I was in the process of getting a mortgage when it was brought to my attention that XXXX XXXX had on my report that there was an uncollected medical debt. I was completely unaware of such a debt. I called XXXX XX/XX/XXXX or XX/XX/XXXX and spoke to the first person, I told them this is the first time I am aware of this debt. They said it was from a visit at a hospital in XXXX in XXXX of XXXX. I was at a hospital during this time so I asked what for contact information they had on me, the person stated that they had an address in XXXX, XXXX and that is all they had. I gave them my current email address and phone number. I asked how did they try to reach out to me to resolve this alleged bill. She stated they sent several notices to the address they had on file. While I was asking for more information the person hung up on me. I immediately called back and spoke to a different person. I explained what happen and asked them how their company made attempts to reach out to me. She said according to what she saw no attempts were made to reach out to me. I asked how could her company hit my credit if they hadn't even reached out to me to notify me of this collection. She said that they would give me a discount if I settled the debt that day, I told them I was not going to settle anything without seeing a copy of this alleged bill. I gave the person my proper mailing address to send me a copy. She then told me she could email it to me if I prefered and that it would take XXXX to XXXX hours to receive the email. I told her if she could email it to me it would be great. On XX/XX/XXXX I received the email and all it was was a settlement letter and had no copy of the actual bill. I called XXXX XXXX and explained to the person what had happened previously and stated that there was no bill in the email I received only a settlement letter. They told me that the actual bill was being emailed to me. I could see on the settlement letter that the bill was from a XXXX XXXX XXXX of XXXX so I asked the person if they happened to have the doctors name. They gave me the name of a XXXX XXXX XXXX. As we were going over this informantion she said that something didn't look right on my account and was going to talk to her supervisor, she placed me on hold and I was disconnected. I didn't call back because I figured I would wait to see if I would actually receive a copy of this bill. While I was waiting for the bill, I reached out to the hospital and asked if they had sent anything to collections from my visit because I had paid the hospital bill that was sent to me. They stated that they did not sent anything to collections and that my account was paid off. I asked if they had a record of who seen me that day they said it was their XXXX radiology department and the XXXX was a XXXX XXXX. None of which is who XXXX said the bill was from. On XX/XX/XXXX I still had not received a copy of this alleged bill and called the company. I asked the person if I could speak with a supervisor. They asked what it was in regards to and I explained the situation and they connected me to a supervisor. I told her that I still had not received a copy of the alleged bill and further explained that there must have been some sort of mistake and don't beleve the bill is for me. She told me that it takes up to XXXX days to receive a bill request and that they sent the request on XX/XX/XXXX so obviously the XXXX person didn't send the request. Then she said I requested them send a settlement letter, which I clarified that I didn't request a settlement letter but requested a copy of the bill. I asked her how did their company try to reach out to me she mentioned they tried calling. I asked how could they have called me when they didn't have a phone number on file until I gave it to them the first time. She also stated that she did try to call me after my third phone call to the company when I was disconnected. I asked if she left a message she said she did not. At this point I told her I would give their company a week and if I didn't receive a copy of this alleged bill I would call back to have this removed from my credit. Then she changed her story to it takes XXXX to XXXX days to get the bill. At which point I told her I was going to file a complaint. This company has done nothing but not provide requested information and give me a different story every time I talk to someone. I don't beleive this is a legitiment bill and that they should never have put this on my credit without first getting a hold of me.

On XX/XX/XXXX I received a notification from my credit tracker indicating that I had a new trade line opened. After further researching it wasn't a new trade line rather a collection reporting made by Elevate Recoveries , LLC. I was shocked because I had never heard of the company nor had I received a validation of debt. I researched the company and located the toll free number online. I contacted the company the same day to inquire about the validation of debt. The representative indicated that they mailed out a validation of debt notice on XX/XX/XXXX. I asked why had the debt been reported if I hadn't been contacted or had the opportunity to dispute the debt and the collector indicated they had attempted to reach me and my voicemail was full and the account had been on their system since XXXX. I disputed the fact that any calls had been made or contact had been established and wanted to understand how the credit reporting took place without sending a validation of debt or establishing contact. The collector indicated they were not in violation of FDCPA which I disagreed and advised I would be filing a complaint. She indicated she would transfer me to a supervisor and ultimately XXXX transferred me back into the collection queue. Additionally, the IVR nor the collector provided the mini-miranda when I called nor during the phone call. The next day XX/XX/XXXX I received a settlement offer/validation of debt that was mailed to my parents address. The letter was dated XX/XX/XXXX and provided me with the opportunity to settle the debt in addition to validate it at the same time. The validation of debt notice was not clear and conspicuous, referring me to the back of the notice for all consumer related information. The notice did not include a tear-off form to send back to the debt collector to dispute the debt or take other actions, nor did the creditor wait a reasonable amount of time prior to submitting negative information to the credit bureau XXXX The reporting to the credit bureau, not only negatively impacted my score, decreasing it by XXXX points, the reporting also impacted my ability to move forward with a transaction that I was currently in the process of finalizing. The company acted reckless and not only provided me with false information when I called, they also are proceeding with credit reporting without allowing consumers the opportunity to dispute the debt in addition not providing a fully compliant validation of debt notice. The company did not provide an itemization of the debt owed, just a full amount and doesn't list what services were provided. My credit is damaged because of this reporting and I would like it removed immediately and will be reaching out to my attorney for information on how to file suit against the company and collector for failing to provide the mini-miranda and disclosing that their collect efforts were met because they attempted to contact me and my voicemail was full and never made established contact.

Truthfully, I wasnt sure which box to check because this company has stooped to almost every single one of them. 1 ) They have communicated after my asking them to stop via verified letter. 2 ) They have made false statements and misrepresented themselves 3 ) They sent a letter to my parents address thus improperly sharing my information 4 ) They have attempted to collect a debt not owed as I have no record of this bill and they said that they had closed the account due to an inability to verify it I submitted a complaint against XXXX for the same matter earlier this week. While their response was satisfactory, it is clear that it was not true as I continue to be contacted by Elevate Collections for the same alleged debt that XXXX attempted to collect and then subsequently discharged according to their response to CFPB complaint number XXXX. XXXX initially mailed a statement to my parent 's address in XX/XX/XXXX. When asked, the representative at Elevate said that they mailed a letter on the same date as XXXX despite me having no record of a second separate letter. Upon hearing this, I realized that Elevate and XXXX must be part of the same group and that he was talking about the same letter, but your representative assured me that that is not the case and that he has never heard of XXXX. When pressed further, your representative said that the debt no longer belonged to the original medical group he had initially said that it did and that XXXX XXXX XXXX XXXX was the new owner of the debt. On XX/XX/XXXX, I called XXXX myself and asked the representative what their relationship to XXXX and Elevate was. She candidly said " Oh- that's us! We're all the same! '' Knowing this, it is clear that XXXX XXXX, Elevate, and XXXX are all engaging in deceitful and fraudulent collections practices. XXXX assured me that the debt had been written off as of XX/XX/XXXX and that they would no longer contact me in large part because they could not produce sufficient proof to verify the debt, but it is clear that XXXX XXXX/Elevate have resumed their contact albeit under a different name and that they have escalated to lying about their practices. Should these deceitful tactics continue, I will have no choice but to escalate the issue further.

I had gone to an ER for treatment on XX/XX/XXXX. I was billed twice for this visit. One came from the hospital to my current address and I paid my portion after they submitted it to my insurance company. The other came from the doctor/ER and that was sent to an address I hadn't resided for over 10 years. I didn't learn about this other bill until I checked my credit report in XX/XX/XXXX and discovered it had been sent to a collections agency ( Elevate Recoveries LLC ). After speaking to the agent, she said they tried to contact me ( at the old address/phone number and I had no knowledge anyone was trying to contact me ). I tried to explain the hospital made an error and should have sent that bill to my insurance company instead of billing me directly. The hospital never went through my insurance company. Again, I tried to explain that I knew nothing about this and the agent said it was too late to dispute this and I had to pay the amount of {$1000.00}. Although, she did add they would settle it if I paid less ( around {$700.00} ). Since I wasn't getting anywhere with this agent, I contacted the company that sent the bill to collections ( XXXX XXXX XXXX XXXX ). I explained to them that they did not submit the bill through my insurance company as they should have. Since this was billed in XXXX, the woman helping me wasn't able to do much anything else. I contacted my insurance company and explained all of this to them. That same day ( XX/XX/XXXX ), my insurance company sent a Certificate of Coverage to XXXX XXXXXXXX XXXX XXXX. XXXX XXXX XXXX XXXX confirmed they received it on XX/XX/XXXX and sent notification to Elevate Recoveries LLC that it was being handled by my insurance company. Elevate Recoveries LLC still has the collection amount of {$1000.00} listed on my credit report. I have tried talking to them on several occasions but they keep transferring me to the same agent assigned to my case. I tried explaining this to her but all she would say is that I had to pay it and it was too late to do anything else. She would not let me speak to a supervisor when I requested to do so. I feel that I can not reason with them so I am reaching out to you for your assistance in clearing up this matter.

Good evening. I am reaching out for help because of the unresolved medical collections that continue to remain on my credit report in the amount of {$2000.00} in spite of me disputing having no knowledge of these companies or services rendered with all 3 credit bureaus in XXXX of XXXX and XXXX, and {$940.00} medical reported today. I also disputed unsuccessfully the loan balance appearing for {$4100.00} which should be XXXX balance. I contacted the bank and was told they reported my XXXX balance in XX/XX/XXXX to all 3 credit bureaus. There is also a repo reported as a charge off for XXXX XXXX on XXXX but does not appear on XXXX, this was disputed in XX/XX/XXXX and XXXX as incorrect but has not been corrected. These unresolved items is attributing to a lowered credit score impacting my ability to attain proper housing and a wheelchair vehicle to transport my XXXX mother. Additionally, In accord with law 15 U.S.C which is the FDCPA section 1692c ( b ) please help remove any and all negative items reported by all three credit bureaus as these reportings are a violation of law. No prior written consent was ever given to any debt collector to communicate any debt. 15 U.S. Code 1692c Communication in connection with debt collection ( b ) Communications With Third Parties- Except as provided in section 1692b of this title, without the prior consent of the consumer given directly to the debt collector, or the express permission of a court of competent jurisdiction, or as reasonably necessary to effectuate a post judgment judicial remedy, a debt collector may not communicate, in connection with the collection of any debt, with any person other than the consumer, his attorney, a consumer reporting agency if otherwise permitted by law, the creditor, the attorney of the creditor, or the attorney of the debt collector.

I hope this letter finds you well. I am writing to raise an important issue that pertains to the Fair Credit Reporting Act ( FCRA ). Under the provisions of the FCRA, specifically 15 USC 1681 section 602 a, there exists a crucial need to ensure that consumer reporting agencies, such as XXXX and XXXX, fulfill their obligations with utmost fairness, impartiality, and respect for consumers ' right to privacy. However, it has come to my attention that certain actions undertaken by these consumer reporting agencies may be in violation of the aforementioned principles. This is deeply concerning and raises questions about their commitment to upholding the rights of consumers. Given the importance of consumer reporting agencies in maintaining accurate credit information and protecting individuals ' privacy, it is crucial that appropriate measures are taken to address any potential breaches or misconduct. I kindly request that you thoroughly investigate the practices of XXXX and XXXX to ensure compliance with the FCRA. It is imperative that these companies fulfill their responsibilities in a manner that prioritizes the rights and interests of consumers. I appreciate your attention to this matter and your efforts in holding consumer reporting agencies accountable. I trust that you will take the necessary steps to address these concerns and maintain the integrity of the credit reporting system. I am writing to bring to your attention a matter of utmost concern, which involves a violation of the law. As per the provisions of the Fair Credit Reporting Act ( FCRA ), specifically 15 U.S. Code 1681s2 ( A ) ( 1 ), it is unlawful for any person to provide inaccurate information about a consumer to any consumer reporting agency, if they have

TODAY : XX/XX/19 I RECEIVED A THREATENING PHONE CALL ABOUT A MEDICAL DEBT THAT OWE FROM A REPRESENTATIVE AT ELEVATE RECOVERIES. I HAD NO IDEA THAT THE DEBT WAS EVER OWED DUE TO THE FACT THAT I MEDICAL INSURANCE COVERAGE AND HAD NEVER BEEN CONTACTED ABOUT THE DEBT IN THE PAST. I ADVISE THE REPRESENTATIVE THAT ONCE SHE SENDS THE STATEMENT TO ME IN THE MAIL I CAN PROCEED TO LOOKING OVER MY FINANCES TO SEE IF THIS IS SOMETHING THAT I CAN AFFORD TO PAYOFF OR MAKE PAYMENTS ON. SHE BEGAN TO BADGE ME OFFERING SETTLEMENTS, ADVISING THAT THE ACCOUNT WOULD IMPACT MY CREDIT, AND JUST REFUSED TO ACCEPT THE FACT THAT I WAS NOT ABLE TO PAY ON THE ACCOUNT AND I HAVE NO KNOWLEDGE OF EVER OWING FOR THIS DEBT. I ASKED THAT SHE REMOVES MY ***CELL PHONE NUMBER*** FROM THEIR DATABASE, SHE PROCEEDED TO BEING VERY CONDESENDING, PRACTICING DECEPTIVE COLLECTING ON MY ACCOUNT, SPEAKING OVER ME AND TELLING ME THAT SHE WOULD CONTINUE CALLING ME ABOUT THIS ACCOUNT BECAUSE I OWE THE DEBT. SHE WAS RUDE, YELLING, AND KEPT REMINDING ME THAT I OWED THE DEBT AND BECAUSE I PROVIDED MY CELL PHONE SHE WOULD CONTINUE TO USE IT. WHEN I ASKED FOR A SUPERVISOR SHE SAID THAT NO ONE WAS AVAILABLE AND EVENTUALLY JUST HUNG UP ON ME. I CALLED BACK SPOKE WITH A SUPERVISOR AND HE TOO KEPT BADGERING ME ABOUT PAYING OFF THE DEBT. HE OPENLY ADMITTED THAT ELEVATE RECOVERIES **DOES NOT REPORT TO CREDIT BUREUAS** ; I ASKED IF YOU DO NOT REPORT TO CBR ; WHY ARE YOU ALL USING THAT AS A REBUTTAL TO SCARE P [ PEOPLE INTO PAYING THESE DEBTS? HE SAID THAT WE JUST HAVE TO MAKE CUSTOMERS AWARE. I TOLD THE SUPERVISOR, IF YOU ALL ARE NOT THE ONES TO DO ANY REPORTING THEN THREATENING ME TO PAY BECAUSE ITS IMPACTING MY CREDIT IS A DECEPTIVE COLLECTION PRACTICE.

I received my credit report and noticed Elevate Recoveries , LLC placed an open collection on my report in the amount {$2200.00}. First, I do not have a legal contract with this company. Secondly, Elevate Recoveries is in violation of consumer law code 15 USC 1681a Section 3. Per 15 USC 1681a Section 3 Restriction on sharing of medical information.Except for information or any communication of information disclosed as provided in section 1681b ( g ) ( 3 ) of this title, the exclusions in paragraph ( 2 ) shall not apply with respect to information disclosed to any person related by common ownership or affiliated by corporate control, if the information is ( A ) medical information ; ( B ) an individualized list or description based on the payment transactions of the consumer for medical products or services ; or ( C ) an aggregate list of identified consumers based on payment transactions for medical products or services. This company has also violated H.R.2537 - Consumer Protection for Medical Debt Collections Act per Congress.gov. that states This bill restricts the inclusion of medical debt on a consumer credit report. A consumer reporting agency is prohibited from including on a consumer credit report, and a person is prohibited from reporting to such agency ( 1 ) medical debt arising from a medically necessary procedure, or ( 2 ) other medical debt less than a year old. Lastly, the Elevate Recoveries violated the HIPPA law. The HIPAA law protects patient privacy, including third-party debt collectors accessing your information. I never provided a HIPAA release form.

This debt had previously been removed by another company. I received a notice from elevate stating I owed a debt for a bill that was previously disputed and removed with another company. " XXXX XXXX XXXX, FL XX/XX/XXXX Elevate Recoveries , LLC XXXX XXXX XXXX Re : XXXX Dear Elevate Recoveries LLC, I am responding to your contact about collecting a debt. You contacted me by mail, on XX/XX/XXXX and identified a debt. I do not have any responsibility for the debt youre trying to collect and has previously been reported to Consumer Financial Protection Bureau. If you have good reason to believe that I am responsible for this debt, send me debt verification via mail with the documents that make you believe that. Stop all other communication with me and with this address, and record that I dispute having any obligation for this debt. If you stop your collection of this debt, and forward or return it to another company, please indicate to them that it is disputed. If you have reported it to a credit bureau report that the debt is disputed. Thank you for your cooperation. Sincerely, XXXX XXXX " Elevate failed to provide information to verify this date and did not respond but instead reported negative collection agency. I disputed the collection on my XXXX report however elevate state it was a valid debt.

I have previously submitted a XXXX Complaint to XXXX XXXX XXXX XXXX XXXX back a few years ago for a medical bill which I accepted the resolution on the XXXX website and resolved my debt with them. However, XXXX XXXX unexpectedly went out of business and instead of reconciling the debt, they sold my debt to XXXX XXXX and then XXXX sold my debt to Elevate Recoveries. I explained the situation to XXXX years and they left me alone. Now 2-3 years later, I now have Elevate harassing me about a debt that I don't owe and now placing the debt on my credit report. Ive explained this to numerous Elevate associates and asked repeatedly if this issue was on my credit. They said no. I have at least requested that Elevate provide me a debt validation letter on 2 separate occasions that I remember, because I do not owe this amount. They have not complied and only have provided me with their invoice stating that I owe them an amount. There is no breakdown of services rendered, dates services occurred, what the insurance paid, what I paid, nor what XXXX XXXX agreed to and resolved prior to going out of business. Elevate Recoveries is reporting inaccuracies to my credit report. Your responsibility as a recovery agency is to provide me with the proof that I have requested on previous calls with your company. This is a violation of FCRA and FDCPA. This seems like a scam and the ongoing harassment needs to stop.

My former insurance carrier ( via my parents ) was not billed for services when I received medical treatment at XXXX XXXX in XXXX, Texas ( XXXX ). Instead, the facility listed me as self-pay when my valid insurance information was provided. The former insurance XXXX, XXXX XXXX XXXX ( XXXX ), refuses to take calls or correspondence regarding prior coverage, although they were the active carrier at the time services were rendered. XXXX is also known for not changing emergency visits to in-network to avoid paying valid medical expenses, since when seen in urgent or emergency care, you have no control over the network a physician uses for billing. My parents have also had difficulty with XXXX not paying bills or taking calls, since my mother 's employer chose another carrier, but, in this case, I have no record that they were ever billed. Elevate Recoveries placed a collection on my credit report, listing a different creditor than the facility that I visited, but for the same bill amount. I also deserve to be treated fairly by Elevate Recoveries, XXXX and XXXX-XXXX, and should only be responsible for any remaining balance after my insurance pays based on my family 's coverage at the time of service. Additionally, XXXX-XXXX did not advise me that I would be billed a premium totaling approximately 20 % more ( {$210.00} ) for visiting the 24-hour facility late at night.

Elevate Recoveries contacted me as early as XX/XX/XXXX. When they called, they didn't disclose who they were and the purpose of their call. The young lady on the phone began asking me for personal information such as my name, address, the last digits of my social security number. And when I didn't give the information and asked who they were and the purpose of the call. The lady was very rude and obnoxious. Over a period of 3 months I continued to get phone calls from Elevate Recoveries. I noticed from a credit card statement that my credit dropped from XXXX to XXXX. I contacted XXXX to find out why. XXXX mailed me out paperwork to show me what was on my credit report. I received the report and I seen that XXXX Recoveries submitted a claim of {$1800.00} for XXXX XXXX XXXX against my credit. Damaging my credit. I contacted XXXX XXXX XXXX XXXX Department. Come to find out, the bill from XX/XX/XXXX from XXXX XXXX XXXX was already paid in full. The paid in full letter is dated XX/XX/XXXX. Elevate Recoveries claim they didn't know the debt was a workmans compensation claim. Which is not an excuse. Because the bill was already paid in full at the time Elevate Recoveries tried collecting for this bill. Yet Elevate Recoveries immediately placed a claim against my credit without first having the correct information.

Company called me stated I owed a medical debt. They wanted me to pay over the phone so it would not go on my credit report. I was contacted by this company on XXXX XXXX which I was informed I had 45 days to pay in order it being reporting to credit report regarding a medical debt. They wanted payment over the phone without any written verification showing a detailed information over the debt owed. I told them no, not until I get something in writing. I asked them to email a letter to my email address the rep stated that was unable to email or provide any documents by mail. By law I told them I would need some type off documentation showing the debt is owed and the payment being provided is accurate. I asked the rep if I make a payment will they be able to provide proof. They said they cant even send proof of payment out to me. They said they dont send letters or emails out. I have called this company numerous time to get the issue resolved. Well, on XXXX XXXX I called and spoke to XXXX representative of this company. This individual has no idea about collection laws. She basically, told me to pay the balance to avoid any other issues I may occur with credit reporting. From my understanding I was thinking the bill was paid in full by insurance company XXXX and XXXX since this was apart of an accident.

I never received any notices from Elevate Recoveries. I only became aware of this fraudulently attempt to collect the amount of {$660.00} after it appeared negatively on credit report. The collection notice states the original debt is from XXXX XXXX XXXX, which has been closed for almost a year. I called Elevate Recoveries and the several weeks ago and was emailed the statement, that I will later upload in this compliant later. Elevate Recoveries has placed this collection on my credit report and has caused my credit score to drop down more than XXXX points. I have been in communication with XXXX, my insurance provider at the time, and was giving the following information. On XX/XX/XXXX, Claim # XXXX, services provided was for {$1400.00}, {$660.00} Contracted amount was for {$250.00} XXXX paid {$83.00} and my co-pay was for {$170.00}, which I paid. Please not that on XX/XX/XXXX, Claim # XXXX, services was for the amount {$660.00} and was from XXXX XXXX XXXX OF XXXX XXXX, and not ( XXXX XXXX XXXX ). I went to the hospital in XXXX and received 3 different bills as expected. I had paid all the bills and was making payments on 1. I made the payments as required and when it was all done I received a refund of XXXX for over payment. In XXXX this account was filed on my credit report, to which I filed a dispute, proved my payments, and it was removed. This same account is now back on my credit report from a different collection agency. On XX/XX/XXXX I received a phone call from Elevate recoveries LLC saying I owed XXXX, I said that can not be true and I have confirmation numbers of payments. They said confirmation numbers do not help them because it was not their numbers. They can not give me any information that I ask and have no idea who what when or where, they just know I must pay them. I said I would like a copy of everything they had and to send it to my new address. She said she would not do that and that she could send me a statement but with no payments or anything accounted for. Within 15 days they put it on my credit report without sending me anything. I called them was put on hold and no one ever picked up.

A {$500.00} collection account from Elevate Recoveries LLC showed up on my credit report out of nowhere. It says XXXX XXXX XXXX I keep track of all of my bills and receipts whether I have the money or not, and I dont have an invoice, bill or open balance from any person, business or healthcare provider for the amount of {$500.00} dollars. I havent had health insurance in a long time so I know that every appointment, prescription, service I get I need to pay for OR I go through the billing dept and always qualify for low income. Not sure why this company is reporting on me. All reports go to my lawyer and Ive brought this up when it first came up and also provided information. I tried getting removed through XXXX XXXX back then ( mistake apparently ) and nothing happened, they said no. Also, if this is a medical/healthcare provider invoice or bill, isnt it against HIPPA law to report this, especially as a collection account? I would hate if my medical records were put out in the public for financial gains. I would like this investigated and removed from my credit collection reports.

This company is engaging in the illegal practice of DEBT PARKING. I was never notified of this debt and I have not lived in Texas for over 7 years. This is a junk debt buyer and they have not provided validation of the debt to me. They also continue to illegally validate the debt to the credit bureaus and this has detrimentally affected my credit scores. They are violating the Fair Debt Collection Practices as I was never notified of this fraudulent debt in writing before placing on my credit file. I could not dispute the validity of the debt within 30 days. I had no medical debt when XXXX Texas XXXXXXXX XXXX XXXX years ago and I was not notified of any purported debt during my residency there. I have disputed this with the credit bureau and this company continues to validate the debt with them. No one will send any paperwork to me about this debt.

I had a XXXX debt added to my credit report that I have never received any communication about and was not allowed the opportunity to dispute the item or ask for information to verify. I moved recently and have attempted to communicate with company through email about disputing the item but never received a response or call. I believe the company is not following laws by not sending communication to me and allowing me to respond to it, instead they reported it to the credit buries dropping my near perfect XXXX XXXX down due to a small debt. If the item would have been verified and I received the documentation necessary showing it was owed, I would have worked with the company. Again, I believe the medical debt is incorrect and I do not owe a balance. I ask it is immediately deleted from my credit and the company sends the required documentation and allowed me to dispute the item without punishing me.

Dear collection agent, I noticed a negative reporting on my credit report and as allowed under the Fair Debt Collection Practices Act ( FDCPA ), I am requesting that you allow me to validate the alleged debt. I am aware that there is a debt from Elevate Recoveries LLC, but I am unaware of the amount due and your crediting reporting does not include a breakdown of any fees. Additionally, I am allowed under the Health Insurance Portability and Accountability Act ( HIPAA ) to protect my privacy and medical records from third parties. I do not recall giving permission to XXXXXXXX XXXX Hospital and/or give them authority to release my medical information to a third party. I am aware that the HIPAA does allow for limited information about me but anything more is to only be revealed with the patients authorization. Therefore my request is twofoldvalidation of debt and HIPAA authorization.

Elevate Recoveries fraudulently reported a debt from XXXX XXXX XXXX, amount {$650.00}. I contacted Elevate Recoveries today - XXXX - to inquire about this account. They stated that they sent out 1 letter on XXXX. When I informed Elevate that I had not received this letter, they informed me that it went to the wrong address. When I asked why they would then report it to XXXX on XXXX, the representative claimed that they do not report to the credit bureaus. I told her that I had contacted XXXX, who informed me that this was reported by Elevate. She still claimed that they do not report to the bureaus ; so I asked her if they didn't report it and XXXX did not, how did it get on my credit report. Her response was that I should file a police report and immediately disconnected the call.

A collections account was filed on my credit report from a creditor named Elevate, LCC. The original creditor was XXXX XXXX XXXX, XXXX. I had no knowledge of this bill until it was reported on my credit report. I called the collections agency and got the date my daughter visited the ER. It was XXXX. I called the insurance company and no bill was submitted to them for payment from the original creditor. Ive tried to find information for contacting the original creditor. I have found no contact info and the collections company refused to give me any contact information and told me to look it up on my own. I have no documentation to submit because the only contact theyve had with me was on the phone on XXXX. Ive never even recieved a bill in the mail from the original creditor.

XX/XX/22 A female agent called to discuss a collection that I have. I told her that I can't make any payments until after the 1st of the year. She offered a settlement option and I told her again I don't have any extra funds until after the 1st of the year. Then she began harassing me and continuing to question why I can't make a payment, that I've owed the amount for over a year. I told her I didn't care about that I'm not making any more payments until I can afford to. She begin to mock me, be very disrespectful and unprofessional. I called back and got the same type of response from a different employee. I can't believe people like that represent a collection company.

I received a call from ELEVATE RECOVERY. The call was today XX/XX/24 at XXXX to XXXX. When the phone rang there was a pause which led me to believe it may be an automated dialer. I asked the rep if that was the case, and if so, please remove my number from the automated dialer system. She then advised, " ok so you don't want to pay your bills. '' I'm not sure who the company is, nor did I EVER identify myself as the right party contact. I believe this to be a violation of the FDCPA due to the fact the rep advised that there was a debt owed without establishing or confirming who she had made contact with.

Elevate Recoveries LLC XXXX XXXX XXXX XXXX. XXXX TX. XXXX Placed a fraudulent medical collection debt on my XXXX Credit Report. I have never been notified by the accuser that I am obligated to render service to any debt as is required by Federal Law. If I had been, I would have promptly disputed the debt as is my legal right and demanded full " Validation '' of the debt in question. Attention not to confuse legal " Validation '' with mere " Verification '' under the law as the demand of the former outweighs the latter. This alleged debt was never bought to my attention because it is invalid.

Received a notification about this hitting my credit report, when I have no idea who they are, it says a medical debt, but I am a XXXX XXXX XXXXXXXX veteran and have been under Va care since 2010. ALL my medical is covered by the VA, so not sure how they're saying I owe them anything.
