XX/XX/XXXX : - I opened two ( 2 ) medical debt collection notices I received from Credit Center , LLC ( aka XXXX ). - The first billing notice ( Account # XXXX ) was for a total of {$100.00} for the following charges, and dates of service ( which I later verified with the XXXX representative ) : o {$100.00} on XX/XX/XXXX o {$30.00} on XX/XX/XXXX - The 2nd billing notice ( Account # XXXX ) was for a total of {$80.00} for the following charges : o {$20.00} on XX/XX/XXXX o {$30.00} on XX/XX/XXXX o {$30.00} on XX/XX/XXXX - I then called their listed local contact number ( XXXX ) and spoke to one of their representatives. - After verifying the dates of service, I agreed to pay the amount in full, which I did, using my VISA ( bank ) debit card, but asked that they be paid separately in order to receive confirmation from them that I had paid EACH notice in full, to which their representative agreed. - After completing the transaction, the representative agree to send an email to me with confirmation that each billing notice was paid in full. We then pleasantly ended the call. XX/XX/XXXX : - I noticed that I hadnt received the email from XXXX - Before calling, I checked to see if the payments had been processed through my bank account. - I found that I had been charged 6 times by XXXX on behalf of XXXX ( the medical billing office ) for a total of {$200.00} -- {$20.00} more than I was supposed to have been charged. The charges to my bank account ( in their listed order ) are as follows : o {$79.00} o {$30.00} o {$30.00} o {$20.00} o {$30.00} o {$20.00} - I called XXXX at approximately XXXX, and spoke to the same representative as she indicated to me, UNPROMPTED. I asked her if she could tell by the number from which I was calling and she said, No. Because I remember your name and that you wanted me to charge the two payments separately., or words to that effect. - I immediately agreed that my bank statement confirmed that to b true, but that XXXX also charged me an additional {$20.00}, for a total of {$200.00}, instead of the {$180.00}, I was supposed to have been charged. - She then said Let me check the system. while she continued chatting about what might have happened. After offering a variety of explanations ( while still checking ) she offered that, Yes. It was paid in full. I then asked whether it showed that I had overpaid by {$20.00}, since the amount take by them from my bank account had to be somewhere. She then ( eventually ) said something like, Oh, yes. It is showing that you paid an extra {$20.00}, but it hasnt been applied to your account. Or words to that effect. I then asked where the {$20.00} had been applied and she implied that it was there, but it was just not applied to my account. I then inquired, confused, why there would be a record that I had been overcharged, but that it hadn't been applied to my account. She replied that she didn't understand but that was what their system was showing. I then asked her to confirm that XXXX owed my {$20.00}, to which she said something like this is very confusing. Let me check something. Then put me on hold. She came back a couple of minutes later and made some explanations that were nonsensical to me including one that implied that I still owed the {$180.00}. I replied that was definitely NOT the case and that XXXX owed me {$20.00} AND that the bill was paid in full and that she had already told me that it had been paid. She then apologized, and said she had to talk to the person who inputs the payments and put me on hold again. I was on hold for 10-15 minutes. During that time I kept hearing a clicking sound over the music playing, like someone was typing. Then, for about 10-15 seconds, I heard a high-pitched screeching noise that would have been unbearable in my ear if I hadnt been using the hands-free option on my phone. By this time, it was almost XXXX and she came back on the phone to say that I was going to have to send XXXX a letter, along with a copy of my bank statement, requesting to be reimbursed for any overpayment. I said I was not going to do that. That I had called because it was clear that I had overpaid, that my bank statement verified it, she confirmed it over the phone, despite XXXX never having sent a confirmation that it was paid. I told her that I wanted to speak to whomever was responsible for inputting the payment and sending the confirmation letter, and although she had just come back from supposedly talking to whoever was responsible, she said they had already left for the day. She said then said I would need to talk to the Manager, and put me on hold. She came back and said, He must have left for the day, hes not answering. She then tried to get me to agree to send the letter and bank statement as a request for reimbursement of the overpayment. I CALMLY said, No. This is how its going to happen. You are going to have someone from XXXX call me back tomorrow, no later than XXXX, with a full explanation why I was overcharged by {$20.00}, and also have a solution for reimbursing me for that amount immediately. Or words to that effect. However, Im not sure how much of that she heard, because when I finished there was silence at the other end. And said, Hello? Hello?. The call had been disconnected. I then checked their stated hours and saw that their office closed at XXXX. Thats when I went to this website to find out how I could report what appears to be some sort of shell game relating to this apparent SURREPTITIOUS overdraft of my bank account WITHOUT MY PERMISSION and without explanation of that overdraft of my account not even an admission that it may have been and inadvertent error on their part.

My son had an XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX The team 's medic called for an ambulance and he was taken to XXXX XXXXXXXX XXXX ER XXXX XXXX XXXX XXXX There were XXXX separate bills for that event : The ambulance, ER visit, doctor, and XXXX. I had private insurance ( primary ) and I was told to file a claim with the school 's insurance company ( secondary, ) which I did. As I got the bills I called the hospital and was told to wait until both insurance companies paid, and that then I would get my bill for the remainder. I paid the ambulance in full and put the other bills on a payment plan. During the time I was paying off the balance, XXXX Hospital changed companies in charge of payment/billing and that's when mistakes started to happen. Payments that I was making to one account got credited to the wrong account. The hospital 's bills to me didn't reflect the secondary insurance payments which were made -- and I had confirmation numbers from the secondary insurance company to prove it -- but they only noted the primary insurance company 's payments. I was in touch with both insurance companies and the hospital during this time trying to correct the records. I finally confirmed with all parties that the bills were paid in full and filed away my records and copious notes. Thankfully I kept all of them because in XXXX I got a notification from my credit-monitoring service that I had a new negative report on my XXXX credit report from a collections agency in XXXX XXXX ( CCI. ) I had no idea what this was for. ( My credit score is over XXXX. I have no late payments on any of my credit reports from all 3 agencies. ) I was told the collection was for XXXX XXXX. Here is where the action was improper : I had no communication from either XXXX XXXX or CCI that this bill was due or that it was going on my credit report. In fact, if I didn't pay for a credit monitoring system, I would not have know this was on XXXX. I talked to the manager at CCI countless times, emailed him proof that the hospitals billing company had not entered the secondary insurance payments, and he admitted that I should've been notified before this went to XXXX. He said as a " courtesy '' he would take the stain off my record, to which I replied, " it's not a courtesy, it's your obligation, and you harmed me in an unlawful way. '' To make matters worse, I just got another bill from CCI for over {$200.00} for the XXXX Hospital bill. It isn't itemized, it's just a demand that I make a random payment. After everything I've been through with them I don't trust nor believe them.

I had a XXXX performed at XXXX XXXX on XX/XX/XXXX with a short office visit prior to receive prep instructions.. To date I have received the following bills from XXXX XXXX for it : 1 ) {$67.00} dated XX/XX/XXXX : This is now 74 days past due and the hospital was notified in writing that I was delaying payment while the amount was under appeal with Medicare. 2. {$120.00} dated XX/XX/XXXX which contained 2 charges : the {$67.00} mentioned above and a new charge of {$54.00}- which I PAID by a check that cleared XX/XX/XXXX, but to date this is not reflected in the online bill pay portal. I have repeatedly messaged the Hospital to correct the amount in the bill pay portal but they have not done so, making it impossible for me to pay the amount actually due via the portal without incurring double payment. 3. {$180.00} on XX/XX/XXXX- which consists of : the {$120.00} above, of which {$54.00} was already paid, i.e. this amount is incorrect. {$66.00} in newly billed charges which are not yet overdue I am unable to make payment of the amount I actually owe ( $ XXXX {$54.00} = {$130.00} ) because the online bill portal will accept only {$180.00}. I am out of the US at this time so the online portal is the only means through which I can remit payment. I have repeatedly communicated this to Nuvance to no avail and with no response. Meanwhile, despite the fact that no charge is 90 days overdue, the hospital turned this over to a collection agency, Credit Center. Further, they turned over not just the {$67.00} which is overdue, but the amount already paid and the amount just billed on XX/XX/XXXX. From the amount in the collection agency letter it appears there are other charges as well for which I have not yet even been billed. According to the Hospital 's Financial Policy Credit And Collection : " it is the Networks policy that accounts not paid within ninety ( 90 ) days will be reviewed for appropriate collection action. This amount, and the amounts below, were not overdue 90 days. '' My complaint is three fold ( your menu made me select just one ) : - Unjustified initiation of a collection action in clear violation of the company 's own policy, with attendant negative consequences to my credit rating. This is my main concern -Double billing for an amount already paid - Failure to adjust the amount in the online bill pay portal to reflect payment already made, making it impossible for me to use that channel for the amounts actually due, despite repeated requests and being informed that I am overseas so can't mail a check, have to use the portal.

I personally sent a Debt Verification Letter via Certified Mail and First Class to CREDIT CENTER LLC on XX/XX/XXXX. This letter was delivered on XX/XX/XXXXXXXX XXXX XXXXXXXX pm as reported by USPS. In addition, I have emailed CREDIT CENTER LLC requesting verification on XX/XX/XXXX. I received an email back stating " Your message has been received, is in the process of being reviewed, and will be responded to within 24 hours. '' However, I was never responded to via email, mail, or phone. I also followed up on this with the same response but was once again never contacted. Prior to sending any Debt Verification Requests to CREDIT CENTER LLC, I had not ever heard of this debt. I never received a letter via USPS or otherwise. I also did not receive any phone calls. I contacted the original creditor and was told that there was no information on my file regarding collections or CREDIT CENTER LLC. CREDIT CENTER LLC reported collections in my name to all three credit bureaus. I do not recognize this debt in any capacity, but I also noticed that Credit Center LLC reported this collection account as " past due ''. With that said, per my legal counsel I have been advised that a collections account is past due by default therefore they have failed to list this accurately. This is a violation of the FCRA and is also in violation of requisite XXXX XXXX Compliant reporting standards. By the provisions of the Fair Credit Reporting Act and per the standards of reporting compliance implemented with the CRSA enacted CDIA XXXX XXXX COMPLIANCE regulations, I have the right to a fair and accurate credit report, and XXXX is required to remove any information that is not true, accurate, compliant, and verifiable. Additionally, I am allowed under the HIPAA law ( Health Insurance Portability and Accountability Act of 1996 ), to protect my privacy and medical records from third parties. I do not recall granting XXXX XXXX XXXX XXXX permission to release my medical information to a third party.

Credit Center , XXXX is reporting a delinquent medical collection account to the credit bureaus under my name. I have sent multiple debt validation letters to Credit Center , XXXX. I sent the original request for debt validation letter on XX/XX/21 via USPS mail. I sent a second validation letter on XX/XX/21 via USPS Certified Mail. ( See attached receipt ) I did not receive correspondence from Credit Center XXXX after my first validation letter was sent out. They waited for me to file a complaint with the Consumer Financial Protection Bureau at which point they still did not provide CFPB or myself with sufficient validation of this alleged debt, instead they attached an XXXX spreadsheet with a manually typed " timeline '' and a notation that states they'd be happy to validate this debt if I contacted their office. I went ahead and sent out a second validation letter on XX/XX/21 via USPS Certified Mail. ( See attached receipt. ) This company responded to my Certified letter but failed to legally validate this alleged debt. Instead, they sent me copies of sensitive, protected, private information from my medical records including specific medical tests and procedures that were performed as well as prescriptions that were prescribed. This is a direct VIOLATION of HIPAA regulations. I did not sign nor did they provide me with a HIPAA release form authorizing them to have access to this information. Credit Center , XXXX has violated my HIPAA rights. Please terminate this account and cease all collection activities or I will be forced to take legal action in response to this violation. I have attached the forms that were sent to me for your review.

This is related to my debt collection from XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX which was sold to some other company at the end of my XXXX .. All the bills were paid until then. But since the company was changing they were not able to collect anymore and i was told that I will get bills to my home.. This never happened, instead it directly went to Debt collection.. The debt collection agency ( Credit Center LLC ) reached out to with a higher amount which was not matching my insurance records.. I asked for entire statement since start including billed codes to the insurance.. which was sent to me till date by the debt collection agency.. They created 2 accounts for the debt collection and reported 2 late payment, which is unethical.. Reasons : Incorrect/higher amount requested, no attempt made by hospital to collect in the initial stages, information requested with additional billing proofs never provided Regards, XXXX.

I received a letter from " XXXX '' of XXXX stating that I owed {$20.00} due to an unpaid medical bill from " XXXX XXXX XXXX XXXX. '' After calling and speaking with them and making it clear that I have never visited a place called " XXXX XXXX XXXX XXXX '' she said " Well, we have your name, your birth date, how else would we get it? '' I made it clear that I was not paying until I found out if this was legitimate. She gave me the date of service but said she " had no other information. '' I called my doctors office, confirmed that I had visited on that date, paid on that date ( as per my credit card statement ) and that I had no balance due. I believe this company is simply attempting to collect money from anyone who will give it to them, regardless of whether debt is owed or not. The letter I received from them states that there will be a delinquency on my credit report if this is not paid.

I received notification through a credit monitoring service that I had a past due balance of {$70.00} for a medical appointment several months prior. At no point in time did I receive notification ( written or verbal ) from either the medical office involved or the collections agency. The debt collector fraudulently reported the total amount owed as 4 separate past due balances totaling {$70.00} to the credit bureau, seriously affecting my creditmoreso than one derogatory mark. Since I first discovered the debt through my credit monitoring service, I should not be expected to have paid earlier, nor penalized with derogatory marks. Upon discovering it, I did verify it, and paid it as soon as possible. The CFPB needs to step in here to prevent this company from fraudulently and viciously trashing my credit, and reach out to XXXX to remove these marks ASAP.

XXXX XXXX XXXX XXXX XXXX CT XXXX XXXX sent XXXX SEPERATE bills to collection office of about XXXX $ XXXX $ EACH BILL for same appointment. I was not notified of these XXXX XXXX medical bills and actually had paid a payment plan to this same doctors office prior to this that I had paid. I have medical debt from various collections agencies. I called to talk to someone at the same agency ( XXXX XXXX XXXX ). I spoke to three people who gave me three different prices. I will be starting law school in the fall and wanted to take care of my medical debt so that I have good credit for my student loans and get an apartment. Although I recently set up a payment plan, I am confused about my amount and frustrated by getting three different total amounts owed from three representatives. Additionally, I mentioned that amounts that were below {$500.00} should be removed and they told me that wasn't true/had n't been passed yet.
