I am a XXXX % permanently and totally XXXX XXXX with a service connected XXXX The VA is obligated to pay for my treatment if I am scene on an emergency basis at a local hospital due to XXXX symptoms. In XX/XX/2012, such was the case. The hospital intake worker for billing was informed and placed the information on the face sheet of my folder. All other service providers followed the practice and were paid by the Central Fee Basis department at XXXX except XXXX of XXXX ( Virginia ). After several months, they began sending me bills for their service on this occasion. While I notified them numerous times to file with the VA using the appropriate code and form, they ignored me and just kept trying to bill me. I returned a copy of every bill they sent me with the same instructions clearly marked in red type. They still ignored me. Recently, after XXXX years and XXXX months, I received yet another bill from them with interest charges on top of the original charges. The total is now over {$600.00}. I do not owe them XXXX red cent. They failed to follow simple and repeated instructions which would have led to payment from the XXXX. In a recent call to them I laid out the facts of the case, and XXXX, supervisor of collections admitted they had made a mistake. I asked her what they were going to do to correct it, and she simply responded, " Nothing. '' They apparently reported the debt to the XXXX major credit bureaus in XX/XX/XXXX of this year, and again in XX/XX/XXXX through their collections affiliate, XXXX Solutions. I had contacted the latter and disputed the bill, cautioning of legal action against their client if this was not straightened out. They said the did an investigation and decided to stick with the client. Thus, they become drawn in to this complaint because they did NOT do a careful investigation and proceeded to take additional action to damage my credit. Earlier this summer I had excellent credit, but it has recently been reduced to fair, and only because of this XXXX creditor 's actions. I am trying to find a house to buy with my VA loan, but the damaged credit rating will hurt that effort severely. My XXXX Credit Card availability of credit was reduced by my bank to the tune of {$11000.00}. I had a total of credit availability from them of {$15000.00}, and they wrote to tell me that due to the decrease in credit rating, they were cutting my total available XXXX XXXX to {$4000.00}. All of this damage is primarily the fault of XXXX XXXX of XXXX and their refusal to do the right thing. I have several conditions that are service connected in addition to the 100 % for my XXXX. I suffer from XXXX and interstitial cystitis and chronic pericarditis ; all of which are worsened by stress. I was hospitalized again as recently as a month ago with symptoms of cardiovascular issues. While I now personally believe that XXXX XXXX of XXXX 's actions indicate deliberate and purposeful fraud, Given the evidence of wrong doing they appear to admit to, I can afford to do little more than take them to XXXX XXXX. Any action there is not binding on either party, and I can not afford an attorney to sue them. I hope you will help me resolve this. As I have a copy from XXXX of the face sheet that outside providers use to do their billing and it clearly shows that the XXXX was supposed to be charged, I hope you can help me get them to accept responsibility for their error and stop harassing me. XXXX more associated matter. In a more recent incident, when I was seen at XXXX and gave clear and explicit details about billing, XXXX. refused to follow those instructions and billed my medicare instead.

The error to receive proper payment was in coding this bill properly in order for the insurance company to pay the bill. The improper coding on the bill has created the insurance company to deny payment. The medical procedure of XXXX is not optional like a XXXX but mandated by the County of XXXX as well as the United States department of XXXX control. XXXX XXXX in Delaware has billed properly and was paid properly given the fact that they coded it as mandatory versus optional. Our insurance company XXXX has informed you on more than XXXX occasion as well as us that your coding of this treatment was in error by the XXXX XXXX nevertheless XXXX XXXX refused to change the code from an XXXX to a XXXX. My wife did not seek a XXXX as a way of not XXXX in the event she should someday be exposed. She went to XXXX XXXX to receive a subsequent vaccination in her series as the result of having been exposed to a XXXX over 2 nights and waking with a XXXX that had not been noticed before. As you have been previously notified, she was told to get this XXXX by public health officials. Nobody in their right mind would go through this XXXX unless it was absolutely necessary! This lack of changing the code has created a denial for this bill to be paid timely. My wife and I are willing to have a court of law decide what to do or an arbitrator of choice. This is true that you billed our insurance company and it is also true they did not cover the " XXXX of non-covered charges '' due to your billing code error as stated in the XXXX XXXX letter. Secondly, according to contract procedures that XXXX XXXX has with my insurance company XXXX proper billing and protocol was not adhered to thus coverage was denied. Billing for coverage for services over one year old is normally not allowed and is a breach of the covenants within the contract in addition to the coding error. If you re-read the letter sent by registered mail on XXXX XXXX XXXX the bill was sent with the wrong code therefore your claim for insurance was denied. If XXXX XXXX had taken proper action in correcting this billing in XXXX near the service date then the bill would have been paid but no timely action was adhered to thus creating a denial of coverage from the insurance company. I had requested arbitration as well as proceeding to court to have a judge decide this matter however you have decided to harm the XXXX family by reporting this to the credit bureau. This malicious act of harming our credit rating will have a great effect on obtaining credit, getting a home loan at a reasonable rate.

My son went to the XXXX XXXX XXXX XXXX sometime this year. The hospital been sending him a bills saying that he owes them {$240.00}. After not getting a response from my son about this debt, they then sent it to the collection company Focus Recover Solutions to see if they could collect the money owed. Focused Recovery Solutions did not contacted me about this debt to see whether it belongs to me or my son because we have the same name. Instead the went ahead and place this debt on both of our credit report. I contacted the collection agency to let them know that I did not owe this money and that the debt belongs to my son. I also contacted the hospital about the debt and they also stated that this debt does not belong to me and that I should contact the collection agency again. I have contacted the collection agency 5 time already to see if we can resolve this issue.The collection agency states that they have already notify the all three credit reporting agency to have this item remove from my credit profile. XXXX and XXXX removed the item but XXXX will not remove the item and insisted that the item belongs to me. I have repeatedly disputed this item but XXXX still say that the item belongs to me. I even try to get the collection agency on the phone with XXXX, but the representative from XXXX say she ca n't speak to third parties and will not take the information from the representative from collection agency with the control number showing that they already send the request back in XXXX XXXX to have this Item remove.I have also asked Focused Recovery Solution to send me a letter stating that this item belong to my son and that they have request removal of this item, but i did not get that request full fill. I again contact the billing department at the hospital and they stated that they will have the collection agency send me a letter showing that they corrected the mistake, still no letter. I did not go to the hospital and I do n't owe them the {$240.00} that is placed on my report. I need this Item remove from my credit report immediately.

Recently while conducting a monitor of my credit report I discovered that FOCUSED RECOVERY SOLUTIONS has furnished some items that have unverified information. I believe under the Fair Debt Collections Practices Act, I have the right to request that you allow me to validate the alleged debt. I am aware that there is a debt from XXXX but I am unaware of the amount due and your bill does not include a breakdown of any fees. Additionally, I am allowed under the HIPAA law [ HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 ], to protect my privacy and medical records from third parties. I do not recall giving permission to XXXX to release my medical information to a third party. I am aware that HIPPAA does allow for limited information about me but anything more than is to only be revealed with the patients authorization, therefore my request is twofold and as follows, Validation of Debt, and HIPAA authorization. 1.Please provide breakdown of fees including any collection costs and medical charges 2.Provide a copy of my signature with the provider of service to release my medical information to you. 3.Cease any credit bureau reporting until debt has been validated by me. 4.Please send this information to my address listed above and accept this formal complaint as my formal debt validation request, which I am allowed under the FDCPA. Please note that falsification of the information you received from any medical provider can be percieved as a HIPAA compliance violation as well as a violation of the FDCPA because you will be deceiving me after my written request of debt validation. I request full documentation of what you received from the provider of service in connection with this alleged debt. Additionally, any reporting of this debt to the credit bureaus prior to allowing me to validate it may be a violation of the Fair Credit Reporting Act, which can allow me to seek damages from a collection agency

This is a follow up complaint to XXXX-XXXX, in which 1 ) XXXX, working on behalf of XXXX XXXX Hospital, claims I never revoked TCPA consent and 2 ) Focus Recovery Solutions, collecting on behalf of XXXX XXXX Hospital contacting me illegally with an auto-dialer. I am writing to share additional information and to contradict inaccurate statements in the company responses. I have a recording of my conversation with XXXX on XX/XX/18 where I unequivocally revoked TCPA consent. Here is a transcript : Agent - " Do you give us permission to contact you on your cell phone using the automated dialing system or pre-recorded voice for future inquiries? '' Me - " No. '' Agent - " OK. '' This TCPA revocation was reasonable and valid, yet was not documented across my accounts with XXXX which, at the time, were accounts ending in XXXX and XXXX. The representative did not ask if my revocation was subject to one account or the other, yet a future representative notified me on XX/XX/XXXX that I had to call in separately for each account. My inclination, which I fully intend to prove through discovery, is that XXXX was managing and collecting on both accounts, but XXXX XXXX Hospital spelled my name incorrectly on one. That is the reason why they did not notate the account. When account XXXX was recalled from their office by XXXX XXXX Hospital and placed with Focus Recovery Solutions, XXXX 's failure to honor my revocation resulted in me being contacted illegally by Focus Recovery Solutions via autodialer on XX/XX/2018.

I pay more than {$3000.00} per year for health insurance through Medicare and XXXX but XXXX XXXX Medical Center has never billed my insurance for an amount of {$3300.00} due to ER care. Further they tell me that the Insurance company representing the party that rear ended me has already settled with me but they have not bothered to document that claim. I am represented by an attorney and the claim is pending so the debt is legally that of the Insurance company for the other party. The visit to the ER is billed out in XXXX parts-one for the use of equipment and XXXX for the medical treatment. XXXX XXXX XXXX provided treatment, billed my insurance and has been paid. At the present time XXXX XXXX Medical Center has placed their account for collection and I have been reported to XXXX and XXXX. In my opinion, XXXX XXXX has denied due process ( filing with my personal insurors ), has unfairly damaged my credit rating which dropped XXXX points because of this reporting, is ignoring Medicare provisions for special payments to medical facilities in cases where the insurance payments are delayed in processing, and are engaging in a form of black mail to effect payment from me that I am not responsible for. I contacted the billing department at XXXX XXXX on XXXX XXXX, 2016 with full documentation of everything above but have not received any information back from them. I should not have to pay for someone hitting me from the rear when I was stopped at a traffic light. This debt is not mine.

I am filing a complaint against the company for violating my consumer rights. I initiated a dispute and request for validation through the credit bureau and sent the company a written request for proof of this alleged debt. In this communication to them, I gave a reasonable time frame for the company to respond in writing ( 30 days ) and also requested that they cease and desist any verbal communication. The company has ignored my request for proof of this debt and has also violated the FDCPA by continuing to place calls to me. On XX/XX/19, A representative contacted me by phone against my wishes. I explained that I do not believe this to be a valid debt and advised her that I sent a request for validation with an updated address for all communication to be sent to me in writing. The collector told me they are not required by law to provide proof of the debt and would not be providing validation. I explained that the law states if they choose not to validate the debt, they must stop all collection activity which includes credit reporting. She proceeded to try and argue with me and started to raise her voice and speak to me in a condescending tone even while I remained calm and professional. This company is blatantly disregarding the FDCPA and FCRA. They are refusing my rights and also reported back to the credit bureau that the information is certified as accurate. They have yet to send me any documentation to validate this claim.

Dear Federal Trade Commission : This is a letter serves as a formal investigation request investigation for a breached agreement by Focus Recovery Solutions. On XX/XX/2018, Focus Recovery Solutions mailed a counter-offer in response to my original settlement letter dated XX/XX/2018. On XX/XX/2018 I faxed a letter accepting their counter offer in the amount of {$390.00} contingent upon deletion and any negative remarks on this account. Despite my financial hardships due to their willingness to resolve this account I paid the counter offer as agreed on XX/XX/2018. I feel as though their eagerness to help me resolve this account was a form of deception get me pay with no intentions of removing from my credit after settlement. On XX/XX/2018 not only did they update this account on my credit report as a new collection account they added a negative remark indicating this account was settled for less than the original amount owed. Their actions not only dropped my credit score 21 points but has cause other revolving accounts to affirmation actions to include lessening my credit limits based off current credit score. I am requesting the immediate removal of this account that i am no longer liable for. Should this account status remain on my credit report, I ask that my payment of {$390.00} be returned for breach of contract and any further communications cease. A medical bill for XXXX XXXX XXXX was setup on a credit card to be charged each month. At some point, as happens frequently to us, our card was compromised and canceled and re-issued. If yo have ever had this happen, is it not easy to forget all the payees that may have your card? It is very easy to forget. No new letters, no calls were received. While in the process of buying a home for my elderly Mom to live near us, we were alerted of a collection. I immediately called : FOCUSED RECOVERY SOLUTIONS Agency Address : XXXX XXXX XXXX XXXX. XXXX, VA XXXX ( XXXX ) XXXX and inquired about the account being on my credit report. I politely, asked questions, and confirmed that the credit card payments had been interrupted by the credit card breach. The very next day, I paid this in full and Focused recovery explained my flex spending would be charged, and the record on the CRA would be deleted. Well, they issued a XXXX balance to the CRA 's, but not a deletion request as expected. An overnight letter, XXXX, was sent, with no response and no deletion order sent to the CRA 's. I have this tracking number if required, please contact me. This should not impact my ability to get a good rate on a mortgage, because no-one called me about the credit card payment issue, no letters were received. I have no collections, and never missed a payment on any accounts.

I was renting a salon space from XXXX XXXX inc. I was allowed to break my lease because they had a tenant ready to take my space over. I moved out and was at a XXXX balance. One week after move out I went back to the space and another tenant was in there working. I was told I would be responsible for rent if no one moved in. So fast forwa rd 2 yr s later I get a letter of collection, stating I owe one month rent plus late fees. I never once received a bill. I requested my statement from XXXX XXXX before I made any disputes. My billing statement was closed out XXXX owing {$250.00} fore cleaning and repainting fee. I moved out in XXXX no where on the bill was I charged for XXXX XXXX rent or late fees. Now all of a sudden I owe almost {$1400.00}. I have spoken with the owner who did not have a answer as to why and then told me just deal with the creditors. I have dealt with the creditors which are n't of any help and the forged a document stating what I owe because I requested documentation from them more recently and it 's absolutely not the original document I received before I went into disput with them.

I had a medical bill for {$45.00} that was paid by check to the company that provided the service, XXXX XXXX. Months later on XX/XX/2019, I get a notice from a credit monitoring agency saying a deliquent account for {$45.00} was reported by " Focused Recovery Solut '' to ONE of the credit bureaus. I called the number to investigate what it was for and then see it had already paid but call the bank and the check didn't show cleared so I mailed another check to the company, XXXX XXXX, and carbon copied the letter to the collection company, Focused Recovery Solutions, on XX/XX/2019. I never received any correspondence from Focused Recovery informing me of the debt or trying to collect until yesterday, XX/XX/2019 when they said they received my dispute and it was infact valid and they did report it to the credit agencies and are still demanding payment. Furthermore, they reported it to the other two agencies the first of XXXX. The appropriate steps were not taken to collect the debt and it was paid again on XX/XX/2019 by check # XXXX and cleared the bank on XX/XX/2019 -- -so it's NOT VALID!

On XX/XX/XXXX, I went to the emergency room at XXXX XXXX Hospital in XXXX County VA. In early XX/XX/XXXX, I received a bill for over {$3000.00} for services from XXXX XXXX XXXX ( XXXX ). My medical insurance provider told me not to pay the XXXX bill as it was a double-billing issue. XXXX sent the billed amount to a debt collector in XX/XX/XXXX and the amount in dispute was listed on my credit report as a collections. The issue was not with my medical insurance provider but with XXXX. XXXX was attempting to collect an amount not owed by me. My medical provider stated that XXXX was contractually bound to accept agreed upon payments for services rendered and nothing more. I feel that there is an on-going fraudulent business practice by XXXX by trying to collect additional dollars for services covered ( and paid ) by medical insurance providers. XXXX finally accepted payment in late XX/XX/XXXX by my medical insurance provider but not before I was hounded by dept collectors and a negative collections account was placed on my credit report.

Focused Recovery Solution ( FRS ) - I paid off the attached debt to the creditor. Once my check cleared I contacted the account to verify if my account was updated and information forwarded to the credit bureaus. I was informed FRS does not update the credit files with partial payments. I advised the account I was only aware of the balance of {$750.00} and inquired of the difference. The FRS collector advise she wasn't sure of the difference therefore I disputed the balance of {$130.00} with the account. To date I have to received an explanation of the remaining balance. Once FRS advised they did not update partial payments to the credit bureaus I forwarded the information to the credit reporting agencies directly ( XXXX, XX/XX/XXXX and XX/XX/XXXX ). All three had updated my file to reflect the correct balance however when I checked my report with XXXX today I've noticed the full amount of the account was re-established on my credit report ; which is erroneous.

I have been trying to have this incorrect report removed from my XXXX Credit Report since this first took place in XXXX. I have tried REPEATEDLY to communicate with managers and representatives alike. Back in XXXX I was finally able to talk to a manager about the situation - which is that this was not billed to my insurance even though I had Insurance - and I was told it was going to be removed and MY ACCOUNT NOTED. Yet this never happened. I was NEVER even notified of this debt and AS SOON AS I RECEIVED A PHONE CALL I WORKED WITH THE HOSPITAL AND MY INSURANCE TO HAVE THE BILL PAID IN FULL. It was. The hospital has since changed ownership and they are not able to provide me with any records or notes because I was told their system has been changed and there is no way to access the old XXXX XXXX XXXX XXXX. Under the CRNR, a client has the right to be notified before a negative report can be made on their credit and I was not.

I recently received a copy of my credit report and noticed some erroneous items on my credit reports. Focused Recovery Solutions is handling the collection of the accounts mentioned above. However, after reviewing my records, I am unable to find any documentation of any contractual relationship between Focused Recovery Solutions and me which makes them entitled to enforce a commercial claim against me. This is not a request for verification or validation. This is a request for strict proof of contract to substantiate their claim. Provide me with a certified copy of an original contract with my signature, specifically naming Focused Recovery Solutions entitled to enforce a commercial claim against me. Certification can be done through the presence of a notary public, who duly swears the copy made is in fact a copy of the original paper contract in question.

On XX/XX/2017 I paid {$220.00} to XXXX XXXX via debit card online ( as I was directed to do by focused recovery group, to whom the account had been turned over to for collections ). I have a receipt that was sent to my email that shows the account paid in full available for upload, if necessary. This information has been shared with all 3 credit bureaus as well as Focused Recovery Group. I continue to have my credit dinged every 30 days by Focused Recovery Group for a debt that was paid OVER 2 YEARS AGO! When I have disputed it ( repeatedly ) they drop it off my report for 30 days and then file it again so I have had to continuously dispute a debt that is not owed and furnish evidence that they themselves have acknowledged is accurate. This is a predatory tactic used to try exploit additional money from people that is not owed.

On XX/XX/XXXX, I sent Focused Recovery a " Validation of Debt letter that my attorney gave to me. I would like to see the proof that I actually owe these debts as the credit report shows the original creditor to be named only as " Medical ''. I have no idea who " Medical '' is. I also don't recognize having accounts with those balances. I also don't recognize the date of services that the credit bureau shows for these accounts. There are so many things that are confusing about this. Account # XXXX {$330.00} and Account # XXXX {$1000.00} What my attorney has told me is that this is not a legitimate debt. So I re-sent the request to validate the debt, and they refused to respond. I don't understand why they did that. I am very concerned about my rights being violated and am going to pursue all legal action to get justice.

I disputed a medical bill on my credit report. i disputed it with the credit bureaus and with the collection agency who reported the bill to the credit bureaus. in attempting to verify the debt, thr collection agency sent me an itemized list of medications with a total that was well below the total they claimed that I owed and haphazardly used and shared sensitive and private medical information in the process. I sent the collection agency another letter of dispute, and also disputed the information with the credit bureaus again. HOWEVER, the credit bureaus have marked the bill as verified and are no longer allowing me to dispute the bill with them, due to the fact that they have already " investigated and verified the debt '' but can not disclose the sensitive information that proves the debt has been verified.

Focus Recovery advised us of the debt after placed on Credit Report. They didn't attempt to collect ( The 30 days ) until after already on credit reports. Then advised that our Insurance statements that say " This is NOT a Bill '' and " You May Owe '' are considered notification of a debt owed to the original creditor. When asked to verify the dept and was disputed they then forwarded all medical records from the hospital. This obtained all information of treatment and medications administered by the hospital. Focus Recovery also informed our XXXX year old son on his cell phone thinking it was mine that this was a medical debt with all the information thinking he was me. They informed him they would take legal action.

FOCUSED RECOVERY SOLUTIONS is being unfair and negligent. This company has RE-AGED the account and is reporting discrepancies across bureaus. I sent them a debt validation letter back on XXXX. They failed to respond or even mark the account as disputed. They didn't even investigate the disputes. Basically, I was ignored by this company. I have complained about this company XXXX through the CFPB and wrote both times that they were to stop calling any number they have for me. I saw a negative mark on my credit report from FOCUSED RECOVERY SOLUTIONS. XXXX XXXX lists XXXX XXXX HOSPITAL as the original creditor. This company needs to remove all information from my credit reports. To my knowledge, all medical bills were paid by insurance and up to date. Focused Recovery solutions claimed I owed a medical company in XXXX VA, XXXX was paid long ago. I 'm not sure how this company ended up with the account. I never got correspondence from this company.
