RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT METRO 2 data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX , XXXX , and XXXX. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with The XXXX XXXX XXXX, Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C. 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. Finally, you are reporting the INQUIRIES that are the result of the same type of fraudulent activities. It must be noted that I did not apply for credit accounts with the below-listed companies and creditors, some of these are the result of attempts to open accounts, but the companies did not approve the accounts. Therefore, these inquiries must be deleted from within my credit file. You can not delete the fraudulently opened accounts without also deleting the fraudulent inquiries. It should be noted that an inquiry that was not made by me is not a factual record, just as a fraudulently opened account is not a factual record and not referred to the creditor to be disputed directly with them. The credit reporting agencies have attempted to address an inquiry as a factual record of file access, but if it was done through fraudulent means, it is not a factual record. As a consumer, I should not be required to dispute each fraudulent inquiry directly with each company. These inquiries are the result of fraudulent activity and therefore, inaccurate in which the credit reporting agencies or the creditors can not verify, meaning that these inquiries are unverifiable information and must be deleted from my credit files. 1. VALLEY CREDIT SERVIC bal. {$1800.00} I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from alleged identity theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer tha t the information relating to the transaction by the consumer. I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. On XX/XX/2017 I had a scheduled appointment with my XXXX, I had previously had a XXXX XXXX done by her ( which that portion was paid in full ) and was going to get a XXXX XXXX. I called three weeks prior and spoke to secretary ( I have names of everyone involved ) about my XXXX insurance changing from XXXX XXXX to XXXX XXXX and that I wanted to make sure that they accepted that insurance because if not I would have to cancel and get the XXXX from a provider under my new insurance. I gave XXXX all of the information on the new XXXX insurance and was contacted back and told yes I was covered and they accepted that insurance. The day that I went for the XXXX, I walked out to the desk as I have done at each and every appointment and secretary who was filling in for normal secretary told me that there was no bill left. Every time time prior to that I would have a procedure done and would pay right after. I have total prior to this have spent well over {$3000.00} at this location and have bank statements to prove, in addition my children and husband have been, my child had a XXXX XXXX and I paid the remaining ( after what insurance was paying ) the same day. Fill in secretary said that she would let secretary know my concern and when she was back in the office she would give me a call. The very next week secretary did call me, she called to let me know that my insurance was not covered, that she goofed. She told me that she would speak to the XXXX and would return my call. I explained a XXXX with insurance through my new insurance was pennies compared. secretary called a couple days later and told me that they would charge me what I would have paid through the prior insurance, that being {$480.00} I told her that this was their fault and they should charge me what I would be charged through my new insurance because I would have simply gone somewhere else. Again she told me she would call me back. A week went by and she called back and left a voicemail that they would " take care of the bill '' Now it has gone to collection, on top of that they sent {$9.00} for my husband to collect under may name as well, which should not be a charge at all because all of our XXXX and XXXX with the old insurance was all preventative and NEVER charged. They also had a credit under my children that they used for my husband that we NEVER knew about. On top off all of this I NEVER received a bill until I fought it with credit monitoring site and marked it as a dispute. Last week they creditor sent me the bills an everything. To say the least I am not happy. This is not ok. I would have never went back to her, I have ALWAYS paid every bill to her the day of service. Now they are trying to collect {$480.00} for what they say I owe, {$140.00} for collection fees, and my husbands {$9.00} which if we were told at the time and actually owed it we would have paid for my husbands bill.

I have recordings of XXXX different poeple giving me XXXX different reasons for a XXXX. ** amount. First was a remander amount of XXXX. When I called in XXXX tobpay amount in full. My charge was for XXXX including fees and interest on the full XXXX amount. My questions I wanted answered on this has yet to even be addressed and is where my dispute is. The other reason for this XXXX amount was said to be a bounced check ( which I am neither confirming nor denying because it was not my check. ) Also the third reason for this amount was a separate XXXX dollar bill in itself. I have these recorded and XXXX separate companies of which my medical debt was through who were told XXXX of these reasons on the wxact same day. I have requested reciepts in regards to what my payments have went to in detail so I can either agree with them or shiw a mistake was made. I am told i will be sent one and never received one. However, I am refused help. When asked not to be transferred and to speak with a manager I am denied. When asked to leave voicemail I was told mabager does not have one. When asked to handwrite a message for manager they refused to. All of this is recorded and I can prove it so there is merit. I was told yesterday that the manager was out then received call from XXXX who was helping me and said she just talked with her. My issue is the lack of answering questions, a lack of giving me rightful procedures to follow to complain on one in offuce person in paticular who transfered me twice when I ask to please not to. How can I deny nor agree to this debt when they refuse to address any of it? My first concerns popped up when new amounts arose. Im not saying they havent been there the whole time. Im saying they were not in the amount when I myself first tracked vcs down XXXX to start paying my debt off. I asked to pay XXXX lowest amounts first. That XXXX was not mentioned. Later it shows up.i call just asking for more details and that is when I was transferred out of office, not guven proof of payments to certain accounts, and the run around in regards to any manafer of any kind. They made a issue when they have failed to give me proof of all this new stuff. So yes I wanted more details befire paying off. I am not satisfied with there answer. I received notice from a collection company Valley Credit Service XXXX in XXXX XXXX stating that I ( XXXX XXXX XXXX owed Md. XXXX XXXX XXXX XXXX XXXX XXXX, Md. XXXX {$210.00} for services in XXXX XXXX. Firstly, I have never been a patient of this group. Secondly I do n't know where this XXXX XXXX the visit comes from??? A little background. My wife, XXXX, was a patient of this group while XXXX XXXX XXXX XXXX XXXX in XXXX up until XXXX XXXX when she was XXXX. Because she lives in XXXX XXXX Md she is told to get a new nephrologist because they do not practice in that area. My wife did so and her new Doctor is XXXX XXXX, XXXX XXXX XXXX XXXX, XXXX XXXX, XXXX XXXX. He was her doctor from XXXX XXXX through XXXX XXXX, XXXX. At which time my wife had an XXXX XXXX at XXXX on XXXX XXXX, XXXX. XXXX spoke to her original doctor, which is no longer with this group, regarding the supposed charge for XXXX XXXX. XXXX XXXX XXXX told her he never put this charge in my file. In the meantime this group sent to my XXXX and XXXX and were paid {$270.00} This is a fraudulent claim. My wife did not have an office visit!!! As for me, how did they get my social security number, etc and file a bogus debt on my credit report? XXXX XXXX billing person for Md. XXXX group said she never sent it to them in my name ( XXXX ) ; however, the fact is I have a sworn affidavid from the collection agency showing her signature that she did in fact send it to them. No one has yet to apologize they just told me to wait and pull your credit report to see if it is gone in XXXX. All I ask is a letter from them stating the truth about this debt and the collection agency. They all tell me they do not have to do anything for me as it is the law. They made the mistake why do I have to jump through hoops to get this resolved. It was their error and done without my knowledge. Lastly, why did my wife, XXXX, never receive any kind of bill from Md. XXXX XXXX for over 4 years??? And when one came it was in my name not hers.??? Please note each letter states a different amount owed, i.e. {$270.00} - {$220.00} - {$210.00} so which is correct????

A credit collection company claims I have a debt with them from XXXX when I was in college under the financial support and insurance of my parents. I called the original creditor to understand where this XXXX debt is coming from or why I never received any form of them attempting to collect the debts 1,2,3 or 4 years later ( it is now XXXX ). They claim to have contacted me in XX/XX/XXXX when it was turned over to them but haven't followed up since. Now that the collector has it they have been reporting it to the credit reporting agencies, but not attempting to collect it. Many complaints on this site, consumerfinance.gov, state that consumers are constantly contacted about a debt that they owe by collectors. One can assume that this is to reinforce that the consumer owes this collector a payment for a valid debt and that the collector still needs this payment. Until served a cease and deist letter or request, the collector can continue to contact the consumer. I never served this collector with a cease and desist letter, so I am unaware why they would stop contacting me for this debt that they still needed payment for. They stopped contacting me, but yet still reported to credit reporting agencies as needing to being paid. I only know they were reporting all these years after being provided a credit report which I received after being turned down for a loan due to the credit score this company has ruined. I firmly believe they are preying on the mistakes of financial uneducated and un-experienced XXXX year old faced with medical bills they needed help paying and are now just out to destroy my credit. They show little worry about collecting the actual debt.

I was originally contacted by collection agency in XX/XX/XXXX for a debt that I did not recognize, and agency threatened to report it on my credit. I requested validation of the debt and received back from them information about an XXXX XXXX my adult daughter had. My adult daughter had XXXX insurance under my name. I was listed as who the insurance information is under. I was not the patient. I did not agree to pay for the patient. I did not sign the financial agreement. The copy they sent me was signed by the patient, not me. I returned the documents to them pointing out I was not the patient, the insurance information is under my name and that I did not agree to pay for the patient, nor did I sign the financial agreement they enclosed a copy of. I never heard back from them again. I received a notification that my credit score dropped in XX/XX/XXXX and I requested a copy of my credit report and found the collection agency is reporting this debt as a collection record against MY credit. I immediately disputed to all 3 bureaus. The collection agency falsely confirmed to all 3 that it is indeed MY debt! I also wrote to the collection agency at the address listed on the credit bureau and received my letter returned not deliverable as addressed. They are ruining my good credit and my job could be jeopardized by my credit.

In XXXX of 2010 I opened a contract with XXXX, under the terms that I can break the contract at anytime due to them wanting my business and I was preparing for a transplant. The manager their signed me up for a membership, and stated with proper documentation they will not charge me anymore once it came time for my XXXX. However after moving, and deciding to review my credit this account was sent to collections, and I was indeed charged. I reached out to the corporate office of XXXX of the XXXX where I originally had my membership, and they stated I had no negative account and I was cleared from their system years ago. They advise me to ask the collection agency to send me verification of this account. I reached out to Valley Credit Services and was told by a female customer service rep they do not talk to debtors, nor do they verify, I would have to contact the gym. How? The gym sent me to them, and stated the information was not in their system as me owing them anything.They would n't even give me the name of the person/persons sending them this information. I spoke with the general manager of XXXX, as well as the gym manager and neither said they had negative information on my account.

I disputed a debt collection letter I received on behalf of A Visual Affair. The bill received from the office was for {$160.00}. They reported the debt to be XXXX to Valley Credit Service Inc ( VCS ) a debt collection agency. I contacted the business manager of A XXXX named XXXX XXXX who told me they sold the debt to VCS and that the additional XXXX was added on to as an administrative fee by VCS. I asked for XXXX XXXX to verify for me that VCS added on this administrative fee before I would pay it in a conversation that I told him would be recorded. In that conversation, he assured me he would contact me back and ensure that was the case. I never heard back from him. When I spoke with VCS and paid the {$160.00} I owed, she informed me that their company did not buy debts and that their company did not add on an administrative fee of {$160.00}. I disputed the remaining balance of the debt with VCS. A XXXX XXXX then sent me back the same invoice for only XXXX but certified that I still owed them {$160.00} without providing an invoice explaining the charge.

I lived at a location XXXX XXXX XXXX and moved the place i was renting from kept my security deposit because i broke my lease a month early. I checked my credit a few months ago and seen i owed a debt of {$680.00} for rent and I called to clear it up and got no response on the issue. So I recently checked my credit report on XX/XX/18 and seen that i still owe this amount when i was never notified by the landlord or his staff i owed any remaining rent balance or a judgement from the court. I discovered an unknown account on my personal credit report. I contacted the company by way of registered mail twice and I have not heard back from them. However, the credit bureaus allege that XXXX XXXX XXXX says the account belongs to me. I have know knowledge of what the account is for or whom the original credit is. I was not afforded the opportunity to dispute the unknown allege debt- it was just placed on my credit. I was told they can not do that and its against the FCRA. Valley credit collection agency was paid for XXXX delinquent accounts on XXXX/XXXX/2015. {$56.00}, {$100.00}, {$120.00}, {$25.00}.
