It is essential to note that I have never entered into any contractual agreement or financial arrangement under the name mentioned in your correspondence. Therefore, I request that you provide proper verification and documentation to establish the accuracy and legitimacy of the debt in question. According to my rights under the Fair Debt Collection Practices Act ( FDCPA ), specifically section 15 USC 1692G, I am entitled to receive this information upon my request. Furthermore, I must emphasize that I have not received the complete medical bill from the healthcare provider involved. The bill lacks essential information, including the medical coding, place of service address, and Tax ID number, which are necessary for my insurance company to process the bill appropriately. They said if I didnt pay a substantial amount, to which I said I couldnt afford, that they would garnish my wages with my employer. I informed them that the debt was charged off by a previous debt collector, and that Id been penalized by the IRS for canceled debt that I didnt claim on my XXXX taxes. I also said that I was well within my rights to no longer be in danger of being sued for this debt. I stated that I would need proof of the debts authenticity, and I would respond with what I would do next. I received a letter about a week later, and in the letter it said I had only 14 days to respond or they would take further action. I understand that legally I have 30 days to respond. Unfortunately, just last week, I got a letter from my payroll department, stating that a judgment for wage garnishment had already been issued. It also said I was served with the proper paperwork, on XX/XX/XXXX, but I was never served with anything. Furthermore, I was at another location when the company says they served me. I have still not received anything but a letter from my employer saying that wage garnishment has already gone into effect. Im going to respond within a 60 day timeframe, but the debt has surpassed the statute of limitations in the state of Washington where I live. This debt collector has bought old/expired debt, and is attempting to make money off of it.

My bank where I do business notified me when the underwriter pulled my credit report there was a collection on my credit report for a medical bill for the amount of {$540.00}. They asked me if I was aware of it which I was not. They did not send me a letter. They suggested that I pull my report off of XXXX for review and dispute. To my surprise there it was XX/XX/2023, and a collection was placed on my credit reports with all credit bureaus. I became upset then it was suggested to me to report the agency for bad business practices and deception. They didn't give me the opportunity to investigate the matter. This now explains the people calling all day asking for me then when I say can I help you? They just hang up. On one of my credit reports, there's the threat of having an attorney. I have not given any agency permission to buy or sell my personal information. Today I sent them a letter by mail and faxed it to the address shown on my credit report whenever they receive it for further information. I would like to know more concerning this matter. This has caused me detrimental harm concerning my personal life and business by placing derogatory information on my personal credit report without my written permission. ***I Could not attach a copy of the letter that was sent this evening XX/XX/2023 by fax and another by mail. I have invoice from XXXXXXXX XXXX XXXXXXXX XXXX stating the same. I reached out to Nationwide Recovery Service ( Collection Agency ) why this debt collection was raised. They requested for invoice. I Provided them invoice. Collection Agency told me they will get to their client who raised the debt and will get back. It has been more than a month, and I have reaching to them to check but they are like we have not heard from their client. Instead collection agency is raising it as debt to all credit bureaus which is hammering my hard earned credit score In 5 years I never got any Call/email/ mail about this I asked them any ETA or date I should wait, they have no answer. They told me to tell Nationwide to connect with them, but Nation wide is not ready to listen to my request. I have reached out to Nationwide Recovery Service and XXXX XXXX XXXX XXXX ( under XXXX XXXX ) multiple times over the last 2 years to get these debts removed from my credit report that were already paid. Back in XXXX my XXXX was in the XXXX and XXXX claimed that they provided care for my XXXX for anesthesia and Iv. My XXXX never needed XXXX XXXX XXXX and never saw the doctor listed as the primary care doctor for the XXXX. A year later in XXXX XXXX in charges were placed on my credit report. My insurance ( XXXX XXXX ) paid for these charges but XXXX or XXXX never confirmed with the credit companies that they were paid. After filing multiple disputes they all came back as met fcra requirements Im not sure how this is possible when no one has been able to contact anyone from XXXX and XXXX continues to say they do not own that company and are not aware of accounts in my or my XXXX name. At this point in time I feel like these are fraudulent claims, seeing as I am not the only person who has had issues like this with XXXX or XXXX XXXX XXXX XXXX. I have the EoB proof from XXXX that these were paid, and the checks were cashed 3 days after they were received. Yet still show as I need to pay them on my credit report. I have additional documents with all the charges that were paid.

On XX/XX/2022 Nationwide Recovery Service reported eleven ( 11 ) separate collections accounts on my credit report. I had not been contacted by them prior to this. On XX/XX/2022 I called and spoke to an agent. I did not verify my address, in fact I told them the address they gave me was wrong, did not verify my birthday or any other PIH information and the agent provided me with dates that " I '' was in the urgent care. I staunchly believe that to be a HIPPA violation. Once I explained this breach of privacy as they did not verify it was in fact me on the phone, approximately 20 minutes later re-reported the debt on my credit report. The agent was extremely rude and retaliated when I stated that I would be making a complaint. Secondly, after reviewing the new changes to medical bill reporting, I called and asked why these debts were still on my credit report as all eleven ( 11 ) are under {$500.00}, with the most being {$320.00}. The agent said that because the total was more they won't take them off, however they are each reported as individual accounts. That agent began yelling at me in an extremely rude manner and hung up as I was trying to ask a follow-up question. I am very afraid of this company and what they may do to retaliate.

Upon checking my credit using XXXX XXXX, I noticed a delinquent account from Nationwide Recovery Services for an amount of {$240.00}. The account information states that it was opened in XXXX of 2021. I check my credit regularly and have never seen this attempted collection on my account before, nor do I know what the charge is for. The original creditor is XXXX XXXX XXXX by XXXX. I did not receive any communication from them in regards to this charge prior to the opening of the XXXX account. I contacted Nationwide Recovery Services twice for more information about this account. The first representative was unable to locate the account. They searched their database using my social security number, my first and last name, just my last name, and any addresses that may have been associated with me. None of my information had any accounts associated with it. The second representative was also unable to locate the account. I believe this account has been incorrectly linked to me. Because it's not linked to me, I'm unable to even pay the balance and prevent the delinquent account from appearing on my credit report.

on Wednesday, XX/XX/XXXX I started receiving phone calls from nationwide recovery systems from various spoofed numbers in an attempt I believe to conceal their identity and prevent me from knowing who they were. after getting multiple calls from the same number I decided to call them back XXXX XXXXXXXX XXXX during our 4 min call I asked that they send any info they had to the address on file and that they stop calling me and anyone associated with me via any means. I was transferred to a supervisor who stated that he would note my file and send a notice to the address on file ( which I have yet to receive ) I refused to verify anything as I did not want to have my words twisted and miss construed. in spite of my request on XXXX, XX/XX/XXXXXXXX XXXX XXXX XXXXXXXX the company called me from another spoofed number. The Fair Credit Reporting Act ( 15 U.S. Code 1681 ) says ( 1 ) The banking system is dependent upon fair and accurate credit reporting. Inaccurate credit reports directly impair the efficiency of the banking system, and unfair credit reporting methods undermine the public confidence which is essential to the continued functioning of the banking system. So whenever there is a violation under the FCRA, it impairs the efficiency of the banking system. This is a violation of my privacy with my consumer report, since I control what is listed and what is not. In the event that any accounts are reported without my written consent, that is considered identity theft. This is a violation of both 15 U.S. Code 1681b ( 2 ) and 15 U.S. Code 1681c-2. This leads way to civil liability against the credit reporting agency for negligent and willful noncompliance under 15 U.S. Code 1681n, which allows for {$1000.00} per violation, for account listed without my expressed written consent.

Debt with Nationwide Recovery was discharged earlier this year ( this is in my CFPB file ). The debt was sent to Nationwide Recovery in error by XXXX XXXX XXXX which they admitted to when they discovered that they never filed the insurance claims. I put XXXX and XXXX together on a phone call and this was resolved. The problem here is Nationwide Recovery who were instructed to back off, which they did at the time. Now they are coming at me again and impacting my credit rating. Nationwide Recovery is not operating ethically and I need these folks to clean their records and leave me alone

I disputed with this company a month ago and their information came back verified. But I stated then that I was a victim of identity thief and that should have never came by verified. Nationwide Recovery Services uses dozens of phone numbers that you can not pin down which one they call from, I know I've blocked at least a dozen or more personally. They have called while I am at work and can not be disturbed by it and have called on weekends. I have seen them call at least 5 times per day, with different numbers and different operators from the exact same company.
