I placed an online order with XXXX XXXX XXXX on XX/XX/XXXX, with delivery scheduled ( via the tool on their website ) on XX/XX/XXXX. Every item I ordered was clearly marked as In Stock and Ready to Ship. My order did not ship on XXXX. I contacted the company over and over, and was repeatedly promised shipping dates that came and went.
Finally, on XX/XX/XXXX, I notified them that theyve missed too many deadlines and directed them to cancel the order. On XX/XX/XXXX, I received an email saying that they were shipping my order. I replied within 11 minutes that the order had already been canceled. On the same day, I received an email from XXXX XXXX from XXXX XXXX XXXX, apologizing for this unfortunate situation and assuring me that she would contact XXXX to have the order returned. Per XXXX, the order was returned back to the company on XX/XX/XXXX. Now 6 months later, they've fraudulently sent the account to collections ( a total of {$1000.00} ).

I paid a bill to XXXX XXXX XXXX and XXXX on XXXX/XXXX/2016. This payment was posted to my checking account on XXXX/XXXX/2016. As per XXXX at Patient Services, the payment processing service for XXXX XXXX XXXX and Imaging, their records confirm that my payment was received on XXXX/XXXX/2016 and the account was sent to collections on XXXX/XXXX/2016. As it is unlawful to attempt collection on an account that has been paid in full, I request an immediate investigation of this alleged violation of the FDCPA
