RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT XXXX XXXX data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX XXXX XXXX XXXX XXXX XXXX. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with XXXX XXXX XXXX XXXX, Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C . 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. 1. Creditors Bureau Associates & BAL. {$1500.00} I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency, report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from alleged identity theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer that the information relating to the transaction by the consumer. I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. Ive received a copy of my credit report and you are claiming I have an account with you or I owe you something. I have never entered into any business arrangement and I have never signed any contract to conduct any kind of business with you. Also, because you are a 3rd party collector, you have no permission to furnish any information on my credit report in the first place. The law states that before you collect any information, about me, you have to get my authorization to access my credit report for any reason. You didnt do that first, and theres no way I would ever give a slimy collection company permission to slander me on my credit reports. Heres the law in case you never read it before. ( o ) Excluded communications. A communication is described in this subsection if it is a communication. ( 5 ) with respect to which. ( A ) the consumer who is the subject of the communication ( i ) consents orally or in writing to the nature and scope of the communication, before the collection of any information for the purpose of making the communication ; ( iii ) in the case of consent under clause ( i ) or ( ii ) given orally, is provided written confirmation of that consent by the person making the communication, not later than 3 business days after the receipt of the consent by that person ; Now, I am also exercising my right under the FDCPA to dispute the alleged account and demand validation from you. 604. Permissible purposes of consumer reports [ 15 U.S.C. 1681b ] ( a ) In general. I am writing the CFPB as a final formal complaint against CREDIT BUREAU ASSOCIATES for violating my rights under the FDCPA. They are in clear violation of my consumer rights. I have asked them repeatedly to produce evidence of the fact that I owe them this debt and I am going to postulate the following to you in response to the last dispute I submitted to the CFPB. They replied that they sent me evidence of this debt on XX/XX/XXXX which I have attached the evidence of the medical records they mailed me. XXXX. They did not mail any evidence with the medical records other than the full medical records of the procedure done to this individual. XXXX. Their rebuttal was that I owe the " creditor '' this debt. First of all, it is none of your business who I owe anything to if it does not directly involve you. So since you claim to be collecting this debt, you are required to submit clear evidence that you have a right to have these medical records without a HIPPA release. I know I did not sign a HIPPA release because I did not get any evidence from you proving this. Since you " claim '' you sent me a letter on XX/XX/XXXX to substantiate this debt, then attach the evidence in your rebutal here so that the CFPB can be a witness to the evidence you sent besides medical records which clearly is a violation of PHI and HIPPA according to the OCR and the FDCPA. As for the original creditor, I believe they are in violation on FCRA as well for selling medical records on a patient without a HIPPA contract signed by the patient authorizing you to have access to the private medical procedures performed on a patient, so if this is the case, then CBA XXXX and XXXX XXXX XXXX are at risk of a law suit, where you will have to provide evidence of this in court and you will be required to pay all court costs if you lose your premise in court, which is what I intend to do next, as I have already consulted a lawyer about this matter and am ready to escalate this. The next letter you will get from me is a letter from my lawyer petitioning you on my behalf. XXXX. You are the one who needs to prove to me why you have been reporting this debt on my credit since XX/XX/XXXX and validate the reporting of this debt and I never received anything in writing from you advising me that you were collecting on this debt so that I could dispute this debt with you prior to you reporting this to the credit bureaus to damage my credit worthiness which is causing me great distress and I will be suing you for defamation of character if we end up in court. I am automatic payment and have been for the last two years. One of the accounts is paid off. The other account is still being auto draft on the XXXX of every month. I have advised the credit reporting/collection agency over 5 times, that this debt is not correct. The collection agency advised me that as long as I do not pay them directly they will continue to place this incorrect information on my credit report. I offered to provide them with statements that show I do not owe and that I am on auto-draft. I made an appointment for XX/XX/XXXX to speak to the Dr . I provided the Dr with my bank statements and credit card statements to show I was paying and yet still this account was placed with the collection agency- Credit Bureau Associates- XXXX XXXX XXXX XXXX Georgia I started out owing over {$1000.00} on one account and almost {$900.00} on the other account. XXXX XXXX and XXXX XXXX is the same company. Now that the balance owed is less than {$300.00} I found out today that the original balance has been placed on my credit report, {$180.00}, {$580.00}, {$250.00}. The same original amount. I have spoken with Both Companies in the past about this error. The agent at XXXX XXXX XXXX XXXX XXXX reached out and requested verification on the amount owed. To this date, no response. The agent at XXXX also made a conference call to XXXX XXXX XXXX and XXXX XXXX XXXX and XXXX trying to get a resolution as to why the account is turned over to collection. XX/XX/XXXX XXXX XXXX advised they had some billing issues and changed billing companies and billing software more than 2 times within a 6 months span. Creditors Bureau Associates advised me that they are helping XXXX and XXXX out by collecting the payments for them, my question was do you collect payments while the account is on auto-draft? It was during that conversation that I learned that as long as I was paying the Dr office directly this information would continue to re-appear on my credit report, " you know they are having some issues, I have tried to get you to pay us instead of them, to make sure it stays off your credit report. '' I have been unsuccessful in my attempts to explain that I do not owe the balance that is reported on my credit. I owe less than {$300.00}. I have visited the XXXX XXXX XXXX and XXXX XXXX XXXX for Resolution. Once I was told they did not have my signature for autopay, However, I was already on auto-pay. A few months after that I was told the signature form was in my file and no one knew why the auto-draft discontinued Earlier this year, Creditors Bureau Associates contacted me about the account and I offered to send them my billing statement that shows the conflict with the amount owed and I offered to show verification that I was still on auto-pay. I was told that they would reach back out to XXXX and XXXX XXXX. After a few weeks, I called them back to see what was the outcome because the person they had been speaking with would not return my call. Creditors Bureau Associates have a personal relationship with that company and they are aware of the billing issues the company has, yet they still make attempts to collect debt balances that are not owed. I missed an appointment in XX/XX/XXXX I was charged {$50.00}, that appointment did not happen because I was already there for an injection. The charge was supposed to be removed, instead, they sent me a separate bill for the same {$50.00} that is already on the original billing statement. This separate invoice occurred after one of my Auto-drafts did not end even after the balance had been paid. I received a bill for the {$50.00} they over-collected for that says missed appointment XX/XX/XXXX. However, the overpayment occurred in XXXX. Each time I ask Creditors Bureau Associates to verify what I owe, they are not able to confirm what the correct balance nor can they validate payments made to XXXX and XXXX XXXX XXXX that have yet to be credited to my account.

XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX MN XXXX Creditor Bureau Associates XXXX XXXX XXXX XXXX, Ga XXXX Date : XX/XX/XXXX Re : Acct # XXXX To Whom It May Concern : I have previously sent you a request to validate my debt under the Fair Debt Collection Practices Act and all I received was an itemized list of the alleged charges you claim I owe you. This does not meet the Federal Trade Commissions guidelines of what constitutes proper debt validation In addition, in the case of XXXX Vs. XXXX ( IN THE COURT OF APPEALS OF INDIANA, No. XXXX ), the court ruled that : 15 U.S.C. 1692g ( b ) ( emphasis added ). On XX/XX/XXXX, nineteen days after the date of XXXX debt collection letter, XXXX counsel XXXX sent XXXX a letter declaring that XXXX disputes your debt collection-related allegations, denies the same, and demands strict proof and verification thereof. Record at 21. As such, XXXX should have ceased his debt collection efforts immediately upon receiving that letter. Instead, XXXX proceeded to obtain a default judgment against XXXX on the debt collection claim before he had mailed XXXX the necessary verification and, thus, violated 15 U.S.C. 1692g ( b ). XXXX maintains, however, that there was no violation of the FDCPA because he sent adequate verification of the debt [ to XXXX ] in the XX/XX/XXXX notice of claim. Brief of Appellee at 13. Specifically, XXXX claims that a copy of the consumer credit contract between XXXX and XXXX XXXX attached to the notice of claim provided sufficient verification of the debt within the meaning of 15 U.S.C. 1692g ( b ). We can not agree. The contract in no way provides sufficient verification of the debt. A review of the document reveals that it identifies only the terms of XXXX loan, including a 17.99 % annual interest rate and the original loan amount of {$2500.00}. The loan agreement contains no accounting of any payments made by XXXX, the dates on which those payments were made, the interest which had accrued, or any late fees which had been assessed once XXXX stopped making the required payments. First of all, I do not know who CREDITORS BUREAU ASSOCIATES are, and according to 15 U.S. Code 1681a ( w ) The term Bureau means the Bureau of Consumer Financial Protection, so why are they referring to themselves as a Bureau? Thats deceptive and is a violation of 15 U.S. Code 1692e - False or misleading representations. They put a make-believe false account number on this letter they sent me but I do not have a contract with them. Furthermore, these people are in violation of 15 U.S. Code 1681b ( 2 ). I have not given them written permission to put anything on my consumer report. And I did not give them permission to mail me letters to my address. On XX/XX/XXXX I received a letter from Credit Bureau Associates of Georgia ( CBA ) dated XXXX XX/XX/XXXX. This was for a medical service for XX/XX/XXXX. The letter states to notify the office within 30 days ( XX/XX/XXXX ) to dispute the validity of this debt of {$110.00}. That same day after receiving the letter from CBA I paid the original creditor the {$110.00} and received a receipt from the original creditor were I received medical service in XXXX. That same day XX/XX/XXXX, I write a letter to CBA stating the debt was paid to the original creditor and I attached a receipt of payment. I then state to please respond within 10 business days to request that the debt was settled with the original creditor. A month goes by and I received a letter from CBA on XXXX XX/XX/XXXX dated XXXX XX/XX/XXXX. The letter states that I still owe the debt and returned my original letters back to me including the receipt. CBA also included in my letter an original receipt of payment from the original creditor showing that they I the patient paid the original creditor on XXXX XXXX the {$110.00} which was highlighted by CBA. Since CBA has the receipt from the medical billing department that I satisfied the payment with the original creditor, why would CBA still send me a letter stating I currently owe {$110.00}? I then call the original creditor at XXXX and ask to speak with the billing department. I then explain the situation and she states that she will call them to settle the confusion as she showed that I paid the account on XXXX XXXX on her end. She then tells me that she would call me back once CBA has confirmation from the billing department that the account is settled with the original creditor. She later calls me back and states the account is settled with CBA as they now know my account was paid with the billing department of the original creditor. I submitted a complaint to which the company brushed me off - see below. Credit Bureau Associates of GA located in XXXX, GA reported XXXX ( XXXX ) medical bill balances to my credit report without sending me notices in the mail. According to the Federal Credit Report Act you have to notify in writing prior to reporting to ones individual credit report. I had no knowledge of these balances. I would have paid the balances from the start of the original credit would have sent me the notices in the mail and they fail to do so also. On the other hand, the new law that was passed in XXXX was violated also. A medical collection debt can not be placed on ones credit reporting until after 6 months of medical provider receiving explanation of benefits from the patients insurance company. CBA reported before the 6 months which violates my rights as a consumer. I will be contacting the attorney general and requesting an audit for all their accounts and possibly generating a link for consumers to sign a petition for the negligence this company has caused. Also, I will be filing a claim in court if these XXXX accounts are not removed. Im more than happy to pay the doctor if I owe the amount but creditors should not be allowed to file until they have the resolved info. I filed a grievance with Ambetter insurance back in XXXX for the type of services and treatment I received from the orthopedic. My family of 4 got XXXX Testing done by XXXX XXXX XXXX on XX/XX/XXXX. When I registered each of my family members, I put the insurance member for each person on the registration information. This was verified at the appointment. Unfortunately, the facility NEVER ran my insurance information. I called and asked about it after receiving an invoice and was told that they haven't had to chance to file a claim with the insurance company and will soon. After the 2nd invoice, I called them and my insurance provider who verified that no claims for XXXX testing has been on my insurance. After it was sent to collections, the collection company told me to call my insurance and put a claim for the testing. I did call my insurance company and was told that the provider needed to put a claim and not myself. I submitted a complaint against this debt collector on XX/XX/2021 and they replied on XX/XX/2021 stating that they are not the appropriate company to address my issue. Let me very clear. This is definitely the company I need to be addressing my issue with and by this second complaint against CBA Macon to the CFPB, I am building my case to use against them and the hospital that sold this information to them. This is a CLEAR HIPPA violation! I have the full medical records showing EVERY detail of the patient which is a PHI violation as well. Not only did they send me the full detailed medical record, but they sent no explanation as to why they believe they have a right to report this on my credit without having valid evidence. Now they are lying and saying they are not the company to deal with my issue. If CBA Macon does not IMMEDIATELY remove this reporting from my credit until they can PROVE that they 1 ) Have a HIPPA waiver signed by me and 2 ) that they have a right to collect on this debt and 3 ) that I am the person whom they are damaging their credit worthiness without even sending me information that they were going to XXXX up my credit. I will be turning this matter over to my lawyer and handle this matter in court and not only will I bring charges against the original creditor, but CBA Macon as well. Yesterday I noticed that Credit Bureau Associates of XXXX, GA reported a medical collection on my credit reports. First I called XXXX, to talk to a representative to see if that collection could be reported. As we investigated, it was shown that the company should not have reported to my credit. That the medical debt was not passed due for 1yr with their agency. That law was passed this year 2022. The reprint then informed me to call the collection agency and ask them to remove the collection since it's in violation of th fcra. I called the collection agency and spoke with an representative, she informed me that they can not remove the collection, but can close it if I set up an arrangement. I told her I can set up an arrangement if the debt was verified and not in violation of the hippa law that protects my health information that they are not privileged to. So I asked again could it be removed. She told me no and transferred me to " XXXX '', a supervisor. XXXX continued to say that she can't remove it and it's not in violation and she doesn't have to validate any debt pass 45 days. I asked her how could she not validate a debt when requested? I never knew a company could refuse validation. She continued to say that she doesn't have to do anything and since I disputed it she still doesn't have to validate. She continued to say she can see my visit, and tests that the insurance supposedly paid for, that is information that she should not have. I feel offended. I would like this debt removed from all three of my reports. If validated I will pay collection. I just want to make sure everything that has been done to me is valid.

A collection account from Creditors Bureau Associates was placed on my Credit on XX/XX/2021. I immediately filed a dispute with XXXX only to find out that Creditor Bureau Associates was collecting a paid in full debt from XXXX XXXX XXXX XXXX. Subsequently, during the dispute process, a representative from Creditors Bureau Associates by the name of XXXX XXXX contacted me on XX/XX/XXXX. I explained to XXXX XXXX that this account and any other balance due to XXXX XXXX XXXX XXXX was paid in full via a settlement in XXXX of 2021. XXXX XXXX asked for proof which I forwarded to her an email from XXXX XXXX, XXXX Supervisor, at XXXX XXXX XXXX XXXX. I had not heard back from the agency but on XX/XX/XXXX, I received notification that my dispute with XXXX was complete and the account was verified. Not sure what that means but I had already sent the proof to the collection agency that it was false and should have never been forwarded to them. The agency then proceeds to add the same invalid collection account to my XXXX credit report as well. I attempted to call on Tuesday, XX/XX/XXXX after receiving further documentation from the collection company showing proof of the two payments I made in XXXX of 2021. One payment on XX/XX/XXXX in the amount of {$300.00} and another payment on XX/XX/XXXX in the amount of {$100.00}. The agent I spoke to transferred me to a supervisor where I ended up having to leave a message with no callback. I will be filing a complaint with the credit bureaus as well for allowing inaccurate information to be placed on my credit file.

This is in regard to account number - XXXX, Original Creditor Name : XXXX XXXX XXXX. On XX/XX/2015, I was transported to the emergency room by XXXX XXXX XXXX. I provided my insurance information during transport. XXXX XXXX XXXX did not properly bill my insurance carrier for my transport. I called my insurance carrier ( XXXX ) to confirm my claim status with XXXX XXXX XXXX and was advised that no claim was filed on my behalf. I then contacted the XXXX XXXX XXXX directly and spoke to a representative to advise that a claim was submitted inaccurately or not at all, which caused a billing error/issue. I informed the representative with XXXX XXXX XXXX, after speaking with my insurance carrier ( XXXX ), to re-submit the medical claim, at which point the representative refused to resubmit claim stating that the timely filing period has expired, and I am now the responsible party of the alleged XXXX XXXX bill. I believe XXXX XXXX XXXX balance billed me, which is illegal in the state of GA and then sold the account to Creditors Bureau Associates ( Collection Agency ). Creditors Bureau Association failed to provide me a notice of debt, prior to inserting this alleged account onto my credit report. I was not provided the opportunity to request validation of this alleged debt. I have disputed this alleged account on several different occasions with all major credit reporting agencies, as well as the collection agency, and all parties continue to verify this alleged debt without properly investigating, which is violation of the FDCPA and FCRA.

I am reporting my rights, under the Fair Debt Collection Practices Act, ( FDCPA ), were violated. Under the FDCPA, it states, If you dispute the debt, then anytime the collector reports that debt to a credit reporting agency, then they must report that debt is a disputed debt. As you can see in the attached documentation, from my archived credit report, ( XX/XX/XXXX, XX/XX/XXXXXX/XX/XXXX ), they did not report the debt, as being disputed. They still reported disputed debt, to XXXX and to XXXX, as if it were not being disputed. Attached you will also find a copy of the certified letter receipt ; the collection agency received a letter, asking them to validate not verify, said debt. According to FDCPA, Upon the consumers request written request within the thirty day period, the debt collector will provide the consumer with requested form of validation. A debt collector presented with a timely ( within 30 days of receiving my notice ) demand for validation must stop all collection activities until they provide the requested validation. As you can see from the attached certified letter receipt, from XX/XX/XXXX to XX/XX/XXXX ( today ), it has been 52 days. They have not provided me with requested validation, and they continued to report XXXX and XXXX, which is another direct violation of the FDCPA. In lieu of the violation of my rights under the FDCPA, I am requesting that these accounts be deleted from my XXXX and XXXX credit profile, and that documentation of requested action be forward to me as soon as possible.

Creditors Bureau Associates currently shows a {$370.00} collection for XXXX XXXX XXXX on my XXXX credit report, which I disputed repeatedly WITH EVIDENCE to no avail. Heres what happened. In 2018, I worked at XXXX but was let go as of XX/XX/2018, and according to our separation agreement ( see email copy from HR ), my health insurance with XXXX was valid through XX/XX/2018. When I went to XXXX XXXX XXXX with XXXX XXXX the front desk staff CONFIRMED that my health insurance was active and valid. Then, they sent me a {$370.00} bill. I contacted XXXX XXXX XXXX repeatedly. They said my employer would have to contact them. I contacted my employer who confirmed my insurance had to be valid and active through XX/XX/2018. See attached emails. However, my former employer dissolved business, the HR person was no longer there, the CEO moved to another state with another company, and no matter where I emailed, nothing was emailed back. Phone was disconnected and so was the physical address. See attached emails. The {$370.00} is not the debt I owe for my XXXX XXXX XXXX visit because as you can see from the attached proof, XXXX was responsible for paying it, not me. This must be removed immediately from my credit report. This is false reporting and violentes my FCRA rights. Ive attempted multiple times to resolve it directly with the company to no avail. This medical debt was accrued as a minor. A minor can not legally enter a contractual obligation in the states where this debt was accrued, a parent or legal guardian must sign the service and payment agreement. This agency is attempting to collect a debt NOT from the original owner of the account, but of the minor involved AFTER that minor turned XXXX. The debt was accrued in XXXX of XXXX. The minor was XXXX XXXX XXXX. The collection agency opened a collection account against the ( former ) minor in XXXX of XXXX, when the ( former ) minor was XXXX XXXX XXXX. This collection agency has tried to contact the ( former ) minor multiple times before they eventually blocked all phone numbers associated with the company to stop the harassment. I sent a letter to XXXX, XXXX, and XXXX disputing items on my report that need to be removed. I asked for them to update my personal information ( name, address, job, and no phone number ). I asked for the original contract ( my wet signature on it ), creditor information ( name, address, and phone of the original owner ), payment history and itemized billing of the account, and proof of ownership ( proof of authorization of ownership ) for other items listed on my report. I asked for a copy of my updated credit report. I have not received any of this information from anyone. I have not even received an update copy of my report. I would like for you to look into what is going on with my credit report. I sent the letters XX/XX/2022 and now, it is XX/XX/2022. I still have not heard anything from the credit bureaus. There is at least one that is reported on my credit report twice. The information is also different on each report. There is one derogatory mark on XXXX, XXXX on XXXX, and XXXX on XXXX. The companies names are Creditor Bureau Associate, and XXXXXXXX XXXX XXXXXXXX. I have never received any bills from these companies nor have I ever heard of any of these companies. I was laid off during the pandemic due to a severe injury. I had 3 surgeries to repair in a 4 month period. I was unemployed for 30 days before being rehired. During that time I was eligible for XXXX but unable to pay the {$600.00} premium. The account was sent to CREDITORS BUREAU ASSOCIATES in XXXX, GA. I explained that there was an extension to pay the premium for XXXX eligible individuals to CBA and they removed the report. I have been openly communicating with them while waiting for XXXX to approve the extension. This extension allowance was authorized for the pandemic period only. I was not contacted before they re-applied the collection to my credit on XX/XX/2022 almost a YEAR later. I contacted XXXX to ask them to escalate my request for an extension which they did. I received an email from XXXX stating that my extension request was APPROVED and they would process the documents and payment. On XX/XX/2017 I sent a certified letter to CBA of Georgia requesting they send me verification of the alleged debt they say I owe. The letter was received and signed for on XX/XX/2017 by a representative of their company. I continue to get inappropriate calls from Creditors Bureau Associates of Georgia after I have made numerous electronic payments to pay towards and/or settle the medical debt owed. The agency continues to call requesting sensitive information ( i.e. social security number, mom 's last name, etc. ) after I have requested them to send me information via mail. My payments have been returned after two attempts to pay. I have also called and received inappropriate attitude from the collections specialist after I repeatedly explained that I wanted to pay the debt owed based off my financial capability. The specialist offered to settle the payment and I accepted based on the conditions that the settlement is sent to me in writing ( via mail and email ). To date, XXXX XXXX, 2016 I have not received a letter or email. So, to continue showing that I want to pay the debt, I continued to make payments and the company continues to return the payment. I filed a complaint on XX/XX/2018 and it was closed on XX/XX/2018. I don't understand why my case is closed when the matter isn't resolved. Creditors Bureau Associates of Georgia claimed they have verify that I owed the amount, but refused to send me proof. In XX/XX/2016 I noticed a collection account on all XXXX of my credit reports, so I initiated a dispute knowing it was not mine. XXXX and XXXX corrected the mistake, but it was still on XXXX. I then call the collection company CBA of XXXX. I requested documentation including copy of Signature, ID 's, Social security card to show they had legal right to collect. I also requested documentation showing they had the right to collect for the company that intiated the account. I also requested proof of their license and bond to even collect in the state of GA. They put me to management and I was assured that they would email the requested documents to me as soon as possible. A couple days went by with no email so I called back and then was told they were getting this information and documents from the 1st party and would forward it to me. On XX/XX/2020 I sent Credit Bureau Associates a debt validation letter, in which they received on XX/XX/2020 at XXXX XXXX requesting debt validation for two collection accounts they are actively reporting on both my XXXX and XXXX credit reports. They responded with a vague billing summary that did not include any information to validate the debt. The amounts do not match the amount reporting on my credit reports, there is no breakdown of fees, as well as having a line at the bottom of the billing summary which states, " Total Charge Outstanding as of XX/XX/2020 {$0.00} '' ( the {$0.00} amount is listed in all three payment columns ). I subsequently followed up with another debt validation on XX/XX/2020, in which they received on XX/XX/2020 at XXXX XXXX. To date they have not responded to my request. I received a phone call and voicemail at XXXX XXXX Central Time from ( XXXX ) XXXX. The message was from Creditors Bureau Associates in XXXX, Georgia. When I returned the call, XXXX answered, requested my phone number, asked me to verify my first and last name, once I verified, she then requested my Date of Birth to verify my identity. I have twice demanded validation from this company and they have responded by sending me a bill claiming that it is past the 30 day validation period.
