What is the complaint about?
CBCS attempts to collect on debts, handles disputes improperly. Attempted to collect on debt that doesnt belong to me, wouldnt give me a correct phone number so that I could verify the debt. When I finally found another billing company that had been associated with XXXX XXXX Hospital Physicians, they stated my name is not on that account. Why could I not get in touch with XXXX directly? According to the Tennessee State Department of Commerce and Insurance, CBCS is not licensed to collect in the state.
This is my statement regarding my interactions with CBCS. I have also recorded phone calls. Thank you for your time.
After I received a letter from CBCS regarding an account with XXXX XXXX HSP Physicians, I called and spoke to XXXX on XX/XX/XXXX and I disputed the charges. During the conversation XXXX confirmed that the Department of Veterans Affairs was the insurer on file. When I asked why they did not directly bill the VA, I did not know, and that maybe the balance is my portion of the bill. I explained that the assumption was incorrect and that I would like proof of the debt ( see letter dated XX/XX/XXXX ).
After I received the response regarding my dispute of the debt from CBCS, I called XXXX billing. I spoke to XXXX who confirmed that the Department of Veterans Affairs was on file as my insurer, and the hospital bill had been paid within a month of the claim. XXXX referred me to the physicians billing group ; XXXX XXXX XXXX.
I called the XXXX XXXX and listened to their list of Debt Collection agencies, CBCS is not one of them. On XX/XX/XXXX, I called CBCS again and spoke to XXXX. I asked her for a direct number to XXXX XXXX Hsp Physc and she stated the number is not on file ; the only information they have on the debtor is what is listed on my statement ( name and associated account number ). I then inquired as to how CBCS receives my information to start collection process against me, she stated they dont buy the debt and that basically only an account number, address, name, date of birth with a debtor is needed to start the collection process.
On XX/XX/XXXX, I called CBCS again to try to obtain information about the debtor, such as address and phone number, so that I could verify the debt. XXXX told me that they do not provide that information. XXXX stated that he doesnt have access to information about XXXX XXXX Hsp Phsyc. I told him that since they are the clients of CBCS, they must have company information for billing purposes ; XXXX then referred me to the office in New Hampshire.
I spoke with XXXX in the New Hampshire branch of CBCS and she gave me a phone number and address for my debtor that is seemingly incorrect ( I called the number and its a cell phone, no outgoing message ). After calling the New Hampshire branch of CBCS back, I spoke to XXXX ; who then said the clients name ( my debtor ), was shown as XXXX XXXX XXXX XXXX XXXX XXXX at XXXX. XXXX gave me a different phone number with the same post office box address as XXXX from the first time I called their office.
XX/XX/XXXX, I sent a certified letter to CBCS, disputing all the debt associated with account XXXX ; and asking them to verify the identity, name and address of the company claiming I owed money.
After doing some research on the internet, I found a number for XXXX XXXX Hospital Physicians and spoke with XXXX from XXXX, ( XXXX ) on XX/XX/XXXX. Using the account number on the invoice sent to me from CBCS, XXXX was unable to match my name with the account. XXXX then proceeded to tell me that I had another account holding a XXXX balance and a service date of XX/XX/XXXX, from XXXX XXXX XXXX, XXXX XXXX XXXX ( acct # XXXX ). I then confirmed my name, date of birth that she had on file. XXXX stated the middle initial on the account is XXXX ; I told her that XXXX is not my middle initial, and I obtained the business address of XXXX as XXXX XXXX XXXX XXXX XXXX, MI XXXX. Be advised, I was not in a NY hospital XX/XX/XXXX.
XX/XX/XXXX, I called CBCS and spoke with XXXX. I advised her of the fraud situation her company is participating in and that I was informed by the Tennessee State Department of Commerce and Insurance, CBCS is not licensed to collect in the state of TN.
XX/XX/XXXX, I spoke to XXXX and he agreed to send me an itemized statement from XXXX XXXX Hospital Physicians. The beginning of XX/XX/XXXX, I received the letter stating they have and will stop all collections on this account. I began asking for verification of this debt from CBCS starting XX/XX/XXXX. Why does it take so many phone calls, so much time to get these companies to do the right thing?

XXXX XXXX XXXX claims I owe them {$320.00} for a medication that I did not authorize. The date of the initial disputed transaction was XX/XX/XXXX.
I am on a very expensive medication to help with my XXXX XXXX XXXX that my insurance company only allows me to have shipped directly from XXXX XXXX XXXX. I have been on this medication for years, and up until XXXX, with the help of a copay card from XXXX, I have only had to pay either had to pay {$5.00} for my monthly prescription. If there ever were ever any costs that have exceeded the % 5.00 copayment, XXXX XXXX has always called me and ended up working it out with the Copay assistance. In XXXX, there were some issues with the copay card, so after contacting me to let me know, they contacted the copayer and the copayer authorized a direct payment to them. I told XXXX XXXX every time they called me that if the copayer didn't authorize their payment, then I declined the charges and to not ship the medication. This happened a couple times, during the year, and the copayer paid them 100 %. I paid {$0.00} for my copayment in XXXX.
XXXX XXXX never called me to get authorization for the {$320.00} not covered by the copayer for the XX/XX/XXXX shipment. I did not even know about it until after they called me in XXXX asking for ~ {$2000.00} for the XXXX shipment of my medication. I told them at that time, the same thing I always told them. " If the copay card isn't going to pay for it, then I decline the shipment and the charges. '' I remind them everytime that I can not afford any more than the {$5.00} copayment. They have confirmed that they have this conversation recorded and on file.
I have called them multiple times to dispute the charges, XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX and XXXX gentleman I spoke to the first time told me to the second time told me to call the copayer and give them a sad sob story to see if they would pay the {$320.00}. He told me if they didn't pay it, then to call back to XXXX XXXX, ask for a supervisor, and the supervisor would write it off because there was no record of me authorizing the charges. He said ignore the repeated notices to pay in the mail, and that they would never send this to collections.
The call on XX/XX/XXXX took 2 hours and 2 min. I was transferred around several times. They were not very willing to help me. They said they listened to all the calls they had on file and were not able to find any call where I authorized these charges. However, they claimed that because I did not call them prior to the shipment to cancel it, that the charges were valid. I don't understand how I am supposed to decline the charges when they never called to tell me about them until a month after they charged me. The Supervisor said she would escalate the matter for further investigation and get back to me.
No one from XXXX XXXX ever got back to me. Instead they sent it to CBCS collection Agency, which is exactly what they said they were not going to do. I called the collection agency to tell them I disputed the charges. They said they would make a note, but still said I had to pay them.
Because of this problem, I have not been able to get my medication for my XXXX XXXX XXXX for 7 months, and my health is suffering. This is not right or fair. I need my medication. XXXX XXXX has received $ XXXX {$30000.00} a year from my insurance and the copayer for over 7 years for my prescription, and they are unwilling to admit they made a mistake and write-off the {$320.00}.

I'm contacting you in regards to a medical bill that has gone to collections in error and the company is threatening to report this information to the credit bureaus as a result. The medical bill is from a work related injury dated back in XX/XX/XXXX. After I was injured on the job I immediately reported the incident to my supervisor. At that time, my supervisor was the XXXX XXXX XXXX XXXX XXXX meaning she was over all medical billing for the XXXX on my previous job where I worked and where the injury occurred. After I reported the work related injury my supervisor she then stated she would be over to my work area to give me a worker 's compensation form and I would need to get that form in order to see the doctor. My supervisor then advised me to give the worker 's compensation form to the admitting clerk at the Emergency Room, so they would know how to bill the worker 's compensation company. My supervisor also stated that the worker 's compensation form would cover my emergency room visit she, also gave me a incident report form from the incident. After my visit at the Emergency Room I contacted the hospital call center and requested my billing statement because I haven't received one within a months time. After I made the request to receive my billing statement with the call center. I then received a UB-04 ( Universal Billing Form ) Form in the mail from the call center. The UB-04 form is different from an actual billing statement. You would use a UB-04 Form to file claims with. So, I contacted the hospital call center again this time I requested to speak with a supervisor. I explained to the call center supervisor that I haven't received a billing statement from my date of service from XX/XX/XXXX. I also explained that I worked in the business office where I handle all medical billing inquiries at the hospital location. I proceeded to explain that normally a patient would receive a billing statement within two weeks of the date of the service and I haven't received a billing statement in months. Therefore, I made another request for my billing statement with the hospital call center supervisor. After the request was made I received another UB-04 form later by mail. After several attempts of the hospital call center denying my request for me to receive my billing statement. Several months later, in XX/XX/XXXX I received a collections notice from the emergency room visit from XX/XX/XXXX date of service. The collection notice stated that several attempts had been made by the hospital in regards to them contacting me about my medical bill and I ignored those attempts. The billing statement from the hospital stated, if that amount I owed wasn't paid within a certain amount of time my account would be sent to the hospital 's collection agency. I tried to resolve this issue with the hospital, but everyone I spoke with ignored my response. My account was then transferred to the hospital collection agency where I also sent them documentation showing that I was injured on the job and I submitted documentation of the worker 's compensation information. Once again my request was ignored and denied. Now the hospital has hired another collection company to seek the amount.

I have received calls on an almost daily basis from XXXX XXXX XXXX about a debt I should not owe. In the fall of XXXX, I was injured playing a XXXX Volleyball for my university- XXXX XXXX University XXXX XXXX XXXX PA. Since my insurance ( XXXX XXXX XXXX ) was based in WA and I attended school in PA, my insurance recommended I go to the ER so that I could be covered for an MRI/Xray if the doctor deemed it necessary, which they did. I have the documents from the Doctor explaining his thoughts on the injury. The Hospital administrator was on the phone with XXXX XXXX XXXX trying to get the authorization that night but they assured him it would get covered eventually and a few days later it was cleared so I got both tests done. A month later, I was injured again and my insurance decided they would not cover the new XXXX and were going to back out of paying the first one too. Since I had secondary insurance as an XXXX ( Through XXXX XXXX XXXX ) I submitted my claims to both ( XXXX XXXX XXXX and XXXX XXXX XXXX XXXX so that XXXX XXXX could deny it and then provide an XXXX so XXXX XXXX XXXX could Approve the coverage. The Hospital ( XXXX XXXX ) refused or chose not to bill my insurance in the proper order, meaning that I had to call several times a week ( including calls to XXXX XXXX XXXX and XXXX XXXX XXXX ) to try to get it sorted out. After months of calling, they were able to properly bill my primary and secondary insurance on some smaller charges but still did not do that for the larger amount, regardless of how many times I called. Each of the places I called would all point to the other as the source of the error. For about 6-8 months I did not receive any communication from the Hospital and assumed the bills had finally been distributed properly. Apparently the Hospital decided to send my debt to a collector without my knowledge even though they were at fault for not billing the insurance in the proper order, which I gave them numerous times. Now I get pestered by XXXX for a large bill I should n't even owe. They have told me that it will damage my credit multiple times and have even found a way to contact my family members without my permission or knowledge.

Our XXXX year old XXXX boys were being seen by speech therapist at XXXX XXXX 's hospital in XXXX Ohio. Our healthcare provider ( XXXX XXXX XXXX ) began paying for the treatment for the first 6 months. Suddenly, XXXX XXXX XXXX stopped paying for the speech therapy sessions. We were contacted by the billing department of XXXX XXXX 's Hospital of an outstanding debt. When we contacted XXXX XXXX they claimed that XXXX XXXX XXXX XXXX had never received authorization to provide XXXX to the boys. In addition, XXXX XXXX alleged that the contract with my employer had changed and the boys were no longer covered for XXXX. My employer ( XXXX ) maintains that there was no coding change in the contract between the city and XXXX XXXX. XXXX XXXX 's Hospital claims that authorization was given by XXXX XXXX to treat the boys. At this time we have no idea of who is being forthright. Had we known in advance that the therapy sessions would not be covered, we would have taken a different approach. At this time XXXX XXXX 's hospital has sent our account to collections claiming that we owe over {$2000.00}. Again, XXXX XXXX paid XXXX XXXX 's XXXX for nearly three months while the boys were going to XXXX. If authorization was not give to XXXX 's Hospital than it is my belief that the hospital should have notified us. If a request for authorization was not received by XXXX XXXX then why would they pay close to {$1000.00} in medical expenses. Finally, it would be incumbent upon XXXX XXXX and XXXX XXXX XXXX Hospital ( XXXX ) to notify us that the insurance would not cover the treatment. Especially before it reach the level of thousands of dollars. No one has been able to explain to us how we arrived at this point.

In XXXX of XXXX, I received a letter in the mail from CBCS stating that I owed a {$4300.00} bill for XXXX XXXX of XXXX. I immediately called to dispute the claim. I let them know that the bill could not possibly have been mine because I left XXXX when I was XXXX years old when I moved to XXXX to attend school. I have also spent the past few years abroad, and in fact XX/XX/XXXX was the first time I opened up a utility bill account under my name in the United States. I also let them know that I am currently disputing another fraudulent account that was taken out under my name while I was living in XXXX XXXX in form XXXX-XXXX. They said that they would send me paperwork to fill out so I could file the account as fraudulent. In XXXX, after a month of not receiving anything, I called them again. They said that they had " mis-written my address '' ( although they had been sending me letters to collect the amount they claimed I owed ), and that they would send me the packet so I could file it as fraud. A month later, today, XX/XX/XXXX, they sent me a letter saying I still owed the full amount. I tried to call the number that they listed in their letter ( XXXX ), but it was just infomercials, with another number to call at the end ( XXXX ). This number also just lead me to more infomercials and when I refused the " free trip '' offer, it hung up. I called their offices, which were closed and left a voice mail. I strongly believe that this company is giving misleading information and faulty numbers in order to drag out the 30 day period they claim to give in order to legally come after me, even though this is a fraudulent charge.

RE : Account number CBCS - XXXX Dear CFPB, Please accept this letter as a formal complaint against XXXX Hospital and their 3rd party collection company, Called CBCS. Please know that I have attempted to straighten this out with XXXX Hospital and CBCS several times with no results.
My complaint is as follows, In XXXX 2015, I made XXXX payments to XXXX Hospital as noted below. Proof of all payments is attached XXXX XXXX, 2015, made payment of XXXX. This electronic payment from my XXXX account was made too XXXX hospital, who transferred the money to CBCS. Payment was cashed on XXXX/XXXX/15 BUT NOT APPLIED TO MY ACCOUNT.
XXXX XXXX, 2015, Payment of XXXX hard copy check # XXXX. This was cashed and applied to my CBCS account on XX/XX/2015 XXXX XXXX, 2015 Payment of XXXX hard copy check # XXXX, this was cashed and applied to my XXXX Account on XXXX/XXXX/15 XXXX XXXX, 2015, Electronic XXXX payment of XXXX was made to XXXX who transferred the money to CBCS. This payment was cashed and applied to my CBCS account on XXXX I have requested a statement from CBCS of my account and all transactions, they refuse to send it to me.
I have filed a dispute with CBCS and their investigation deemed that they did post my payment to my account. However, my account balance is not showing that this was done.
NOTE- This is the 2nd time in a year that XXXX and CBCS has lost my electronic H., XXXX payments.
Please assist me in getting this matter resolved and my lost money found along with an apology from XXXX and CBCS.

I had been paying on XXXX Hospital bill ( account # XXXX ) in the amount of {$1300.00} for Date of Service XX/XX/XXXX, religiously, every single month to XXXX XXXX XXXX ( XXXX ) in XXXX, Indiana. Then one day, my husband went in to pay the bill, and he was told by a XXXX representative that " nothing else was owed on the bill. '' The money ( {$50.00} ) was immediately given back to my husband, and we heard nothing else regarding this bill until XX/XX/XXXX. In XX/XX/XXXX, I started receiving calls from CBCS Collection Agency ( phone XXXX ) regarding this bill. However, I NEVER once received any documentation whatsoever from CBCS regarding this account, even though I asked several times for written confirmation that this debt was owed, or what the new amount was. Around XX/XX/XXXX, I started receiving calls from XXXX XXXX XXXX, stating that I owed the exact same amount on the exact same account to their law firm on behalf of the same hospital. Once again, I NEVER once received any documentation whatsoever from XXXX XXXX XXXX regarding this account. *According to the Fair Debt Collection Practices Act, I should have received some sort of documentation within 5 days from both of these collection agencies, but I DID NOT! Now, I am getting multiple calls from both agencies at the same time! Therefore, I vehemently DISPUTE THE VALIDITY OF THIS DEBT!! and I intend to stand up for my rights. Two companies can not legally pursue the same debt at the same time.

As of XX/XX/2019, I checked my credit profile and noticed an account in collection from CBCS for XXXX XXXX XXXX. I immediately contacted XXXX and the collection agency CBCS to notify them that this account was a fraudulent account that did not belong to me because I have never been to that hospital before. On XX/XX/2019 CBCS responded to my dispute and sent me a letter stating that after investigating my dispute they have stopped all collections activity on this account and requested that the credit bureau remove the collection account from my credit report. What occurred after this letter was sent to me has left me confused because according to the credit bureau, CBCS removed the account from collections for a few day then reopened the collections account. On XX/XX/2019, I called XXXX to check the status of the collections removal and the representative stated that they determined the account belong to me. At this time the collection agency and XXXX are refusing to remove this fraudulent account from my credit file. They have also refused to provide me with any type of documentation proving that this account belongs to me. This situation has caused me a lot of stress because I should not be penalized or punished due to the fact that I am a victim of identity theft.

In previous complaint,XXXX I advised that Credit Bureau Collection Services, had contacted my primary number. At that time they had taken part in 3rd party disclosure as advised in complaint listed. This company never supplied me with a validation of debt, nor a chain of custody agreement as I requested. They are now not only violating my rights as a consumer, and my legal verbally cease and desist order I provided them by again calling me, they are now also violating yet another law. They are foreshadowing the validation period, by attempting to collect and taking part in collection activity of the debt prior to myself being provided the validation paper work that I verbally requested on XX/XX/2018. On XX/XX/XXXX I contacted XXXX and put this debt on a payment plan of 12 payments of {$78.00}. This amount was debited on XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX and XX/XX/XXXX for a total of {$310.00}.
On XX/XX/XXXX I received a notice from the company XXXX XXXX stating that this account was overdue and it was sent to collections.
On XX/XX/XXXX I called XXXX ' billing department, spoke with XXXX XXXX ( supervisor 's name is XXXX XXXX ). XXXX was able to verify that I setup a payment plan via phone on XX/XX/XXXX and informed me that due to a XXXX ' internal accounting mistake this debt was sent to collections even though I was making the payments on time, per the agreed upon payment plan.
I believe that this was done in bad faith, and that XXXX sent my debt to collections with the intent to intimidate me, giving me no option but pay the debt in full, even though we had agreed on a 12 month payment plan.
I felt threatened and afraid that this debt would be reported to the credit reporting bureaus and therefore paid the remaining debt in full on XX/XX/XXXX.

Each visit I paid {$100.00} and advised that I would like to set up a payment plan after treatment was complete. The staff was willing to accommodate this request. The fact is I never visited this medical center in XXXX as I was living in a totally different location at the time. Hello. On XX/XX/2020, I received a collection letter of a " past due amount on account that was placed with this office for payment. '' CBCS Company, a XXXX XXXX XXXX.
I am being contacted for a debt that I do not owe. This is a consumer loan with XXXX. I was " coerced '' into being a Co-Signer on this account by XXXX XXXX. XXXX. It is an attempt to defraud me. Verbal promise made to me by Mr. XXXX to pay the loan.
Attached is collection letter from CBCS. I have attached also most recent Consumer Loan Statement, from XXXX, for this account, that clearly shows Mr. XXXX 's name, and Not mine.
Mr. XXXX has a mental issue ; has been stalking me, I have previously taken him to court to stop contacting me. I asked XXXX to remove my name from loan, and it was refused.
I ask that XXXX make the arrangements to remove my name from the consumer loan. If needed I will seek additional assistance to accomplish this, including legal action.

A debt collection company ( CBCS ) continues to harass me an d try to collect on a debt that has previously been paid. I have subsequent billing statements that show the debt as no longer being owed, and that I am 100 % current.
On their first attempt on XXXX XXXX, 2015, I sent a cease communications letter that explained that their charges were in error and that I choose not to work with a third party collection agency.
On XXXX XXXX, 2015, they sent an additional letter demanding payment. I replied stating that I do not wish to work with a third party collections agency and to please cease communication with me in regards to this error.
Today, I received a third harassing letter demanding payment for a debt that I have previously paid in full to the original creditor. I have penned a third cease communication letter and will mail it out in the morning certified mail.
I have copies of the certified mail receipts for the first two letters, and a copy of the letter 's themselves if needed. I suffer from XXXX ( XXXX ). I just want this harassment to stop!

The company XXXX has issued a debt collector, CBCS, to collect on a false debt that has already been paid for. I purchased a ladder from their website online and have records of payment and receipt, they do not ship a ladder unless it is paid for, so there is absolutely no reason for them to try to collect on a debt that has already been paid for. I then got letters from a debt collector indicating I still owe for the ladder that I already paid for. The balance is {$230.00}, but I already paid this and should not even be seeing this debt. I already sent proof of payment to the debt collector and told them I am disputing the debt, while they only replied to me a letter indicating the bill of which I have paid for. Again, you can not get merchandise from this website unless you pay for it, so there is absolutely no reason why I am being billed for a ladder I already paid for.

I contacted the hospital and they did not have any record of the charge with the account number listed on the notice.The account # liisted on the notice was XXXX. We did have two payments for that amount to XXXX XXXX 's last year in XX/XX/XXXX and XX/XX/XXXX, but they were paid. There was a previous fraudulent claim action against this company in which they paid over a million dollars to the FTC for this same type of action. CBCS is trying to collect a debt for {$37.00} for an injection not covered by Medicare Health Plan Insurance.
CBCS represents XXXX XXXX XXXX XXXX XXXX for a service of injecting some medication which was not approved by my Medicare Health Plan Insurance as medically necessary needed for my treatment, consequently, I am not financially obligated to pay the {$37.00}. Medicare said that if not covered by my health insurance plan I don't have to pay it since I did not know at the time of the service for what the physician gave such injection.

I habe received numerous letters from a collection agency regarding a XXXX bill that I do not owe. I noticed a new collection on my credit on or about XXXX When I contacted the gas company who sent this to collections I was told they had a system reboot XX/XX/XXXX an no longer have access to the records. I have repeatedly requested verification of that I owe or how I owe this bill when I did not reside in the residence at the time they claim. I have yet to receive verification of my request. The company has point blank told me that the burden is on me to prove I do not owe them. Receive notice from credit bureau that a collection file was added to the 3 credit bureau, for a XXXX dollar debt. I have yet to receive a written documentation from this collection agency in order for the right to confirm, or reply for verification of debt per. the fair credit reporting act.
