RE : Attentively review my formal writ composed declaration of not proven complaint misinformation that is all or in part : deficient of the adequate current status in the fullness of TRUTH, ACCURACY, COMPLETENESS, TIMELINESS, documented OWNERSHIP, certifiable RESPONSIBILITY, or otherwise irrefutable, complaint obligatory, and regulatory requisite reporting thereby mandating your immediate actions to rectify and remedy and any infractions behavior ( s ) by retaining or returning to demonstrate TRUE, CORRECT, COMPLETE, and COMPLAINT METRO 2 data field formatted reporting!!! I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with XXXX , XXXX , and XXXX. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The Federal Trade Commission, which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act. Failure to comply shall lead to complaints being filed with The Better Business Bureau, Consumer Financial Protection Bureau, and Attorney General stating your companys unlawful collection practices, misrepresentation of creditworthiness, defamation of character, and noncompliance to abide by laws enacted by Congress to protect victims of identity-theft who's credit been impacted by acts of fraud. Because I know my rights under THE FAIR CREDIT REPORTING ACT, CONSUMER CREDIT PROTECTION ACT ( CCPA ), and FAIR and ACCURATE CREDIT TRANSACTION ACT of 2003 ( FACTA ). I will be taking action. I also know my rights under this paragraph of Section 312 of the FACTA LAW : SEC.312. Procedures to enhance the accuracy and integrity of information furnished to consumer reporting agencies. ( b ) duty of furnishers to provide accurate information. -- Section 623 ( a ) ( 1 ) of THE FAIR CREDIT REPORTING ACT ( 15 U.S.C. 1681s-2 ( a ) ( 1 ) is amended ( 1 ) in subparagraph ( A ), by striking " knows or consciously avoids knowing the information is inaccurate '' and inserting " knows or has reasonable cause to believe that the information is inaccurate '', and ( 2 ) by adding at the end of the following : ( D ) DEFINITION -- For purposes of subparagraph ( A ), the term 'reasonable cause to believe that the information is inaccurate ' means having specific knowledge, other than solely allegations by the consumer, that would cause a reasonable person to have substantial doubts about the accuracy of information. Policy states. According to the Fair Credit Reporting Act FCRA 611 ( 15 U.S.C 1681 ), Procedure In Case of Disputed Accuracy. ( a ) Reinvestigation ( 1 ) Reinvestigation ( a ) In general. Subject to Subsection ( f ), if the completeness or accuracy of any item of information contained in a consumers file at a consumer agency is disputed by the consumer and the consumer notifies the agency directly, or indirectly through a reseller, of such dispute, free of charge, conduct a reasonable reinvestigation to determine whether the disputed information is inaccurate and record the current status of the disputed information, or delete the item from the file in accordance with paragraph ( 5 ), before the end of the 30-day period beginning on the date on which the agency receives the notice of the dispute from the consumer or reseller. Therefore, my position is being clearly stated, these accounts do not belong to me, meaning that you are reporting inaccurate account information within my credit file. The fact that these are inaccurate account information means that you, this credit-reporting agency, are reporting incorrect accounts. Finally, you are reporting the INQUIRIES that are the result of the same type of fraudulent activities. It must be noted that I did not apply for credit accounts with the below-listed companies and creditors, some of these are the result of attempts to open accounts, but the companies did not approve the accounts. Therefore, these inquiries must be deleted from within my credit file. You can not delete the fraudulently opened accounts without also deleting the fraudulent inquiries. It should be noted that an inquiry that was not made by me is not a factual record, just as a fraudulently opened account is not a factual record and not referred to the creditor to be disputed directly with them. The credit reporting agencies have attempted to address an inquiry as a factual record of file access, but if it was done through fraudulent means, it is not a factual record. As a consumer, I should not be required to dispute each fraudulent inquiry directly with each company. These inquiries are the result of fraudulent activity and therefore, inaccurate in which the credit reporting agencies or the creditors can not verify, meaning that these inquiries are unverifiable information and must be deleted from my credit files. 1. XXXX XXXX XXXX XXXX XXXX bal. {$75.00} I am well aware of my rights as a consumer. The Fair Credit Reporting Act requires that you, the credit-reporting agency report accurate and/or correct account information within my credit file. My credit issues are very specific, these accounts and/or items do not belong to me. This means that you are reporting incorrect account information within my credit report. It is this in mind that I request that these listed accounts and items be Blocked and Deleted. According to the Fair Credit Reporting Act FCRA, Section 1681c ( 2 ) Block of information resulting from identity theft. 1.Block. Except as otherwise provided in this section, a consumer reporting shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from alleged identity theft, not later than four ( 4 ) business days after the date receipt by such agency of 1. Appropriate proof of identity of the consumer ; 2. A copy of an identity theft report 3. The identification of such information by the consumer ; and 4. A statement by the consumer that the information relating to the transaction by the consumer. I have provided all of the above listed four ( 4 ) items within this package. Therefore, I expect these listed accounts to be deleted and blocked within ( 4 ) business days. This is clearly an identity theft issue. I am also entitled to receive a free copy of my credit report, so I am requesting a free credit report and that all of these accounts, public records items, and inquiries be permanently blocked and deleted. To Whom It May Concern : I, XXXX XXXX XXXX a natural person, and a consumer, received a copy of my consumer report, and noticed and have proof that I am a victim of identity theft as defined under 15 USC 1681a ( q ) ( 3 ). 15 USC 1681a ( q ) ( 3 ) -The term identity theft means a fraud committed using the identifying information of another person, subject to such further definition as the Bureau may prescribe, by regulation. First Federal Credit & Collections has stated that I, a consumer, owe an alleged debt of {$75.00}. This alleged debt has been furnished to my consumer report and is a violation of 1692b ( 2 ). See attached I, the consumer, have not given any prior consent or received any documentation before the communication of this alleged debt. This is a violation of 15 USC 1692 ( c ) ( a ) First Federal Credit & Collections engaged in conduct that has harassed, oppressed, and abused me in connection with the collection of this alleged debt. Oppressing a right would be conducted not allowed under the FDCPA and is a violation of 15 USC 1692d. Credit is my right granted and protected by federal law. First Federal Credit & Collections has caused damages to my financial reputation by furnishing this alleged debt to my consumer report in order to coerce payment. This is a violation of 15 U by SC 1692d ( 4 ) and is known as debt parking described by the FTC. First Federal Credit & Collections has communicated a false representation of the character and amount of this alleged debt. This alleged debt is furnished on my consumer report in a negative amount. This is false and misleading and a violation of 15 USC 1692e ( 2 ) ( A ). First Federal Credit & Collections has knowingly furnished the credit reporting agencies with false credit information which is a violation under 15 USC 1692e ( 8 ) All of these actions were false and deceptive and a violation under 15 USC 1692e ( 10 ) First Federal Credit & Collections has not provided me with any validation of this alleged debt under 15 USC 1692g which should include : 1 ). amount of debt. 2 ). the name of the creditor to whom the debt is owed. 3 ). A law which states unless the debt within thirty days after receipt of the notice, disputes the validity of the debt, or any portion thereof, the debt will be assumed to be valid by the debt collector 4 ). Another law which states if you notify the debt collector in writing within the thirty-day period that the debt, or any portion thereof, is disputed, the debt collector will obtain verification of the debt or a copy of a judgment against the consumer and a copy of such verification or judgment will be mailed to the consumer by the debt collector. And a final law which states upon the consumer 's written request within the thirty-day period, the debt collector will provide the consumer with the name and address of the original creditor, if different from the current creditor. My failure to dispute the validity of this alleged debt may not be construed as an admission of liability! 15 USC 1692g ( c ) The failure of a consumer to dispute the validity of a debt under this section may not be construed by any court as an admission of liability by the consumer. These abusive and deceptive actions have caused a denial of credit and First Federal Credit & Collections is liable to me for actual damages sustained due to violations committed under the FDCPA {$1000.00} per violation.

On XX/XX/XXXX, I contacted First Federal Credit & Collection ( FFCC ), a debt collection agency based in XXXX, Florida, to discuss an alleged medical debt of {$500.00} reported to the credit bureau in XXXX. That same day, FFCC wrote me an e-mail requesting that I provide them with a letter from XXXX stating that my son was covered on XX/XX/XXXX. FFCC also requested a copy of my son 's XXXX I.D . card and I.D . number. One month later, I received a phone call from FFCC regarding the XXXX letter. I advised them that obtaining the letter was taking much longer than expected. On or around XX/XX/XXXX, FFCC reported a " late payment '' to the credit bureaus for the first time in four years. On XX/XX/XXXX, I e-mailed FFCC a letter from XXXX dated XX/XX/XXXX stating that my son was eligible for continued coverage in addition to sending a copy of the XXXX I.D . and I.D . number. My letter also stated that my son resides in a group home and is 100 % the responsibility of the State of Florida, including all of his medical expenses. I immediately received a phone call from FFCC advising that the letter from XXXX was insufficient because it needs to state that XXXX had coverage on XX/XX/XXXX. FFCC also threatened to continue to report the alleged debt monthly until paid. This threat was intended to coerce me into paying the debt by ruining my credit. I told FFCC during the phone call that the letter is sufficient proof that XXXX was covered on XX/XX/XXXX because the renewal Notices cover the entire year. I also advised that I was unable to get a letter from XXXX for a specific date as their computers are not set up to customize letters. Instead, XXXX provided me with a duplicate copy of the Notice of Action taken on XX/XX/XXXX. FFCC 's request for a letter from XXXX is merely a disguised attempt to harass me and force me to pay a debt that I do not owe. Indeed, the billing statement for medical services provided by FFCC shows that their client already billed XXXX for services on XX/XX/XXXX and XX/XX/XXXX. The last page of the billing statement shows that in both instances XXXX denied coverage because the primary insurance paid more than the allowed amount. Therefore, FFCC is already aware that my son was covered by XXXX on XX/XX/XXXX. Requesting the letter and continuing to report the debt to credit bureaus are just mechanisms for prolonging the matter to coerce payment for a debt that is not mine. FFCC is, in fact, a dishonest company. For example, FFCC reported a balance owed of {$500.00} when the final bill shows an outstanding amount of {$390.00}. I am filing this complaint against FFCC to resolve this issue as soon as possible.

Disputed : Reported Account Number : XXXX {$75.00} Creditor/Collection Agent ALCHRO INC. who are the parent company of XXXX XXXX XXXX XXXX XXXX.The mail to XXXX XXXX XXXX XXXX XXXX was sent certified delivery and has been returned on each occassion by USPS as UNDERLIVERABLE. Chain of Events : On XX/XX/2018 I reviewed my credit reports and found that there was an inaccurate report of an account that was not mine. XX/XX/2018 I contacted the company XXXX XXXX XXXX XXXX XXXX who is named as the reporting company on my credit files in writing at the postal address stated as their mailing address from my credit report by letter delivered by certified mail ( see attached ) requesting full validation pursuant to the Fair Debt Collection Practices Act and the Fair Credit Reporting Act. My request for validation included competent evidence bearing my signature and a copy of the instrument used to secure a contractual agreement that proves I am obligated to pay was never made available to me. I made it clear in my letter that the negative marks found on my credit report held by the ( 3 ) reporting bureaus and reported by their company or any other company that they represent for this debt that I do not owe is a violation of the FCRA and FDCPA and I request that if this debt can not not be validated this company must take steps to have all reporting bureaus delete this entry immediately. It is ridiculous that is is now over 250 days since my request for validation ( see attached certified mail verified as delivered ) and I have not received a reply. This is over the 30 days time frame and this collection company is out of compliance. Under advise, I wish to informed XXXX XXXX XXXX XXXX XXXX that I am prepared to take steps to file a small claim case in my county small claims court. I would like to make this one last good faith effort and ask for the assistance of the CFPB in obtaining one of two things 1. that this company provide me with full debt validation as per FCRA & FDCPA 2. deletion of this invalidated account from my credit reports

In XXXX, 2015 I entered a written dispute with FIRST FEDERAL CREDIT & COLLECTION reference account number XXXX as contained on my consumer credit profile at XXXX, XXXX and XXXX XXXX. Specifically alleged, FIRST FEDERAL CREDIT & COLLECTION was attempting to collect a debt for which I had no knowledge and alleged was potentially opened by an unknown third party. Moreover, said account was appearing XXXX on my consumer credit profile notwithstanding the allegation of a single debt owed. FIRST FEDERAL CREDIT & COLLECTION failed to investigate, verify, or validate said debt. Although I also filed a cease and desist demand with FIRST FEDERAL CREDIT & COLLECTION an absolute requirement of the Fair Credit Reporting Act is to report said debt as disputed pending resolution of an investigation. FIRST FEDERAL CREDIT & COLLECTION failed to report said debt as disputed and failed to conduct an investigation into my allegations of potential fraud. As such, my credit worthiness has been negatively impacted by the erroneous reporting of this account. A review of records compiled by the state XXXX XXXX XXXX XXXX in Florida indicates that FIRST FEDERAL CREDIT & COLLECTION is not currently licensed to conduct business pursuant to section XXXX, Florida statute. As such, the imposition of this debt and the erroneous reporting to XXXX, XXXX and XXXX XXXX is an unfair and deceptive trade practice. Several phone calls to FIRST FEDERAL CREDIT & COLLECTION have met upon deaf ears. When I called to inquire on the status of the disputed account, I was advised that Florida licensing was not required. I was further advised that the debt remained due notwithstanding a lack of validation and claim of potential fraud. At this point the call was terminated. My written dispute remains outstanding. Said account remains on my XXXX, XXXX and XXXX XXXX notwithstanding the existence of a dispute, lack of validation, and my allegations of potential fraud.

Once more I come to this agency with the attempt to solve this issue but appears nobody cares! That's the main reason why people get abused, cheated and extorted. But it is my right to dispute a debt I definitely don't own I will exercise my rights to the very end to clean up my name and credit. On XX/XX/2020 I was feeling really bad at night with certain symptoms that for me appeared to be XXXX. At that time everyone was scared and with a lot of misinformation related to the XXXX. I went to the emergency of XXXX XXXX seeking help because that was the first good urgent care facility close to me that came to my mind. Them I immediately reported that if i needed to pay for the services I had no insurance at the time. The assistant told me that I have not to concern about it because assistance related to XXXX would be covered by the govern. They made a series of exams and, prescribed a fogging session and dispatched me home. Thanks XXXX was not XXXX and the following day I woke up better. I couple of weeks later I received a bill from the Hospital requesting payment for the services. I immediately called the financial department and after explaining the situation they told me the bill would be taken care of and I had nothing to be concern. Them I realized they sold the collection to other companies and did not have done once more what they told me. This is outrageous, immoral and anti-ethical, especially when taking advantage of people when they are in a very fragile moment. That's really not fair and that's the reason I insist and dispute this unfair practice that for sure is not only from them, but the great majority of the Medical system. Their main concern should be saving lives.

I previously opened a complaint on this company on XX/XX/18. The complaint # was XXXX. Here is what I indicated : The company XXXX XXXX XXXX XXXX XXXX has listed a debt on my credit reports in the amount of {$75.00}. I've repeated disputed that the debt is mine and they have refused to remove the debt from my files. I have never contracted with this company, nor do I have any knowledge of this company. This company is hurting my credit scores and preventing me from having access to financial products that would help me to better my life. The company responded with the following on XX/XX/18 : Due to XXXX regulations, we are unable to provide consumer with account information. She disputed account through credit bureaus and the account was verified as all the information matched. The consumer may contact XXXX at XXXX ext XXXX to obtain an itemized bill for the charges. ********************* I have called this company on numerous occasions and have yet to get someone to answer the phone. I did a web search on this company and every review has the same response that they will not answer the phone. I question the legitimacy of this company. I called the extension listed and the phone continued to ring for an " XXXX '' that never answers the phone. I tried selecting other extensions and still no answer. I was NOT sued - it 's the closest radio button to what occurred. My FIRST notice of this outstanding debt was an Alert from XXXX credit monitoring on XX/XX/2016. I contacted the collection agency and spoke with XXXX XXXX for information. Turns out its a bill from a drs office - I never received the bill for XXXX b/c they were admittedly mailing it to an address in XXXX XXXX - instead of my City of residence in XXXX. I informed them that they needed to remove this since I never received proper notice. After speaking to the Drs office who was apologetic about the mistake and indicated the collections agency would correct and remove as a mistake. Today I see that they added it AGAIN as UNPAID debt. Meanwhile the outstanding amount was paid YESTERDAY after a discussion with the Drs office. It 's retaliation and harassment. XXXX XXXX was refusing to acknowledge that their reporting was in violation of the FCRA during our phone conversation and intentionally pushed through an " UNPAID '' report to XXXX after I paid. According to the Drs Office - they notified the collections agency to cease and CORRECT the report - Clearly it was pushed through incorrectly on XX/XX/XXXX.

My husband went to our XXXX 3 times. Once was for a XXXX. XXXX. The XXXX went away on it 's own, and while XXXX paid for the XXXX and XXXX, they did not pay for the XXXX. My husband was not made aware of the fact that XXXX would not, or may not, pay fort he XXXX when the XXXX was taken. I believe this should have been done BEFORE the XXXX was taken. His second visit was because he had a XXXX. He paid the co-pay, XXXX, or so she thought. The third visit was because the XXXX. Most XXXX would have made the XXXX without additional cost, but this XXXX charged a co-pay AGAIN, which my husband paid. All of these visits and charges were posted onto MY account with the XXXX, not my husband 's. I do not know if the XXXX correctly submitted the XXXX to XXXX or not, since it is on my account rather than my husband 's. Now, there are XXXX collection notices on MY credit report. On Wednesday, XXXX XXXX, 2017 I contacted XXXX XXXX XXXX XXXXXXXX to pay a past due bill of {$150.00} on my credit report. A bill I did n't know I had, but wanted to pay for. I was informed that the total amount due was {$200.00} that included a " Deletion fee of {$15.00} '' I paid the {$15.00} because I wanted to have the record deleted immediately. This company reported to the credit agency that I did not pay the debt. I explained multiple times I was not the responsible party that was financially responsible, even in their validation of debt that the company sent to me. On XX/XX/2018 First Federal Credit & Collections added a derogatory mark on my credit report per XXXX XXXX. In May a debt I had for a medical service I received last year was sent to collections. I began making payment relatively soon thereafter, eventually landing on $ XXXX towards the {$1300.00} debt. As I was making payments, right after a month where I had actually made two payments, they added the report to collections on my credit. I was under the impression that the debt was being paid so it would not go to collections. I believe that paying off the debt on the payment plan should have prevented me it from going on my credit report. Moreover, I should have received notice before it went on my credit report. Upon receiving my recent Credit report, I have found information regarding debt that is being claimed against me. I've been working diligently to improve my credit, despite having difficulties with collection agencies in the past. I've done my research on debt collection practices and how to manage my personal finances better. I've been saving for quite some time to pay off my accounts and finally did. This was difficult as the collectors were unwilling to work with me at times. That said I was able to pay off the accounts by starting with a new slate on my credit. The collection agencies in question have not removed the accounts from my credit report yet the rest being damaged because of it. I was advised by the Internal Revenue Service that my social security number and other confidential information were compromised. I 've never recieved a bill from this company at all. I did n't even know what it is for. So when I called to inquire about it the supervisor/owner ( XXXX XXXX ) offered me a {$10.00} addition fee on top of what what I already owe to get it deleted as well. Like he was doing me a favor.
