First, I have disputed with XXXX for them to delete the addresses listed below as they are inaccurate and erroneous : XXXX XXXX XXXX XXXX XXXX, XXXX, GA XXXX XXXX XXXX XXXX XXXX, XXXX, GA XXXX XXXX XXXX XXXX, XXXX, GA XXXX XXXX XXXX XXXX XXXX XXXX, XXXX, GA XXXX XXXX XXXX XXXX, XXXX XXXX, VA XXXX XXXX XXXX XXXX, XXXX , GA XXXX XXXX XXXX XXXX XXXX XXXX XXXX , GA XXXX DELETE ALL ADDRESSES LISTED ABOVE!!
I have disputed these inaccurate account entries with XXXX on numerous occasions and they have stated that the items were verified. To that end, I disputed with Darnel Recovery, the third party collection agency that is reporting this inaccuracy as well. Darnell alleges that a balance for 2 accounts is due in the amount of {$1100.00}. One account for {$650.00} and the other {$480.00}, yet they were unable to provide any documentation that bore my original signature for both entries, nor any documentation that reflected the financial responsibility page for both entries. Lastly, they never ceased reporting of these inaccurate entries to the credit bureaus as requested so that I may validate the alleged debts.
To that end, I visited XXXX XXXX XXXX ( since renamed from what Darnel alleges ) and requested a complete and comprehensive document that reflected any charges that I may allegedly owe the facility. See attached documentation that reflects a mere 1 account and a balance of {$350.00}. XXXX has allowed Darnel Recovery to continue to damage my credit worthiness with these inaccurate entries.
DELETE these entries immediately from my credit report per attached documentation that fully justifies the inaccuracy and untruthfulness in these entries being attached to my report.

On XXXX upon trying to refinance my home the mortgage company advised me that there was a lien against my home from a company by the name of Darnel Recovery, I was in total shock as I have never acknowledged this matter or was served this documentation by the local authorities back from 2007. I called to speak to XXXX XXXX and she stated that I needed to send my id through a email and that the document indeed was me, now the problem I have with this is that My HIPPA laws have been violated my information has been released to a third party. They are in violation of the Fair Credit Reporting Act twice over. First by the original alleged creditor giving my information to a third part debt collector and secondly by submitting the Medical debt to the Superior and Magistrate Courts in both XXXX and XXXX county where I do not reside at which there was a lien judgement against my home. I would like to see a validation of Debt and HIPAA authorization because I didn't authorize my information to be shared nor did I receive any information by mail or from any proper authorities.

I received my credit report and notice debt from this agency on XX/XX/XXXX. In XX/XX/XXXX, I wrote a letter and sent it certified mail asking for proof that this was my debt and where I owed the company. The company failed to respond sending the information I requested. In XX/XX/XXXX the company had yet to respond so I sent another certified letter requesting this information to be validated and since it could not be validated to be removed from my credit report. The company will not respond to any of my requests nor follow up and the debt is in the amount of {$150.00} from XXXX XXXX XXXX from XX/XX/XXXX. 1. This balance was paid in full by my insurance company before XXXX XXXX XXXX closed. 2. The hospital was purchase by XXXX XXXX and they did not update my account information to show paid in full. 3. All of a sudden no one can find my bill nor services from that date in XX/XX/XXXX. 4. This company wrote me out a hand bill physically which does not prove anything.

I have asked Darnel Quick Recovery to validate a collection they are reporting on my credit reports on XXXX/XXXX/16 and XXXX/XXXX/16 and they have failed to provide anything that would support the fact that they are attempting to collect on a debt without an agreement or contract. Nor have they supplied anything other than a statement on one of the collections. I told them that I would be reporting them to you but they obviously do n't fear you because they failed to delete themselves or answer questions about the debt ( s ) that they are using to damage me financially. Its damaging my credit and it is important to me.

I mailed this company a formal dispute letter with my current address on XXXX XXXX 2015 hoping to get a copy of the original debt today XXXX/XXXX/2015 I contacted the company to advise them that is has been over 30 days and I should have got the documents back by now I spoke with XXXX and XXXX XXXX and she advised that she sent the paperwork to an old address even though my current address is clearly printed on the dispute letter I sent them. I also advised them several times this is not my debt and also not to send me anymore emails with company jargon that I ca n't understand.

I have made many attempts to contact XXXX XXXX with regards to a debt that they are reporting in collections on my credit file. I have sent many letters asking them to provide me with proof of this debt and to date they have not done so. I have sent them a letter as recent as XXXX XXXX, 2015 via certified mail. I have also disputed this account with XXXX who report this account as verified. I am leary of this since I ca n't get a response back from Darnel Quick Recovery. I mailed the company a formal letter of dispute on XXXX/XXXX/2015 and requested verification of debt. I called today to advised them it 's been past 30 days and I still have not gotten any documents.
