On XX/XX/XXXX, I received out-patient services at XXXX XXXX XXXX XXXX in XXXX, XXXX. The remaining balance of my bill ( after insurance payments & adjustments ) for the services, was {$360.00}. I received the first bill from the hospital and the payment due date was XX/XX/XXXX. On XX/XX/XXXX, I made a {$50.00} payment ( confirmation # XXXX ). After making the payment, I received a letter from the hospital stating that the " payment did not qualify for monthly installments ''. I called to set up a formal AUTOMATIC debit payment of {$26.00} per month to be debited the FIRST of each month. Every single payment has been honored by my bank upon representation with the exception of the XX/XX/XXXX payment. I realized in XXXX that the debit card that I used to set up the automatic payment had expired. Therefore, I initiated a call to pay the {$26.00} ( confirmation # XXXX ). Beginning with the XXXX payment, the {$26.00} resumed automatically ( confirmation # XXXX ). My complaint centers around collection notices that are sent to me each month. I contacted the hospital and was informed that all accounts are sent to a monitoring/collection agency when they are NOT paid in full upon receipt of the bill. The hospital said that I should not be receiving collection notices ( XXXX XXXX XXXX , XXXX, XXXX. XXXX XXXX, XXXX, XXXX XXXX ) and that they are not the ones sending the notices. I contacted the collection company and was informed that collection notices were sent to me because the wrong payment date was in the computer system and the date was now corrected. The company also informed me that though I was in receipt of collection notices, they had not reported my account to the credit bureau. When I asked why I was receiving collection notices, no one could provide a reasonable reply. Is this a new wave? Sending collection notices to consumers who are paying on time? My last attempt to resolved the issue was XX/XX/XXXX. In an effort to resolve the above issue, on XX/XX/XXXX, I submitted a complaint to the XXXX XXXX XXXX XXXX XXXX ( complaint # XXXX ), XXXX XXXX XXXX XXXX, XXXX XXXX, XXXX, VA because I was a resident of XXXX. I am now a resident of XXXX. The XXXX XXXX XXXX ( XXXX ) forwarded a copy of my complaint to XXXX XXXX XXXX XXXX. On XX/XX/XXXX, the XXXX sent me a letter stating that " XXXX has made multiple attempts to contact the business, but has not received a response. Our policy in this case is to close the complaint as No response, which will be reflected in the business 's XXXX Reliability Report. '' Last week I received another collection notice. I contacted the agency on XX/XX/XXXX. The representative said that in XX/XX/XXXX my account was being " monitored '' and that perhaps ( her assumption ) because the XX/XX/XXXX payment was late by 5 days, and the account is considered " old '', it was sent to " collections ''. The representative also said that as long as I continue to pay my account on time, no reports will be filed with the credit bureau. I find it difficult to image that a company looking at the overall payment history of an account would send it to collections because it is 5 days late. I further reiterate that I initiated a call to the collection company in XXXX to ensure that my payment was received. With the exception of the XX/XX/XXXX payment, following are the automatic payments of {$26.00} made to Account Number XXXX with XXXX XXXX XXXX , XXXX, all of which have been honored by banking institution. As a consumer I have not missed any payments nor have I ignored my responsibility. Following are the XXXX payments : XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, and XX/XX/XXXX. As resolution I am respectfully requesting that all collection notices cease and that a letter be sent to me stating that no reports have been made to any credit reporting agency. I furthermore request to know if the conduct of collection agencies is now to send collection notices to consumers who are paying on time. As a consumer, I consider the collection notices being sent to me by XXXX XXXX XXXX as abusive and an unfair business practice, because my account is being paid on time and as agreed.

I had an emergency room visit and my health insurance did not pay all of the amount. - I have a family member who is a XXXX, XXXX XXXX. She was going to try and reach some agreement with the hospital and the two other medical practices that are billing me. She spoke with XXXX XXXX, located at this number ( XXXX ) XXXX, which is the location of XXXX XXXX XXXX XXXX XXXX XXXX in XXXX, AL on XX/XX/2019. She requested the Medicare codes for each billed item. He would not provide this. She explained to him that I could not possibly pay {$8800.00} because I had just started to work after a number of years of unemployment. He offered to spread the bill over thirty-six months. Ms. XXXX told Mr. XXXX she would look at my income and expenses and be back in touch with him within thirty days to inform him of what I could pay. He told her that bankruptcy was probably my best course of action. - However, Mr. XXXX didn't wait anytime until he turned the account over to a collection agency, Professional Account Services, P. O. Box, XXXX, TN XXXX. I sent them a letter letting them know that a XXXX, XXXX XXXX was going to be contacting them for some additional information. The letter was sent on XX/XX/2019, to the address In XXXX, TN and they signed for this letter on XX/XX/2019. - Ms. XXXX contacted them by phone but they wouldn't provide any information that she requested. - Within, a couple of weeks based on my income and expenses we determined a monthly amount and the number of months that I could pay for the hospital bill. We sent the offer to the XXXX, TN address listed above. -What I received was a letter from Past Collectively Different, XXXX XXXX XXXX XXXX, XXXX , TN XXXX. It read " We are in receipt of your dispute letter regarding the above referenced amount, which was {$8800.00}. We have verified with our client that the charges are true and correct. Please contact our Customer Service Department at XXXX to discuss arrangements. Pursuant to the Fair Debt Collection Practices Act ( FDCPA ) Section 14 usc 1692g, we have met the provisions set forth to validate this debt. ". I HAD NEVER DISPUTED THE CHARGE IN ANY CONVERSATIONS OR WRITTEN CORRESPONDENCE WITH THE HOSPITAL OR COLLECTION AGENCIES. - My complaint is that the collection agency refused to accept or even respond to my offer made to them. Rather than respond to the letter of offer sent to the XXXX address, they sent my account to another office for collection, This was obviously done to keep the collection process going. -My financial situation was not one of my own making. I lived in a town in XXXX Alabama, which had been a manufacturing town. However, the recession caused more and more companies to leave the area each year. Unfortunately,, I was tied to that area because I owned a house and could not sell or rent it. I was finally able to move and became employed in the last quarter ofXX/XX/2019. - I need the collection agency to consider the offer I made and to quit sending my account to other branches so they can continue the threatening letters.

XX/XX/2014 I had XXXX at XXXX XXXX XXXX XXXX in XXXX Mississippi. As a result I managed to acquire XXXX separate accounts that I began receiving bills for. I asked the hospital to combine the accounts so I could make larger payments on XXXX account but they refused. The first XXXX accounts I managed to pay off fairly quickly. The XXXX and XXXX accounts I was still paying on when XXXX XXXX turned the larger of the XXXX remaining accounts ( the XXXX account ) over to Professional Account Services ( PASi ) for debt collection. Up to this point, all of my payments had been made directly to XXXX XXXX XXXX Systems. A representative from PASi called me at work and during this conversation I was told that it was ok to continue paying the hospital, but paying PASi was the same as paying the hospital, and that I should pay PASi. Told him I had no intention of paying the debt collector and that all of my payments would continue to go directly to the hospital. I explained to this representative that it was unfortunate that his company had gotten involved in a matter, as I could prove that payments were being made on all of the outstanding accounts, not just the one that had been turned over to his company. At this point the hospital was wanting me to pay XXXX a month on the account that they have not turned over to collections and PASi wants me to pay XXXX on the account that had been turned over to them. I had explained to them on more than one occasion that I could not pay their minimum balances. In XX/XX/XXXX, I received a letter from a law firm saying the account had been refered to them for legal action. I responded to the law firm with a letter of my own and explained to them the same thing I had been telling XXXX, and took the opportunity to remind them that I had been and still was paying the bill. I have now been supoenaed to court on XXXX XXXX. This suit was filed with XXXX County Justice Court on XXXX XXXX, 2014, a month BEFORE I received the letter from the law firm saying they were reviewing it for legal action. Why the law firm bothered to harass and intimidate me further when the debt had already been filed with Justice Court is beyond me. Why I 'm being harassed for a debt I 'm paying and now sued for a debt I 'm paying is beyond me as well. They say to call and make payment arrangements. I 'm already paying them, why should I make payment arrangements on something I 'm already paying? They were n't willing to work with me and combine the accounts. Why should I work with them and make a payment arrangement for something that 's being paid? Now, I have to miss time from work to go to court for this. I do n't understand why I was turned over to collections and sued when I have been and can prove that I have been paying them.

I have been dealing with XXXX XXXX since XX/XX/2016 which required XXXX, major XXXX with complications and multiple subsequent XXXX of which I still have 2 more to go. I had 2 medical accounts that were sent to a collection agency by my hospital when I was trying to set up arrangements with them. They told me it wasn't a real collection agency and wont affect my credit report. So I just went with it and made arrangements with the collection agency to pay XXXX per month on both accounts. The balance on the accounts were XXXX and XXXX approx. I said I would like to pay online and that was agreed upon. From time to time the payment would not be accepted, saying I was not paying enough money even though I was putting in a payment for XXXX as agreed upon. So I had to call. They tried to tell me the payment was late which it was not. This happened a number of times and I got so sick of calling to have them reset things. Sometimes I just paid the additional amount they were asking for but I couldn't continue to do that. I have been paying down 4 other medical accounts, my son also needed XXXX XXXX in the middle of this .... It had become time consuming to handle all the paperwork and I wasn't feeling well. So in the middle of a bad month, it happened again and by the time, I had a chance to call, the payment was then late. As soon as that happened, of course, they reported it to my credit which I only learned about yesterday. I called yesterday and got blamed again for paying late. I was talked to like a child which made me so mad. Especially after the person I spoke with said something that I called her out on being untrue and then she completely contradicted herself. I told her that if this is the way they treated me, I won't be paying them off first and to bump me down to XXXX a month. So today I went to apply my payments and both accounts come back saying I owe more. So I called again and now I'm being informed that XXXX payments won't be accepted online. My options are payments by phone or mail a check. Phone payments are not secure and I do not trust mailing a check for a lot of different reasons. It seems that if they are in the business of collecting money, they would accept anything that was being sent to them. I've been spoken to in a disrespectful and passive-aggressive manner which has tried my patience and reduced me to angry words. Today they hung up on me which left me sobbing. I am trying to get my life back on track from the past 2 years and emotionally struggling and this feels like a kick in the gut.

I was hospitalized in XX/XX/XXXX. The attached form discloses the claim dated XX/XX/XXXXfor a disagnostic image ( MRT ) that was done prior to me being admitted to the hospital. Initially, my insurance company informed me I was responsible for the entire bill of {$8100.00}. I am insured under my mother 's ( XXXX XXXX ) health insurance. She wrote a letter of appeal ( see attached ) regarding this outstanding bill. On XX/XX/XXXX, we received a letter from the insurance company ( see attached ) that approved our appeal and a copy was sent to the original creditor ( XXXX XXXX XXXX XXXX XXXX ). During this entire time, we never received a bill from XXXX XXXX XXXX XXXX XXXX before or during the appeal process. On XX/XX/XXXX, we received a bill ( see attached ) from XXXX XXXX XXXX XXXX. We immeditately called the original creditor to make arrangements directly regarding the revised amount I owed, {$770.00}. We came to an arrangement and made our first payment XX/XX/XXXX ( see attached bank statement ). We received a call from XXXX XXXX XXXX on XX/XX/XXXX. We 're filing based on the following 4 things : 1 ) Dual Tracking abuse from XXXX XXXX XXXX sending this debt to a collection agency and negotiating simultaneously ( 2 ) Professional Account Services attempting to collect on a debt prior to the 30-day validation period ( 3 ) Never received a bill from orginal creditor prior to receiving letter from collection agency ( 4 ) Collection agency collecting on a debt when original creditor agreed to payment arrangement and received first payment. We expressed to the collection agency during the XX/XX/XXXX call that we ( see attached letter ) prefer to deal directly with the original creditor since we made the arrangment directly with them. They refused and implied that I was refusing to cooperate and pay this debt. I told them that is not true as evidenced by the arrangement that was made and she acknowleged payment was made ( 10 % of the balance for the next 10 months ). Further, I believe the original creditor prematurely submitted this debt to the collection agency given the time frame of the appeal process and the new rules that medical organizations must adhere to when placing a medical debt to a collection agency.

I had routine visit 's to XXXX XXXX XXXX center in XX/XX/XXXX I stopped getting bills after normal month and insurance coverage wait. I continueously contacted their billing dept throughout XX/XX/XXXX with the concern. Was abruptly told that their billing system changed and I would get the opportunity to make my payments once things were straightened out. In XX/XX/XXXX I received a total of amount due from their Dept out of state in XXXX with a " Collections XXXX XXXX '' address. I immiediately called that business number and was told I owed payment in full. I reached the local branch office in XXXX business dept to discuss my concern at this point and inability to make a full payment. They set me up on a payment plan. I immiedately made my XXXX payment and every month since, thereafter, to date as of now XX/XX/XXXX. A few months ago I received XXXX " Collections '' notices of an amount due much higher than what I have it already paid down to, along with harrasing calls from a dept collector on the facilties behalf named " PASI ''. This become so frustrating as they would not initially acknowledge the proper amount I yet owed but that I had already been making payments monthly and on time all along to XXXX XXXX XXXX XXXX directly which I have full documentation of proof of. I actually had to go to the local business office in person to ease my mind and was late for work. I eventually lost my job over this and the emotional distress it has put me under. Now I am getting calls yet again by " PASI '' to collect this " Debt '' which is not a Debt as I am still and all along making payment directly to XXXX XXXX XXXX XXXX intially by phone debit payments and most recently via their website as they instructed me that I could do. So I am now without a job, yet still making my payment as per their payment plan directly to them, and still getting harrassed by debt collections " PASI ''. What rights do I have at this point? Again, I have full documentation of all payments and some legal recorded calls regarding the matter as I informed other party on phone that I would be recording the call as per XXXX state law. Can anyone help me? I am emotionally distressed over this and out of work.

I had gotten 3 calls from 3 different numbers all claiming to be XXXX, none of the phone numbers were associated with XXXX and were all previously reported as scams, claiming I owed $ XXXX {$990.00}, it changed each call, and I told them I would pay it when XXXX sends me a bill. I never got a bill in the mail or a call from a legitimate phone number. Then a couple weeks later I received a letter from " Professional Account Services Inc '' with a very vague description, the wrong name, and the wrong address. Then a few days later I received a call from a woman, who had a clearly fake " southern '' accent, claiming to be from the company and saying she was in Tennessee. I told her I was unable to talk as I had to take my daughter to therapy, but she refused to let me off the phone politely. She claimed to have my social security number, which is a threat, and I gave her my email so she would let me hang up. I never received an email. I looked into the phone number because it had a XXXX area code. Turns out this company " from Tennessee '' is using a landline registered in XXXX XXXX, Florida. After that I called the real XXXX service and they looked at my account and said there was no {$990.00} bill ever and they had no idea where that number was coming from. So I called the " debt collectors '' back. A lady without an accent answered and then after hearing me changed to having an accent, then when I said I spoke to XXXX she dropped the short-lived accent and started screaming that " XXXX knows who we are '' and when I said okay but I spoke to them she screamed that I am " a liar '' and starting having what sounded like a mental breakdown and I just hung up. Why would XXXX not send me a bill and immediately send it to a debt collector? I am tired of the harassment and threats and want these people shut down. One of the phone numbers they called me from was XXXX.

XXXX XXXX XXXX, XXXX sent me a debt collection letter. I received this letter on XXXX. On the same day, I responded and asked the collection agency to validate the debt and provide HIPPA authorization. On XXXX, I received another letter dated XXXX from XXXX. The letter states that the charges were verified with the original creditor and included an itemized billing. An itemized billing does not validate the alleged debt pursuant to FDCPA and FCRA. In response, I mailed another certified letter on XXXX requesting validation of the debt and HIPPA authorization. This was after calling the collection agency a few days prior and being told by a XXXX employee that they would keep reporting to my credit unless I paid them. The second letter requesting validation was received by XXXX, but they have yet to respond with proof of validation or any other written communication. The only communication that they have made with me were phone calls demanding that I set up payment arrangements with them. XXXX last called me on XXXX after I had already requested that they not call me. I would prefer written communication. More specifically, I would like written communication reflecting a validation of the debt they claim I owe to them. According to the FDCPA and FCRA, unverified and/or debts not validated should not be allowed to be reported to my credit report without proper proof if such proof is requested. I have contacted XXXX about XXXX XXXX reporting, but XXXX has not provided XXXX with proof of validation or verification either. It is unfair and unlawful for an unverified and unvalidated debt to be reporting to my credit especially since I have made multiple requests requesting proof to resolve this matter. This is a clear violation.

On XX/XX/2018 I filed a complaint they the cfpb with an alleged debt of XXXX XXXX XXXX. The account was closed and removed from my credit report but put back onto my credit report several months later with any formal notice or validation of this alleged debt. I am allowed under the Health Insurance Portability and Accountablity Act ( HIPPA ) to protect my privacy and medical records from third parties. I do not recall giving permission for them to release my medical information to a third party. I am aware that the HIPPA does allow for limited information about me but anything more is only should be revealed with patients authorization. Therefore, my request is twofold, validation of alleged debt and HIPPA Authorization. Please have company note that withholding the information received from any medical provider in an attempt to be a HIPPA complaint can be a violation of the FDCPA because you will be deceiving me after my written request. I also would like a reason as to not being notified of this alleged debt being placed back onto my credit reports without and notice after it had been removed after filing a complaint in XX/XX/2018. According to FCRA I should have been notified or contacted, neither was done. Additionally, any reporting of this debt to the credit bureaus prior to allowing me to validate it is a violation of the FCRA. If this information can be obtained I will need the name of the person providing this data and the way that it was provided. If you can not obtain the information requested with the original creditor within the required time frame ( 30 days ) please delete this account from my credit report with all bureaus and that it can not be sold to another collection agency.

I went to the facility, filled out the paperwork, and paid the {$150.00} fee before seeing the doctor. I left the facility after seeing the doctor and was not asked to pay any additional fees. A few weeks letter I started receiving bills from the facility indicating that I owed an additional {$60.00} for the visit. I attempted to contact the facility by telephone several times but was unable to get through to a billing representative. I left several voice messages requesting a call back to discuss the bill, but was not called back. I sent several letters to the facility along with the billing slip indicating that I was told that the bill was {$150.00}, that I paid {$150.00}, and that them sending me an additional bill for {$60.00} was incorrect. They did not respond to my messages or letters. In XXXX of XXXX I was contacted by Professional Account Services , Inc. - a debt collector attempting to collect the {$60.00}. I wrote them a letter disputing the charges, stating that I never signed anything agreeing to pay {$210.00} for a {$150.00} service. I asked them to provide a copy of the documentation where I agreed to pay more than the {$150.00} I was charged. They responded on XX/XX/XXXX with a generic statement indicating that they received my dispute but had " verified with their client that the charges are true and correct ''.

A few months ago i contacted XXXX XXXX XXXX in XXXX pa regarding a bill that was XXXX. I paid a XXXX deductible and i was told that this was all i would owe at the time of service and if anything the next bill i would recieve would only be a few dollars more. It turned out the hospital didnt tell me that i would get that additional bill of XXXX. They told me the XXXX was all i would owe and billed me for XXXX.i disputed the claim by mail. I never recieced a call or a letter from the hospital after i wrote to them to try to settle the amount since they failed to notify me on what my insurance would cover and any additional cost after. I tried to negotiate it for 1/2 of the price they never got back to me nor did they notify me that they would be putting this on my credit after i attempted to settle the debt. Turns out i moved and still didnt recieve a letter regarding the balance but my credit dropped 30 points and they didnt get back to me as i requested them not to call me my cellphone is broken and to notify me by mail only. No attempt to settle nor resolve a problem that they caused after they told me this would be all that i would owe. I shouldnt owe anything if i paid my bill in front the day of service. They shouldve contacted my insurance 1st.

On XXXX XXXX, 2015 my aunt ( XXXX ) had a follow-up appointment with her XXXX. She had been hospitalized and was currently in a XXXX. The XXXX wanted a XXXX. He faxed an order to the hospital office which was just next door and indeed walked us out to the front of the building and directed us to where we were to go. We went and had it done. XXXX and her supplementary insurance initially paid for it but then withdrew the payments because they said that she should have had the XXXX at the XXXX. Medicare told me verbally that she was not liable for the payment. The physician had received documentation from the rehab so he knew where she was staying. Now the hospital is sending her ongoing letters and indeed a Final notice demanding payment as well as threatening to turn it over to a collection agency and listing it as a bad debt which will be detrimental to her credit rating. I feel the XXXX and those with XXXX are targeted as more likely to comply. I am her power of attorney and we are not going to pay for the mistake of others. To make matters worse, on XXXX XXXX the XXXX was repeated at the rehab because the Physician notes were unclear. My aunt should not have to pay for the mistakes of the professionals.

On XX/XX/XXXX, I went to a XXXX XXXX XXXX Urgent care facility in Tucson, AZ. Urgent care was a covered expense after copay by my then-insurer, XXXX XXXX XXXX XXXX XXXX. I paid the {$50.00} copay. On XX/XX/XXXX, XXXX made two payments of XXXX and XXXX to the medicare center, a document which I have. 2 and one-half years later on XX/XX/XXXX, I received a bill for XXXX for the same claim. I had never been contacted at all in the 2 and one-half years since the visit. I called the medical center for an explanation and was referred to a billing office in XXXX, who did not have the referenced document and did not want a copy, but said my insurer did not pay. I called XXXX, no longer my insurer, who could not locate the claim. I sent a letter challenging the claim with the documentation to XXXX, and received no response. They then referred the claim to PASI Collections in XXXX, TN. I challenged the bill, again sending copies of my correspondence with the medical center and a copy of the document showing payment. They denied the claim without explanation. I am unable to understand what I am being billed for, 2 years later, and sent another letter asking for an explanation. No response at all.

I a hospital bill that was turned over to collection to XXXX XXXX XXXX XXXX XXXX I paid off my bill in full on XX/XX/XXXX. Because they failed to discontinue my automatic payments from my checking account, they withdrew another {$50.00} dollars from my account on XX/XX/XXXX. I called them this same date and told them what happened and they said they would reimburse my account. On XX/XX/XXXX, I still did not receive the reimbursement and called them again. This time they said they didn't show that they had taken the extra money and I needed to send them proof of payment. On XX/XX/XXXX, I faxed them proof of payment. Called on XX/XX/XXXX and they said they received the fax and would reimburse me and that it would take a couple of days. On XX/XX/XXXX I still had not received the money and called them again. They said it would take up to 10 days for the money to be reimbursed. Today is XX/XX/XXXX and I still have not been reimbursed. Every time I call these people they give me a different answer. I think they are just stonewalling me hoping that I will give up. I am on social security and {$50.00} is a lot of money to me. This seems like an unfair and illegal practice to me.

Dear representative, On XXXX XX/XX/XXXX, I went to XXXX XXXX XXXX XXXX XXXX for a bad case of XXXX. According to XXXX XXXX XXXX, I owe them {$4600.00}, however, that is not the problem even though its over a {$1000.00} an hour not counting the Doctors and the Radiologist that I have paid. The problem is that XXXX never sent me an invoice. I have contacted them multiple times and was told I didnt have an account. I finally got in touch with a representative through e-mail and then lots of phone tag. I thought things were fixed. So I made a payment of {$150.00}. I still have never received an invoice. Now to my surprise, I have received a letter from a debt collector. I never received any notification that this step was happening. I have no way to know that this debt collector is legitimate. I called XXXX again on XX/XX/XXXX to talk to the same representative that I thought fixed things. She said she would work on it and get back to me, but never did. The Doctors, Radiologists and Nurses that I had were awesome! They took great care of me even though they got very busy. I wish I could say the same for the medical center.

I received a Collection notice from Professional Account Services , Inc. for a hospital bill in XX/XX/2018. I explained that I have contacted the hospital many times about the fact that my insurance company stated I reached my max out-of-pocket amount in XX/XX/2018. Therefore, I was advised to inform the hospital to bill the insurance company directly. Despite the fact that I have advised the hospital verbally and in writing to bill the insurance company, the hospital nonetheless sent the account to Collections. The Collections agent told me I need to clear it up with the hospital, but when I told her I've already tried many times yet they still sent it to Collections, she told me that's my problem. I asked why they don't contact the hospital ( their Customer ) and she said they don't do that. I let her know that if I am contact by Collections again, I will consider it harassment and make a complaint. She said she will just block my call. I have not had experience with Collection Agents, but this woman fit the stereotype I've seen in movies and television.

I was rear ended in a car accident on XX/XX/2019. I went and received medical attention on XX/XX/2019. I filled out of the form with information from the other insurance company. Today on XX/XX/2019, I received a certified letter in the mail from a collection agency named XXXX XXXX XXXX XXXX, XXXX TN stating they put a lien on my house for medical bills unpaid. First of all, I never received a bill from the hospital to pay anything. I called the other parties insurance company and they stated they never received any bills from the hospital. So the hospital never sent out any bills but immediately turns it over to a collection agency who states they filed a lien on my house. At the top of the letter it states they filed with the county clerk in XXXX XXXX. My house is not even located in XXXX XXXX. I, then call my local property accessors office and ask if there has been a lien placed on my house and they say no there is not a lien on my house. So this collection agency is using scare tactics.

I received a bill today address to Me, except it had my boyfriends name added to the end of mine. The bill is my boyfriends bill. In the body of the letter is states the patient is my boyfriend, account number, service date and amount owed. I called to ask why they are sending me a bill for his account, and advise them that they ca n't just add a name to someone else 's bill and say it now is this persons bill and this person is responsible because they say so. They kept asking me what his dob was. I would n't provide it. First, it 's against the law for them to be discussing 'his ' account with me, it violates his privacy act, and HIPPA XXXX Second, if they sent me the bill and state that it 's mine, they can have my information and we will discuss it. I am not providing someone else information to them. They would n't discuss anything further and second time I called t he manager realeased the call for not providing 'his ' SSN.

Professional Account Services has been harassing my wife and I over a hospital bill that we are currently disputing with them because the state of Florida should be paying via retroactive medicaid. This bill is from XX/XX/2020 when I was laid off due to Covid and my son was born. Insurance paid their portion, the rest was supposed to be submitted by the hospital to the state of Florida for medicaid to cover. The hospital failed to do so and failed to notify us either by mail or by phone for six months after my son 's birth. When they finally did notify us, it was to tell us that the outstanding balance of over {$5000.00} was being sent to a collection agency. My wife and I have been going back and forth between the hospital and the state to solve this problem but the debt collection keeps calling and has used aggressive and abusive language towards my wife and has been calling every few days at random hours and while I'm at work.

I stayed in the hospital for a period of time and was billed by several areas of the hospital. I was sent to collections with PASI who setup multiple payment plans over a 2 year period. After my debit card expired, we established a new plan for XXXX per month out of my husband 's checking account. Payments continued monthly until XX/XX/2021. We never changed the bank account or had a returned payment however PASI sold our debit to a new company and stated it was due to no payment being received. From XX/XX/2021 to XX/XX/2021, no communication was received asking for payment and no notice of the debt being sold was ever sent. I called PASI who said they were not sure why I was not advised or why I was ever transferred. The agent transferred me to Customer Service where I got a recording that their office had just closed 5 minutes earlier. The agent also advised that whoever answered could and would get me a statement.

On XXXX/XXXX/15 at XXXX PASI ( XXXX ), a medical debt collection agency called me. It was my debt and I have paid it off. The problem lies in the fact that when I was contacted the first time, the lady said that if I did not pay in 10 days, the debt would hit my credit and they would garnish my paycheck. I called back the next day and verified that they were a collection agency and not the billing department of the hospital that I went to. Upon verification that they were indeed a collection agency, I informed them that I knew my right under federal law and that they were not allowed to report the debt or garnish my wages at this point in time without taking me to court. This is when I was informed that they would give me 30 days to pay instead of the 10 days originally quoted. Now, it was my debt, i legitimately owed it and I have since paid it. However I do not appreciate the unnecessary threats.

My son and I were both seen in the emergency room at different times last summer and fall. The hospital did not take our copay of {$75.00} at the time of service. We received bills from the hospital and paid them in XXXX and XXXX of 2016. Those debts were paid directly to the hospital.

We have been contacted by this agency repeatedly, and I have told them that the debts were already paid. On XXXX XXXX, I faxed them copies of the cancelled checks and referenced the account numbers related to the charges. A representative from the agency had informed me that the letters and phone calls would stop once they received that information. We then received an invoice, dated XXXX XXXX, asking for payment of this charge. I informed them prior to sending the XXXX XXXX fax that further communication from them after I sent the fax would result in my filing a complaint, but they sent the invoice anyway.

Professional Account Service, INC called me to collect a debt for medical bills that I am currently making payments on and am up to date. Company stated that I was in collections and had not been making payments. Tried to explain that I am making payments and they stated that not all of my accounts were combined and that my payments were n't being put towards the debt amount in collections. I paid the amount. The following day I checked with the accounts payable department where my medical bills were and they stated this has happened to other people and confirmed that I have been making payments on time and my debt should not and is not in collections. This is fraud and is treating patrons unfairly and misrespresenting debts to patrons that are not in collections that can bring undue stress to individuals. This practice should be stopped and they should represent debts fairly and truthfully.

Professional Account Services keeps calling me and will not even tell me why. I know they are a debt collector for medical bills. I do not have any medical bills. They call and have asked for my social security number to identify myself, even though they called me! When I refuse, they then ask for my birthdate - which I wont give them either since I wo n't give any personal information to a stranger on the phone who wo n't say why they are calling. I 've asked them to send something in the mail and I will respond if it is legitimate, maybe a doctor I 've seen. But after a month of my requesting this, they do not send anything stating if I even have a debt. This company has the power to ruin my credit if they are really trying to collect a debt ( there is n't XXXX ), or even if it is a mistake. This seems a wholly illegal way to handle this. they call from XXXX.

I received a collection notice stating that we owed {$140.00}. I request proof of the debt. The collections agency sent me a copy of the original bill. I responded that I would like to see a complete statement of our account as we PAID the hospital {$1100.00} on XX/XX/2019, based upon their estimated bill after the expected insurance payment. On XX/XX/2019, the Hospital REFUNDED {$550.00}. Our thoughts were that we paid based upon an estimate, and after all the insurance settlements, we actually overpaid. I requested a statement of the account that shows what we paid, what the insurance paid, and what the remaining balance is. We can not understand how the hospital can say we were delinquent when they gave up a refund. The agency responded to our request for a statement with a letter threatening to report us to the credit bureaus.

On the same day, I responded and asked the collection agency to validate the debt and provide HIPAA authorization. On XX/XX/2018, I received another letter dated XX/XX/2018 from the collection agency. The letter states that the collection agency has verified the charges with the original creditor and includes an itemized billing. The collection agency failed to validate the debt pursuant to the FDCPA or provide any agreement with the original creditor that would grant the authority to collect the alleged debt. The collection agency failed to provide a copy of my signature with the provider of service to release my medical information to the collection agency.

My hospital bill that I have consistently been making monthly payments on since its origin, was turned over to a collection agency " Professional Account Services Inc. on XXXX/XXXX/XXXX. I sent the collection agency a CEASE & DESIST letter on XXXX/XXXX/XXXX explaining that I was settling the debt directly with the hospital and not to make any further communication to me regarding this account. They recently sent me a letter on XXXX/XXXX/XXXX explaining that they have been trying to unsuccessfully contact me by phone & advised me to pay them the debt by phone with a credit card. They sent me another letter two days later on XXXX/XXXX/XXXX threatening to place negative marks on my credit report unless I contact them to resolve this account.

Professional Account Services continues to report an account that I have previous disputed and sent police reports as well as identity theft packets. This account was previously removed in 2020 when XXXX XXXX XXXX was the collector o behalf of XXXX XXXX XXXX ( also attached ) however it is being repeatedly bought by new debt collection companies even though it is fraudulent. XXXX has been given this information on several occasions yet they have refused to remove this collection after being sent supporting documents showing that this was a result of fraud. I would like to now only file this dispute against both companies for legal documentation but to also get the support of the federal trade commission to help me resolve this issue.

I have tried on several occasions to get the XXXX XXXX XXXX XXXX to rebill my insurance company for the {$1900.00} they were initially charging for the XXXX XXXX XXXX I had done. Instead of them doing so, they hired an collection agency to harass me. I called XXXX XXXX XXXX XXXX this morning to get to the bottom of this and the first rep I spoke to told me that if I did not pay they would sue me! I had insurance and called Arkansas Medicaid to verify it. I even called the Medicaid Claims department and grievance department. I was told by the grievance department that the hospital failed to submit the claim with an extension or with a XXXX referral. This couldve all been solved without the threats and headaches I received.
