On XX/XX/XXXX I received a letter from the Commonwealth of Kentucky Department of Revenue stating, that I owe debt totaling {$1900.00} to XXXX XXXX for date of services XX/XX/XXXX for {$1800.00} and XX/XX/XXXX for {$89.00}. This was the first time that I heard about this debt so, I called the Department of Revenue disputing the debt. I was informed to fax a letter disputing the debt but, this would not stop the Department of Revenue from continuing processing any administrative collection actions unless they received a letter from XXXX XXXX to stop the process so, I faxed the dispute letter and called the Dept. of Revenue to verify that they received my which, they did. After, this on or approximately XX/XX/XXXX I received a telephone call from XXXX a representative from the Department of Revenue explaining that he received a response via e-mail from XXXX XXXX stating, that I owe the debt so, I had to pay but XXXX refused to give me the name and/or the telephone number of the person he received the email from regarding this alleged debt stating, he was told for me to contact my insurance company if I wanted to know what they paid. I ended the conversation with XXXX and took it upon my self to call XXXX XXXX billing department but, was told my account had been transferred to the CKMS which, I had to ask what it stood for " Central Kentucky Management Services. '' I spoke with XXXX a representative with CKMS and she begin speaking to me about another bill for some physician charges separate from what I received from the Department of Revenue that again, I knew nothing about. I explained that I had no ideal what she was speaking about and asked for an itemized bill including what was paid by the insurance company. Please, keep in mind that I never received any invoices in the mail and it is not on my credit report with any of the 3 credit bureaus. Therefore, I requested that the itemized debt be mailed to me showing what my insurance paid for the alleged debt. Upon receiving the itemized reports I noticed that one of the descriptions stated, that I was treated for " coronary care - intermediate CCU. '' However, I have never been treated for coronary care in my entire life. Also, I don't understand why I never received an invoice from XXXX XXXX XXXX and/or CKMS instead of receiving a letter from the Department of Revenue stating, that a lien not limited to funds being held by my bank, wages, real estate property, tax refund or other monies that may become due to the Commonwealth of Kentucky be seized. I truly do not understand this practice allowing a Medical Collection company to have the right to send a debt to the Commonwealth of Kentucky Department of Revenue when I do not owe the Department of Revenue and I never had a chance to review or dispute the alleged medical charges beforehand. Furthermore, I am unsure why CKMS has my medical records and I never signed a release form releasing my personal medical records to them breaking HIPPA laws. Lastly, I have researched this practice and found that this is an unlawful practice that XXXX XXXX has been doing for years. I would greatly appreciate your assistance with this matter.

I had to receive XXXX XXXX and stay in the hospital for multiple days. This resulted in a XXXX dollar medical bill from the hospital, I am uninsured. I attempted to apply for financial aid through the hospital but was denied. This debt was then sold to collections. I have spoken with the company and they were unwilling to offer any reduction to this debt, which is no way will I be able to pay or even attempt to pay off with a payment plan. This was not a voluntarily procedures and if I wouldnt have died without it I would have not opted to get the XXXX for fear of medical debt that I could not pay.

Collection Agency collected on a bill that was not owed. Agency confiscated state tax refund in 2018 - that is first time I knew of ANY bill from hospital. I phoned collection agency and disputed the bill immediately. Asked for verification of the debt. Agency could not verify debt. Agency then levied bank account. With NO JUDGEMENT. I got Medicare involved and they contacted agency stating this debt is not owed. Agency admitted they made mistake but will not return my money. Agency is guilty of illegal billing, double billing, and theft of my bank account.
