The XXXX Office of XXXX XXXX XXXX ( XXXX ), also known as " XXXX XXXX, XXXX, '' located at XXXX XXXX XXXX XXXX # XXXX, XXXX, IL XXXX, has repeatedly and falsely attempted to collect debts from me, even though they have clearly agreed in writing that the amount in question was paid by the now-defunct insurance company, XXXX XXXX XXXX. I am aware that other patients have had similar false claims submitted to collections by this same office, and I am urging federal and state consumer protection officials to take immediate action to stop this office from pursuing such fraudulent collection actions. On XX/XX/XXXX, I visited XXXX XXXX 's office for a XXXX appointment. At that time, I paid a {$20.00} co-pay to XXXX XXXX 's office because the provider was explicitly listed as a " Tier 1 Provider, '' as documented in writing by XXXX XXXX XXXX. In XX/XX/XXXX, I was contacted by a collection agency, XXXX XXXX and XXXX XXXX ( XXXX ) claiming that I owed XXXX XXXX XXXX a {$40.00} co-payment. Land of XXXX stated that this was an error and that I did not owe any amount to XXXX XXXX office. A staffer named XXXX XXXX in XXXX XXXX XXXX later confirmed twice in writing, on XX/XX/XXXX and XX/XX/XXXX, that I did not owe any amount to XXXX XXXX office and that the amount was due for reprocessing by XXXX XXXX XXXX. XXXX also indicated that XXXX XXXX office would immediately remove this inaccurate debt from the collections agency and my credit report. In XX/XX/XXXX, I began receiving inaccurate collection notices from a different collection agency, Armor Systems ( XXXX ). I explained to Armor Systems that the collections attempt is in error and that the debt is not owed by me. Armor Systems agreed to contact XXXX XXXX office on my behalf to resolve the matter. However, when I contacted XXXX XXXX office about the same, they responded on XX/XX/XXXX and refused to release the {$40.00} amount from collections. This is the third time that XXXX XXXX office has wrongfully filed an inaccurate balance with a collection agency, and because of this, it has had a negative impact on my credit score. ( XXXX XXXX 's office has also incorrectly stated that I visited their office on XX/XX/XXXX, a visit that never occured. ) I am deeply concerned that XXXX XXXX office may be in violation of federal consumer protection laws, such as 15 USC 1692f 807 of the Fair Debt Collections Practices Act. XXXX XXXX office may also be in violation of consumer protection laws in the State of Illinois, such as the Collection Agency Act, 225 ILCS 425/9.7, which empowers the Attorney General of Illinois to enforce fraudulent collection attempts. I am urging federal and state consumer protection officials to immediately investigate and resolve this matter, and to investigate and resolve any similar attempts by XXXX XXXX office to collect on fraudulent debts from other patients. I have attached evidence documenting the series of events outlined above. I look forward to a prompt resolution of this matter, including the immediate removal of this fraudulent claim from my credit report and from Armor Systems account in my name.

I am writing regarding several medical bills for which I am not responsible that have been incorrectly and falsely sent to collections by the billing office of XXXX XXXX XXXX in XXXX 2015. The bills submitted to collections by XXXX XXXX 's office total {$160.00}. Despite repeated attempts to have these bills removed from collections, XXXX XXXX 's office has thus far refused to do so. XXXX XXXX 's billing office incorrectly states that I visited them on three occasions XX/XX/2015, when in fact I only visited on two occasions : XXXX/XXXX/15 and XXXX/XXXX/15. At the time of each of my visits, I paid a {$20.00} co-pay per visit per the requirements of my insurer, XXXX XXXX XXXX, and the requests of XXXX XXXX 's office. The insurer stated on their website and confirmed via phone that {$20.00} was the correct co-pay amount. XXXX XXXX is a XXXX Preferred provider under XXXX policy, clearly documented by a screenshot of XXXX website and my phone conversations with their representatives. However, XXXX XXXX 's office inaccurately charged both my husband and I an additional {$40.00} per visit ( including for a third visit that never occurred ) and submitted the difference to collections. I have contacted XXXX repeatedly regarding this matter, who confirmed that the correct co-pay amount is {$20.00}. I spoke with several representatives there recently, who confirmed this again and stated that XXXX has contacted XXXX XXXX 's office with the same information. I have also contacted XXXX XXXX 's office repeatedly regarding this issue and stated in writing that there was no basis on which to submit this matter to collections, and repeated that assertion recently. Per the attached documentation from the collections company, Armor Systems Corporation, XXXX XXXX 's office has cancelled all claims for my spouse, who had the exact same issue with XXXX XXXX 's billing practices. However, my claim falsely remains in collections. Per this documentation, I have asked XXXX XXXX 's office to remove these bills from collections immediately, however they have refused to do so. I have also asked that the office immediately contact the credit reporting agencies - XXXX, XXXX, and XXXX - to have these items removed from both of our credit reports. It is troubling that XXXX XXXX 's office failed to exercise any due diligence in obtaining accurate information before submitting these claims to collections, and has still failed to resolve this matter nearly a year later. Clearly, this is evidence of deplorable and irresponsible business practices that harm consumers such as myself.

In early XXXX I had contacted XXXX in regards to a Medical bill on my credit XXXX. I had talked to a lady in the billing department and she informed the details and what insurance did not cover. Said that I had to make the payment of ( I can not recall the exact figure ) which would have been the discounted rate if I had orginally paid it shortly after the er visit. She said she would let the Collection Agency now but I would have to pay it directly through them as it was in their possession. I had requested with the lady XXXX that the negative mark be removed by the agency and she said that would be no problem as long as the account was paid. I call and spoke with Armor Systems and made the arrangments I asked the collector that it be removed once paid and was told " Yeah Sure '' I asked for it be placed in writing and was informed that it could not be placed in writing but would be removed from all the collection bearus that it had been reported to ... A couple months later when it still was not removed after it had been paid I once again reached out to Armor as the reported it paid but did not remove the negative mark. I was told then they would not so I took it up with the CB and still they did not remove it. I call XXXX/XXXX/XXXX was transfered to a woman in the credit department who after a couple mins answered and she did not even ask for my information and had obviously pulled my info off call id. Said that they would only marked as paid asked me hold and after XXXX mins of silence the phone was hung up on their end. I called back from a different number and asked if I could have the name of the female who is in the credit department they said they could only transfer me and after XXXX mins longer the same woman picked up and once I told her my name she placed me on hold for another XXXX mins and once again the line was disconnected on Armor 's end.

Upon checking my credit report in XX/XX/XXXX, I noticed there was an error in status on one of my accounts. Armor Systems Corporation indicated I had an unpaid account. A phone call to them verified that the balance was indeed paid. I asked that they update my account to indicate such. I also asked for written confirmation. I never received the written confirmation. On XX/XX/XXXX I again checked my report and the status went unchanged. On that day I sent a letter to Armor Systems Corporation informing them that the status of my account with them was incorrect, asking again for a correction. No change and no response. I sent one more letter to them on XX/XX/XXXX asking for validation. Again, no change and no response. The incorrect information represents a very serious error in their reporting, and is also one reason I have recently been denied credit. By not replying in a timely manner to my initial letter on XX/XX/XXXX they have not only violated federal and state laws, but have also failed to provide me with a copy of any viable evidence, bearing my signature, showing the account is being reported accurately. At that time I decided to try going through XXXX to get this updated and on XX/XX/XXXX I sent XXXX a letter asking for validation and proof of my 'unpaid ' balance. XXXX sent an automated response indicating the account had been verified?!? This is either a blatant lie by Armor Systems Corporation and/or XXXX, or extremely negligent reporting. Either way, I now want this account completely removed from all 3 of my credit reports.

I learned that XXXX Systems reported account # XXXX to the credit reporting agency. On or about XXXX XXXX, 2016, I contacted Armor Systems and I stated that I was disputing the debt owed. I was told to put it in writing and email a letter to them. The letter was emailed. On or about XXXX/XXXX/16, I contacted Armor Systems XXXX and XXXX XXXX from the credit department hung up on me stating that she does not have to assist me. Called back and spoke to XXXX XXXX again and she stated that they never received the letter emailed to them on XXXX XXXX, 2016. I again stated to XXXX XXXX that I was disputing the debt and will forward another letter to them. I asked XXXX XXXX what contact information did they have on file for me. XXXX XXXX stated that they tried contacting me at XXXX. I explained to XXXX XXXX that the number is disconnected. XXXX XXXX also stated that they sent a letter to XXXX XXXX XXXX XXXX, XXXX XXXX. I told me XXXX XXXX that I 've never received a letter nor did I receive a phone call. I was not notified by Armor Systems about the debt until it was unfairly reported to the credit bureaus. I should 've been given an opportunity to dispute the debt but instead Armor violated the law by not allowing me the opportunity to dispute it instead the reported it to the credit bureau.

Armor Systems Corporation sent notice to me on XX/XX/XXXX that a debt for an unpaid ticket issued by the XXXX Police Department had been assigned to them and they were attempting to collect. They did not advise what this debt was about. I had to research the debt with the help of the XXXX Police Department, which revealed that this debt was for a XXXX XXXX XXXX that is owned by an ex-girlfriend. I had completed the paperwork months before this to take my name off this vehicles title and registration. Notice of this debt triggered legal action against my ex to rectify the title and registration issues. My ex confirmed that this was her debt and that she had contacted Armor Systems Corporation to both pay and reassign the debt to her. Meanwhile, onXX/XX/XXXX, I sent a certified letter advising I was formally disputing the debt and requesting a formal response within 15 business days of my letter. Armor Systems Corporation did not respond to this dispute and did not reassign the debt. They sent notification on XX/XX/XXXX that the debt was paid. They have not provided any rectification for the assignment of the debt to me or even a response to my dispute.

A few weeks ago I checked my credit score and noticed an open account that I did n't recognize under the name " Armor Sys '' or something similar. A little googling resulted in me finding Armor Systems Collections so I figured I would give them a call. They found the account for {$170.00}, which is what my credit report showed as well. I asked for verification and also the company which reported it and they provided both. I called the original company that reported the debt and they have no record of me being a patient there ( it 's an XXXX I believe ). They were very suspicious of the charge and suggested I look into it as it sounded fraudulent. I do n't know what to do as calling Armor Systems Collections only results in them telling me I need to pay the debt plus interest and them putting me on hold for a long time.

I called Armor Systems Corp., a debt collector, to inquire about the contact information for XXXX XXXX. XXXX ( the debt holder ) so I may proceed with verifying the actual debt. I spoke with a manager, XXXX XXXX. She refused to give me the contact information for XXXX XXXX, belittled me, used a condescending tone, was nasty, mean spirited, and down right verbally abusive. Her level of unprofessionalism was shocking. Her abusive words brought me to tears. XXXX told me that she " would not give any information until I pay them some money ''. Thousands of people do not have health insurance/coverage in this country. Having medical debt should be handled with care and empathy, not ridicule/judgement or threats.

I have consistently reported fraud on my credit regarding several accounts. After countless times, XXXX deleted the numerous accounts due fraudulent/inaccurate information. I called both XXXX and XXXX and explained XXXX removed the accounts and I needed to get the steps to remove it on the reports as well. They explained they investigated the same information and it was reported accurately. At this time I need the accounts removed because it has tarnished my credit for years. I have been trying to get verification of this debt from Armor Systems. They have refused to send me documents that fully verified the debt. I have got some paperwork from them but it was only screenshots and no paperwork showing a contract of myself actually owing this debt. Also, failed to show a wet signature proving that this is my alleged debt. I will attach my mailed requests to this complaint. Armor Systems has been taking my state tax returns for 3 or 4 years without full verification of this alleged debt. They will take this current year 's state tax refund from me as well.

I believe that this company violated HIPPA because they discussed this account and my medical history with relatives. I dont think this is something that they are allowed to do. I am asking that they withdraw this account and send it back to the original medical provider. I am appalled that they shared any information about my account or medical history to anyone they get on the phone. I do not wish to deal with them and I will get an attorney involved if I have too. I would like for them to put in a request for them to remove this account from my personal credit reports.

I was notified via email by my credit monitoring company of 2 new collections accounts added to my credit report.
