I ha d medical services rendered in XX/XX/XXXX . At the time I had insurance and the provider was in-network. The provider group was subsequently bought out by another company several months later. In XX/XX/XXXX I received a bill from the old group 's billing company XXXX XXXX XXXX XXXX XXXX XXXX for the full amount of services that were rendered. I attempted to call the company several times and finally received a call back from a man there that stated that he was the only person in the office working these accounts. I asked him if it was sent to my insurance company, he said it was. He then stated that there was no response from the insurance so he placed it in patient responsibility. I explained to him that this was adjusted to my in-network deductible and that he needed to call my insurance and obtain an EOB and adjust my account so that I can pay. He then proceeded to be very rude stating he did n't have time to contact my insurance company and that it would be my responsibility. I tried calling the doctor 's office that was bought out but they said they could n't help due to the buy out. Because of that I mailed him a copy of my EOB mailed to me by my insurance. I did n't hear back about this unt il XX/XX/XXXX wh en I received a collection letter fro m Fidelity National Collections. I formally disputed the amount owed. I explained to them that I did n't owe the full amount in question and faxed them a copy of the EOB that I had printed from the insurance company 's website. They said they would go back to the original creditor to see what they wanted to do. After many numerous follow up calls that I made to them ( they never contacted m e ), I fi nally received a response on XX/XX/XXXX . They gave a very ambiguous statement made by the original creditor of why I owed the full amount and said that I still owed the full amount even though I sent proof that I had an in-network deductible and they were balance billing me the full amount. No where did Fidelity or the XXXX XXXX XXXX XXXX state or provide in writing to me that the provider was out-of-network. I then contacted several managers, who I ended up speaking to finally was XXXX I believe his name was on XX/XX/XXXX . He said that they have to stick with what their original creditor tells them. I told him that I do not owe the full amount in question and would pay that day but only what my in-network deductible was. He said that the original creditor probably would n't go for that. That day, I involved my company 's insurance broker, XXXX , on the matter and she was very helpful. We spoke to XXXX together and he stated he would get the original statement and would get back to me the next day. That never happened so we called Fidelity t ogether the next day. We spoke with a man who said that XXXX was out and that he had no direct line, no voicemail or email and said he would contact us in a week. After several weeks with no response XXXX suggested that I contact you for help resolving this claim. I am more than willing to pay what I legally owe. I have verified with the insurance company that that provider was in-network at the time services were rendered. They are trying to balance bill me for what I do not legally owe and XXXX XXXX XXXX XXXX and Fidelity National Collections a re both neglecting the legal contractual obligation of in-network services. I would appreciate any help in the matter.

There is a collection account opened in my name by FIDELITY PROPERTIES IN. They claim my original debt is owed to XXXX XXXX. The collection was opened on XXXX XXXX 2017. I do not know how this charge came under my name. It could be a case of Identity theft? I did not go to XXXX hospital that year or the preceding year for any purpose! There is nothing in my medical records to reflect this alleged charge. I tried to dispute this charge through all 3 credit bureaus. XXXX and XXXX both removed the collection from my account. XXXX did not. They contacted the collection agency and claim the charges are valid. That is impossible. There must be some record of me visiting the hospital or getting some procedure done to account for these charges. No one has produced any evidence to validate these charges. There must be something. An emergency room admission, test results, medical records, something. I know for a fact they can not have any of these things as I had no reason to go to the hospital. I am healthy. I have had no accidents. I have not been ill. If they have something in my name the only logical explanation is someone else used my name falsely to avoid the charges themselves. Even if that is the case there should be video evidence to prove that the person seeking medical attention was in fact not me. I do not know how else to dispute these charges. If this is not something that your organization handles could you please direct me to the proper channels that I may have this removed from my records. It has significantly damaged my credit rating. There is about XXXX points difference between the 2 valid reports and the one with the false charges. This keeps me from building my credit and gives me higher interest rates. Please help me in this matter. I would like to know where these charges came from and how they were placed under my identity. They can not possibly be valid as the hospital visit never happened. I visited XXXX XXXX XXXX once in XXXX of XXXX. I believe the date of service was XX/XX/XXXX. During my visit I was instructed to do some home care and was told I didnt need any follow up visits or further treatment. I received my bill and started to post on it. I made several payments and in XXXX of XXXX I called XXXX XXXX to pay my bill in full and paid the exact amount I was quoted over the phone and was advised my account was paid in full. Fast forward to XX/XX/XXXX I get a letter from a collection agency advising I owe an additional {$75.00}. This was strange considering I was told a year prior my account was paid in full and received no other bills or contact from XXXX XXXX. I called the business, the billing agency, and the collection agency and requested my account statements showing what I originally owed, what my insurance paid, and what I paid. A few weeks later I received what I believed to be my complete account statement showing what I asked for and insinuating that I still had a balance due of {$75.00}. Per the letter it asked me to provide cancel checks or a written explanation if I believe that I had already paid this amount. So very quickly I went through my emails, and printed off copies of my bank alerts showing that I made a minimum of three different debit card transactions in XXXX however only XXXX were accounted for on the statement. With third payment being accounted for the company would then owe me {$25.00}. I sent them a letter and the printouts. A few weeks later they sent me another letter and a completely new account statement with more charges and visits. I believe their book keeping has failed and they are just throwing statements together attempting to get me to pay what Ive already paid. I am refusing to pay anything further and would like them to stop harassing me.

My complaint is against Fidelity Properties. They placed a Derogatory Comment on my Credit Report. They stated that they have been trying to collect/contact me about a bill with XXXX XXXX XXXX XX/XX/XXXX.
I have never received any communication ( written or oral ) from Fidelity Properties about this matter. I have Informed Delivery from the USPS and have never received any communication from Fidelity via the USPS.
I wrote to Fidelity on XXXX XX/XX/XXXX inquiring about the bill that they mention on my Credit Report ( Account # : XXXX in the amount of {$2700.00} ). They responded about another Account # and bill amount. Not the one that's on my Credit Report.
I asked them to show me proof of the bills that they supposedly sent to me. They failed to show such proof.
In additional info on this XXXX XXXX. I have never received a bill from XXXX XXXX.
I have been trying to get them to send me one since XX/XX/XXXX.
I have sent them countless emails, called them and even wrote them. XXXX failed to send me a bill.
I even worked with the XXXX XXXX XXXX for 2 months to try to get XXXX to send me a bill. All they did was send the XXXX and I in circles. The XXXX closed the account due to XXXX 's negligence to resolve the matter.
I do have two written documentations ( I can send these to you upon request ) from XXXX admitting to me that they sent my bills to an incorrect address.
Due to XXXX 's negligence, I was placed in Collections falsely.
I am more than willing to work with Fidelity to resolve this matter.

On XX/XX/XXXX, I sent a letter to XXXX XXXX XXXX whose address is XXXX XXXX XXXX XXXX, XXXX, Ohio XXXX. I sent a letter in reference to multiple collection accounts I had found from them on my XXXX Credit Report. I asked for verification and validation. I received a letter from them dated XX/XX/XXXX which included simply a list of all of the supposed debts that they were attempting to collect on. They never provided documentation from the original creditor or anything at all that had my signature on it. They provided nothing to me that proved they have a legal right to collect this debt. It is not enough for to simply send you a printout of the amount owed. In fact, shortly after their response letter to me on XX/XX/XXXX, they began listing these supposed debts on my XXXX Credit Report. They were never listed on my XXXX Report at all prior to this time, and most of these supposed debts show open dates three or more years ago. It would appear to me that after my request for validation/verification that they not only did provide to me the required documentation but also retaliated by adding the supposed debts to another credit bureau report. Since XXXX XXXX XXXX has not provided any of the requested documentation to me, I insist that all of these account be deleted from my XXXX, XXXX and XXXX reports immediately.

I owed XXXX on a medical bill, I told this company I would be more than happy to pay once I received an itemized bill explaining where the charges came from. At first they agreed. But a bill never came that stated the debt. Instead all I received was a bill with the date and the amount. I called again asking for an itemized bill and was refused. I was told that they would not and could not request another copy and thay if I wanted to know what it was pertaining too I would have to call the hospital myself. I also received a letter threatening legal action if I did not pay, and to notify my employer of the nonpayment. When I confronted them that this was illegal the response was " I did n't know that '' I have since made the payment on the bill but do not believe it is truly owed. As no proof has ever come to me. I did call the hospital and they claim it is for a XXXX, yet no bill has come yet.

I recently obtained a copy of my credit report. Shockingly there were 7 different accounts placed in collections by the same company on different dates for varying amounts that I was never sent a bill or account statement for. Fidelity properties INC which I believe collects for XXXX XXXX. None of these dates coincide with medical treatment that I have received and the amounts make no sense. XX/XX/XXXX there are 2 accounts one for {$100.00} and the other for {$160.00}. XX/XX/XXXX has 3 separate collections accounts listed from the same place {$160.00}, {$180.00}, {$220.00}???? I did not get any type of medical treatment 3 times in one day! Nor did I get any treatment at all. No bills, just showed up on my credit report. My credit has been significantly destroyed by this.

Early this morning I was contacted by an agent, who did not identify herself as a collector. I figured it out during the conversation. She was trying to collect a debt that I believe I have already paid. I am In the process of getting my reciepts in order. How ever I have had no payment agreement with this company even though the send me letters stating this. I have paid their clients directly. Many of there letters list the original creditor as Various Accounts. I know this is in violation of the FDCPA. This agent refused to let me talk and continued to talk over me. I first noticed a collection account from Fidelity Collection on my credit reports alleging that I owe them {$250.00}. I just recently did a credit report on myself and there's an account owned and owed that I've never seen or heard of before. The collector says " Fidelity Collections ''. XXXX XXXX XXXX turned me into fidelity collections for medical bills that where paid by the XXXX that I have through my insurance. XXXX has repeatedly proven to fidelity that this money was paid but wont take it off my credit report because XXXX XXXX will not respond to Fidelity about this matter.
