXXX I still had several questions regarding my account with them and asked that he would please allow me to speak and ask my questions instead of disconnecting the call, or to get me into contact with a Supervisor. The agent became very agitated and was dismissive of my questions. I had asked to speak to a supervisor and the agent refused. I advised the agent that he was being unhelpful and I was the customer attempting to understand how my credit will be impacted. XXXX then advised that " XXXX is the customer, not you '' and I asked where I can forward my complaint to the attorney general and XXXX then advised " You can do whatever you want '' and disconnected the line, again. I am appalled by the actions taken by HCI and how they represent themselves as a healthcare collection agency, as this agent refused to assist me with my questions and educate me on how their process works, so that I could avoid derogatory marks on my credit report and close out the account accordingly. I am genuinely concerned as we are in the midst of a deadly pandemic and I am sure a lot of patients are being sent to collects and I fear they will also be treated in such a manner. Their company website : XXXX XXXX XXXX XXXX that they take a pledge every day that states the following : " I believe that every person has worth as an individual. I believe every person should be treated with dignity and respect. I will make it my personal responsibility to help consumers find ways to pay their debts. I will be professional and ethical. I commit to honoring this pledge. '' Neither was the agent assisting me with my account in a professional manner, nor was the agent acting in an ethical manner by refusing to assist me and continuing to disconnect the line. How are patients able to get actual assistance with their medical debts when the company allows for their agents to act in such a manner and have zero escalation procedures and practices? I respectfully ask that this company be placed under an investigation in order to protect patients of vulnerable populations, as this is inhumane.

On XX/XX/XXXX I returned from a vacation to find a collection bill in my mailbox. I was shocked because I did not know about this outstanding bill. When I called the collection agency on XX/XX/XXXX, I found out that the bill was from service that was performed on XX/XX/XXXX. This was an XXXX that I had for treatment of my XXXX XXXX. My physician had obtained a prior authorization prior to beginning the XXXX in XXXX of XXXX. During the conversation with the collection agency, the representative stated that he could see that I was not billed for the date of service except right after the service was completed in XX/XX/XXXX before it was billed to the insurance. The representative stated he could not see any additional bills being sent to me. As I continued to investigate, I found that the last denial from the insurance company was done on XX/XX/XXXX. The collection agency admitted that they had received the file from XXXX XXXX in XX/XX/XXXX but did not send anything to me until the end of XX/XX/XXXX. The issues that I have with this is that I had prior authorization giving us the approval to have the infusions completed. The insurance denial on XX/XX/XXXX showed that the billing department did not include the authorization number and on XX/XX/XXXX the denial stated not deemed medical necessity yet I received prior authorization stating it was approved. The facility failed to bill the insurance with the necessary documentation allowing it to be paid in full. Because the collection agency sat on the bill for the 180-day parking time it took away my ability to appeal the claim with the insurance company and get the bill paid. Additionally, if there were any uncovered costs the facility was to bill it to the pharmaceutical company who would have picked up any uncovered costs but because of the failure to bill the insurance company the pharmaceutical company was unable to pay the uncovered costs as well. I am including with this letter the documents showing the approval prior authorization letter, the denial letters from the insurance company, additional EOBs showing different dates of services for the same infusion showing payment and information on XXXX XXXX from the pharmaceutical company. I do not feel that we should have to pay this bill since it was an error on the facilities part in billing it to the insurance correctly as well as their delay in notifying me prevented me from being able to appeal it and get the services covered by the insurance. Also as soon as I contacted the Collection company they immediately placed it on my credit as a derogatory claim going back to the time that they received the claim in XX/XX/XXXX. Any help you can give me to get this claim off my credit and preventing the facility from pursuing this further would be appreciated.

I received a collection attempt from HCI collections dated XX/XX/2019 for goods or services that I never requested. I mailed HCI a letter via certified mail saying per fair debt collection act they have to prove that the bill is mine. They sent me a copy of "itemized bill" with someone else's name on it and consider that to be proof, and they are notifying the credit bureaus to affect my credit score. They have absolutely no proof that I requested these goods or services. They have to prove it, I don't have to prove that I didn't. P.S. I worked that day, all day. The company responded with the below information but I don't have a XXXX XXXX policy. XXXX is also attempting to collect this from my wife. It's not identity theft, they are clearly trying to collect the wrong thing if they don't even have the right insurance or responsible party. Please do something about this instead of saying "well they say he owes it so he owes it".Company's ResponseMr. XXXX, The account in question, per XXXX XXXX records, is for your son XXXX XXXX. The insurance used was your XXXX XXXX policy. He arrived at XXXX XXXX on XX/XX/2019 and was discharged at XXXX XXXX on XX/XX/2019. It appears that your wife XXXX XXXX was the person who brought your son to XXXX XXXX. XXXX even reached out to XXXX XXXX on XX/XX/2019 asking that an itemization of the charges and explanation of benefits from the insurance be mailed to your residence. These two documents were mailed out on XX/XX/2019. We have requested the condition of admission from XXXX XXXX and will mail you a copy once it is received in our office. If neither is your wife or son, then we ask that you fill out the attached ID Theft Packet and return it to our office. Thank you XXXX XXXX

A medical debt I owed to XXXX XXXX for {$560.00} was put on a monthly payment arrangement directly with XXXX XXXX* for $ XXXX/month. I paid the debt off in full, early, with the final payment being In XX/XX/XXXX. Payments were as follows : XX/XX/XXXX : {$57.00} XX/XX/XXXX : {$57.00} XX/XX/XXXX : {$57.00} XX/XX/XXXX : {$100.00} XX/XX/XXXX : {$95.00} XX/XX/XXXX : {$200.00} Total payments : {$560.00} Receipts for all payments have been attached to this complaint. No notice of placement with a collection agency had ever been received by me - not by phone, mail, nor email. The only bills I received were directly from XXXX XXXX. This is confirmed and evidenced by XXXX XXXX setting up the payment arrangement *with me directly* for $ XXXX/month ( see attached evidence ), the debt being paid directly to XXXX XXXX per their monthly payment arrangement instructions ( see attached payment receipts ). The payment arrangement I made with them had never been late nor past due, and the debt was paid off in full two months prior to the false reporting by Healthcare Collection Inc. As of XX/XX/XXXX the debt had been paid in full. On XX/XX/XXXX a new collection appeared on my credit reports with all three credit reporting agencies from 'Healthcare Collection Inc ' and showed ' {$57.00} past due as of XX/XX/XXXX '. There is no past due amount and there never was a late payment. After this item appeared on my credit reports, I lodged a dispute with all of the credit reporting agencies ; XXXX has responded stating 'The information you disputed has been verified as accurate. ' confirming that Healthcare Collection Inc is violating the Fair Credit Reporting Act as the information they are reporting is indeed inaccurate and false.

After exercising my rights under the FCRA Fair Credit Reporting Act, and countless letters sent certified mail via USPS, XXXX, XXXX and XXXX have failed to properly validate and verify collection activity on my account. XXXX, XXXX, and XXXX were advised of additional accounts reported by Healthcare collections. I have further disputed that one account made by Healthcare collections is outside of statue of limitations within my state ; original balance reported XX/XX/XXXX that is still being reported against me which is against the law. Other accounts are also being reported by Healthcare Collections have also not been validated or verified. I have disputed several accounts reported by Healthcare collections on XX/XX/XXXX and XX/XX/XXXX via certified mail with XXXX that I have carefully kept record of and no response has been the result of this request for validation. I have yet to receive any documentation bearing my signature showing any form of legal binding or contractual obligation to Healthcare collections debt collector. After being unsuccessful with Healthcare Collections dispute, I have formally requested XXXX, XXXX and XXXX on XX/XX/XXXX to further manually validate and investigate the accounts, in which they have simply updated as accurate without any further clarification on how this was completed. I have received no documentation on any records provided by the credit bureaus that also validate and verify the debt is accurate and mine. I was in the hospital XXXX/XXXX/14 to XXXX/XXXX/2014. There were XXXX different bills that resulted from that visit : XXXX XXXX {$37000.00}, XXXX XXXX XXXX XXXX {$9900.00} and XXXX XXXX XXXX {$550.00}. We have insurance that pays 100 %. Both XXXX XXXX and XXXX XXXX XXXX XXXX submitted their bills to our insurance and were paid in full. XXXX XXXX XXXX, however, said that we did not have insurance and and now have submitted this charge to Healthcare Collections Inc as our responsibility and posted negative to our credit. They will not submit to the insurance and will not remove it from our credit bureau. This bill for {$550.00} was to be paid in full like the {$47000.00} that was paid to the other XXXX bills. We should not be penalized for their lack of processing and someone dropping the ball in their billing department. When I contacted them and asked why they did n't submit to the insurance, they said " We do n't have time to research and find out if people have insurance ''. I advised them that we are 100 % covered by insurance and we submitted our insurance information to the hospital upon check in on XXXX/XXXX/14 and that all the other bills were paid. They said now it is too late and refused to help us. It has almost been a year since I was in the hospital and this charge is barely posting to the credit report?! This office obviously has billing problems. We were never notified of any issues with the billing and did n't expect to be since our insurance covers 100 %. We are currently trying to buy a house and this is severely affecting our credit. Please help us resolve this situation. Thank you.

At XXXX XXXX on XX/XX/2021 I received a call from a number I wasn't familiar with. The lady on the phone asked for me by name. I asked wgo she is and where she was calling from..she gave me her name but no business name ... she wouldn't give me the name of her business..she just kept asking if I was XXXX. She even insisted that they have spoken with me before..I asked a THIRD TIME for the company name ... at this point I told the representative that when asked the name of her company, she HAS to give me the name. She then said " nevermind '' and hung up on me ... I called the number back and the answered buy a different name then hci ... I talked to a rep and explained what happened and told her its and fdcpa violation and she snickered like I was wrong ... I asked for their dept that handles these types of violations and the said they have no such dept..and they have no ways of handling these types of complaints. I have called them several times to ask them how the handle these violations and none of their reps can answer me other than giving me another number to call to speak to a " manager '' ... I am appalled that there are collection company still allowed to break with the fdcpa laws..and have no repercussions ... something needs to be done here..this is completely illegal behavior..THEY DID NOT SWEAR AT ME ... but I selected the 'communication option ' as they broke the law by not giving the name of their company when asked three times and hanging up on me

I got a letter from this collection agency XXXX XXXX XXXX the letter state that I owe them a debt for {$250.00} was dated XX/XX/XXXX. They said I must notify this office in writing within 30 days from receiving this notice that if I dispute the validity of this debt or any portion thereof, this office will obtain verification of the debt or obtain a copy and mail it to me. I then sent them a certified letter with a signature receipt on XX/XX/XXXX asking them to do so. I request they send me something in writing with my signature verifying that I owe them this debt. I then received another letter from them stating that they got the okay from their client to add this to my credit report again I have not received anything from them but yet I get alert on my email on XX/XX/XXXX telling me a new collection has been added on my credit report. How can they add this to my all my credit report without first sending me something with my signature verifying this is my debt? I feel the company violated my rights under the FDCPA. I have not received any letter from this company to explain or etc and I notice they placed this collection on all 3 bureaus without validation letter. I am going to report this action to Attorney General and the Better Business Bureau today to see if can get this off of all my credit reports. This needs to be resolved immediately as this is majorly affecting my credit scores.

I do not recall giving permission to Healthcare Collections or XXXX XXXX XXXX XXXX XXXX for them to release my medical information to a third party. I am aware that the HIPAA does allow for limited information about me but anything more is to only be revealed with the patients authorization. Therefore my request is twofoldvalidation of debt and HIPAA authorization. Please provide breakdown of fees including any collection costs and medical charges. Provide a copy of my signature with the provider of service to release my medical information to you. Attached please find a copy of the response from Healthcare Collections Inc., included in the response is the letter I sent to the company and initials indicating they recieved it. I have blacked out any personal information contained on the bills and am attaching them to this complaint for the only purpose to show the HIPAA violations. I have not recieved nor have I ever signed paper releasing my medical information. Also I was covered under the Arizona Medicaid program XXXX health care at the time of this hospital bill. The insurance information was provided to the company at the time of service. I was not notified that this account was in arrears or that it would be sent to collections. The bottom line is that XXXX should have been billed for this service, and companies are not allowed to balance bill a patient with Medicaid.

I asked for validation of this account to include the original contract with my signature and identification and address. i never received a response from HealthCare Collections LLC within 30 days.They are still reporting the account to my credit report as accurate which is a violation Under the FDCPA, you have the right to debt validation. This means a consumer can demand that a creditor reporting information to the credit bureaus prove the account is really your responsibility and that the balances are accurate. Once your letter is received, the collection agency must stop all collection activity, including reporting and verifying, until they supply proper validation of the debt. If the collection agency failed to validate the debt and has sent a notice of the debt to a consumer reporting agency, the notice of the debt must be removed. If not, send a notice of the failure of the validation to each consumer reporting agency that lists the debt. Unless the credit reporting agency can validate the debt, it should be removed from your credit report. The original debt was with XXXX XXXX. XXXX XXXX had set up a payment plan to pay {$43.00} due XX/XX/18 payment made XX/XX/18. Letter from HCI medical received was dated XX/XX/18. I started receiving calls before the 30 days given to dispute the debt. The times called they were rude and would not disclosed nature of call. I spoke to XXXX on XX/XX/18 and informed her of the repeated calls and rudeness. She apologized and we set up a payment arrangement of {$50.00}. I made another payment on bv XX/XX/18 of {$50.00}. Dear Sir/Madam, XXXX ( healthcareinc ) is attempting to collect a debt. Their tactics are either laughably incompetent or connivingly sinister in their methods. I received a letter of collection from them that clearly stated how to pay. I WILL NOT REGISTER with them or call them, I believe every transaction and communication should be documented in writing. When attempting to pay at XXXX XXXX XXXX I was unable to do so. The account number had ambiguous text or numbers, and even what looks like capitalized and lower case letters. After trying every single possible variation ( even improbable ones ), I came to the conclusion that they do not want you to be able to pay using their " Want To Pay Without Enrolling?, One-Time Payment '' option. Interestingly, there is NO WAY to contact them by email or messaging. Please try to connect to their website with the information on the letter they sent me, I'm sure you'll find it to be impossible. If this company has the ability to place a derogatory mark on one 's credit history, then some minimal level of competence should be met, otherwise it SHOULD BE ILLEGAL! I shudder to think what additional information they may have and in what ways they may be misusing them.

HCI Healthcare Collections Inc has responded to a complaint I made through CFPB with a violation and you just closed the complaint. They say I owe {$750.00} which they reported to the 3 credit bureaus as well negatively impacting my credit score and life. On XX/XX/XXXX of XXXX I send them a request for validation of debt letter. They responded on XX/XX/XXXX of XXXX with a verification letter and an itemized XXXX bill from a XXXX, not a validation of debt. On XX/XX/2022 I sent them a request of my consent of HIPAA proving they have the right to have my medical information. They did not respond. They responded to the complaint I made to CFPB on XX/XX/XXXX stating they have a signed authorization from me. I have submitted in a complaint before and have provided the requested documentation in a addition have disputed these two accounts for two years. On XXXX/XXXX/XXXX I went to XXXX Hospital XXXX XXXX, XXXX, Arizona. Services were rendered, I paid the {$100.00} copay, the hospital sent XXXX XXXX the electronic file for the claim. On XX/XX/20, I paid all outstanding medical debts by phone to Healthcare Collections Inc. I was offered a cash discount ( I did not ask for this ) and accepted it. I was given a confirmation number : XXXX. I was told that the items would be removed from my credit report XX/XX/20. On XX/XX/20, HCI reported the cash discount as an outstanding debt AND as a late payment. I spoke with them again today, and was informed that I paid it off at the wrong time - that they require two weeks to charge off the discount THEY offered, and that the fraudulent item they have reported to XXXX will be removed XX/XX/20. When I stated that I was trying to buy a house and this fraudulent item was affecting that, the collection agent said " Well, that's the way it goes. '' When I informed him that I would be filing a complaint with CFPB, he hung up on me. I find this medical debt reported on my credit report, but I do not remember ever owing a bill with a remaining balance of {$7.00}. It looks like the company dated the opening of this debt XX/XX/2010. I looked for the collection company on line, but it is as though it does n't exist. It appears to have impact on my credit score. I am willing to pay it if I could get an itemized bill showing the date of the procedure and when and what my medical insurance company reports about it. I have recently checked my credit report over the last few months after years of never trying to get credit nor looking at my credit report. I was awarded XXXX in XXXX and they back dated it as XXXX as of XX/XX/XXXX and I was covered under Medicare ( Primary ) and Medicaid ( Secondary ) as my Health Insurance. The 2nd lette r indicates date of service as being X/X/16 . I was NO longer in the hospital at the time. As I indicated on my XX/XX/16 letter, I had XXXX XXXX but as soon as I no longer had insurance due to some error on their part I was released from the hospital although I was still in a compromising condition I consider this a negligent behavior on part of the medical center.

This was NOT a request for verification or proof of my mailing address, but a request for VALIDATION I respectfully requested that their offices provide me with competent evidence that I have any legal obligation to pay them. < P/> Their offices have reported invalidated information to any of the 3 major Credit Bureaus ( XXXX , XXXX or XXXX ) this action might constitute fraud under both Federal and State Laws. Hi, I was sent to collections because I moved and never received the medical bill. As soon as I found out about it, I paid it back in XXXX. The collections company ( see below ) informed me that they sent all 3 credit bureaus a letter back in XX/XX/XXXX asking them to remove the collections from my credit file. A debt collection account with Healthcare Collections showed on my credit report for {$2000.00} from a 2017 medical debt. When I called them, they said they were unable to locate an intemized statement and directed me to call the hospital for information. When I called the hospital, they said the bill was from my adult son 's care in XX/XX/2017, but that it seeemd a bill would not have been received in the mail as they had symbols in error on the address line. I was unable to receive further information about this bill, as Healthcare Collections said they didn't have the itemized statement, and the hospital said they wouldn't discuss it with me since it was for another adult. In the meantime, it is on my credit report, and has caused all my student loans to be denied this semester, and will result in my student loans being denied going forward.

This debt collector, HCI Healthcare Receivables Management Company, in spite number XXXX, having absolutely no proof that the debt is mine, continues to attempt to collect a debt account from me for services on XX/XX/2019. They have reported it to credit reporting agencies and now I'll have to take the next step, contacting the reporting agencies directly. I was not there and they do not have my signature or any proof that this is my bill. If this is not corrected and soon, I will additionally sue this unethical debt collector and Consumer Financial Protection Bureau for failing to help me. I contacted the credit agency after viewing on my credit report that a bill had been sent to collections that I was not aware of years prior. I researched the company and called them. I paid the debt immediately and asked for a letter stating I had paid the collections and was requesting it be deleted from my credit report. They had not done so. Recently I called and spoke to a supervisor who advised me that I would not get a letter of deletion now even though I had requested one. He stated there was no way he would send a letter of deletion because my account was not paid in full, however, I had asked for the debt to be paid and when on the phone with the collector, it was communicated that I needed the debt off of my credit report and I therefore paid the amount they requested I pay. This is the third time I have complained about this exact same issue. The first time the collection agency said " oh he owes it '' and that satisfied CFPB. Last time I said that it's not my insurance and the collection agency admitted that they had nothing signed by me and that satisfied CFPB. I reviewed my credit reports with XXXX, XXXX and XXXX and found out that Healthcare Collections LLC had been attempting to a collect a medical bill debt for {$880.00} against me. I never received a notice to dispute from the collector or original creditor. I sent a letter to Healthcare Collections LLC on XXXX/XXXX/15 letting them know I dispute their claim under the FDCPA and am allowed to request validation from third party collection agencies. On XX/XX/2020, I mailed a request to validate letter to the collector, HEALTHCARE COLLECTION INC, with certified mail receipt. They had until XX/XX/2020 to reply within the 30 days. I did not receive any response from the collector and therefore submitted a dispute with the three credit bureaus for investigation. I received a notification from my bank in mid XXXX that there was a change in my credit report.
