I was in an accident rear ended on XX/XX/XXXX. My pain management doctor, XXXX XXXX XXXX. XXXX, started billing my PIP through XXXX XXXX claim # XXXX. I had NO CHANGE from preexisting conditions or treatment. I never got even a single bill from the doctor, but when the PIP ran out they demanded money to be seen. They then placed the Illegal debt with Frost Arnett, who I called, and formally disputed this account. It has been over 60 days and they have not responded to the dispute, or addressed the dispute, or sent any mail whatsoever, but just did recently send the billings that XXXX XXXXXXXX Sent them. THEY ARE OUT OF TIME FOLLOWING A DISPUTE TO CONTINUE AND MUST CEASE. ATTACHED AND FOLLOWING HERE BELOW IS THE LETTER I SENT FROST ARNETT : XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX, Florida XXXX XXXX XXXX XX/XX/XXXX CL # XXXX claim # XXXX XXXX XXXX XXXX XXXX XXXX, TN XXXX Phone XXXX XXXX Frost Arnett account number www.frost-arnett.com XXXX XXXX XXXX XXXX, Tennessee XXXX On XX/XX/XXXX I was rear ended. I filed a claim and went to the ER to get checked out. Shortly thereafter, upon my regular visit, I informed my XXXX XXXX doctor, Dr. XXXX Office, about the accident, and gave them a release to get the ER information. I had minor XXXX XXXX, and apart from that, no new injuries. My doctors office began to bill my PIP through XXXX XXXXXXXX. I have been informed this PAYS MORE to the doctor for most procedures. I WAS NOT TREATED FOR ANY OF THE TEMPORARY DISCOMFORT IN MY XXXX. I got no procedures, no medication, no examinations beyond the initial cursory, brief examination in early XXXX. Further, at no time, to date, have I ever gotten a single billing, or notice of billing, or indication I had been getting billed, in any manner, either verbal, written, or electronic. For all purposes, and in all respects, my treatment through XXXX XXXX XXXX and XXXX XXXX in XXXX or XXXX XXXX, there was nothing to indicate that my treatment for my lower back and other issues, which were pre-existing and ongoing, had changed in any manner, shape, or form. In XXXX of XXXX I assume the PIP funds had run out, and was told that I would not be seen until over {$1200.00} was paid. I was not given or offered continuity of service and care. I was told I needed to talk to the office manager, and made multiple calls and left multiple messages about this situation, but was never called back. My account was handed off to a collection agency. I asked for copies of billing ( from Frost-Arnett ), which I have just gotten. I also asked for these documents from XXXX XXXXXXXX, which came much sooner. I had requested the documentation from XXXX XXXXXXXX on XX/XX/XXXX. It came within 10 days. I requested the same from Frost-Arnett, which has just come via email approaching 60 days after the request and initial dispute of this bill and account. This more than exceeds Frost Arnett fulfilling their duty to respond, in writing, to this dispute within 30 days, and exceeds the 15 day window following that date. Frost-Arnett has not verified the debt or addressed the dispute. Under the FDCPA I have the following rights : # 1. Right to notice of the debt. Frost-Arnett has not sent any mail or communication other than one email which was late. I contacted them after the client, my doctor, told me that was who would handle it. # 2. Right to contest the debt. I exercised this right MORE THAN TWO MONTHS AGO, and none of the merits of my dispute, clearly laid out to Frost-Arnett over 60 days ago, were addressed. # 3. Right to verify the debt Frost-Arnett has not verified the debt. They have not checked with XXXX XXXX or XXXX XXXX, nor addressed that the things that their client, XXXX XXXXXXXX XXXX, billed XXXX XXXX for under my PIP coverage had been and were PRE-EXISTING and that no change in my care or treatment had taken place subsequent to my claim with XXXX XXXX. Under the FDCPA Frost-Arnett has lost all legal right to pursue this matter. Frost-Arnett, and their client, XXXX XXXXXXXX XXXX, are hereby notified that any negative credit reporting will be considered to be HARM and undue harassment and will be met with litigation for same, and it is my view that the billing of my PIP under XXXX XXXX is either an egregious mistake, gross negligence, or fraud. I demand that XXXX XXXX XXXX return all monies claimed under XXXX XXXX PIP from XX/XX/XXXX forward be returned to XXXX XXXX forthwith, that all claims for any monies from this matter be withdrawn. As stated, as of this writing, I have not received not one single billing or notice of billing, or collection of said bill ( ings ) from any insurance agency, or XXXX XXXXXXXX XXXX and Dr. XXXX, or Frost-Arnett. I certify that I did call Frost-Arnett about this matter the very day Dr. XXXX office told me that they had assigned it there and entered a dispute at that time, which is evidenced by the extremely late and out of time email containing the billings from XXXX XXXXXXXX XXXX. In that email or PDFs the dispute itself is not addressed. I have also, again today, informed Frost-Arnett phone representatives as to the dispute and merits, and demands within, my dispute. I asked that agency for their legal department, for a Fax number or email address, and was directed to their website. It appears they can send emails late, but not receive emails through regular email logistics, or even send mail concerning this matter. I will authorize Frost-Arnett and/or XXXX XXXX to contact my providers, XXXX XXXXXXXX XXXX, & XXXX XXXX to attain past billings that PROVE that all lumbar treatments and procedures were PRE-EXISTING AND SHOULD NOT have been submitted to the PIP coverage of The General, but should have gone to XXXX XXXX I have lost continuity of care and am in excessive pain because of this situation. I am also highly stressed out, can not sleep properly, and have lost enjoyment of life, my relations with my wife, and interactions with my child, due to the break in care and medication. I will be addressing this matter with my Primary Care Physician shortly, as XXXX said there is nothing they can do, and I am seeking the proper avenue to proceed on state regulatory levels. Our letter also indicated that we are disputing the validity of the debt in the amount of {$1300.00} because XXXX XXXX Medicare was in full effect as of XX/XX/XXXX. We further explained that Medicare had denied many of XXXX claims initially because of an error in their system, but that the error has since been corrected and that Medicare is now paying all claims. Continued to receive various collection calls from Frost-Arnett XX/XX/XXXX Received a collection call from XXXX at Frost-Arnett. I explained that weve sent three certified letters, but XXXX said they did not have any letters from us. I asked to speak to a supervisor. XXXX XXXX XXXX took my call. She also said that they had no record of receiving certified letters and asked what address we sent them to. I told her we sent them to XXXX XXXX XXXX, XXXX, TN XXXX ( as listed on their collection invoice ) and I had the USPS return receipts ( with signatures ) in my hands from all three letters. She confirmed that the address was correct, but couldnt explain why our letters werent in their system. XXXX XXXX requested that we mail all the letters, back up and copies of return receipts to her attention at their physical address at XXXX XXXX XXXX XXXX XXXX, XXXX, TN XXXX. I again explained to XXXX XXXX that XXXX XXXX Medicare coverage was in full effect on the date of service ( XX/XX/XXXX ) and that we asked XXXX XXXX to rebill Medicare because his coverage was in effect on XX/XX/XXXX. I explained that Medicare did have issues with XXXX account ( due to Medicare not setting it up properly ) but that all claims that were initially denied were now being paid. XXXX XXXX also gave me her telephone number XXXX and said that she will closely follow this case and I can call her at any time. XX/XX/XXXX Send certified letter # 4 sent to XXXX XXXX XXXX at Frost-Arnett. XX/XX/XXXX Called XXXX XXXX at XXXX. She did not answer so I left a detailed message ( with master account number XXXX and my telephone number ) and asked that she return my call. XX/XX/XXXX XXXX XXXX had not yet returned my call, so I called her again and left another detailed voicemail. Continued to receive collection calls from Frost-Arnett.XX/XX/XXXX XXXX XXXX had still not returned my call, so I called her again and left another detailed voicemail. XX/XX/XXXX Because XXXX XXXX was not returning my calls, I called the main number at Frost-Arnett at XXXX. I spoke with XXXX. XXXX confirmed that they did in fact receive our certified letter that was sent to their physical address and that it would take 30 days to review the account. XXXX said that we need to get a letter from Medicare stating that they will cover, because as of XX/XX/XXXX XXXX XXXX stated that Medicare still will not honor the claim. XX/XX/XXXX At approximately XXXX XXXX, I called and spoke to a supervisor named XXXX XXXX. XXXX gave me her direct line of XXXX. XXXX asked that I give her 24 hours to review the file and said it would be helpful to her client, XXXX XXXX, if we could provide Frost-Arnett with a letter from Medicare confirming coverage. XX/XX/XXXX I contacted XXXX XXXX to let her know that I had a letter from Social Security indicating that XXXX is covered by Medicare, effective XX/XX/XXXX. I also told her that I found the benefit summary on Medicares website, and it appears that XXXX XXXX billed the wrong Medicare number. XXXX XXXX instructed me to upload the documents to their website and requested that I follow up with her on XX/XX/XXXX. The documents were successfully uploaded at approximately XXXX XXXX. XX/XX/XXXX I contacted XXXX XXXX to confirm that she received the documents I uploaded to their website. She said she did receive them and forwarded the documentation to XXXX XXXX.

On XX/XX/22, I received a letter of debt owed to XXXX XXXX from Frost-Arnett Company XXXX XXXX, TN. On XX/XX/22, I called and made a payment of {$25.00} for ( 5 ) small accounts from a XXXX, thinking these are so small and I could have overlooked these items as I healed. However, I questioned the {$260.00} bill to XXXX XXXX due to meeting maximum out of pocket and copays pre- and XXXX I explained to Frost-Arnett on XXXX XX/XX/22 call that I will be reaching out to XXXX XXXX XXXX XXXX to resubmit the XXXX XXXX bill and never had any follow-up from Frost-Arnett after this conversation XXXX No additional correspondence or phone call from Frost-Arnett led me to believe the XXXX bill had been paid. On XX/XX/22, I received a credit alert that an unpaid debt was reported. If you pull all of my credit reports, you will see ( XXXX ) late payments for over two years. Please compare this to a XXXX search for Frost-Arnett with 72 reported ratings, leaving their agency a customer service satisfaction of two stars. It appears I am not alone with the following complaint, with other consumers reporting similar issues with Frost-Arnett. I went online and submitted a complaint to Frost-Arnett on XX/XX/22. I followed up with a phone call to Frost-Arnett on XX/XX/22, speaking with XXXX. XXXX did not see my submitted complaint in their system. XXXX saw in their system the letter from Frost-Arnett dated XX/XX/22, my call on XX/XX/22, and no further correspondence or phone calls to me from Frost-Arnett after this XXXX I asked for an itemized bill from XXXX XXXX and for someone to pull my call to Frost-Arnett from XX/XX/22. XXXX said she can not pull a call, but can ask a supervisor to do this, and will email an itemized bill to me. On XX/XX/22 I called XXXX XXXX and spoke to XXXX due to not receiving an email from Frost-Arnett as promised on XX/XX/22 by XXXX. XXXX transferred me to her supervisor, XXXX. XXXX was very kind and said she was going to speak to her supervisor and call me back at XXXXXXXX XXXX. about the limited communication regarding this debt. XXXX did return this call and stated that her supervisor, XXXX, told her to relay to me that the Credit Bureau stated on XX/XX/XXXX that all reported debt with a {$0.00} balance will be removed from credit reports. Now I'm all the more concerned. Does this mean that Frost-Arnett does not report paid in full to credit bureaus for which they have turned consumers over for bad debt after a debt has been paid in full? Frost-Arnett just expects consumers to have any negative credit debt ( legitimate or not ) reported by Frost-Arnett to be auto-corrected by the credit bureaus themselves? My complaint is that I, once again, request the call to Frost-Arnett from XX/XX/22 be pulled. Frost-Arnett states each phone call is recorded and XXXX agreed to ask her supervisor for this. There was no follow-up to this request. The request was ignored. I request, once again, an itemized bill from Frost-Arnett for XXXX XXXX XXXX I request, once again, clarification as to why I was never contacted by phone or in writing after one letter was generated by Frost-Arnett on XX/XX/22. I called Frost-Arnett immediately upon receiving this letter on XX/XX/22 and this is documented in their system. During this call I state that I'm reaching out to XXXX XXXX XXXX XXXX to pay this bill since I met maximum out of pocket. I thought this was noted on my account during the phone call with Frost-Arnett XXXX The employee handling my collections case did not discern that at least one additional phone call or one follow-up letter would be necessary, informing the me that the debt remained, that XXXX XXXX XXXX XXXX XXXX not pay as we discussed during my last call to Frost-Arnett? Due to Frost-Arnett 's lack of communication with consumers, I was left to believe the debt was no longer an issue and XXXX XXXX XXXX XXXX XXXX XXXX XXXX. I request to be shown that this debt is owed in writing and not just dismissed by the manager of Frost-Arnett XXXX XXXX, who relayed to me through another employee to simply pay the debt and the credit bureaus will automatically remove the debt from my credit reports because the credit bureau stated such on XX/XX/XXXX. What? His consumer service message does not address any of my requests to prove the debt. His consumer service message does not address the issue of only one correspondence from Frost-Arnett regarding this debt after the consumer, XXXX XXXX XXXX called Frost-Arnett for guidance about this debt. XXXX 's consumer service message/solution completely dismisses the consumer 's concerns about this debt, leaving the consumer no choice but to pay the medical bill if the consumer wants the credit reports corrected and that Frost-Arnett will not ensure XXXX negative affect on the consumer 's credit report is correctly reported as paid. That is on the consumer and the credit bureaus to correct.

I had a scheduled checkup last year ( XX/XX/XXXX ) at her XXXX for which I had to be put under general anesthesia during the checkup. We had scheduled this appointment after assurance by the doctor 's office [ which is in network of our health insurance provider ] that everyone involved in the checkup procedure will be billed as in-network. We were given the name of the XXXX who gave general anesthesia to me on day of the appointment, Anyway the check up went fine and waited for the explanation of benefits ( EOB ) from the health insurance and bill from the providers to pay for what were expected to be in network charges. Sometime mid-year ( XX/XX/XXXX ), we ( me and my spouse ) got an EOB letter from the health insurance saying that the charges from the XXXX will be calculated as out of network since the XXXX is out of network. This was the first time we even came to know the name of the XXXX. Facing a bill of around {$3000.00}, we called the XXXX office and asked them why they did not inform us prior to the procedure that they were out of network of my health insurance.They told that they were working with the health insurance to claim the bill as in network since the procedure was performed at a doctor 's office location that was in-network. Since the health insurance did not pay to the XXXX office for the claim that the XXXX office sent them, the XXXX office sent us a bill last month ( XX/XX/XXXX ) with adjustments and asked us to pay around {$500.00} to settle the bill after adjustments. We asked them to provide me breakdown of how they calculated the {$3000.00} bill and when they sent me the details, we was astonished that billed me incorrectly stating the time for the procedure was 87 minutes, even though we clearly remember is being under 30 minutes. When we asked them on the phone about why they mischarged the bill, the XXXX spoke very rudely with us said that they were not going to discuss any further details and threatened us to either pay up the {$450.00} ( since we had paid {$50.00} on the day of the procedure itself to the XXXX office ) or they would send my bill to a collection agency. We asked them to work with us and correct the bill for the service they provided but they just hung on us and stopped responding to our phone call. Recently we got a chance to speak with them again on the phone and they told us that my bill was handled by a different office and gave me a XXXX number to call. When we called the provided number, the person responding to the call mentioned that they are a collection agency. Now we believe that my rights as a consumer have been violated since there were mistakes done by the XXXX office at multiple levels which I have listed below : 1 & gt ; Since the checkup as a scheduled checkup, the XXXX office should have informed us that they are going to charge me as a out-of-network service which they did not do. 2 & gt ; I believe that they billed me incorrectly stating the time for the procedure was 87 minutes, even though this was a simple procedure which definitely did not last 87 minutes. 3 & gt ; When we was trying to work with the XXXX office after receiving their bill with adjustments to pay {$450.00}, I was trying to figure out how they arrived at the original {$3000.00} bill that they claimed, but they refused to provide me any further information and sent the bill to a collections agency to make it impossible and inconvenient for us to challenge or question the bill they they charged. I believe is again a violation of my rights as a consumer of health services and the XXXX office did not handle my account properly as required by law.

I received a call regarding an unknown debt and letter on XX/XX/XXXX. Subject : IDENTITY THEFT STATEMENT, DEMAND FOR SETTLEMENT Account Number : Disputed Debt Collector Account # XXXX and/or Original Creditor Account # XXXX To Whom It May Concern, Under the Fair Credit Reporting Act ( FCRA '' ), Fair Debt Collection Practices Act ( " FDCPA ), Fair and Accurate Credit Transactions Act ( " FACTA '' ) I am exercising my lawful right to request the removal of any trade lines of this account on my credit report, including inquiry records and collection notices being reported to the four National Credit Reporting Agencies ( `` CRA '' ) : XXXX, XXXX, XXXX, and XXXX. This request is due to the fact that the above-referenced account is fraudulent and a case of identity theft. I did not put this in writing sooner because I was unaware of the fraudulent activity until I contacted the original creditor. I also received, in writing, a partial debt validation from your agency. Besides, I did not know what recourse was available to me as an identity theft victim. I HEREBY AFFIRM 1. I did not authorize anyone to use my name or personal information to seek the money, credit, loans, goods, or services described in the account listed above ; 2. This began when I had to find a new XXXX who performed XXXX / XXXX because my doctor moved out of state. When I initially began seeing this doctor it was from a referral of a XXXX that did not perform XXXX. Due to the XXXX I can experience, at times it requires I take XXXX. As this clinic deals specifically with XXXX, it would be clear that their patients may, at times, be in a XXXX and XXXX the information on the forms they complete for the physician. At no time was I verbally informed, or any signage present, to make clear this doctor, who was XXXX out of a freestanding office building, was operating and billing his practice as XXXX services. After a few visits, I saw my medical EOBS and immediately discontinued seeing this physician. Then I began speaking with their billing service about making a lump payment arrangement for the XXXX visits once all medical bills have been run thru and paid out accordingly by my insurance company. Their billing company supposedly agreed. However, they did not file the last visit, with my insurance co, in a timely manner and decided to send my balances to a collection agency. That agency, Frost-Arnett, sent request for money without any documentation other than provider name and lump sum. I paid XXXX visits and those were supposedly resolved. After I began reviewing my insurance EOBS against their collection claims, I realized they were starting out with amounts that were not valid according to my insurance company. My doctor is contractually only allowed to collect from me what he agrees with insurance. Frost -Arnet was hired to collect more than I was contractually obligated. Since we had come to agreement on a reduced amount for the XXXX visits, I felt that fair and did not pursue adjustment. However, on the unpaid visit that eventually was processed thru insurance, they are attempting to collect 3 times what the EOB states I am required to pay. The final visit had no obligation for the insured as I met all out of pocket/deductible expenses. Since I knew they were attempting to collect an invalid amount, I requested Frost-Arnet provide an itemized statement from the doctor. What was provided, did not match what was valid financially for that visit. I sent, via confirmed fax, a letter of dispute with insurance EOB proof that the amount was invalid. I requested they contact doctor and correct amount for me to consider pmt. They ignored this request and sent another pmt request with the suggestion I use tax refund money to pay it. In addition, the wording lies in that it states they have not been able to contact me. I have regular documentation of names, dates, and fax confirmation to prove there has been LOTS of contact. Therefore, I submit they are operating under lies, misrepresentations, and character XXXX. As they are contracted to provide this service for the doctor 's billing, I hold all parties involved equally responsible and they need to cease this fraudulent collection attempt and lies that I have not made effort to resolve this matter. I will make payment against the valid amount and wish to be done with all parties related to this matter. I refuse to do so until the amount of collection reflects the amount the doctor can contractually collect. Please help get them to comply with how they should be operating under the Fair Debt Collection practices.

1 ) I was contacted by a company which I have never given any permission to my personal medical or personal information with regards to a Debt. I strongly feel that my HIPAA privacy and sercurity has been violated with regards to this company by the name of FROST-ARNETT COMPANY ; their address being XXXX XXXX XXXX XXXX, TN XXXX. Their phone number listed XXXX & XXXX. 2 ) They contacted me trying to collect on a debt, asked me for my social security and full name saying that they have all my information of an existing medical debt that they need to collect asap. I was seen at XXXX Texas XXXX XXXX for a XXXX exam on XX/XX/XXXX. I was initially charged an amount of {$75.00} which I paid off in full, I then continued to check my account to make sure no new charges were added over the next year. All of a sudden on XX/XX/XXXX, more than 22 months after the date of service, a {$120.00} charge was added to my account for the XXXX exam almost XXXX years prior. In going through my previous statements, I found none of them contained this charge or any amount due and that the XX/XX/XXXX statement was the first time I was billed for this and notified. The Texas " Timely Billing '' law ( TX XXXX Practice and Remedies Code CH. XXXX ) requires health care service providers to bill a patient no later than the first day of the XXXX month after services were provided. If the bill is not sent within the timeframe in the law, the health care service provider can not try to collect payment for certain charges. On XX/XX/XXXX I called Ascension to inform them that this bill was sent in violation of the aforementioned Texas XXXX XXXX and likewise dispute, to which they replied that their negotiations with my insurance where why it was charged late and claimed that meant they could still charge for it. The Texas XXXX XXXX makes no such exceptions for insurance negotiations, and when I informed them that they still hadn't given any reasoning to why the bill was still valid despite being sent out 22 months after the date of service ( almost twice the allowed 11 month window ) they said that they would still charge it anyways. Eventually it was sent to XXXX XXXX where I was contacted by Frost-Arnett XXXX I submitted a dispute online on XX/XX/XXXX detailing the above information and asking them to verify that this bill was not sent out in violation of the Texas Timely Billing XXXX. I then received no confirmation messages or emails from them and had to call XXXX times over the next week to verify that it had been received. Once I got confirmation from them that it was being processed I had to wait until XX/XX/XXXX until I received the dispute response, with the letter being dated for XX/XX/XXXX but only postage stamped a week later on XX/XX/XXXX. I had asked that they send any statement showing from the 11 month window after the date of service, however they instead sent me a statement for 25 months after the date of service. This actually provided more evidence that this bill was sent in violation of the Texas Timely Billing Law however for they still listed it as verification and upheld the charge. I then called on XX/XX/XXXX to inquire further and discuss their verification, however nobody picked up and I had to leave a message. As of XX/XX/XXXX I still have not received any response nor has the company been adequately able to rectify the stated issues. As a result I have had no choice but to turn to the CFPB to file a complaint in the hopes of having the charge removed.

The company provided the itemized bill and said the debt was validated, but they did not have records of the payments and did not accept our bank records showing the payments were made. In XXXX of 2021 I had blood work done, in XXXX of 2021 I received a bill from XXXX XXXX XXXX. I entered a payment arrangement with them and made {$25.00} payments in XXXX, XXXX, XXXX, XXXX, XXXX, and then in XXXX they took a little over {$13.00} in the automatic payment and then discontinued the payment plan. To me that means it was paid off. I didnt cancel the payments and I fulfilled my end of the agreement to pay {$25.00} a month until the balance was paid off. A few months ago we got a notice in the mail that the account had gone to collections and stating no payments had been made on the account. I disputed it, requested verification of all payments, sent bank statements showing two of the payments, and notified them of the violation to the FCRA in that XXXX XXXX XXXX sent the balance to collections without first notifying us there was an issue with their billing. From a consumer protections standpoint this appears to be fraud, as we made regular payments and they are claiming no payments were made. They sent an itemized bill on XX/XX/XXXX and said the debt was valid and they would continue collection attempts. I called them and asked about the payments we made, they said they didnt have any record. I asked them to pull up my dispute, they verified they had records of two payments but refused to continue the dispute, stop collection attempts, or request additional information from the original creditor unless we sent a record of all payments that were made. This is a third party with protected health information that was illegally sent by XXXX XXXX XXXX without a release of health information signed by me in violation of HIPAA, they have evidence that the debt is incorrect, even if just the balance is incorrect due to billing errors by XXXX XXXX XXXX in administering the payment arrangement and recording payments, and they are refusing to cease collection attempts or investigate the payments made. If there was a billing error on the part of XXXX XXXX XXXX, they should have contacted me to correct the error prior to going to collections. This cavalier treatment of my credit report is in violation of my rights under the FCRA. The fact that my payments were not recorded is evidence of gross negligence on the part of XXXX XXXX XXXX in the treatment of my financial information. Where did my money go? Did they apply it to another account? Did they likewise lose any of my protected health information that they so readily shared with a third party who couldnt even provide a phone number for XXXX XXXX XXXX? Please investigate these companies and help us with this illegal collection practice.

In XXXX of XXXX I had procedures done with XXXX XXXX XXXX within the XXXX XXXX of XXXX XXXX. At the time, I was under the impression I had 100 % coverage with my insurance company, XXXX, and was not charged or billed for services. Later, we discovered an issue with my insurance coverage and I was billed by XXXX XXXX and XXXX XXXX XXXX. While I was working with the XXXX XXXX to get documents to help provide medical assistance for these charges, it was reviled that no entity can explain why I have a balance of {$1600.00} from the debt collection agency of Frost-Arnett. '' I asked for contact information for this company, so I could review itemized charges. They have NO contact information to give me. They are " not allowed '' to contact the company on my behalf. XXXX XXXX XXXX is a contracted group for XXXX XXXX XXXX XXXX. I obtained their XXXX XXXX due to a left shoulder operation in XX/XX/2022. They sent me a bill which I was in monthly communication with XXXX billing and advised them to correct the payment amount because they did not apply the XXXX XXXX XXXX XXXX XXXX Assistance XXXX XXXX. All vendors of XXXX XXXX XXXX XXXX must abide by this discount program to reduce costs for patients requesting financial assistance. In XXXX and XX/XX/2022, I supplied XXXX with copies of the XXXX XXXX XXXXXXXX XXXX via email and XXXX XXXX XXXX. I requested that they adjust the payment so that I can pay the correct amount. On XX/XX/2022 I recieved a letter from Frost Arnett Company who was hired to collect the debt. As a result, I contacted XXXX and spoke with XXXX ( XXXX ) and advised her that I have been in full cooperation and willing to pay but the amount was incorrect due to the missing discount. XXXX researched and confirmed the errors and that I was not given appropraite notifcation or time to pay the corrected amount with appropriate discounts after the adjustment. This was confirmed again in a later conversation with XXXX. On XX/XX/2022 I spoke with XXXX representatives from Frost Arnett, a debt collector. I advised the representatives that there was an error and that I dispute the collection as per their letter. Their letter indicates that I can dispute verbally or in writing before XX/XX/2022. The representatives ignored my request for dispute. They attempted to collect the debt. I referred them back to XXXX to verify the error. I also advised them that I feel that they are violating my Fair Debt Collection Rights. They both indicated that they had no obligation to contact XXXX and that they had a debt to collect. They also requested proof of the financial assistance letter. I again directed them to XXXX who has all of the documents and determined the balance to be XXXX. Within the past 30 days, I started receiving robocalls from Frost Arnett. I didn't answer the calls because my XXXX call filter app marked the incoming calls as a robocaller and potential scam. There were never any voicemails left to indicate who was calling and what business they had with me. I also blocked each call as it came in, but the calls were always from a different number to evade the blocks. At this point, the robocalls were coming in regularly so I reverse XXXX searched the numbers and it came back as Frost Arnett. So I called them to find out why they were calling and I was told I owed money to XXXX XXXX. I had been paying according to an agreed-upon payment plan on their website since I got the first bill. I was never late or short on any of the payments. At the time Frost Arnett contacted me, the debt was down to {$30.00} something dollars. I told the agent I was paying on the XXXXXXXX XXXX website and would continue to do so. I had received nothing in the mail from Frost Arnett at that point, and the agent recited my correct and current mailing address. After getting off the phone with the agent, I immediately went to the XXXXXXXX XXXX website and paid off the balance. There was no indication on the billing website that the account was late, in arrears or placed with collection. I had also never received any mail from the XXXX XXXX office reflecting a negative account status. I had been paying as agreed, then the sudden robocalls began. Today, on XX/XX/XXXX, I received yet another robocall from Frost Arnett, again from a brand new number to evade the blocks. I told the agent I paid the debt and it was paid directly to the XXXX. The agent said it could take 30 days for something to update in their system to reflect that. The agent then attempted to get my place of employment and email address after she was told the debt was paid. At this point, I do not believe Frost Arnett is going to cease robocalls and other collection attempts, though they have been notified the debt is paid. They have a multitude of negative reviews online, outlining shady and criminal collection practices just as I have unfortunately experienced here.

Subsequent to being injured at work in XX/XX/XXXX some of the medical providers failed to code it as a Workman 's Compensation Injury, thus placing the debt under my name. For the past 18 months, I have not been successful in having my claim resolved. I found it necessary to have the Worker 's Comp attorney intervene. The law firm has mailed the proper documents several times to the medical providers, however, they ( medical providers ) have not followed the protocol ( necessary forms ) required by Workman 's Compensation to pay the claim. I received a notice via the mail on XXXX, Sent a written notice to Frost-Arnett on XXXX. In accordance with Title 15 of the United States Code Chapter 41 Section 1692, I requested validation of the debt on more than one occasion within the 30-day notice. However, Frost-Arnett has failed to provide validation of this alleged debt within 5 days of contact/communicating with me regarding the debt. Instead, they have practiced aggravated identity theft under 18 USC 1028 ( A ), by accessing my personal information without my express consent. Altering the account numbers on each notice to indicate new debt ( see exhibit A ). In accordance with FDCPA 8012 furnishing deceptive forms, the monetary remedy is owed to me. When my credit was pulled on XXXX it came to my attention that there where XXXX medical collections reporting from the same collection agency that I was completely unaware of. I had XXXX XX/XX/XXXX and received numerous medical bills which I was under the impression I had made arrangements or paid all of these in full when they were received. Never did I receive any notices from any collection agencies or that would 've been an obvious sign they were n't paid and I would 've paid them. When my wife called this collection agency on either Thursday or Friday of last week, she asked if they had ever sent me notice of either of these supposed debts and the representative responded stating they had never sent anything but the hospital did so they do n't have to. She also stated they had just sent XXXX of the collections back to the hospital so they no longer had it anymore but the other they did have and I needed to pay it. I was under the impression however that they have to send notice giving the consumer 30 days to dispute the validity and to request verification. Without sending that notice, it was illegal to report the account on my credit report. I was also under the impression that 5 days after the first point of contact, they had to send a letter and it 's now been 7 days and I 've still yet to get anything from them. I 'm unsure as to whether this debt is truly owed or not because I 've not been given the appropriate time to investigate the account per law and being that I 've paid so many of these bills, it 's quite possible it has been paid especially since there were obvious mistakes with the XXXX collection they supposedly sent back to the hospital. If the debt is truly owed, I have no problems with paying but I do feel that the correct procedures need to be followed so that I could 've paid it to prevent it from hitting my credit report and I was not given that opportunity.

That day i sign the quote that i was given 1 month prior and the agreed upon price, of {$4500.00}, was paid that day as well. The paperwork i signed and agreed to says An additional charge will be applicable if the XXXX requires special supplies and or patient may receive a bill from XXXX for XXXX, XXXX or any other supply used by your surgeon. If that were the case then the price of {$4500.00} is what i should only pay unless extra supplies were used. Which they were not. Two months after my procedure, XXXX XXXX XXXX reached out to me stating that they never charged me for a third code for {$2300.00}. That code being XXXX again. My doctor, the one who performed the XXXX, spoke to XXXX XXXX XXXX saying he only charges once for that code and that is what he quoted me. XXXX XXXX on XX/XX/XXXX handed me that quote which they agreed to charge me, i signed, paid for it, then received my XXXX. I spoke with XXXX, XXXX XXXX collection company, over the phone and theyre saying its now {$3400.00} and the charge is for XXXX. I already paid for the XXXX, {$800.00} on the quote. Im very confused with why they keep trying to charge me for a XXXX that I paid in full and XXXX i paid in full as well. Im in no right liable to pay this collections. Every time I receive such billing letters, I visit XXXX XXXX - XXXX online Patient portal, which is the only portal available for patients, and recheck if I really Owe any amount, if yes, I pay it online, just to avoid Fraud and to avoid exposing my credit card information to fraud websites, As it is common in USA for data leak and we keep getting fraud letters. Later after 6 months, suddenly I see a debt collection reported to XXXX, from XXXX XXXX, and it says original payment originator is XXXX XXXX, but I dont see this amount in online portal of XXXX XXXX. then again on today XX/XX/XXXX, I received another debt collector letter from company, Frost-Arnett with a due amount of {$100.00}. To make sure, I called XXXX XXXX customer care again, and they could not see this amount due anywhere. Conclusion is, How do I Identify the bills I am receiving are not fraud, if XXXX XXXX is not disclosing the list of 3rd parties involved in the surgery, anywhere, neither online nor on any billing summary. So, if XXXX XXXX informs me about all 3rd parties/ laboratories participated during XXXX, then I would confidently go ahead and pay the bills as soon as I received. Hence, if there are any outstanding debt remaining from my side, that is purely not my mistake, and it is a purely a bad reporting of billing from the XXXX XXXX

I received XXXX bills of owing a debt, but I have insurance and had insurance during the time of treatment and well after I called a few times and was told the debt was waiting for approval of payment.. XXXX debt was taken care of because I called XXXX XXXX after a notice in the mail and called my insurance company on XXXX way of which I should not have had to do. I asked about the other debt in the amount of XXXX and was told it should be taken care of and they are waiting on my insurance company to pay it.. I have not got ta a phone call A letter. Or any other type of attempt to collect this debt from XXXX. I only got a letter stating that it was in collections. I have provided my insurance information on every call I have made to XXXX XXXX I assumed it was take care of since I had not heard anything for months. This is unprofessional and unfair The lack of communication between them and the insurance company is not my fault and they are causing me a great inconvenience as I am trying to transition financially to be able to qualify for certain things coming up soon. This needs to resolved and XXXX XXXX needs to take the proper action to resolve this matter instead of sitting and waiting on the insurance company. I have been receiving calls from Frost - Arnett. At the beginning of the call they advise that the call may be monitored or recorded. They ask if I am ____ ____, I say yes you are speaking with _____ _____. I then ask them what this is about. They then say they are a financial company calling me about a financial matter. They then tell me I will have to verify my date of birth and social security number before continuing to disclose the reason for the call. I advise that I do not release this information over the phone, and I do not believe I have any financial dealings with them. I further ask the name of there company, and who I am speaking with, as this was not provided at the beginning of the call. I was given the name of the company Frost - Arnett, and first name only of the person calling. I then further ask them if they are a debt collector, and are they attempting to collect a debt. The representative tells me they will not disclose the matter until I further verify myself. 1. I have not received any notices from the company 2. I thought they had to disclose this as an attempt to collect a debt upon the person stating that they were speaking with the person they were calling. 3. I had a difficult time to reach a manager to voice my concerns regarding their practices. 4. I advise them that they must disclose the reason for the call, and they failed to do so. 5. Therefore, I requested no further communication from them, and that they dissolve any debt that they think that I may owe them, as they will not provide me any information. On XX/XX/2022 I had a CT scan done at XXXX XXXX XXXX XXXX in XXXX, Alabama. My insurance is XXXX XXXX XXXX XXXX. That means that I have full Medicare and full Medicaid. When a provider bills United Healthcare they pay what is covered by Medicare. They then instruct the provider to bill Medicaid for any remaining balance. That is not what happened in this case. I have contacted XXXX XXXX 's billing office repeatedly. I have also contacted XXXX XXXX at least 8-10 times. The representatives have contacted XXXX XXXX 's with me on the phone. They have been given the Medicaid number numerous times. A representative and myself have been on a call together and contacted Alabama Medicaid. We were informed that no request for payment has ever been submitted by XXXX XXXX 's for the remaining balance of {$110.00}. Instead they chose to turn it over to a collection agency that continuously call daily, shortly after XXXX XXXX. I have told them to bill Medicaid because I do not owe. XXXX XXXX 's has told XXXX XXXX repeatedly that they will pull the file back from collections and bill Medicaid, yet they tell me that I don't meet their criteria. I have health problems and a husband that is currently hospitalized. I want them to stop harassing me. I can not bill Medicaid for them. They told me to request a XXXX, which I did, yet they still continue to harass me and not do their job which is bill Medicaid. If I did not have the insurance coverage it would be understandable. But I do have the coverage. Frost-Arnett needs to stop their harassment. I have asked them to stop calling me, yet they continue to do so daily.

On XX/XX/22 I filed a complaint against Frost Arnett, complaint ID # XXXX. Frost Arnett replied, stating they would submit an itemized bill and remove the collection from my credit reports. I just received the itemized bill this past Saturday, XX/XX/22 ( although the letter and bill are dated XX/XX/22 and XX/XX/22 ). Please see attached. I thought I had 60 days to reply and was waiting on the requested documentation to appropriately reply, but the case was closed without being successfully resolved. You will find the attached itemized bill reflects {$580.00} owed, {$110.00} in payments made, {$460.00} in adjustments, and a {$0.00} balance. I called Frost Arnett today, XX/XX/22, at XXXXXXXX XXXX. and spoke to XXXX, asking her to review the bill and {$0.00} balance. She told me she could not see the bill and that another department could assist, but never transferred me to someone who could assist, and ended the call by telling me to call XXXX XXXX. Did Frost Arnett not save the medical bill in my Frost Arnett file for follow-up? So, here I am again with Frost Arnett refusing to work with me and now having a {$0.00} balance. From very many online reviews there appears to be a pattern of miscommunication and a consistent lack of professional courtesy from Frost Arnett for those consumers who are attempting to work with Frost Arnett 's debt collection practices. Due to this as well as the multiple comments from consumers regarding debt collection being executed against them by Frost Arnett for medical bills that they did not owe, I am apprehensive to pay this bill without proof of debt.

I request a thorough debt validation from XXXX XXXX after explaining to them that I paid XXXX XXXX the full amount and do not owe any debt. They did not send me any debt validation. From the attachment you will be able to review what was sent. They show XXXX line item of what they claim I owe, but they are unable to show my past charges and my pas payments that I made on the account. I also wanted the paper work that I signed at the office and they were unable to provide this. On the attachment from XXXX XXXX it shows a line item from XX/XX/2023, with a total due of {$85.00}. I have attached a bank statement from a payment that I made in their office on the day of my final office visit where I made a payment of {$340.00} on XX/XX/2023. This is a payment that came out automatically from my debit card. This is the main reason I wanted to get all of my past charges and payments from XXXX XXXX and XXXX XXXX before moving forward. I believe based on how it looks that they are double charging and have done so multiple times in the past. It looks as if they charged my insurance, then charged me, but did not put my payments in the books at all. This would be fraudulent. I would appreciate if they can send me an itemized list of all of my past payment history so I can audit and make sure everything lines up with my insurance and bank statements. To conclude what they sent me is not a debt validation, and you can review this from the attached document. Upon visit to XXXX XXXX on a Saturday, I attempted to pay in-person at the time of service. Because it was a Saturday, and they could not contact my health insurance to verify my {$50.00} copay amount, they would not let me pay in person at my visit the day of service. I have never paid a bill late before so for me to receive a collection notice ( which will negatively affect my credit ) is devastating to me. Immediately after I received this collection notice, I paid the bill as soon as possible on XX/XX/20 ( as I always do when I receive any bill ). I don't know if someone had tampered with my mail, but this is not my fault and should not negatively affect my credit in any way since I had no control over paying this bill on time. Frost Arnett Company are continuing to try to collect on a debt that I have plainly told them was as a result of identity theft. I have mailed a certified letter and I have made a phone call to Frost Arnett to tell them that the debt that they are attempting to collect on was due to someone using my personal information that they obtained when my identity was stolen. When I spoke to them on the phone they told me that they did not care if the debt was from identity theft that I just needed to pay it. I told them that I was not going to pay a debt that I did not owe, and they told me that if I did not pay the debt that they would have me arrested. I told them that I was not going to pay for a fraudulent debt and they told me they would report the item to all XXXX credit bureaus so that they would prevent me from being able to obtain credit until I paid the fraudulent debt. I told them that it was unlawful to defame someone 's character, but they told me that they would do what they wanted to do until I paid the fraudulent debt. The debt was from XXXX Hospital the account number for the fraudulent debt with Frost Arnett is XXXX. I have also contacted XXXX XXXX Hospital and told them this was not my debt and was as the result of identity theft, but they refused to speak to me and told me that I had to contact the collection agency. I told XXXX XXXX that Frost Arnett was verbally abusive to me and were threatening me, and they told m that they did not care that I had to contact them and pay the fraudulent debt.

Please reference CFPB complaint IDs XXXX ( XXXX ) and XXXX ( XXXX ) as this is a follow up to the same issue. From previous complaint : On XX/XX/XXXX, I scheduled a phone appointment with XXXX XXXX XXXX of XXXX XXXX XXXX XXXX. I talked to him at XXXX at the scheduled time and informed him of my symptoms. He told me he was unable to assist until I performed a XXXX XXXX and XXXX XXXX and that he would send the referral. I waited for the referral but nothing was sent over. Due to the urgency of the situation, I visited another doctor who handled the situation more promptly which led to the resolution of the medical issue. Later when I received the medical bill from XXXX, I called them to dispute the amount and file a complaint as I did not receive any medical care. I did so multiple times and XXXX consistently never responded. Finally in the latest communication, I spoke to XXXX XXXX XXXX ( XXXX XXXX XXXX XXXX ) and described to her the issue. She told me she understood my concern and would investigate further internally before providing me with a response. I ultimately received no response from XXXX XXXX in writing or a phone call. Finally I received this collection notice from XXXX XXXX XXXX XXXX on XX/XX/XXXX. The previous complaint was closed with XXXX closing the file and XXXX XXXX still not responding. XXXX XXXX, I receive a 2nd letter from you with some paper stating that you have verified such debt by sending copies a bill from some emergency room that I never been to. I am requesting proof that I am the correct party, and there is some contractual obligation which is binding on me to pay this debt. In late XXXX XXXX of XXXX. I received a notice of collection from Frost-Arnett Co XXXX The notice was regarding a medical bill from XXXX XXXX, in the amount of {$210.00}. I called Frost-Arnett in XXXX to resolve this debt. I was informed that my balance was no longer {$210.00} and it would only be {$100.00} to resolve this, since Frost-Arnett took over the debt from XXXX XXXX XXXX I was told that the payment could be made online, and was directed to do so at XXXX. I paid the balance of {$100.00} on XX/XX/XXXX, received a reference number for proof, and a balance of {$0.00} on the website. However, in late XXXX XXXX, Frost-Arnett still reported this balance to my credit report, resulting in a XXXX point drop in my score. I immediately disputed via my credit bureau. Upon speaking with representatives at Frost-Arnett, they admitted that some sort of misstep or mistake was made, and they released incorrect information to my credit report. Showing the debt still owed. On XX/XX/XXXX, I received a letter stating that I had unpaid debt from a 3rd party debt collector from a medical bill. The amount owed was {$46.00} from XXXX XXXX XXXX which was passed to Frost-Arnett, the debt collection agency. However, I had not known about this bill because I was not told about this payment due. So I contacted the company, the same day I got the letter, asking for verification and what had happened. I was told that in XXXX I was billed that amount. However, I remember that day and my doctor never told me about any bills to pay or I was never billed that amount. There was no phone call, email, or actual mail saying that I needed to pay. I even have voicemails going back to XXXX with no one contacting me. I lived in the same place from almost 7 months, until XXXX when I moved back to California, so I should have received a letter by that time. Until XXXX, when this letter came in. So I was really confused, and in that same phone call, asked them to send me an email confirmation the information needed. InXX/XX/2018 I received a letter from Frost Arnett for an attempt to collect a debt for {$350.00}. I spoke with them and asked what this debt was for. They stated that the debt was for money owed from XXXX XXXX XXXX XXXX hospital stay. On XX/XX/2018, I reached out to XXXX XXXX XXXX concerning this debt. I explained to a female represetatie that my XXXX insurance was supposed to cover my debt, and if they did not cover in full, that my Medicaid would. She told me to call and get my ID number. I called Medicaid on XX/XX/XXXX, and they provided the number to me. I also called Frost Arnett on XX/XX/XXXX which they have record for at XXXX. I explained that I spoke with the original creditor, and provided my Medicaid information. I asked BOTH parties if I needed to follow up with them concerning this debt. Both said that if there were any issues, I would be contacted via phone or email. On XX/XX/XXXX, I received an alert on my phone stating that this account was in collections. I immediately contacted both the collection agency, original creditor, and XXXX XXXX XXXX Hospital. Everyone has a record of me contacting them concerning this debt except XXXX XXXX XXXX. XXXX XXXX XXXX stated that it is now too late to apply my Medicaid ID, and they had reached out multiple times although I have not received any communication from them.

I had XXXX XX/XX/XXXX at XXXX Hospital XXXX in XXXX XXXX, Florida, with anesthesia services provided by XXXX XXXX, XXXX ( XXXX ). The claim was sent to our health insurance, XXXX XXXX, and we were found to owe {$1000.00}. We received a bill from XXXX for this amount, which we paid in full online on XX/XX/XXXX via American Express. On XX/XX/XXXX we received a collection notice for this service from Frost-Arnett ( XXXX ) in the amount of {$1000.00}. We explained at that time that the bill had already been paid in full. Since then, we have received 4 or 5 additional calls from Frost-Arnett attempting to collect on this debt that had been paid in full prior to receiving any communication from a collections agency. After contacting XXXX several times to correct this situation, we received a letter from them dated XX/XX/XXXX stating we had a XXXX balance on the account and were assured that the issue had been taken care of. We received another phone call from Frost-Arnett today, XX/XX/XXXX, again attempting to collect on this previously paid debt. We would like for Frost-Arnett to cease and desist in their harassing attempts to collect on an already paid debt. Additionally, we are requesting a letter in writing stating that this debt collection attempt was made in error and will not affect my credit.

Disputing : Account : Frost Arnett On XX/XX/2018, I disputed the following account in writing due to me having no recollection of ever having an account with Frost Arnett who is reporting negative information to my credit profile. In my first dispute, XXXX and Frost Arnett failed to provide Verifiable Proof such as, an original Consumer Contract with my Signature on it for the account listed above. Their failure to positively verify these accounts prior to reporting them, has hurt my ability to obtain credit. Under the FCRA, unverified accounts must be removed and if they are unable to provide me a copy of verifiable proof, they must remove these accounts. I believe that they have failed to follow the law ( s ) on this matter. Also, according to the Fair Credit Reporting Act, Section 609 ( a ) ( 1 ) ( A ), they are required by federal law to physically verify the original signed consumer contract, and all accounts they they post on someones credit report. Otherwise, anyone paying for your reporting services could fax, mail or email in a fraudulent account. XXXX and Frost Arnett 's refusal to follow the law has left me with no other option, but to file this complaint.. I am submitting my dispute letter that I sent to XXXX on XX/XX/2018.

This company FROST-ARNETT is sending letters and calls after I notified them that this account is NOT mine even violating the HIPA law in my person I sent them a certified letter on XX/XX/2022 and received a response on XX/XX/2022 but they date the letter like XX/XX/XXXX?? obviously in an attempt to justify if I delay under the law in responding to the evidence which is requested and what they proved does not show anything of my signature, only a document with my name and date of birth, indicating that I was a victim of identity theft in in the past even my credit report shows that information this account is NOT mine I have not been in any surgery, in addition to this action of collecting personal medical information is a violation of my HIPA rights the only time I was in the hospital was for XXXX XXXX and I did not have any surgery. They called repeatedly and asserted I owed medical bills for an adult child asserting that they were a minor when they were not. I asked for them to provide proof of age when the services were rendered and in the collection calls they could not. They refused. I did not convey my XXXX XXXX XXXX number to them or authorize payment on XX/XX/2002 in the amount of XXXX. Yet it was charged to my XXXX XXXX. Collection companies should not be able to use financial card information provided to a medical provider with your consent to pay for a debt you did not incur. The addition of carrying your adult child on your insurance should not result in this behavior.

Sir - The debt was originally with XXXX Hosptial which I was paying off in installments and reduced to {$1000.00} in XX/XX/2018. Then without mentioning to me, the debt was transferred to Frost Arnett ( account XXXX ) on XX/XX/2018. However on XX/XX/2018, I paid XXXX Hospital {$200.00}. The letter from Frost Arnett arrived on XX/XX/XXXX When I got the letter I called Frost Arnett immediately and informed them that my debt was only {$800.00}. The person said that they would check about the payment of {$200.00} to XXXX and never called back which he said they would do. I called XXXX also I filled a complaint already with Frost Arnett asking them - why dont you verify the {$200.00} which I paid and reduce my debt by {$200.00}? No Answer. I received an email on XX/XX/2023 from XXXX XXXX telling me that I was one day past due on my bill. I logged into their website and paid my entire bill on XX/XX/2023. The website did not offer a receipt or any documentation that I could use to submit to my FSA. On XX/XX/2023 I received a letter from Frost-Arnett debt collections dated XX/XX/XXXX informing me that I owed {$370.00} and that I had been sent to collections. Please note that the letter was dated after I paid my debt and the XXXX XXXX website said I was current. I went back to the XXXX XXXX website which confirmed I was fully paid and had no debt. I then filled out a debt dispute form on Frost-Arnett which did not give me a receipt or any evidence that I had filed a complaint. I called XXXX XXXX to check on the charge. I had an open balance with XXXX XXXX XXXX XXXX. I thought the amount was too high so I wanted to read through my insurance policy and check the math on the amount owed. Unfortunately, the debt slipped my mind and became long overdue. Frost-Arnett sent me a letter and I was reminded of it. I paid {$400.00}, the total amount due, directly to XXXX XXXX XXXX XXXX on XX/XX/XXXX. The letters and phone calls from Frost-Arnett are still continuing. I have explained on four separate calls that the debt was paid directly to XXXX. After the third call around XX/XX/XXXX from Frost-Arnett I called XXXX and was told my outstanding balance was cleared months ago and owed XXXX. They did state that they or Frost-Arnett were having technical issues communicating with each other but it had be resolved. I would highly recommend that if anyone were to receive a call from Frost-Arnett that they not pay anything until after confirming with the party the debt originated from the amount owed is correct. Frost-Arnett 's competency, from my experience, is subpar and relying on them appears to be risky.

This medical bill was never received by me because XXXX XXXX XXXX admitted they did not have an accurate address for billing, even though I submitted proof of current address on admission to the ER in XXXX and our insurance company which paid most of the bill provided them our current address. When I realized the bill was due via my insurance company ( not through an actual provider bill ), I reached out to them for payment, but they refused to accept it. I subsequently received the bill owed to collections. My file number when I tried to work with XXXX 's billing is XXXX. They apologized for the address issue and acknowledged to me on the phone that this shouldn't have been sent to collections. This bill should never have been sent to collections due to the error in address and no indication to me that a bill was due. XXXX 's refusal to accept payment was also indicative that this process was flawed as the bill could have been paid a long time ago. We have paid the bill to collections ( our only option ) but filed a dispute with them and have the billing complaint on file.

In XXXX 2015, I paid {$340.00} to remove an item I discovered on my credit report. It was my belief, at the time, that paying it was the only way I was able to get the tradeline removed from my credit report. The collection company told me the account belonged to me and I believed them. My sole reason for paying the debt was to remove the trade line. The company did not make me aware or offer an opportunity to request verification of the account they claimed I owed prior to paying it. Even though, I questioned the validity, I did n't know I could request verification of the debt - nor did they offer. I paid the amount the company stated I owed to remove the trade line, but the company did not remove the trade line as I had requested they do once they received my payment. Representatives of the company were very rude once they got my credit card number. After getting nowhere and becoming frustrated, I asked to be transferred to a manager. The manager was also rude and told me they would not remove the trade line and ultimately hung the phone up in my face.

Hi this collection company called me during a covered pandemic hardship that my son and I were facing as me being a XXXX mother raising him alone and it seem to be a problem with insurance company not processing their in of the payment to the hospital but I also explain to them that we were going through this and then today they decided to go ahead and report this to the credit bureaus when they know that were going through a pandemic crisis theyve been informed by phone when they called me and the situation that were facing as I have notified all of the other companies that we have accounts with as well including all of our credit cards and all of our loan company and the loan companies all of which I have unemployment insurance with that still have not reported that properly to the bureaus all the way because they were notified on XX/XX/XXXX at the start of this situation and there should be reporting no late payments or miss payments. As I got all of them three loan companies the proper and appropriate paperwork right away as of XX/XX/XXXX of 2021.

Frost-Arnett , a debt collector, continues an attempt to collect a non-existent debt in the amount of {$92.00}. This amount was paid promptly at the office of the provider, on the very same day of service, XX/XX/2023. Weeks later, I received a lengthy phone call from someone representing the provider, XXXX XXXX, seeking the amount in question. I told them that invoice had been paid and advised them to check their records for the payment, which was made directly with the service provider. There was an obvious failure by them to perform any due diligence in this respect and are now harassing me by issuing collection letters which require my time be spent on contesting something that could have easily been dismissed. I have receipts from the provider for the amount being requested. It appears they would rather spend their time trying to collect twice rather than checking their own records! I find this type of business practice both predatory and intolerable, requiring some sort of legal remedy to put a stop to it.

We have been making monthly payments on all three accounts and all of a sudden they were sent to collections without notice. The monthly payments varied when my husband lost his job but payments are still being made consistently. No notice was sent to put on a payment plan to pay the bills down to avoid collections. I have repeatedly asked that my full name including middle initial be used on all bills, medical files/records and correspondence yet in over two years this hospital and this particular doctor have verbally promised but then refused to follow through. If my middle initial is not included then it causes confusing and is often added to my father 's credit history because he shares my fist and last name, just not my middle named. I also can not use the billing receipts that the hospital and doctor provides as proof of my medical expenses on my yearly taxes because my middle initial is not included. The woman speaking barely gave me any time to speak! It was difficult to just get a little information from her about this so called debt. She wanted me to send a check in the above amount to their company, so they could forward it. I refused, and hung up. Before this account went into collections, I had asked to hospital to send me an itemized bill due to not knowing where my {$1700.00} payment, on top of my insurance, went. I attempted to negotiate a reduced amount for a full and final settlement of my debt. The collection company, Frost-Arnett Company, told me that I had to front the money that I was offering and they would take that amount to the creditor and see if they would accept it for settlement. I provided my account information and the funds were withdrawn. I was contacted three weeks later and told that the creditor had refused to accept my offer of settlement. This company Frost Arnett uses all sorts of tactics being a debt collector. I actually had another number at the time and told them to cease and stop contacting me. I never gave them permission to contact me I am not even sure how they got my number. I changed my number and they still contact me multiple times throughout the day all day long. It is a automated prerecording. They used to leave voice messages but that stopped. Under the Telephone Consumer Protection Act they restrict the use of automated phone equipment and this is a violation also under the Fair Debt Collection Practices Act because this is harassment.

After noticing the derogatory remark on my Credit report I contacted them. to try to resolve the matter and was made to feel very uncomfortable and threatened to pay. So I made a payment to have it removed to only see that a month later I is back on my report but with another collection company .It went from Frost to XXXX XXXX XXXX . I feel like my personal medical information is being passed around and used to get money from me in these tough times. I received an unsolicited text message on my cell phone from " Frost-Arnett '' noting it was an effort to collect a debt. After searching to ensure the firm was legitimate, I called their office to ask about the notice. I spoke with a service representative who checked the file and informed me it was a " wrong number.
