RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT METRO 2 data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX XXXX XXXX XXXX and XXXX. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with The XXXX XXXX XXXX, Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C. 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. Finally, you are reporting the INQUIRIES that are the result of the same type of fraudulent activities. It must be noted that I did not apply for credit accounts with the below-listed companies and creditors, some of these are the result of attempts to open accounts, but the companies did not approve the accounts. Therefore, these inquiries must be deleted from within my credit file. You can not delete the fraudulently opened accounts without also deleting the fraudulent inquiries. It should be noted that an inquiry that was not made by me is not a factual record, just as a fraudulently opened account is not a factual record and not referred to the creditor to be disputed directly with them. The credit reporting agencies have attempted to address an inquiry as a factual record of file access, but if it was done through fraudulent means, it is not a factual record. As a consumer, I should not be required to dispute each fraudulent inquiry directly with each company. These inquiries are the result of fraudulent activity and therefore, inaccurate in which the credit reporting agencies or the creditors can not verify, meaning that these inquiries are unverifiable information and must be deleted from my credit files. 1. CORNERSTONE CREDIT S bal. {$0.00} I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from alleged identity theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer that the information relating to the transaction by the consumer. I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. Complaint against Cornerstone Credit Services. Background Information In early XXXX of XXXX, I contacted cornerstone credit realizing that they were claiming I owed them close to {$900.00}. After researching the matter, it turned out that XXXX XXXX XXXX ( XXXX ) in Alaska did not terminate my cable and internet accounts when I left Alaska. On XX/XX/XXXX, I presented my military orders to XXXX so that they could terminated my contract with the terminations fees waived. At this point, my account indicated {$0.00} with no forwarding charges. However, there was still a balance on the account, but XXXX invoice system indicated {$0.00} due to a poorly design system. I went on with my life not knowing that XXXX never processed my request completely, and continued to charge me terminations and late fees etc. While XXXX is one of the company responsible for this issue, this complaint is solely against Cornerstone credit services due to their lack for morals, poor judgement, accountability, and frankly laziness. 1. Failure to communicate and take initiative/research Cornerstone credit services did not make a satisfactory attempt to contact me. I am aware that collection companies are limited with how and when they can contact people who owe them, however, Cornerstone made no attempt to contact me. I have lived at the same address for the past five years ( XXXX ). Yes, my phone number changed a few times ; however, my correct phone number is recorded on all three credit bureaus. Additionally, I am the only person in the United States with my name. A simply search with the U.S. postal service would have indicated my correct address because I am a service member and I have to update my address every time I move. Most importantly, Cornerstone needed to verify amount owed with the XXXX. This was not done. Cornerstone did not even have my invoices. Moreover, XXXX had my department of defense ( DOD ) identification number which Cornerstone could have utilize to locate me. Over the past five years, Cornerstone made zero efforts to contact me concerning this debt. Instead, then waited for incorrect balances to accumulate interest, over time. Lastly, Cornerstone credits services is listed as Cornerstone Bank on my credit report. This is misleading and resulted in me not recognizing them as a collection agency. 2. Takes advantage of unfortunate situation/mistakes After intense research with supporting documents, the amount I actually owed XXXX was about {$430.00}. However, Cornerstone demanded that I pay them {$900.00} due to interest. They wanted to charge me almost double the principal owed due to several failures on their end and XXXX. XXXX reported my name incorrectly to Cornerstone ( first name written as last name and last name written as first name ) was not my mistake. XXXX reporting a largely inaccurate balance to Cornerstone because they did not process my termination request and close my account was not my mistake. Cornerstone failing to do their research with contacting me was not my mistake. The only mistake I made was, not recognizing that Cornerstone was not a bank on my credit report, but was a collection company. I take responsibility for this. Additionally, I contacted Cornerstone and XXXX immediately when I realized I had a collection issue. I expressed repeatedly that I am willing to work with both parties to correct the issue. Yet, Cornerstone demanded that I am still required to pay the interest that was double the amount owed. I presented myself as a service member that has a stable income and a trustworthy financial history. I pointed out that it is not of character for me to deceive anyone, especially when it comes to paying bills. I also pointed out the chains of mistakes made on both companies that led to this situation. However, Cornerstone, repeatedly told me that it is not their mistakes and I deserved to be charged double the amount I owed. 3. Harsh negotiations. I offered to pay {$500.00} to settle the account to which they initially rejected. One supervisor made me a counter offer of about {$870.00} who was not able to explain why I should pay this much. Another supervisor supported the {$870.00} offer and bashed me for not paying my bills. Cornerstone did not attempt to accept or apologize for their failure to contact me, or research the issue. After several pleadings, I eventually spoke with a manager who accepted my offer, but I was still denied any form of settlement letter or confirmation to the credit bureaus that the issue was revolved. Overall, no collection agency should seek to take advantage of someone willing to pay what they actually owe. Cornerstone needs to recognize that mistake happens due to poor management of records and customers service which leads to situations like my case. People who owe debt are not always intending to rob a company, especially service members. Cornerstone lack the ability to recognize this. 4. Impact of Cornerstone mismanagement of my case. Cornerstone has cause me unnecessary financial and mental stress. Having a collection agency on my record almost cost me my job in the military and could have severely affect my qualifications in my field of work. Moreover, Cornerstone poor management of my case limited my credit score, and will remain so for the next 7 to 10 years, causing other financial hindrances such as buying a property at a lower interest rate.

I was seen in the Emergency room at " XXXX XXXX '' in XXXX, AK. I was suffering from a severe XXXX/XXXX XXXX ( for background information I am a XXXX XXXX XXXX veteran with severe XXXX XXXX due to my service in the military ) The doctors treated my symptoms like a XXXX, despite my trying to communicate to them what it was. Later, as I was beginning to feel better and had been treated, I gave my info to the nurse/billing individual. At the time I had XXXX insurance as well as VA coverage ( 100 % for XXXX related issues ) Months later I began receiving letters saying I owe some {$300.00} to the hospital. After a quick look at one of these bills I was able to ascertain that the hospital was not/ did not charge the VA , just XXXX . When I was able to get in contact with XXXX billing department some months later, ( I had moved from Alaska and was nearly homeless ) I explained what their mistake might be. The employee immediately saw their error and promised to right it immediately. Several months later I received letters from the VA stating that there had been an attempt to bill me more than 6 months after a hospital visit. They will not cover these things after 6 months. When at the hospital, I did my due diligence and gave all my information for payment at that time. When contacted by the hospital I did my due diligence and explained their error, which their representative admitted it was their error, and again, I had done my due diligence. The hospital employees failed to read over all the information provided by me at the time of the visit. They failed over and over again until the VA period of payment passed. They did not do their due diligence. Instead of accept that their employees failed to get their jobs done right, they sent my " bill '' to collections. " Cornerstone Credit services '' is the collections agency. They first contacted me on or around XX/XX/2021, @ XXXX. As I have mentioned, I have severe XXXX and other XXXX related to my service; I usually don't take phone calls. I chose to anyways that day. I was informed of a debt, and I frankly did not believe the caller. I explained I had moved to XXXX and wanted to see everything in writing before I would even believe the caller wasn't a phone scammer. I gave my new address ( and confirmed it that day and during another call ) and began politely waiting for the billing information. I even explained to the caller how this is not my debt, the hospitals employees screwed it up, not me. I was promised a letter within 10 days. Finally, after months of not receiving ANYTHING but calls claiming they had sent out plenty of copies to me in XXXX ( suggesting I was the one avoiding payment ) I wrote a letter with my information on it, included a Self-addressed Stamped Envelope for their own good, and sent it to them clearly marked " COLLECTIONS '' via registered mail on XXXX XX/XX/2021. Today ( XXXX XX/XX/2021 ) I FINALLY find a letter from the collections company in my mailbox. Since I received confirmation of my debt only today, I am invoking my 30 days to dispute these charges. They are not mine to pay, as I have laid out : 1. I did my due diligence the entire time when communicating to the hospital. From initial giving of BOTH my insurance coverages, to helping correct the already too late error their employees made. It's not my job to make sure the billing is done right, that's their employees job. 2. I did my due diligence when contacted by the collections agency. Made sure my story was told and gave a correct updated address. The length of time it took to get me a hard copy, and the fact I had to send registered mail to them before I even got one statement shows that their employees too are not doing THEIR due diligence. 3. All of the individuals involved failed to do their jobs correctly, and in a timely manner. This is not my fault as they are not my employees nor did I train them. So how can they press a debt on my person when their peoples mistakes caused it? 4. Lastly, the collections agency tries to call me semi-frequently, despite my XXXX issues being made known to the original caller in XX/XX/2021. I find this to be harassing, and severely detrimental to my XXXX health as well.

Accordingly, to the Fair Debt Collection Practice Act, a debt collector can not add fees are interest to the amount owed on a debt. In my case this is what is happening in which makes the debt unlawfully to be collected. Section 804 and 809 - Validating Debts of the Fair Debt Collection Practices Act and Texas Finance Code section 392.101 which prohibits a third-part or credit bureau from engaging in debt collection in Texas unless the third party debt collector has obtain a surety bond and file a copy of the bond with the Office of the Secretary of State, Also, a debt collector or Debt Buyer can not hire a third part debt collector to collect debt in Texas on their behalf and the debt buyer is not registered with the State of Texas. I respectfully request that you provide me, in writing, the following : What the money you say I owe is for ; Provide me with itemized calculations on what you say I owe ; breakdown all fees Provide me with copies of any papers that show I agreed to pay what you say I owe ; Identify the original creditor ; and original contract from original creditor Provide a verification or copy of any judgment from the original creditor ( if applicable ) ; Show me that you are in compliance with Texas Finance Code section 392.101 with my state, and provide me with your bond information and registration. Provide to me the full name, address, city, state, zip code, and phone number to the original creditor Provide the original account number in the contract from the original creditor Provide me with the open and close dates on the on the original creditors account number Provide the month-day-year of the last payment received from the original creditor Provide me with the contract agreement between the original creditor and your company to prove you been hired to collect on their behalf Be advised that I am fully aware of my rights under the Fair Debt Collection Practices Act and the Fair Credit Reporting Act. For instance, I know that : You can not add interest or fees accept those allowed by the original contract and state law. You do not have to respond to this verification except to tell me that you either intend to cease your collection efforts or to pursue other legal means of collecting this debt. Should you pursue a judgment without validating this debt, I will inform the judge and your case will be dismissed based on your failure to follow the FDCPA. Any attempt to collect this debt without validating it, violates the FDCPA. Can be taken up with the consumer themselves. Reporting information that you know to be inaccurate or failing to report information correctly violates the FCRA 1681s-2. At this point the collection company is report the inaccurate amount I owe on my credit report.

I have disputed a dental bill on my XXXX credit report. It was denied due to dental office provided record of my visit. I called dental office as well as cornerstone credit to explain my dispute. I did not deny visit my problem is this. 1 ) the bill was sent to an address I lived at in XX/XX/XXXX so I did not know it existed. My husband and I currently use this dentist and they have our updated address so I dont know what they mailed something to an address from XX/XX/XXXX. 2 ) the dentist at the time ( who is no longer with Office ) saw me to look at a XXXX he made that was giving me problems. He told me there was no charge due to circumstance of it not fitting properly. I was given the option to send to the lab for a few days with charge of around 500.00. I declined at the time because I couldnt afford it and did not want to be without it for 3 days at work. He put a temporary repair on it no charge until I could get a permanent fix. I did not know of or expect a bill for this. I disputed and was denied due to dental office providing proof of service I resubmitted dispute under the information I just provided to you On XX/XX/XXXX The estimated time for dispute under XXXX web sight for investigation to be complete was XX/XX/XXXX. Checking the status of that dispute it was never completed by XXXX. As of today XX/XX/XXXX conformation # XXXX Is still showing a pending status and has he ever been investigated. I did resubmit a dispute XX/XX/XXXX for the same debt and it was denied again. I believe my dispute should be granted due to the fact that the XX/XX/XXXX dispute was never completed properly according to my rights as a consumer their job was to investigate and complete investigation within a timely manner. It has not been completed as of this day. As far as my most current dispute on same credit issue I am disputing the conversation the doctor and I had about not charging me for the visit, as well as the mail not being sent to the appropriate address. The staff in the office currently are all mostly new as they had a huge turnover since my XX/XX/XXXX visit. When I talked to a lady in reception at dental office she encouraged me to dispute this, as according to her files are so messed up from old staff, I am still trying to fix things, we have found so many errors its a nightmare. I wanted the dentist no charge promise investigated and the problem regarding mail not being accurately mailed to me to be looked at I would like my disputed granted due to incomplete investigation from XX/XX/XXXX and would like proof of current dispute that was denied was investigated properly.

To whom it may concern : Both my children were enrolled in the XXXX XXXX XXXX XXXX in XXXX Alaska From XXXX until XX/XX/XXXX. I paid consistently and on time twice a month. At the beginning of XXXX I put in a request for termination of service. I also received assistance for Child Care from the military as I am an XXXX XXXX XXXX. They paid a portion and I paid my normal XXXX on the XXXX and XXXX .. this was automatically took out of my bank account.. first week of XXXX my XXXX was deducted from my account. The children ended their time at XXXX XXXX on the XXXX. There should not have been a balance due at this time. XXXX XXXX proceeded to tell me I owed another XXXX. I argued with them that was false and showed them the record where I paid the first half of XXXX. Everything is pre pay! I had to pre pay them for them to start. They insisted I owed them more money. I agreed to pay half even though it was completely unfair and robbery but they said no. The bank even told me they had other clients with the same issue from this business. XXXX XXXX sent this bill to collections. I am currently XXXX in the XXXX XXXX with limited access to email and phone. I received the collection while in country with a notice saying if I did not dispute this account by XX/XX/XXXX I would have to pay in full, seeing as today is the XXXX and I just received the letter that is impossible. I would like to hold this company responsible for trying to over charge me and as well as the collection agency for the harshness of not being able to dispute while I am XXXX to a foreign country. any help or insight you can provide would be greatly appreciated! I am not sure what else to do. I know I have the right to see the justification for the charge but how and why does it make sense for me to pay a full month child care when my children were only in attendance for 2 weeks and I was instructed it would be prorated. I checked my credit reports and contacted the people it said I owed the Debt to. Several of the companies no longer exist. The ones that still existed I paid the debt and it was thousands less than what was shown on my credit report. I had an attorney contact Cornerstone credit about the ones the company no longer existed on. Cornerstone credit said adding 15 % interest to medical debt a month was allowed. So debt that was XXXX dollars as now several thousands of dollars. When asked if they would make a deal, they refused even though they knew that several thousands of dollars is all interest, for a company who probably took a deal as it was being bought out by another company. I recently have been divorced I guess several of the medical bills went to my ex-husband 's address. I did not know I owed them until I had seen them hit my collections. I contacted both of the debtors because it was only 2 new debts that I had missed. I paid off one debtor and it should show as paid at the end of the week. ( XXXX hope. ) The 2nd debtor says they do not show I owe them anything, nor would they because they sell their debts to Cornerstone. I have tried to make payments to a specific account before to cornerstone and they just add it to whichever one they feel like so they can restart a 6-year-old account to a company that no longer exists. I called the other debtors who say I have paid all my back bills and their affiliates in full. The Attorney and my divorce Judge have told me not to contact Cornerstone because of the stuff they have been doing but I would like to pay off the recent bill so it is not on there when the others one fall off nor do I want it to start collecting the 15 % interest. Is it allowed for the original debtor to refuse you to pay off your back debt? If I want to make a payment to a collections agency on that specific account are they allowed to put it towards whichever account they wish? Is the collection agency allowed to keep stuff on my credit they can not prove I owe? ( I can not find the information or the bill and when the attorney asked them to send me the proof of the debt, they only send me their debt bill on not the original. They said this would a breach of confidentiality. ) With the above explanation, How is it a breach of confidentiality for a debtee to know who is claiming the debtee owes them ( the debtor ) money? How am I to know it is not fraud?

An Alaskan physician egregiously billed for labs to my insurance company that were well above industry standards. This occurred in XXXX, XXXX at which time I sent a letter requesting additional information to the physician. I never received a response, and in XXXX, XXXX found the charges in collections. My insurance company paid above usual and customary. For example, the physician charged {$130.00} for a XXXX XXXX Panel. This test cost under {$20.00} in the industry. I am being billed for charges that greatly exceed industry standards. I 've written several times to the physician and collection firm disputing the debt and asking for them to substantiate their charges. I 've asked where do they get their charge data to what do they accept as payment in full from all payors because that is the true value of their services. For example, if they accept {$15.00} for the XXXX XXXX from XXXX XXXX, they are charging up at an enormous rate for out of network patients. The collection company and physician refuse to offer any of this information and have never responded to any of the specifics of my dispute. All they have sent is an itemized statement showing their over-billed lab charges, a copy of the form I signed regarding paying the remaining charges, and refuse to recognize and place my account in dispute. Although I did sign the form, I had no idea what the physician would have charged for services nor what would be considered reasonable for the services. The rates for XXXX are under 20 % of what they 're charging for an example. The collection firm has spoken to me as if I am a child in that they 've " gone above and beyond to get this resolved. '' That is false. They have provided nothing beyond an itemized statement. I will pay extra for the labs that my insurance company did n't cover but the charges must be reasonable. The charges are unregulated and excessive and they will not even consider negotiating. My professional experience has been in health insurance and health care clinics. In my role, we worked with patients/consumers to collect outstanding debts, sent monthly statements, and treated our consumers with respect and dignity. Cornerstone has done none of it, nor has XXXX XXXX office. I am asking for your help in resolving this and having my account recognized by the collection firm as being in dispute.

I have tried to communicate with both the debt collection agency and the medical group about my dispute regarding there inability to provide adequate notice of this debt. I am well aware of local and state consumer protection statutes and am asking the CFPB for assistance regarding the credit reporting of the above referenced account. Specifically, I have sought advise as to recently discovered adverse credit remarks on my credit report. I was informed that both the Fair Debt Collection Practices act and the Alaska Unfair Trade Practices Act protect consumers against unfair collection practices and malicious persecution. I would like to point out that creditors or collection agencies are prohibited from submitting information relating to the consumer to any consumer reporting agency if they have been notified by the consumer that the information is inaccurate pursuant to 15 U.S.C 1681 ( S-2 ). I hereby remain adamant that should I have been given the opportunity to dispute or pay, I would have exercised my rights as prescribed by law. For the record : I was NEVER in receipt of your original dunning notice allowing me 30 days to dispute this debt, or satisfy this debt and avoid any negative marks on my credit record. Therefore, the information that was submitted is inaccurate and must be removed. In my communication from XX/XX/XXXX with this debt collection agency, they claimed that they spoke to me in XX/XX/XXXX and that I hung up on them. This is completely false. I have never spoke to this debt collection agency. A few years ago I cancelled my phone contract with XXXX because I found out it was cheaper if I just bought a pre-paid phone. Well in order to get a pre-paid phone and get out of my current phone contract I had to pay off what was left on my current phone contract. I'm pretty sure I was told I paid off everything with them. To someone in a position of authority, I am making a formal complaint because Cornerstone Credit Services has placed information on my report without my consent, and without validating the debt. I reached out to this company over 4 months ago and I still have not received a response. I informed them that they were violating 15 U.S.C. 1681a ( 2 ) ( B ) as they were supposed to exclude information from my report that was authorized by a credit card. I informed them of this violation and they have not responded to my correspondence. Additionally, I asked them to validate the debt they are reporting on my consumer report with XXXX ; I made a formal complaint with XXXX, Cornerstone Credit Services, and XXXX and they all failed to show proof that this debt that they are reporting belongs to me. Because they failed to validate the debt with sufficient proof, they are in Violation of 15 U.S.C. 1692g. As the original creditor, only I can validate this debt. If I continue to be ignored about this matter I will proceed to seek litigation and remedy for {$1000.00} per violation for defamation of character, Negligent Enablement of Identity Fraud, and Violations of 15 U.S.C 1692g. For the entire year of the dates of service from XXXX XXXX XXXX XXXX, I was receiving daycare vouchers from the state of Alaska. Those vouchers were sent directly to them, from my caseworker at the time. Her name was XXXX XXXX. When approached to pay the amount by XXXX XXXX XXXXXXXX XXXX, they informed me they hadn't received my payment. I told them that the payment had been arranged through my case worker, prior to the kids coming. They said they would contact office. The next time they asked, they asked me to pay it and then if payment came from state they would reimburse me. By this time, it was 2 months worth so I informed them I could not pay, but I could pay in installments, if it would keep the kids in daycare while they sorted out the vouchers. They declined my payment attempts, and said the kids could no longer attend until balance paid. I contacted my caseworker again, who stated she had already submitted payment to XXXX XXXX XXXX XXXX for the amounts owed. I've asked to have an invoice or listing of months that were submitted for payment from XXXX XXXX XXXX XXXX to my case worker. ( Which months did you send the bill to the state for? ) Or did they never submit for payment? In order to know which charges are due, I need to know which months were paid from vouchers from the state. Re : Cornerstone Credit Services.L.L.C ( XXXX ) I have spoken to representatives at Cornerstone regarding notices to pay client XXXX XXXX XXXX the total amount of XXXX regarding statement XX/XX/2022. When I called the Cornerstone representative today, XX/XX/2022, they notified me the total in now {$180.00}. Even though I still have not received a bill from XXXX XXXX. I called XXXX XXXX today and I paid my bill directly and paid the {$150.00} bill. ( paid on XX/XX/2022 ref # XXXX ) On XXXX, XXXX Cornerstone sent a notice on behalf of XXXX XXXX XXXX for the amount of {$44.00}. I also paid them directly. ( attached statement paid on XX/XX/2022 ref # XXXX ) Cornerstone wanted me to pay the {$44.00} and {$180.00} today. One amount has interested applied and the other does not. When I last spoke to Cornerstone I was told the total amount included both the {$44.00} and {$150.00} to get me pay the {$180.00} that the XXXX XXXX statement was at the time knowing the {$180.00} would not cover the {$44.00} amount. Both attached statements are from Cornerstone Credit Services. Cornerstone wanted me to pay a total of XXXX today but I declined stating both Alaska XXXX XXXX and XXXX XXXX have been paid. To whom it may concern, I have on more than one occasion to verify two items XXXX XXXX XXXX XXXX # XXXX specifically on XX/XX/2019, XX/XX/2019 asked this company to validate the information on my credit report under Section 609 ( a ) ( 1 ) ( A ), that they are required by federal law to verify - through the physical verification of the original signed consumer contract - any and all accounts you post on a credit report. When I was residing in XXXX, Alaska in XXXX I had been taken by ambulance from XXXX XXXX to XXXX XXXX. While the Dr was tending to me my fiance ' provided the billing department with my Drivers License as well as my XXXX card and XXXX XXXX card. XXXX XXXX never sent me a bill for the body scan they performed on XX/XX/2022. I paid {$230.00}, then they said they would send to my insurance, XXXX XXXX. I never got a bill from XXXX XXXX or from XXXX regarding my XXXX scan. So this collection agency has broken the law by not properly communicating these issues with me. XX/XX/22 a most recent credit report revealed an account in collections. A letter was sent to XXXX XXXX asking yo verify debt on XX/XX/22 certified mail ; no response was received providing any of the information requested. Pursuant to 15 USC 1692 ( d ) .1, Cornerstone Credit Services, a collections company, has violated the FDCPA by damaging my reputation which is my credit history and score as their alleged debt has now been reported. I have never been contacted about this debt nor spoken to anyone and was unaware of the existence of said alleged debt. The reporting of fraudulent information by Cornerstone Credit, whom I have no contract or agreement with regarding any alleged debts or services, is defamation and stands as damages to my personal reputation. If Cornerstone Credit can produce a contract I have with them, I will gladly review and respond and I will be sending communication to them as well.

We were customers of XXXX and upon closing out our internet services we werent notified that we had a router that needed to be turned in. Instead a debt collector called us and told us we had a {$150.00} unpaid debt from XXXX. I immediately gathered the router and brought it to XXXX and received a receipt of proof that the issue was returned and taken care of. I emailed the copy of the receipt to XXXX XXXX XXXX on XX/XX/23 and still this debt is showing on our credit history, hurting our credit score. We need this removed. My family and I have since moved from Alaska to Arizona on military retirement orders and no longer have any ties to gci. I went to purchase a vehicle yesterday and this populated as a derogatory impact to our credit with should be resolved with no impact. When I paid this debt, I was promised by an agent, that the account would be removed from my credit in full. they have since reported a {$0.00} balance on the account for many months, I was under the impression that after I paid the debt, they would report the {$0.00} balance for 1 month, then remove the account entirely! I have asked the company to investigate this matter many times, and each time they have refused because I paid them! I paid them because they promised to delete the account, I would have had to consider my actions more carefully had they not misrepresented the situation! I know paying an older debt can hurt my credit scores, so I would have taken more time to consider my actions, but since they promised FULL ACCOUNT DELETION, I paid that day, thinking it would benefit me the most! Why are they still reporting?

i was contacted by an agent asking about person who doesn't live with me or at my address, when i asked who was calling they refused to give any information. only to have person who doesn't live at my address or use my phone, call back number on caller id. i researched number found out it was Corner Stone Credit services. i called them back and explained to them that said person doesnt live at my address, doesn't use my phone, and told them to put my number XXXX on a DO NOT CALL LIST. that is when the lady got rude and said my number was put down as contact number. i said no my number is listed as an emergency contact not debt contact. and do not use it as such. that is when she got mad and said i am placing you on hold. that is why i am filing complaint as this is not good debt collections from these blood suckers.

I had a large amount hit collections for medical debt. I called the collection agency- they said XXXX send me paperwork. I never received paperwork. I called my insurance company- they said absolutely Im covered and shouldnt have any debt. I called the hospital that supposedly sent my account to collections. They told me IMMEDIATELY that some sort of mistake was made, and that they will have the collection agency delete the collections off of my account immediately. My biggest issue with this is, 1. ) I disputed this account MULTIPLE times and it came back as all good thats a huge violation 2. ) Even once they delete this off my credit, it wont fall off until next credit cycle. So my credit will remain affected for even LONGER- when it should have never been affected at all.

I am a victim of identity theft. Due to the Corona Virus Pandemic, we are all facing which has me sitting still at home and I saw the recent news about the multiple XXXX Data breaches. I was diagnosed with XXXX while studying medicine out of state in Alaska. Since the insurance was through a Washington state coverage ( my home state ) I had to get prespptoval for treatment out of state. I thought bills had been paid by insurance and later learned the had failed to cover the initial XXXX XXXX performed in Alaska as well as a number of follow-up visits which were in Washington state, where the major treatment was performed. I received a summons to small claims yesterday for two emergency medical bills accrued during the pandemic and that also fell in a grey area when looking at what my Medicaid covers retroactively. I spoke with the debt collectors in XXXX of this year to explain that. After no further contact, I thought Medicaid covered one or both of the accounts. I definitely did not think Id be sued, given I had made recent payments on other accounts and had long standing unpaid accounts that werent attempted to be collected as aggressively. I contacted XXXX XXXX XXXX in XXXX, AK when I discovered I had an old cable account from 2008 which was sent to collection. I spoke with a " XXXX '' at Cornerstone regarding this old account, which does not report on my credit report. Asked the balance, amount and where I could make payments when I am ready. He called my work today at XXXX from an unknown number during my work business hours to ask if was still going to pay that account. I did NOT give him my work number as a contact number or permission to contact me here, EVER, nor did I tell them my employers information. They have my current mailing address, that is sufficient. This is getting frustrating to the point that if I can't find resolution this time, I'll have no other choice but to file suit against them! Just so happened to see this on my credit report. I sent a dispute letter and the company came back as it did n't have enough information to investigate. XXXX, They recieved my name, birthdate, so called origional creditor ... If these people can add this collection to the credit bureau, then they can find me in their system. If they " ca n't find me '' as they say, or fully validate any account, they have NO right to try to collect. It 's my right to request valadation or they must delete collection from report. Cornerstone credit is attempting to collect an incorrect amount on a medical bill from XXXX XXXXXXXX XXXX associated with XXXX XXXX. XXXX XXXX was made aware of the billing error, it wasnt fixed as they stated it would be and instead sent the bill to Cornerstone. Cornerstone has been made aware of the error. Our health insurance company states that XXXX XXXXXXXX XXXX needs to re-submit a corrected bill. Neither Cornerstone nor XXXX are making the corrections and instead we keep getting repeated phone called from Cornerstone continuing to collect an incorrect amount.

Cornerstone Credit repeatedly calls my phone regarding a debt they claim I owe. I paid the debt through the original creditor ( XXXX XXXX ) and have given that information to Cornerstone Credit, which they acknowledge the debt was in fact paid and they can not report anything to my credit report because it was paid through original creditor. They repeatedly call me to pay their fee, which the bill should never have been sent to them in the first place. I have requested they stop calling, which they do, then start calling again 2 months later.

XXXX XXXX XXXX was the place I went a few years back after a XXXX XXXX XXXX XXXX, for XXXX XXXX. I got a lawyer, XXXX XXXX XXXX, and they said " this was never found in the discovery period. ". I have TRIED to pay this to XXXX XXXX XXXX, but their phones are off and business closed! There is no way to contact them. Two different medical institutions sent bills to XXXX XXXX XXXX as they did not bill to insurance, I paid these amounts directly to the individual healthcare entities, thus the two entries from XXXX should be removed from my reports, as this was never a collection issue it was the problem of the billing of the healthcare entity that did not bill insurance
